La récente publication de Bernard Lahire sur les structures fondamentales des sociétés humaines constitue une contribution d’ampleur témoignant d’une ambition forte pour les sciences sociales. Il est ici proposé de rendre compte de manière détaillée de ses propositions épistémologiques et théoriques, sur la production de connaissance dans les sciences sociales, le dégagement des structures fondamentales (comparaison homme/animal ; différence entre le social et le culturel), et les effets du biologique sur le social (rôle majeur de l’altricialité secondaire).

Plusieurs normes internationales expriment la nécessité pour les États de renforcer les mesures de protection des droits de la femme, particulièrement en matière d’éducation. Cette étude, effectuée à l’aide de recherche documentaire et par l’observation des incidences des normes de scolarisation obligatoire en Côte d’Ivoire, démontre l’apport des initiatives législatives ivoiriennes dans l’implémentation d’une dimension égalitaire du droit par les dispositions sur la scolarisation obligatoire des deux sexes.

Introduction: the occurrence of breast cancer in men is an indication for genetic counseling and must be systematically sought. The objective of this work was to describe the histopathological and molecular aspects of human breast cancer. Material and Methods: This was a cross-sectional study with retrospective collection from January 2007 to September 2021, including all cases of male breast cancer diagnosed by histology with immunohistochemical examination. The variables studied were histopathological and molecular. The samples were fixed in 10% formalin and processed using the standard hematoxylin-eosin and immunohistochemistry technique. Results: A total of 8 cases of male breast cancer were collected. Each case underwent histological and immunohistochemical examination. Age ranged from 8 to 77 years with a mean age of 50.63±20.79. Consultation time of less than 12 months was common with the presence of a breast nodule as the reason. The left breast was as affected as the right breast. Tumor size T3 (5 cm and more) predominated with 3 cases. The nature of the sample received was: 04 biopsies and 04 lumpectomies. Infiltrative carcinoma of non-specific type was common: 7 cases (or 87.5%) with Elston and Ellis grade II in 75% of cases. Luminal A and triple negative were predominant with 3 cases each (37.5%). On dit souvent du rire, d'une part, qu'il est le propre de l'homme, d'autre part qu'il exprime la joie, le plaisir, en!n, qu'il est un comportement automatique, réIexe. Tout cela témoigne d'une méconnaissance fondamentale de sa complexité qui ne peut être explorée que suivant une approche pluri et interdisciplinaire comportant les dimensions biologique, psychologique et historico-socioculturelle. Ainsi, avec l'éthologie, nous abordons la phénoménologie du rire, son ontogenèse, puis sa phylogenèse, facteurs de son historicité. Puis, avec la psychologie, la psychanalyse et la neurologie, en particulier, nous explorons les causalités, externes et internes, contribuant au déclenchement et à la fabrication du rire qui est aussi complexe. J'en distingue trois secteurs: psychologique, avec ses composantes cognitives et psycho-affectives inconscientes, cérébral et sa réalisation motrice. En!n, nous envisageons une approche socio-anthropologique du rire. A partir de faits historiques et ethnographiques, nous avons élaboré notre propre représentation des relations du rire et du risible formant un système de communication « risible-rire ». Le rire occuperait alors une position intermédiaire entre ce système de communication risible et celui des expressions émotionnelles, individuelles et sociales. Par-delà les insuffisances et carences diverses, le sous-développement de la République Démocratique du Congo s’explique par l’ancrage de la « corrimpunite », la mentalité collective tordue, le tribalisme et la distraction. Ces cancers sociaux, qui annihilent les politiques publiques de quête de développement, persistent du fait de l’homme politique, l’homme de médias, « l’homme de Dieu » et l’homme du son agréable à l’oreille, dont les pratiques donnent également accès aux maux de l’environnement international. L’on a ainsi affaire à des réalités qui escortent le non-développement de ce pays, le plaçant hors d’atteinte des efforts transformateurs. La compréhension, ici livrée, de ces réalités permet de les combattre et envisager un processus de développement prometteur.

