Introduction: We aimed to develop a classification method to discriminate ventricular septal defect and atrial septal defect by using several hemodynamic parameters. Patients and Methods: Forty three patients (30 atrial septal defect, 13 ventricular septal defect; 26 female, 17 male) with documented hemodynamic parameters via cardiac catheterization are included to study. Such parameters as blood pressure values of different areas, gender, age and Qp/Qs ratios are used for classification. Parameters, we used in classification are determined by divergence analysis method. Those parameters are; i) pulmonary artery diastolic pressure, ii) Qp/Qs ratio, iii) right atrium pressure, iv) age, v) pulmonary artery systolic pressure, vi) left ventricular sistolic pressure, vii) aorta mean pressure, viii) left ventricular diastolic pressure, ix) aorta diastolic pressure, x) aorta systolic pressure. Those parameters detected from our study population, are uploaded to multi-layered artificial neural network and the network was trained by genetic algorithm. Results: Trained cluster consists of 14 factors (7 atrial septal defect and 7 ventricular septal defect). Overall success ratio is 79.2%, and with a proper instruction of artificial neural network this ratio increases up to 89%. Conclusion: Parameters, belonging to artificial neural network, which are needed to be detected by the investigator in classical methods, can easily be detected with the help of genetic algorithms. During the instruction of artificial neural network by genetic algorithms, both the topology of network and factors of network can be determined. During the test stage, elements, not included in instruction cluster, are assumed as in test cluster, and as a result of this study, we observed that multi-layered artificial neural network can be instructed properly, and neural network is a successful method for aimed classification., Giriş: Bu çalışmada ventriküler septal defekt ve atriyal septal defekti, çeşitli hemodinamik verilerden faydalanarak ayırt edebilecek bir sınıflama yöntemi geliştirmektir. Hastalar ve Yöntem: Kalp kateterizasyonu ile hemodinamik ölçümleri alınmış 30 atriyal septal defekt ve 13 ventriküler septal defektli toplam 43 (26 kadın, 17 erkek) hasta çalışmaya dahil edildi. Sınıflandırma amacıyla hastaya ait çeşitli kan basıncı değerlerinin yanı sıra, cinsiyet, yaş bilgileri ve Qp/Qs oranları kullanılmıştır. Öncelikle, sınıflandırmada işe yarar öznitelikler diverjans analizi yöntemiyle elde edilmiştir. Bu öznitelikler sırasıyla; i) Pulmoner arter diyastolik basıncı, ii) Qp/Qs oranı, iii) Sağ atriyum basıncı, iv) Yaş, v) Pulmoner arter sistolik basıncı, vi) Sol ventrikül sistolik basıncı, vii) Aort ortalama basıncı, viii) Sol ventrikül diyastolik basıncı, ix) Aort diyastolik basıncı ve x) Aort sistolik basıncı olarak belirlenmiştir. Daha sonra, hastalardan elde edilmiş olan bu öznitelikler çok katmanlı yapay sinir ağına sunulmuş ve ağ genetik algoritma kullanılarak eğitilmiştir. Bulgular: Eğitim kümesi, atriyal septal defekt ve ventriküler septal defekt sınıflarından yedişer adet olmak üzere toplam 14 eleman içermektedir. Tüm başarı ortalaması %79.2 olup; yapay sinir ağının uygun eğitilmesiyle %89'a kadar çıkabilmektedir. Sonuç: Yapay sinir ağına ait ve klasik yöntemlerde kullanıcı tarafından girilmesi gereken parametreler genetik algoritmaların yardımıyla otomatik olarak bulunabilmektedir. Genetik algoritmalar ile yapay sinir ağının eğitilmesi sırasında hem ağın topolojisi hem de ağa ait ağırlıklar belirlenebilir. Test aşamasında, eğitim kümesi dışında kalan elemanlar test kümesi olarak belirlenmiş ve çalışma sonucunda çok katmanlı yapay sinir ağının başarılı bir biçimde eğitildiği ve sınıflama yapabildiği gözlemlenmiştir.

