This article summarizes nursing practice of a patient with decompensated liver cirrhosis (liver depression and spleen deficiency type) who received comprehensive auricular acupoint therapy combined with guided music imagination technology to improve fatigue symptoms. Based on the comprehensive evaluation on primary symptoms of the patient, auricular acupoint massage, scraping, and pressing therapy at the appropriate time, combined with guided imagination techniques, were carried out to alleviate the severity of symptoms such as fatigue, sleep quality, and anxiety of the patient, improve overall quality of life and overall therapeutic effect, and provide clinical basis for clinical nursing. (本文总结1例耳穴“三序”疗法联合引导性音乐想象技术改善肝硬化失代偿期(肝郁脾虚型)患者疲乏症状群的护理经验。入院后完善护理评估, 基于患者主要症状, 采用耳穴按摩、耳部刮痧、耳穴贴压“三序”疗法联合引导性音乐想象技术进行干预, 缓解患者疲乏、焦虑、失眠症状, 提高其总体生活质量。)

Objective To investigate the improvement effect of adding heparin sodium saline at different time points on the occurrence of severe dialyzer coagulation in maintenance hemodialysis (MHD) patients during dialysis. Methods Patients who underwent MHD treatment at the dialysis center of Xiyuan Hospital from November 2021 to November 2022 and experienced severe coagulation with dialyzers within 2 hours of treatment were included in the study. Electronic medical records were consulted to collect baseline data, data on various dialysis parameters, and relevant case records. According to the time point of adding heparin sodium, it was divided into two groups: the treatment group and the control group. Both groups were treated with 200 mL of physiological saline first, while the treatment group was treated with 1 mL of 625 U/mL (5 mg/mL) heparin sodium saline added to the dialysis machine heparin pump before re infusion of blood, and a maintenance dose of 1 mL/h was given until the dialysis was stopped 30 minutes before the end of dialysis; The control group received the same dose of heparin sodium salt solution added by the same method after re inducing blood. Compare the improvement effect of two sets of processes on severe coagulation dialyzers. Results A total of 12 MHD patients were included in this study. The treatment group consisted of 6 patients who completed 4 hours of normal dialysis treatment, while the control group consisted of 4 patients who completed normal treatment, and 2 patients who returned to the machine early due to severe coagulation in the dialyzer and vascular system. After dismounting, the coagulation level was evaluated. In the treatment group, 4 patients with no coagulation were classified as 0 level; Two patients had grade 1 coagulation on the dialyzer. In the control group, two dialyzers showed grade 3 coagulation, and there were small amounts of clots in both the arterial and venous ampulla; Three dialyzers showed grade 2 coagulation; One dialyzer showed grade 1 coagulation. There was a statistically significant difference (P＜0. 05) in the evaluation of coagulation grade between the two groups. Conclusion The method of adding heparin sodium saline before re infusion of blood can effectively alleviate the severe coagulation condition of the dialyzer, restore the function of dialyzer, and avoid blood loss, which is better than the method of adding heparin sodium saline after re infusion of blood to start extracorporeal circulation. (目的 探究不同时间点追加肝素钠盐水对维持性血液透析(MHD)患者透析中发生透析器重度凝血的改善作用。方法 纳入2021年11—至2022年11月中国中医科学院西苑医院透析中心进行MHD治疗, 在治疗开始2 h内发生透析器重度凝血的患者, 查阅电子病历收集患者基线资料、透析中各参数资料、相关病例记录。根据追加肝素钠的时间点分为治疗组和对照组两组。两组均采用先回冲200 mL生理盐水, 治疗组采用再次引血前通过透析机肝素泵追加浓度为625 U/mL(5 mg/mL)的肝素钠盐水1 mL并给予维持量1 mL/h, 直至透析结束前30min停止; 对照组采用再次引血后通过同种方法追加相同剂量肝素钠盐水。比较两组流程对重度凝血透析器的改善作用。结果 本研究共纳入12例MHD患者, 治疗组6例患者均完成4 h正常透析治疗, 对照组中4例完成正常治疗, 2例因透析器及血管路重度凝血提前回血下机。下机后凝血等级评估, 治疗组中, 4例患者无凝血为0级; 2例患者透析器1级凝血。对照组中, 2例透析器为3级凝血, 动脉壶、静脉壶均存在少量凝血块; 3例透析器为2级凝血; 1例透析器为1级凝血。凝血等级评估经比较两组存在统计学差异(P＜0. 05)。结论 于再次引血前追加肝素钠盐水的方法, 可以有效缓解透析器重度凝血的状况, 恢复透析器的功能, 避免血液丢失, 优于再次引血开始体外循环后再追加的方式。)

This article summarizes nursing practice of 1 patient with stuffiness of stomach treated with Bian-stone ironing and rubbing combined with Traditional Chinese Medicine penetration and Baduanjin exercise based on the theory of supporting-yang. On the basis of routine nursing, through the assessment of symptoms, psychological status and sleep quality of the patient, under the guidance of the "Fuyang" theory, the Bian-stone ironing and rubbing combined with Traditional Chinese Medicine penetration and Baduanjin exercise were carried out to relieve the stuffiness of stomach symptoms and improve the health status of the patient. This paper summarized the nursing practice of Traditional Chinese Medicine emotion regulation in relieving anxiety of a patient undergoing total laparoscopic hysterectomy. Based on the routine care, a comprehensive plan of emotion regulation including psychological support, relaxation training; music-guided imagination and interview-based interventions were carried out to relieve the anxiety before and after surgery, and improve the patient’s sleep quality and postoperative recovery. This article summarizes the nursing practice of balanced cupping method based on the theory of Du Meridian-regulating in relieving clinical symptoms of a patient after stroke. On the basis of routine nursing care, the balanced cupping method based on the theory of Du Meridian-regulating was used to select