Cet article nous lance dans la philosophie de l’être de Martin Heidegger. Il pose la question essentielle du rapprochement ou non du Dasein à l’homme. Martin Heidegger refuse d’identifier le Dasein à l’homme. En effet, il est préoccupé non pas par l’étant-homme en tant qu’homme, mais par la saisie en cet étant-homme de l’être qui est l’être en commun à tous les étants. Si le Dasein est loin d’être l’homme, s’il n’est pas à identifier à l’homme, qu’est-il donc ?

Background: Adult acquired buried penis (AABP) is a condition of entrapment of the phallus resulting most commonly from obesity. This condition has a significant negative impact on the quality of life, including micturate difficulties, sexual dysfunction and recurrent infections. Many techniques exist to treat AABP; however, do not necessarily include a suspensory ligament release. The ligamentous system supporting the penis is widely called the "penile suspensory ligament" and is composed of four ligaments., Objectives: We wanted to explore the clinical outcomes on an exploratory patient cohort for penis-lengthening in patients with Grade 1 buried penis using a partial release of the penile suspensory system associated with fat transfer combined with a suprapubic retraction., Methods: Under general anesthesia, an abdominal liposuction was performed, followed by a direct defatting of the pubic mons. Then the fundiform, the suspensory and the anterior part of the vertical dense ligaments were severed. Furthermore, several anchoring sutures were added and a retropulsion of the supra pubic area was performed. Finally, the purified fat was injected into the penile shaft in a superficial plane. Files from patients who underwent surgery for stage 1 buried penis between 2015 and 2020 were retrospectively reviewed., Results: Twelve patients with a buried penis underwent a lengthening procedure. La gynécomastie est la déformation mammaire la plus fréquemment retrouvée chez les hommes. 148 patients (âge moyen 24,7 ans) opérés dans notre service ont été revus avec un recul moyen de 5 ans. La gynécomastie était le plus souvent apparue en période pubertaire (77,7 %), bilatérale (86,5 %) et idiopathique (89,9 %). Son importance était évaluée selon la classification de Simon, basée sur le volume glandulaire mammaire et l'excès cutané. Dix sept (11,5 %) stades 1, 77 (52 %) stades 2A, 32 (21,6 %) stades 2B et 22 (14,9 %) stades 3 ont été recensés. L'examen clinique associé à un examen d'imagerie, le plus souvent une échographie mammaire, ont permis de déterminer la consistance de la gynécomastie : adipeuse ou ferme. Quatre prises en charge chirurgicales différentes ont été réalisées : 17 (11,5 %) mastectomies sous-cutanées, 4 (2,7 %) lipoaspirations, 110 (74,3 %) lipoaspirations associées à une mastectomie sous-cutanées, 17 (11,5 %) mastectomies totales. Toutes les techniques ont donné de bons résultats morphologiques. 2 patients ont nécessité une reprise pour insuffisance de résultat. Les auteurs recommandent l'association «lipoaspiration et mastectomie sous-cutanée» pour de nombreux avantages : pertes sanguines limitées en peropératoire, redrapage cutané satisfaisant, courte durée d'hospitalisation, examen histologique du matériel prélevé. Clinical examination and mammography determined the consistency of gynecomastia: adipose or firm. 4 different surgical managements were used: 17 (11,5%) subcutaneous mastectomies, 4 (2,7%) liposuctions, 110 (74,3%) liposuctions associated with subcutaneous mastectomy, 17 (11,5%) total mastectomy. All techniques gave good morphologic results. Nonetheless, the authors recommend the combination «liposuction and subcutaneous mastectomy», as this technique presents many advantages: small intraoperative blood loss, good skin redraping, short hospital stay, complete histologic examination of the material removed. Shenaz Patel, auteure mauricienne, nous introduit à la cause environnementale dans les albums intitulés La toile bleue (2010) et Le bestiaire mauricien (2016) tout en mettant en exergue la couleur locale. À travers la co-écriture des bandes dessinées Histoire de Maurice (TI et T2) avec Jocelyn Chan Low, elle souhaite promouvoir l’histoire de l’île Maurice. Si la dynamique de l’écriture fait entendre des sonorités exquises pour un habitué, ce sera à travers la représentation picturale, accompagnée par son texte, que l’histoire et la cause environnementale semblent s’exprimer le plus. Le fait que la notion de frontière-démarcation soit réinventée par une sorte de chevauchement de territoires, permet à une certaine identité littéraire de se former.

