There is little research evidence about how the mental health professionals are coping with the complexity of everyday practice in psychiatric acute care. The aim of this study was to explore mental health professionals' reflections on their work on an acute psychiatric ward. Data were collected using participant observation and interview methods. Three core themes were identified from a qualitative hermeneutic analysis. The first core theme, coping with uncertainty, uncovered a dialectical pattern of the factors contributing to thriving and strain in the working situation. The second core theme, caring for the patient, included the caring process, patients' pathway to acute psychiatric care, as well as the patients' needs and roles on the ward. The third core theme, coping strategies, included five different methods the primary nursing system, concealing versus integrating, milieu therapy, seclusion and the medical orientated model. It was concluded that good mental health care is a result of collaboration between health professionals and the health services. This study highlights the need for support to professionals and for establishing structures that will enable collaboration to take place. Taken together, this may contribute to enhancing the care of the patient and their families.

This study addressed the factors that nursing staff perceived as creating job satisfaction in their working environment in addition to addressing the ethical dilemmas that staff experienced within an acute psychiatric care setting. It also addressed how clinical supervision contributed to job satisfaction among staff as well as the differences between staff who attended and staff who did not attend to clinical supervision. Data were analyzed using descriptive statistics. Overall, the results of this study showed that the factors influencing nurses are related to areas of dissatisfaction, for example, stress and experiences with shortcomings. Factors that contribute to job satisfaction or dissatisfaction were found to be related to the nurses' value systems. The ethical dilemmas that were specifically addressed involved how to care for patients and handle work in relation to patients' autonomy, how to approach the patient, how to provide care against the will of the patient, and what action was ethically right for each particular patient.

During the last decade in the Nordic countries, the qualitative research interview (QRI) has become a fruitful method for the study of how people experience their life-world, especially in social and professional contexts. The essential features and structural components of the QRI have been well developed, and various aspects of the interpretation of interview texts have been analyzed and theoretically assessed from the hermeneutic-phenomenological point of view. The validity, reliability, and fruitfulness of the method also have been constructively scrutinized. The present study, which examines the choice of appropriate QRI strategy and technique as illustrated with concrete examples, focuses on the specific area of psychiatric nurses' professional thoughts and experiences in community psychiatric nursing. This scientific article seeks to stimulate and illuminate the right use of QRI.

Mental health services are in transition. The question is to what extent professional training programmes within the health and social sectors have taken into consideration claims and expectations on professionals following this organizational transition. The purpose of this descriptive study was, by means of a semi-structured questionnaire, to evaluate Norwegian postgraduate programmes of psychiatric nursing with respect to what extent they had met the psychiatric nurses' need for competence in their clinical work. Only few evaluations of this type are reported from Scandinavia, and, consequently, it is important to conduct such studies to elucidate strengths and weaknesses in existing programmes. In all 154 psychiatric nurses participated (response rate 48%). The study revealed that psychiatric nurses perceived their training as significant to their practice--and that their professional identity and competence had been strengthened. This was connected to the possibility of studying their own professional field in depth--and to clarify their field of responsibility. Thus, the respondents regarded an alteration of the postgraduate programme in an interdisciplinary direction as unfortunate. Such an alteration might contribute to a weakening of their specific competence and professional perspective. The respondents held that productive interdisciplinary co-operation presupposes a multi-disciplinary approach. The postgraduate programmes seem definitely to qualify psychiatric nurses for working in psychiatric institutions, but their training is also relevant for working in the community health services.

In the last decade the qualitative research interview, based on hermeneutic-phenomenological understanding, has become a successfully used method in the Nordic countries, in the study of how persons experience their life-world (Ger. Lebenswelt) in social interaction and or in relation to their professions. The essential features of the qualitative research interview and its structural components have been quite well developed in these countries. Various aspects of the interpretation of interview texts have been theoretically analyzed from the hermeneutic-phenomenological viewpoint, and also its validity, reliability and fruitfullness have been carefully and constructively scrutinized. Necessary conditions to stimulate the 'right' use of qualitative research interview are, e.g., scientific articles which clearly describe how the method has been successfully applied on, inter alia, mental health and research dealing with public health. In this respect it is desireable to describe 1) how the choice of different strategies and techniques within the domain of qualitative approach are made and and illustrated, by means of concrete examples, and 2) to show how they are systematically and effectively applied in the research process. The purpose of this article should be seen against the background of 1) and 2), i.e., the article purports to show how the qualitative research interview was systematically used in studying psychiatric nurses' professional thoughts, experiences and practices in community psychiatric nursing, a study which attempted to do justice to the attainment of both goals 1) and 2).

The authors offer a holistic model for psychiatric nursing based on a existential concept of man. They assert that the biomedical model of health and sickness is not sufficient to understand the reality in which psychiatric nurses practice. Within this model, understanding clients in relation to their life contexts becomes the focus, as well as trying to comprehend what effects suffering has on their ability to function. The relationship between nurse and client is discussed from a point of view that considers central existential precepts, such as: equality, encounter/being present, self-esteem/significance, responsibility and choice, objectives, meaning and clarification of values, guilt and atonement, openness and insight, conformity versus rebellion and finally hope.

The purpose of this descriptive study was by means of a questionnaire to obtain data on how 13 nursing schools, offering a postgraduate specialization in psychiatric nursing in Norway, have incorporated a public health perspective into their curricula. Since the mid-1980s, national health policies and Health for All by A.D. 2000 have placed emphasis on decentralization of all health care services, including services for the mentally ill. The Community Health Law of 1984 guarantees all Norwegian citizens such services in their local communities. The schools have traditionally emphasized curative services in hospitals, rather than health promotion and preventive measures at community level. Are the new graduates prepared for psychiatric work at community level? A content analysis of a semistructured questionnaire revealed that a social psychiatric perspective has indeed been incorporated in today's curricula. This was supported by reviewing the required reading lists for each of the 13 schools. Roughly one can say that the schools represent two perspectives: 1) those focusing primarily on relational aspects, and 2) those focusing on relational aspects within a social psychiatric framework.