cupping methods such as foot sun bladder meridian flash cupping, Du meridian moving cupping, and specific acupoint retention cupping. Combined with changes in cupping time, speed, negative pressure inside the cupping, and manual coordination, different degrees of stimulation are caused, thereby achieving the effect of tonifying deficiency, reducing excess and regulating the Du meridian, improving post-stroke symptoms and improving patient's quality of life. This paper summarized the preoperative nursing practice of a patient undergoing endovascular exclusion for thoracic aortic ulcer. This article summarizes the nursing practice of a lung cancer patient with constipation after chemotherapy treated with Bian-stone ironing and rubbing therapy combined with Traditional Chinese Medicine penetration therapy. Through scientific nursing evaluation and syndrome differentiation, the patient's nursing plan was determined. Based on the basic theory of Traditional Chinese Medicine, the Bian-stone ironing and rubbing therapy combined with Traditional Chinese Medicine penetration therapy were used to relieve constipation. At the same time, nursing interventions such as defecation guidance, dietary regulation, exercise, and medication guidance were adopted to further improve clinical symptoms and achieve significant therapeutic effects on constipation. To investigate the effect of auricular acupoint combined therapy on gastrointestinal dysfunction caused by abnormal changes of cerebral intestinal axis in 1 case of intracranial infection. This article summarized the nursing experience of a patient with gastroesophageal reflux disease of liver-stomach disharmony syndrome treated with auricular acupoint comprehensive operation techniques based on the theory of ascending and descending. This paper summarized the integrated traditional Chinese and western medicine nursing bundles for a patient undergoing percutaneous coronary intervention for acute myocardial infarction. The basic nursing plan was formulated based on a comprehensive nursing risk assessment. Integrated traditional Chinese and western medicine nursing bundles included monitoring of vital signs, postoperative health education, intra-aortic ballon pump-associated catheter maintenance, skin care, Psycho-Cardiology care and interventions on sleep quality, abdominal distension, chest tightness and difficulty breathing syndrome. The incidence of type diabetes mellitus tends to increase with gradual aging of the population, leading to the increasing number of patients with diabetic foot ulcers. Patients may suffer from emotional disorder at any stage of disease. The emotional status is highly related to the development of diabetic foot ulcers, and has a major influence on following treatment and quality of life of patients. The current therapy model combines two evidence-based treatments: drug therapy including anxiolytics and newer antidepressants, non-drug therapy including cognitive-behavioral therapy, psychological care, music therapy and Traditional Chinese Medicine emotional nursing. This paper reviewed studies on pathogenesis, development and interventions on diabetic foot complicating anxiety disorder. By searching ancient books and literature, this paper summarized the development history of Traditional Chinese Medicine hot ironing therapy and its treatment methods, therapeutic effects and application progress in spleen and stomach diseases, and to understand the research status of Traditional Chinese Medicine hot ironing therapy in the treatment of spleen and stomach diseases. Objective To observe the effect of Bian-stone hot ironing-massage Chinese herbal medicine penetration combined with auricular acupoint sticking and pressing on relief of incomplete intestinal obstruction. Methods Totally 60 patients with incomplete intestinal obstruction were randomly divided into the treatment group and control group, with 30 cases in each group. All patients received symptomatic treatment and routine care, and those in the treatment group were given Bian-stone hot ironing-massage and Chinese herbal medicine penetration combined with auricular acupoint sticking and pressing. The time to relief of abdominal pain, abdominal distension and difficult defecation were observed, and the treatment efficacy was measured and compared between two groups. Results The time to relief of abdominal pain, abdominal distension, nausea and vomiting and difficult defecation in the treatment group was shorter than that in the control group (P＜0. 01). This paper summarized the care bundles of Traditional Chinese Medicine (TCM) for a super-aged patient after interventional treatment for acute arterial embolization of lower extremity. This paper summarized the nursing experience of a patient with chronic gastritis complicated with anxiety treated by Bian-stone hot ironing-massage and Chinese herbal medicine penetration combined with music-guided imagination therapy. The music-guided imagination therapy was used to relieve patient’s negative emotions. Bian-stone hot ironing-massage and Chinese herbal medicine penetration were carried out to relieve the gastrointestinal symptoms of the patients. This paper reviewed the experience of acupoint application combined with Chinese herbal medicine warm compress therapy and nursing cooperation for a patient with