Introduction: Comparer les caractéristiques démographique, diagnostique, thérapeutique et évolutive de la polyarthrite rhumatoïde de l’homme et de la femme. Matériels et méthodes: Il s’est agi d’une étude transversale et comparative avec collecte de données rétrospectives allant du 1er janvier 2006 au 31 Octobre 2018 dans le service de rhumatologie du centre hospitalier universitaire de Bogodogo. Les données épidémiologiques, cliniques, paracliniques, thérapeutiques et évolutives ont été évaluées à l’aide d’un questionnaire à partir des dossiers médicaux des patients. Résultats: 120 patients ont été colligés dont 40 de genre masculin et 80 de genre féminin. L’âge moyen chez les hommes était de 53,33ans ± 15,431 contre 45,79ans ± 12,021 chez les femmes (p= 0,02). Quarante-sept femmes présentaient une atteinte articulaire initiale mixte contre 25 hommes (p=0,23). Un syndrome sec oculaire était noté chez (10%) des hommes contre (1,2%) des femmes (p=0,02). Les nodules rhumatoïdes ont été observés uniquement chez les hommes (2 cas) (p=0,04). L’échelle visuelle analogique (EVA) patient moyen était de 63 /100 chez les femmes et de 72/ 100 chez les hommes (p=0,01). Le DAS28 initial moyen était de 5,18 ± 1,39 chez les femmes et de 5,16 ± pour les hommes (p=0,94). Une carpite fusionnante était notée chez 8 femmes contre 12 hommes (p=0,02). Conclusion: Les caractéristiques cliniques de la PR étaient différentes dans les deux sexes avec un âge de survenue plus tardif, une EVA patient plus sévère ainsi qu’une prédominance des manifestations extra-articulaires chez l’homme.

In order to restore fertility in patients with preserved testicular tissue from patients cured of paediatric cancer in particular, different approaches of in vitro maturation of (pre)pubertal testicular tissue have emerged in order to reproduce in vitro the complete process of spermatogenesis and have continued to evolve over the last 50 years. The aim of in vitro spermatogenesis is to generate in vitro spermatozoa from isolated spermatogonial stem cells (SSCs) or intact fragments of testicular tissue. Isolated SSCs, obtained after enzymatic and/or mechanical dissociation of testicular tissue, can be grown in liquid medium (twodimensional culture) or within a matrix (three-dimensional culture). The culture of testicular tissue fragments or organotypic culture, most often performed on agarose gel in gas-liquid interphase, allows the preservation of the interactions of SSCs with their microenvironment. Progressively, different models are emerging with the aim of mimicking the physiological conditions of spermatogenesis, particularly its spatio-temporal organisation. This research has led to the production of functional spermatozoa and viable and fertile offspring in mice. Cet article met à jour les rapports de pouvoir et les inégalités entre femmes et hommes au sujet de la pratique successorale chez les Bamiléké du sous-ensemble yemba du Cameroun, ainsi que les répercussions de ces inégalités sur l’implication du genre au développement. Il découle du constat selon lequel les Yemba n’accordent pas aux frères et aux sœurs les mêmes privilèges de succession quand leurs ascendants