Introduction: The traditional dual chamber cardiac permanent pacemakers are widely used for symptomatic bradycardia. Pulse wave velocity (PWV) is an index of arterial stiffness and a marker of cardiovascular events. This study aims to investigate arterial distensibility using carotidfemoral (aortic) PWV measurements in patients with traditional dual chamber cardiac permanent pacemakers and angiographically normal coronary arteries. Patients and Methods: We recruited 17 paced patients and 17 age and sex matched controls. Aortic PWV was determined using an automatic device, the Complior Colson (France), which allowed on-line pulse wave recording and automatic calculation of PWV. PWV is calculated from measurements of pulse transit time and the distance travelled by the pulse between two recording sites, according to the following formula: PWV (m/s) = Distance (m) / Transit time (s) Results: The carotid-femoral PWV (10.20 ± 2.00, 9.06 ± 0.94 m/s, p= 0.04) was increased in patients with dual chamber pacing as compared with age and sex-matched control group. Multiple regression analysis between PWV and clinical parameters (age, sex, weight, height, systolic blood pressure, diastolic blood pressure, pulse pressure, mean blood pressure, heart rate) showed that PWV correlated positively with age (r2= 0.31; p= 0.007). Similar results were obtained in the paced patients (age, r2= 0.36; p= 0.03) and control (age, r2= 0.33; p= 0.04) subgroups when analyzed separately. Conclusion: Arterial distensibility is increased in patients with traditional dual chamber cardiac permanent pacemakers and angiographically normal coronary arteries, as compared with age and sex matched controls, Giriş: Klasik çift odacıklı kalıcı kalp pili semptomatik bradikardide geniş bir şekilde kullanılmaktadır. Nabız dalga hızı (NDH) arteriyel sertleşmenin bir indeksi ve kardiyovasküler olayların bir belirtecidir. Bu çalışmanın amacı, klasik çift odacıklı kalıcı kalp pili ve anjiyografik olarak normal koroner arterleri olan hastalarda karotis-femoral (aortik) NDH aracılığıyla arteriyel distansibiliteyi araştırmaktır. Hastalar ve Yöntem: Çalışmaya 17 kalp pili hastası ile cinsiyet ve yaş yönünden uyumlu 17 kontrol grubu alındı. Aortik NDH, online nabız dalgası kaydeden otomatik Complior Colson (Fransa) cihazıyla belirlenip otomatik olarak hesaplandı. NDH, nabzın ilerlediği iki kaydedici bölge arasındaki mesafenin nabız geçiş zamanına oranı olarak; "NDH (m/s) = Mesafe (m) / Geçiş zamanı (s)" formülü ile hesaplandı. Bulgular: Karotis-femoral NDH (10.20 ± 2.00, 9.06 ± 0.94 m/s, p= 0.04) cinsiyet ve yaş yönünden uyumlu kontrol grubu ile karşılaştırıldığında, çift odacıklı kalp pili bulunan hastalarda daha yüksekti. NDH ve klinik değişkenler (yaş, cinsiyet, vücut ağırlığı, boy, sistolik kan basıncı, diyastolik kan basıncı, nabız basıncı, ortalama kan basıncı, kalp hızı) arasında çoklu regresyon analizi yapıldığında NDH ile yaş arasında pozitif ilişki gözlendi (r2= 0.31; p= 0.007). Alt grup analizi yapıldığında benzer sonuçlar kalp pili bulunan hastalarda (yaş, r2= 0.36; p= 0.03) ve kontrol grubunda (yaş, r2= 0.33; p= 0.04) da izlendi. Sonuç: Arteriyel distansibilite, klasik çift odacıklı kalıcı kalp pili ve anjiyografik olarak normal koroner arterleri olan hastalarda, yaş ve cinsiyet yönünden benzer kontrollere göre daha yüksektir.

WOS: 000354772200006, PubMed: 26033652, BACKGROUND: Conduction disturbances including type 2 second-degree atrioventricular block (Mobitz II) and third-degree atrioventricular block following blunt chest trauma are probably rare. Moreover, the pathophysiological mechanisms responsible for this rare dysrhythmia following trauma are not well understood yet. In this study, it was aimed to identify the frequency of this dysrhythmia associated with trauma. METHODS: Two hundred and fifty-three consecutive Mobitz II block and third-degree atrioventricular block patients admitted to the Emergency Department of Internal Medicine between January 2012 and March 2013 were evaluated. Only four patients with Mobitz II block and third-degree atrioventricular block associated with trauma were enrolled into the present study. The level of atrioventricular block was defined according to electrocardiographic characteristics. RESULTS: Only four (mean age: 40.2 +/- 19.7 years, two male) of 253 patients were associated with trauma. All patients had normal coronary arteries in coronary angiography or multislice computed tomography. Permanent pacemaker was performed in two patients with third-degree atrioventricular block. None of the patients had coronary artery disease or hypertension. CONCLUSION: Rare clinical cases in the literature confirm that blunt chest trauma can cause conduction defects, which are usually transient. However, patients with blunt chest trauma must need an electrocardiographic evaluation for atrioventricular block upon admission and in the follow-up period.