chronic gastritis and abdominal distension. Based on Traditional Chinese Medicine theory of syndrome differentiation and cause analysis of abdominal distension, appropriate technology of TCM nursing including acupoint application and warm compress therapy were selected to relieve the clinical symptom of the disease and comfort of the patient. Objective To observe the effect of using the therapeutic apparatus for music therapy based on Traditional Chinese Medicine (TCM)syndrome differentiation in the treatment of functional abdominal distension. Methods Totally 60 patients with functional abdominal distension were randomly divided into the control group and the intervention group, with 30 cases in each group. All patients received a routine treatment and nursing (7 days per course, two courses), and the music therapy based on TCM syndrome differentiation by using a therapeutic apparatus was performed in the intervention group. The Gastrointestinal Symptom Assessment score and overall effective rate were compared between two groups. Results The Gastrointestinal Symptom Assessment score decreased in both two group after two courses of treatment, and the Gastrointestinal Symptom Assessment score in the intervention group was lower than that in the control group(P＜0. 01). The overall effective rate was 93. 33%(28/30) in the intervention group, which was significant higher than 73. 33%(22/30) in the control group(P＜0. 01). No serious adverse reaction was reported in either group. Conclusion The therapeutic apparatus for music therapy based on TCM syndrome differentiation is safe and effective in the treatment of functional abdominal distension. (目的 观察运用中医辨证移情音乐胃肠治疗仪改善功能性腹胀的效果。方法 将符合纳入标准的60例功能性腹胀患者随机分为两组, 各30例。对照组接受常规治疗和护理, 干预组在常规治疗的基础上采用中医辨证移情音乐胃肠治疗仪。两组均以7 d为1个疗程, 共2个疗程。记录与评价患者腹胀症状, 对比两组干预效果。结果 干预后, 两组胃肠症状评分均较干预前下降, 且干预组胃肠症状评分低于对照组, 差异有统计学意义(P＜0. 01)。干预组总有效率93. 33%(28/30), 高于对照组的73. 33%(22/30), 差异有统计学意义(P＜0. 05)。两组均无严重不良反应。结论 中医辨证移情音乐胃肠治疗仪改善功能性腹胀具有较好的疗效, 且安全可行。)

This paper observe the effect of music-guided imagination on relief of anxiety in a patient undergoing high ligation and stripping of the greater saphenous vein. Keys issues of preoperative preparation, intraoperative cooperation, music-guided imagination and postoperative observation were summarized. The music-guided imagination is potentially effective to relieve the patient’s anxiety and improve the surgical compliance. This paper introduced the design of the device for stone needle and scraping of auricular acupoint, and analyzed its clinical application in specific cases. The scraping technique and related acupoint were selected by syndrome differentiation before treatment. The combination treatment of stone needle and scraping therapy proved to be a safe and effective treatment. As a traditional medical method in China, auricular therapy has simple operation and accurate curative effect. Chronic gastritis belongs to the categories of "epigastric pain", "hiccup", "acid vomiting" and "stupidity" in Traditional Chinese Medicine. I ncreasing attention has been paid to health condition of the elderly as the fast growth of the elderly population. The Traditional Chinese Medicine external application has advantages of easy-to-use and low cost, and is highly accepted among patients. Objective To explore the effective nursing service and technical nursing methods in "One Syndrome and One Product" specialized ward in the Department of Spleen and Stomach Disease. Methods To carry out the humanistic care service of Traditional Chinese Medicine(TCM) and the characteristic techniques of TCM: shaking-hands nursing shift, the whole process of accompanying inspection service, TCM needle and stone ironing therapy, and so on. Results Through effective TCM nursing service and techniques, patients' satisfaction with nursing service, patients' satisfaction with nursing technology and nurses' satisfaction with nursing plan were improved, and the average hospital stay was reduced. Conclusion TCM humanistic care service and TCM characteristic nursing techniques can realize the close combination of TCM nursing techniques and clinical application, and the close combination of clinical service ability and " One Syndrome and One Product ", thus establishing an effective joint mode of medical care collaboration and TCM humanistic care. Objective To investigate the effect of " One syndrome and One Product " combined with medical care integration mode in improving the quality of nursing care for osteoporotic patients with spleen-kidney deficiency and blood stasis syndrome. Methods Totally 60 osteoporotic patients with spleen-kidney deficiency and blood stasis syndrome were randomly divided into control group and observation group, with 30 cases in each group. The control group was given routine nursing intervention, and the observation group was given nursing intervention based on " One syndrome and One Product " combined with medical care integration mode. The patient’s level of knowledge on disease, degree of pain, nursing satisfaction and activity of daily living were measured and compared between two groups. Results After intervention, patients in the observation group achieved a higher level of knowledge on disease, lower score of pain assessment and higher score of nursing satisfaction compared with those in the control group (P＜0. 05).