viennent à trépasser. Des veuves sont souvent expulsées des foyers qu’elles ont partagés avec leur époux alors vivant, une pratique contraire à la Constitution camerounaise du 18 janvier 1996 et qui affecte par ricochet les orphelins et les prive de leurs droits. Pourtant, la loi fondamentale du pays consacre le principe d’égalité entre hommes et femmes ; il est raffermi par des textes internationaux prônant la parité de droits, y compris lors des pratiques de succession. Malgré ces dispositions, les discriminations sont des faits. En appréhendant la succession sous le prisme du genre, ce travail rend compte des inégalités dans la transmission des biens et des rôles sociaux d’un ascendant à ses héritiers et son partenaire restés en vie. Il questionne l’implication des pratiques successorales ségrégationnistes sur le quotidien des personnes et des familles. La réflexion ainsi développée s’inscrit dans la perspective d’une recherche qualitative fondée sur l’observation, les entretiens guidés et sur l’analyse de contenu appliquée aux données de terrain. Celles-ci ont été collectées auprès de 52 individus dont 24 hommes et 28 femmes ayant expérimenté aussi bien la perte d’un parent ascendant ou d’un conjoint, que des pratiques de succession au sein du groupe ethnique yemba. En cherchant à élucider cette formule énigmatique, on tente ici de reconstituer, à travers toute l’œuvre de Sartre, le puzzle de la solitude, le philosophe tenant à son sujet un discours cryptique à l’aune duquel se mesurent un athéisme méthodologique double autant qu’une anthropologie phénoménologique trouble.

El presente documento, abreva la combinación de un análisis teórico-metodológico de la posmodernidad y el rol que desempeña el hombre del Siglo XXI, en una sociedad caracterizada por la virtualidad y el híper-individualismo, que obliga a replantear las pautas para hacer política y habitar bajo un contrato social en el que el otro sea tan importante como el sujeto que observa las transformaciones de la realidad en pandemia. Este ensayo científico consiste en analizar tres variables: 1) posmodernidad, 2) hombre digital y 3) pandemia, por medio de una revisión sistemática de literatura científica-política. Se analizaron planteamientos teóricos escritos por varios pensadores que poseen un común denominador al estudiar los retos de las sociedades del Siglo XXI, los territorios y los nuevos escenarios post-pandémicos. Los principales hallazgos confirman que existe una nueva realidad en la que la expectativa de tener prácticas solidarias dista de los rituales sociales, que se erigen en un individualismo exacerbado y con ello, un reblandecimiento del tejido social a causa de tender al otro como una potencial amenaza para la sobrevivencia de uno mismo. El mundo pos-democrático deberá obedecer al nuevo zoon politikon que, desde el autoaislamiento recurre a exigencias de ser tomado en cuenta para vivir en la ficción de una comunidad-mundo al interior de cuatro paredes, por tanto, estos elementos interconectados obligan a replantear las tesis sobre el presente y los procesos de pensamiento concomitante en relación con una tolerancia en el edificio de la sociedad humana.

