Abstract

Celiac disease (CD) is an intestinal malabsorption syndrome in which diarrhea, malnutrition, and bleeding are common. CD is reversed by removal of gluten-containing cereals from the diet. CD has a genetic component that has been associated with several histocompatibility antigens. These are specific proteins present on most cells in the body. Their presence is controlled by genes that code for them, and they recognize 'self' tissues for the immune system. Thus, transplantation of foreign tissue presents new antigens to the immune system, and antibodies against the new antigens are promptly made. Human antigens are called human leukocyte (white blood cell) antigens (HLA) and different HLA subsets are associated with the occurrence of many diseases. The relationship between the subclass HLA (class II), HLA-DP, and coeliac disease, as well as the association with insulin-dependent diabetes mellitus (IDDM) and dermatitis herpetiformis (DH, another gluten-related disease), was studied in 254 subjects with these diseases or who were in a family with CD. The results confirmed previous studies which showed that people with CD frequently had an alteration in the alpha portion of the HLA-DP structure. Further, people with DH also tended to have this alteration, although less often, while people with IDDM and healthy subjects did not. An additional alteration in the beta portion of HLA-DP was found in people with CD; many of the CD patients with the alpha alteration had the beta alteration. Alterations in HLA-DP alpha structure were not associated with particular forms of two other class II HLA proteins, HLA-DR and HLA-DQ. Further, some CD patients did not possess the HLA-DP alpha alteration. The study indicates that susceptibility to CD may be related to more than one HLA characteristic, or to a complex mixture of HLA factors. Abstract

Cervical cancer in older women appears to have many of the same characteristics as in young women. Researchers analyzed cervical tissue samples from 39 women 62 to 85 years old and 104 younger women. Human papillomaviruses 16 and 18 were found in similar percentages of the tumor samples. The DR*1501 HLA gene was also present in 33% of the older women and 28% of the younger women, compared to a frequency of 20% in the general population. Thus, cervical cancer in older women is similar but appears to have a longer latency period.

HLA-class II (human leukocyte antigen) incompatibility between mother and child may reduce the severity of rheumatoid arthritis (RA) symptoms during pregnancy. Researchers immunologically monitored 45 pregnancies among 33 RA patients and 89 pregnancies among healthy volunteers, and found that DQB1 and DQA1 incompatibilities were associated with remission or a reduction in flare-ups of RA. DQ antigens may play an important immunologic role in the development of RA.

Particular genes may be associated with increased risk and severity of systemic lupus erythematosus (SLE) in Spanish people. SLE is an autoimmune disease. Researchers in Spain compared genetic profiles of 85 people with SLE and 186 healthy volunteers. Particular human leukocyte antigen (HLA) genes were more common in SLE patients, and associated with kidney damage from the disease. The genes which related to SLE in Spanish patients were different than those associated with the disease in other ethnic groups.

It does not appear appropriate to offer routine genetic screening for rheumatoid arthritis (RA) until more is known of the results' implications and the tests are more reliable. Researchers have found a strong association between RA and a section of chromosome six called the HLA complex that codes for immune responses and contains a sequence called the shared epitope. However, studies are inconclusive as to whether patients who test positive for this genetic complex will develop a worse form of this disease or would benefit from early treatment.

A combination of genetic factors probably determines the cause and severity of rheumatoid arthritis. Researchers have identified some common genetic patterns in patients with rheumatoid arthritis. One of these patterns is a sequence of genetic material found in many rheumatoid arthritis patients. Researchers call this sequence of genetic material the susceptibility cassette. However, the expression of the susceptibility cassette varies greatly in patients of differing ethnic backgrounds. Many patients without the susceptibility cassette also develop rheumatoid arthritis. Some research suggests that there may also be a genetic sequence protecting healthy persons from developing rheumatoid arthritis.