FUNDAMENTO: A extensão da doença arterial coronariana aterosclerótica em pacientes com angina estável tem importantes implicações prognósticas e terapêuticas. Em modelos atuais de evolução de placas, os trombócitos desempenham um papel importante no crescimento de placas. O volume plaquetário médio é um marcador facilmente determinado, com evidência de correlação com a agregabilidade plaquetária in vitro, além de valores comprovadamente maiores após eventos vasculares agudos. OBJETIVO: No presente estudo, investigou-se a relação entre o volume plaquetário médio e a extensão angiográfica da doença arterial coronariana em pacientes com angina estável. MÉTODOS: Foram analisados prontuários, hemograma completo e dados angiográficos anteriores de 267 pacientes elegíveis com angina estável. A extensão angiográfica da doença arterial coronariana foi avaliada à luz de dados angiográficos, com o uso por um especialista do escore de Gensini em uma cardiologia invasiva. Os valores para o volume plaquetário médio foram obtidos a partir de hemogramas completos, obtidos um dia antes da angiografia. Com relação ao intervalo populacional para o volume plaquetário médio, os pacientes foram agrupados dentro (n = 176) e abaixo (n = 62) do referido intervalo. Foi realizada uma comparação entre grupos e uma análise correlacional. RESULTADOS: Não houve correlação linear entre o escore de Gensini total e o volume plaquetário médio (p = 0,29), ao passo que a contagem total de trombócitos apresentou correlação inversa com o volume plaquetário médio (p < 0,001, r = 0,41). Os pacientes com volume plaquetário médio abaixo do normal apresentaram um escore de Gensini (36,73 ± 32,5 vs. 45,63 ± 32,63; p = 0,023) e doença coronariana triarterial (18% VS. 36%; p = 0,007) significativamente inferiores se comparados com aqueles apresentando valores de volume plaquetário médio dentro dos intervalos populacionais. CONCLUSÃO: Nossas constatações não demonstraram nenhuma relação linear entre o volume plaquetário médio e a extensão da doença arterial coronariana, ao passo que os pacientes com volume plaquetário médio abaixo do normal apresentaram uma extensão reduzida da doença arterial coronariana. BACKGROUND: Extent of atherosclerotic coronary artery disease in patients with stable angina has important prognostic and therapeutic implications. In current models of plaque evolution, thrombocytes play an important role in plaque growth. Mean platelet volume is a readily obtainable marker that was shown to correlate with platelet aggregability in vitro and increased values were demonstrated after acute vascular events. OBJECTIVE: In this study, we investigated the relationship of mean platelet volume and angiographic extent of coronary artery disease in patients with stable angina. METHODS: Past medical records, complete blood count and angiographic data of 267 eligible stable angina patients were reviewed. Angiographic extent of coronary artery disease was evaluated form angiographic data using Gensini score by an expert in invasive cardiology. Mean platelet volume values were obtained from complete blood counts that obtained one day before angiography. Patients were grouped as those within (n = 176) and lower than (n = 62) population-based range for mean platelet volume. Comparison between groups and correlation analysis was performed. RESULTS: There were no linear correlation between total Gensini score and mean platelet volume (p = 0.29), while total thrombocyte count was inversely correlated with mean platelet volume (p < 0.001; r = 0.41). Patients with lower than normal mean platelet volume had significantly lower Gensini score (36.73 ± 32.5 vs. 45.63 ± 32.63; p = 0.023) and three-vessel disease (18% vs. 36%; p = 0.007) compared with those mean platelet volume values within population-based ranges. CONCLUSION: Our findings show no linear relationship exists between mean platelet volume and extent of coronary artery disease, while patients with lower than normal mean platelet volume had reduced extent of coronary artery disease.