This article aims to reflect on the unconscious dynamics that sustain the shocking increase in feminicide in the world. It is observed that psychological and physical violence by the intimate partner has numerous facets, especially when the woman gains more or has more professional success than her partner. This violence can take a subtle form as rejection and betrayal in order to hurt the woman and destroy her self-esteem and success. Based on numerous studies and bibliography it is demonstrated that the main cause of violence by the intimate partner is the fear of the power of the feminine that has been present throughout history. Despite the evolution of women, historical violence reverberates in the 21st century as an intergenerational trauma causing great suffering in families and disturbances in interpersonal relationships. We cannot deny the role of women in the development of French-speaking Maghreb literature. Their presence is all the more intense as they have often evoked through their works what they did not have the right or the courage to address elsewhere, either sensitive issues such as the status of women, or the relationship between men and women in a society dominated by patriarchal traditions. Hawa Djabali, in her novel Agave, far from being trapped by clichés or trying in vain to prove the superiority of one over the other, draws with the help of metaphorical images and a particular treatment of the space–time, the relationship between a man and a woman, and describes their quest for identity, to present at the end a mode of peaceful coexistence leading to the personal and social fulfillment of both genders. This article aims to analyse and dissect these literary images, to retrace the symbolic and psychological journey pursued by the author in this romantic creation. This article shows that this study on the evolution of the romantic relationship within a couple is presented above all as a questioning of oneself, and offers the opportunity for an introspective reflection on the human, and encourages secondly, to set aside stereotypes and to break the chains of hold. Résumé Les plantes médicinales et la phytothérapie sont étroitement liées à l’histoire de l’humanité, elles jouent un rôle important dans la recherche et le développement de nouveaux médicaments, et représentent une branche alternative dans la prévention et le traitement de certaines pathologies. Le pourpier Portulaca oleracea L. (Po), est une plante succulente largement répandue et utilisée à travers le monde pour ses propriétés médicinales et gustatives. Plusieurs travaux menés chez les animaux et l’homme lui attribuent des effets préventifs et curatifs sur plusieurs facteurs de risque cardio-vasculaire. Une synthèse bibliographique a été menée sur différents moteurs de recherche scientifique, tels que Pubmed, Scopus, ISI Web of Science et Google Scholar, afin de souligner les travaux de recherche marquant l’effet de Po sur le diabète, l’hyperlipidémie, le stress oxydant et l’inflammation. La présente revue suggère que Po peut avoir des effets bénéfiques significatifs chez le modèle animal ainsi que chez l’homme en améliorant les paramètres glycémiques, lipidiques, antioxydants et anti-inflammatoires. Medicinal plants and phytotherapy are closely linked to the history of humanity, they play an important role in research, and development of new drugs, and represent an alternative branch in prevention and treatment of certain diseases. Purslane, Portulaca oleracea L. (Po), is a succulent plant, widely distributed and used worldwide for its medicinal and gustatory properties. Several studies in animals and men attributed preventive and curative effects in cardiovascular risk factors. A bibliographic synthesis was conducted on Pubmed, Scopus, ISI Web of Science and Google Scholar, to highlight studies that describe the effect of Po on diabetes, hyperlipidemia, oxidative stress, and inflammation. The present review suggests that Po have significant beneficial effects, in animal and human models, by improving glycemic, lipidemic, antioxidant and anti-inflammatory parameters.]

ABSTRACT: In the literary production of the Maghreb men writers, their view of women and the feminine condition is characteristic. The writers highlight in their texts a devaluing look focused on the woman in the patriarchal society. In this study we will try through different perspectives of male authors to question the situation that women live in a traditional society because it is often reduced to the lower rank and therefore it is perceived as an object. RÉSUMÉ : Dans la production littéraire des écrivains maghrébins hommes, le regard porté par eux sur la femme, sur la condition féminine est caractéristique. Les écrivains mettent en exergue dans leurs textes un regard dévalorisant porté sur la femme dans la société patriarcale. Dans cette étude nous tenterons à travers différents regards d’auteurs masculins d’interroger la situation que vit la femme dans une société traditionnelle car elle est souvent réduite au rang inférieur et donc elle est perçue comme objet.

