AbstractThe determination of serum hepatitis B Virus (HBV) DNA is of paramount significance in the comprehensive management of HBV infection. It aids in diagnosis, disease staging, and treatment monitoring, ultimately improving patient outcomes and reducing the burden of HBV-related morbidity and mortality. In this work, serum HBV DNA detection was performed using a novel nucleic acid screening system. The sensitivity, selectivity, repeatability, cross reactivity, and HBV DNA stability were evaluated and compared with published data. The detection limit of 95% was 29.12 IU/mL and the 95% confidence interval was from 23.71 to 34.55 IU/mL. Blood samples that contain free hemoglobin (1 g/L, 2 g/L, 4 g/L) and triglycerides (3 mmol/L, 6 mmol/L and 9 mmol/L) were also analyzed. The results demonstrated that the system can also be used in the presence of free hemoglobin and triglycerides. The mean of the cycle threshold (Ct) in 5-day replicates was 35.71 ± 0.35 with a coefficient of variation (CV) of 0.97%. The results of negative samples in the anti-cross contamination experiment showed no response. The coincidence rate for 3001 samples over 20 days of real-time stability measurements and external quality assessment samples was 100%. The results demonstrate the effectiveness of the nucleic acid system for HBV-DNA, providing a theoretical basis for other small and medium-sized blood stations or hospitals to these devices. Background/aims Virological factors associated with hepatitis B virus reactivation (HBV-R), following chemotherapy for cancer in hepatitis B surface antigen (HBsAg)-negative patients, are not well known. Materials and methods HBV strains from 16 patients presenting HBV-R following chemotherapy were studied and compared to those obtained from 51 HBV chronically-infected patients. Results HBsAg variability was significantly increased within the major hydrophilic region, the a determinant and the C-terminal region. Amino acid substitutions were more frequently found in HBV-R patients as compared to controls at 17 and 11 positions within HBsAg and HBV-RT, respectively. This resulted in atypical serological testing in 56% of patients and detection of resistance mutation to nucleoside analogs in 12.5%. Conclusion HBsAg and HBV-RT mutations are frequently encountered in patients with HBV-R, resulting in atypical serological testing and emergence of HBV strains resistant to nucleos(t)ides analogs. Primary liver cancer, mostly hepatocellular carcinoma (HCC) is the most common cause of cancer-related deaths around the world. Hepatitis B virus (HBV) DNA is the dominant factor that influences the progression of HCC. In this work, a novel electrochemical sensor triggered by a sandwich hybridization reaction has been developed for the ultrasensitive detection of HBV DNA. The multi-walled carbon nanotubes (MWCNTs) and hydroxylatopillar [5]arene (HP5) stabilized Au nanoparticles are used to modify the electrode to immobilize Rhodamine B-labeled DNA probes and improve the electron transfer efficiency. A supramolecular aggregate was synthesized based on pentaethylenehexamine (PEHA) induced self-assembly behavior of water-soluble pillar [5]arene (WP5) stabilized Ag nanoparticles through host-guest interaction, which serves as signal materials. The sensitivity of the sensor has enhanced on account of the electrochemical oxidation from Ag to Ag+ to yield an electrochemical response greater than that of the single silver nanoparticle. Linear sweep voltammetry (LSV) curves illustrate that the response has a good linear relationship with the logarithm of HBV-DNA concentration in a wide range from 0.1 fmol/L to 0.1 nmol/L, and the detection limit is 0.19 fmol/L according to the 3σ rule. Besides, the sensor shows good reproducibility, stability and selectivity, providing a promising prospect for application in disease diagnosis and prognosis.
