6. Monitoring complaints about food marketing to children under the Australian industry Codes 2015–20: a qualitative analysis

Abstract

: Objective: To analyse the case reports of complaints regarding food marketing to children over six years and compare Australian marketing Codes to best practice recommendations. Methods: Case reports on complaints about food marketing to children under the five industry Codes – the Responsible Children's Marketing Initiative, the Quick Service Restaurant Initiative and the Australian Association of National Advertisers Code of Ethics, Code of Advertising and Marketing to Children and Food and Beverages Code – were qualitatively analysed. Reports on the Ad Standards website in the food/beverage groceries and food/beverage venues categories from 2015‐2020 were investigated. The most common clauses from the Codes were identified and quotes from reports used to illustrate the determinations. Codes were compared with World Cancer Research Fund recommendations on policy to protect children. Results: Only 14 of 119 complaints resulted in a reported breach of industry Codes. The most common reason for dismissing complaints involved clauses requiring advertisements to be ‘primarily’ directed to children. The Codes did not align with best practice recommendations. Conclusions: Complaints by the public show concern for food advertising to children but the Australian industry Codes fall short of addressing those concerns. Abstract

Background: Nutrition and health claims influence consumer purchasing. Claims include content claims, which refer to the amount of a nutrient contained in a product, and health claims, which refer to health benefits of foods or nutrients in a product. Products that display a health claim must meet the Nutrient Profiling Scoring Criterion (NPSC). The present study aimed to explore consumer perceptions of content claims used on food and beverage labelling and advertisements. Methods: Semi‐structured focus groups were conducted with Australian consumers. Analysis involved an inductive, reflexive approach to thematic analysis. Results: Six focus groups involving 26 participants were conducted. Four main interconnected themes were generated: (1) complex factors influence food choice; (2) content claim scepticism; (3) the difference between content and health claims is unclear; and (4) the regulation of content claims is not common knowledge. Content claims were used, although generally viewed through a lens of scepticism and mistrust, and seen as a promotional tool for the food industry. Product complexity appeared to increase content claim use as a result of consumer uncertainty of the content of complex products, such as ultraprocessed foods. Most participants were aware that content and health claims were in some way regulated. Overall, they did not know further detail, including the relevant regulatory body. Conclusions: For content claims to support the consumer they need to be accurate and their use limited to healthier foods. This can be achieved by requiring products with content claims to meet NPSC thresholds, as required for products making health claims. Abstract

Purpose Delirium is a neuropsychiatric disorder delineated by an acute change in cognition, attention, and consciousness. It is common, particularly in older adults, but poorly recognised. Frailty is the accumulation of deficits conferring an increased risk of adverse outcomes. We set out to determine how severity of frailty, as measured using the CFS, affected delirium rates, and recognition in hospitalised older people in the United Kingdom. Methods Adults over 65 years were included in an observational multi-centre audit across UK hospitals, two prospective rounds, and one retrospective note review. Clinical Frailty Scale (CFS), delirium status, and 30-day outcomes were recorded. Results The overall prevalence of delirium was 16.3% (483). Patients with delirium were more frail than patients without delirium (median CFS 6 vs 4). The risk of delirium was greater with increasing frailty [OR 2.9 (1.8–4.6) in CFS 4 vs 1–3; OR 12.4 (6.2–24.5) in CFS 8 vs 1–3]. Higher CFS was associated with reduced recognition of delirium (OR of 0.7 (0.3–1.9) in CFS 4 compared to 0.2 (0.1–0.7) in CFS 8). These risks were both independent of age and dementia. Conclusion We have demonstrated an incremental increase in risk of delirium with increasing frailty. This has important clinical implications, suggesting that frailty may provide a more nuanced measure of vulnerability to delirium and poor outcomes. However, the most frail patients are least likely to have their delirium diagnosed and there is a significant lack of research into the underlying pathophysiology of both of these common geriatric syndromes.

Abstract

Transitioning to motherhood is recognised as a significant point in women's lives and, whilst many women make this move reasonably well, a significant number of women can suffer emotional distress. Women with diabetes can carry additional anxieties due to their fear for their unborn baby's well-being and are reported to carry higher levels of depression than other women during pregnancy. This article explores the challenges that can be faced by women with diabetes in pregnancy and suggests an underpinning approach to care called salutogenesis. Developed by Aaron Antonovsky in 1979, it offers diabetes specialist nurses and other health professionals a pathway to support women in their transition to motherhood. [ABSTRACT FROM AUTHOR]

Abstract

Recent statistics highlight that a significant number of babies continue to require specialist neonatal care post birth when the mother has diabetes. Admission to the neonatal unit causes stress and anxiety to parents and research indicates that attachment and bonding with the baby can be affected. This article explores the important role that diabetes specialist nurses can have in supporting parents, both during pregnancy and when their baby is admitted to the neonatal unit for care. The article emphasises the importance of promoting early attachment during pregnancy, and the role that continuity of care can have on lowering maternal stress and anxiety. The importance of listening and communication skills when parents are negotiating caring for their baby in the neonatal unit is also identified.

Abstract

Congenital cytomegalovirus is the most commonly occuring global congenital infection (Joseph et al, 2018) with significant morbidity to the baby and yet, research identifies that antenatal education remains weak in this area. This systematic review identifies that midwives have a limited understanding of the effect that cytomegalovirus infection can have on pregnancy and the simple measures to reduce risk of infection. Childbearing women also demonstrate a limited awareness of the risks of cytomegalovirus. It is recommended that public health messaging in this area needs to be considered by midwives. Data was analysed for a total of 370 midwives and 1717 women.

For midwives to feel visible and valued, it is vital to capture how their role has developed over time. The electronic Midwifery Ongoing Record of Achievement (eMORA) embodies this by not only detailing every facet of a midwife's role but equipping students to learn effectively in a digital era. Digital literacy has become an expectancy for midwives and an acknowledged developing strategy within healthcare (NMC, 2019). The pursuit of leadership opportunities and quality improvement skills embedded in the eMORA, alongside the consolidation of essential clinical skills ensures that midwives get the recognition they deserve as being well-rounded change makers.