• Premières recommandations françaises sur la prise en charge et le traitement de l'ostéoporose masculine. • Le risque de fracture et de mortalité après fracture sévère augmente avec l'âge, rapidement à partir de 70–75 ans. • Les indications thérapeutiques dépendent de la présence ou non d'une fracture, du type de fracture et de la valeur la plus basse du T-score. • L'évaluation du risque de chute et sa prise en charge doivent faire partie de la prise en charge de l'ostéoporose. Nous proposons des recommandations destinées à tous les médecins amenés à prendre en charge des hommes ayant une ostéoporose ou à risque d'une telle maladie. Le contenu de ces recommandations a été rédigé en conformité avec la méthode HAS et validé par un groupe de travail. Lorsque les données de la littérature étaient insuffisantes, les recommandations ont été basées sur un accord professionnel prenant en compte l'état des pratiques et les opinions d'experts. Les recommandations ont été soumises à un groupe de lecture. Nous proposons 15 recommandations détaillant la population cible, les moyens d'évaluation, le traitement et le suivi. Nous insistons sur les points suivants : 1) une fracture spontanée ou provoquée par un traumatisme mineur doit faire rechercher une maladie osseuse fragilisante ; 2) le risque de fracture et de mortalité après fracture sévère augmente avec l'âge, rapidement à partir de 70–75 ans ; 3) les indications thérapeutiques dépendent de la présence ou non d'une fracture, du type de fracture (sévère/non sévère) et de la valeur la plus basse du T-score de la DMO ; 4) l'évaluation du risque de chute et sa prise en charge doivent faire partie de la prise en charge de l'ostéoporose ; 5) le suivi clinique est indispensable pour adapter la prise en charge à l'évolution du risque de fracture. Ces premières recommandations françaises, que nous avons souhaitées proches de celles concernant les femmes ménopausées, visent à améliorer le dépistage et la prise en charge de l'ostéoporose masculine. On se concentre ici sur l’analyse des rapports entre l’âme et le corps, à la fois dans le domaine théologique et dans les images, pour montrer que les conceptions dominantes de la personne humaine au Moyen Âge sont duelles mais non dualistes. Même si opère une pesanteur dualiste, s’affirme plus nettement encore, notamment aux xiie et xiiie siècles, une dynamique anti-dualiste qui tend à articuler positivement le corps et l’âme. On fait l’hypothèse que cette dynamique est étroitement liée à l’affirmation de l’institution ecclésiale, dont la position dominante repose sur sa capacité à spiritualiser les réalités matérielles.

La dissection coronaire spontanée est une forme particulière de syndrome coronaire aigu (SCA) touchant préférentiellement la femme généralement non prédisposée à la maladie athéromateuse. L'homme est exceptionnellement concerné. Nous rapportons le cas d'un homme jeune présentant un SCA avec sus-décalage du segment ST en inféro-latéral en lien avec une dissection coronaire spontanée du tronc commun gauche (TCG) étendue à l'artère interventriculaire antérieure (IVA). Il est pris en charge initialement par la fibrinolyse compliquée d'un choc cardiogénique. La coronarographie effectuée sous ballon de contre-pulsion intra-aortique (CPBIA) révèle une double occlusion aiguë de l'IVA proximale et de l'artère circonflexe (CX) ostiale. La thrombectomie permet de mettre en lumière un aspect fortement évocateur du TCG-IVA. L'imagerie endocoronaire par OCT (Optical Coherence Tomography) confirme la dissection du TCG-IVA proximale en individualisant le voile intimal ainsi que les vrai et faux chenaux avec une forte charge thrombotique. Une angioplastie du TCG-IVA avec mise en place de 3 stents actifs est effectuée avec succès. Les suites sont marquées par la persistance du choc cardiogénique sous amines et CPBIA indiquant la mise en place d'une ECMO dont l'implantation est grevée de complication hémorragique fatale. Spontaneous coronary artery disease (SCAD) is a particular form of acute coronary syndrome affecting preferentially female patient with few or without traditional cardiovascular risk factors. Male patient is exceptionally concerned by SCAD. We report a case of a young male patient presenting with anterolateral STEMI in relation with SCAD of Left main and left anterior descending artery (LAD). He was initially managed by fibrinolysis, which is then complicated by cardiogenic choc. Coronary angiogram covered by intra-aortic balloon pump (IABP) showed an acute double occlusion of proximal LAD and the ostium of the left circumflex artery (LCX). After thrombus aspirations, the angiographic pattern recalled a SCAD, which is confirmed by OCT (Optical Coherence Tomography). The latter highlighted the intimal flap with true and false lumen involving both Left main and proximal LAD with huge thrombus burden. PCI was then performed successfully with implantation of 3 DES (Drug Eluting Stent). But given the cardiogenic shock persistence despite Dobutamin infusion and IABP, ECMO (Extracorporeal membrane oxygenation) was indicated. L'objectif de cette recherche est d'évaluer chez des hommes auteurs de violences conjugales les changements intrapsychiques apportés par une prise en charge encadrée par des professionnels. La méthodologie repose sur l'administration de questionnaires relatifs à trois caractéristiques du fonctionnement psychologique de l'individu (alexithymie, dépression et impulsivité) avant le début de la thérapie en groupe de responsabilisation (T1), et à son terme (T2). Des 56 hommes ayant participé au T1, 24 ont accepté de finaliser le protocole de recherche. Les résultats indiquent une diminution de l'alexithymie et de la dépression au sein de l'échantillon. L'impulsivité reste, quant à elle, stable dans le temps. Il semble donc nécessaire d'accompagner les auteurs de violence à développer leur capacité à se représenter leurs états émotionnels ainsi que de les comprendre pour les aider à mieux réguler leur comportement. The aim of our research was to examine the association between impulsivity, alexithymia and depression and the perpetration of physical and psychological intimate partner violence. We focused on these particular psychological characteristics in order to understand this phenomenon and its prevention and/or treatment. Our initial study of fifty-six male perpetrators showed domestic violence was associated with a high level of alexithymia and depression. Analyses showed they reported more depressive feelings, and more difficulties to express their emotions than did general population. To determine if reductions were noted about these psychological characteristics at the end of intervention program, the present study examined longitudinal data. Twenty-four male offenders (voluntary or judicialized) were surveyed using self-report questionnaires and structured clinical interview to collect sociodemographic data and to assess specific psychological variables, namely emotional distress (Beck Depression Inventory), impulsive behavior (Barratt Impulsivity Scale 11) and alexithymia (Toronto Alexithymia Scale 20), the inability to experience and express subjective emotions. These men were examined before the start of therapeutic program (T1) and at the end of it (T2), after 42 hours of psychological support. Findings showed both alexithymia and depression were reduced after therapeutic management. Analyses showed male intimate partner violence offenders reported less difficulty to express their emotions and less depressive feelings at the end of their intervention program. However, we only observe a significant result for alexithymia and its dimension "difficulties identifying feelings". Impulsivity, on the other side, remains unchanged between the two evaluation times for all participants after the therapeutic program. Our findings suggested a strong link between alexithymia and domestic violence and showed the positive effect of specific therapeutic interventions on alexithymia and depression. We pointed out the fact that with the decrease of alexithymia, participants may have a better awareness of themselves and of their behavior. Objectives: The aims of our study were to assess the sexual function in men with bipolar disorder type 1 in remission and to determine the various sociodemographic, clinical and therapeutic factors associated with sexual dysfunction., Methods: We conducted a cross-sectional study over an 18-month period (January 2020-June 2021) in which we included men followed up for bipolar disorder type 1 in the euthymic phase strictly defined by a score <8 on the Young Mania Rating Scale and a score ≤7 on the Hamilton Depression Rating Scale. Sexual function was assessed using the Arizona Sexual Experiences Scale (Asex) in its Arabic-validated version., Results: Sixty patients were included in the study. The mean age was 42.5 (SD=11.1) years. Among the patients, 68% had sexual dysfunction according to the total score of the Asex scale. According to univariate analysis, several factors were significantly associated with sexual dysfunction in patients with bipolar disorder type 1: age (P=0.001), total number of hospitalizations for thymic relapse (P=0.015), total number of depressive episodes (P=0.006) and depressive dominant polarity (P=0.046). The factors identified as modifying sexuality according to the total score of the Asex scale by multivariate analysis were age at first antipsychotic prescription: P=0.01; ORa=1.109; 95% CI [1.021-1.206] and number of hospitalizations for thymic relapse: P=0.015; ORa=1.259; 95% CI [1.046-1.546]., Conclusion: Studies assessing factors associated with sexual dysfunction in patients with bipolar disorder type 1 in euthymia were mostly concerned with the effects of psychotropic drugs on sexual function, with factors inherent in bipolar illness itself not widely addressed in the literature. Le vieillissement de la population pénale en France pose un véritable problème de santé publique. Or, il existe très peu de recherches sur la santé mentale des détenus âgés. L'objectif de cette étude était d'évaluer l'effet de l'âge et de la durée de détention sur les troubles mentaux et sur les performances cognitives de sujets âgés incarcérés. Nous avons recruté 138 détenus de sexe masculin âgés de 50 ans et plus dans sept établissements pénitentiaires français. Le protocole de recherche comprenait un entretien semi-directif, le Mini International Neuropsychiatric Interview (MINI DSM-IV) pour l'évaluation des troubles mentaux ainsi que la Batterie Rapide d'Efficience Frontale (BREF) et le Mini Mental State Examination (MMSE) pour l'évaluation des performances cognitives. Les résultats ont mis en évidence une prévalence très élevée des troubles mentaux, notamment de la dépression et de l'anxiété, et des troubles cognitifs. Toutefois, la probabilité de survenue de certains diagnostics psychiatriques diminue avec l'avancée en âge (épisode dépressif majeur, agoraphobie, état de stress post-traumatique et anxiété généralisée). Par ailleurs, les estimations effectuées par régression logistique n'ont montré aucun lien significatif entre la durée de détention et les troubles mentaux. En revanche, il existe un lien significatif entre la durée de détention et la déficience cognitive. Ces résultats mettent en évidence une très grande vulnérabilité des détenus âgés sur le plan de la santé mentale et soulignent la nécessité de mettre en œuvre des mesures adaptées de dépistage et de soins pour répondre aux besoins spécifiques de cette frange de la population pénale. Inmate aging is considered to start at the age of 50, which is early in relation to aging in the general population. The aging of the criminal population in France poses a real public health problem. There is very little research on the mental health (mental and cognitive disorders) of older inmates. The objective of this study was to evaluate the effect of age and time spent in prison on mental disorders and cognitive performance of elderly prison inmates. We put forward the hypothesis that age and amount of time spent in prison are associated with the deterioration of older inmates' mental health, that is, an increase in the prevalence of psychiatric disorders and a decrease in cognitive performance. We recruited 138 men aged 50 and over in seven French prisons. The research protocol included a semi-directive interview, the Mini International Neuropsychiatric Interview (MINI DSM-IV) for the assessment of mental disorders, as well as the Rapid Battery for Frontal Efficiency (BREF) and the Mini Mental State Examination (MMSE) for the evaluation of cognitive performance. The average age of the inmates (N = 138) was 59.7 years (range 50–84, SD = 8.02). The average sentence was approximately 13.5 years (SD = 7) and the average time spent in prison was 6.9 years (SD = 5.9). The results showed a very high prevalence of mental disorders, notably depression and anxiety, and cognitive disorders. However, the probability of occurrence of certain psychiatric diagnoses decreases with age (major depressive episode, agoraphobia, post-traumatic stress disorder and generalized anxiety). In addition, logistic regression estimates showed no significant relationship between time spent in prison and mental disorders. However, there is a significant link between time spent in prison and cognitive impairment. Our hypothesis is partially validated. Indeed, age is not associated with mental or cognitive disorders. However, the amount of time spent in prison has an effect on the deterioration of certain cognitive functions. It appears that after the age of 50, it is not chronological age but environmental factors that mainly explain cognitive decline. Our study shows that the longer the detention period, the greater the inmate's cognitive decline. These results highlight the very high vulnerability of elderly prisoners in terms of mental health and emphasize the importance of implementing appropriate detection and care measures to address the needs of this segment of the criminal population. Routine screening for cognitive impairment in all older prisoners should be carried out by caregivers in penitentiary institutions. Furthermore, better follow-up and cognitive assessment of prisoners aged 50 or more and whose length of incarceration exceeds five years could make it possible to detect subjects at risk and to propose appropriate activities to reduce and/or delay the effects of aging in detention.
