Nonalcoholic fatty liver disease (NAFLD) is one of the most common causes of chronic liver disease and its incidence is rising worldwide. Cyclophilin A (CyPA) is a protein, which is secreted under the presence of oxidative stress and hyperglycemia, and it plays role in proinflammatory signal reduction. In this study we investigated serum levels of CyPA in patients with biopsy proven NAFLD and examined their association with clinical and histological phenotypes.In this study, we identified serum levels of CyPA in patients with NAFLD (n=52) and healthy controls without evidence of any liver disease (n=44). The levels of CyPA were measured by enzyme-linked immunosorbent assay and were compared between two study groups. Furthermore, serum levels of CyPA were assessed in relation to the clinical characteristics of the study participants.Serum levels of CyPA were significantly higher in patients with NAFLD (3,8±2,6 µg/ml, P=0.03) compared to healthy controls (2,8±1,8 µg/ml). Moreover, concentrations of CyPA were 2,8±1,8, 3,4±2,3, and 4,2±2,9 µg/ml in control group, non-diabetic and diabetic NAFLD patients, respectively. The difference between the groups was statistically significant (P=0.04). There was significant correlation between the serum concentrations of CyPA and glucose levels (P=0.01), but there was no significant correlation with other clinical and histologic parameters.Our data suggest that CyPA levels are elevated in patients with NAFLD, especially in patients with diabetes. In this study, new heterocyclic ligands, 2-(3-chloro-phenyl)-1,2,3,4-tetrahydro-quinazoline-2-carbaldehyde oxime,(HL1) and 2-(3-bromo-phenyl)-1,2,3,4-tetrahydro-quinazoline-2-carbaldehy- de oxime,(HL2), and Co(III) complexes of their open-chain tautomer,(HL1′ and HL2′), containing oxime, imine, and amine donor groups resulting from the reactions with Co(II) ion have been synthesized and characterized by spectral methods, elemental analysis, magnetic susceptibility, and thermal analysis (TG, DTG, and DTA) techniques. The obtained results show that the ligands undergo a ring-opening reaction upon complexation with Co(III) ion. The analyses confirmed the following molecular formulae: [Co(L1′)2]Cl.H2O and [Co(L2′)2]Cl.H2O.

Introduction: During and after adjuvant therapy of breast cancer organ toxicity could exist. Lung toxicity was the one of these frequent treatment complications. In our study the impact of the tumor localization to the lung toxicity after adjuvant therapy of breast cancer was investigated. Material and Method: A total of 78 subjects from Kayseri Education and Research Hospital and Mersin State Hospital were included in the study. For each breast the total number of patients was 39 (right and left breast). All of the patients were in remission and treatments of the patients had finished 6 months before inclusion. All patients were examined with respiratory function test. Results: There was no statistically significant between FEV1, FEC, FEV1/FVC values in the patient groups for the right and left breast cancer. Discussion: Despite the small number of patients included, our study showed that tumor localization does not affect the lung toxicity due to the chemoradiotherapy, Giriş: Meme kanseri adjuvan tedavisi sırasında ve sonrasında organ toksisiteleri oluşabilmektedir. Bunlar içinde akciğer toksisitesi sık görülen tedavi komplikasyonlarındandır. Çalışmamızda meme kanseri adjuvan tedavisi sonrası oluşabilen akciğer toksisitesine kanserin lokalize olduğu meme bölgesinin etkisi olup olmadığı araştırılmıştır. Materyal ve Metod: Kayseri Eğitim ve Araştırma Hastanesi ile Mersin Devlet Hastanesinden toplam 78 meme kanserli hasta alındı. Her iki meme lokalizasyonu için (sağ ve sol meme) hasta sayısı 39 idi. Hastalar remisyonda idi ve tedavileri 6 ay önce bitmişti. Tüm hastalara solunum fonksiyon testi uygulandı. Sonuçlar. Yapılan istatistiksel test sonucunda hastaların solunum fonksiyon testleri açısından sağ ve sol meme karsinomlu hasta grupları için FEV1, FVC ve FEV1/FVC değerleri arasında anlamlı fark yoktu. Tartışma. Hasta sayıları az olsa da çalışmamız, meme lokalizasyonunun meme kanserli hastalarda, kemoradyoterapiye bağlı akciğer toksisitesini etkilemediğini göstermektedir

Chondrosarcomas are malignant tumors of chondrocytes that affect bones and joints, and it represents the third most common type of primary bone tumors. Chondrosarcoma is difficult to treat because it is relatively resistant to both chemotherapy and radiation. Thus, surgery remains the best available treatment. It is important to find new diagnostic markers and improve treatment options.miRNAs are small non-coding transcripts (19-25 nucleotides) that regulate gene expression via targeting complementary sequences within messenger RNAs (mRNAs). miRNAs have been shown to be involved in regulation of many biochemical pathways. Dysregulated expression of many miRNAs has also been associated with multiple human diseases, such as cancer.18 surgical chondrosarcoma specimens were obtained from patients. RNA extractions were performed from decalcified paraffin embedded tissues. The aim of this study was to investigate the expression levels of miR-181a and miR-371b in patients with chondrosarcoma by using RT-PCR and to evaluate the relationship between these miRNAs and chondrosarcoma.miR-181a was found to be upregulated in chondrosarcoma specimens whereas no significant alteration was found for miR-371b expression.It has been proposed that miRNA expression studies might be used as diagnostic, prognostic marker in cancer. miRNA expression data produced in our study may contribute future chondrosarcoma diagnosis and therapy.

Nous rapportons le cas d’un patient de 70 ans atteint d’un adenocarcinome bronchique ayant presente un syndrome polyuro-polysdipsique. Hospitalisation initiale en cardiologie pour dyspnee aigue avec epanchement pericardique, decouverte recente d’un adenocarcinome bronchique metastatique. Le patient presente depuis 48 h un syndrome polyuro-polydipsique. Diurese entre 4 et 7 L par 24 h, hypernatremie a 153 mmol/L lors de la restriction hydrique en cardiologie. Bilan a l’entree dans le service : natremie a 144 mmol/L, glycemie a 1,35 g/L, osmolalite plasmatique : 315 mOsm/kg, osmolarite urinaire a 240 mOsm/kg. ADH basse en regard de l’osmolalite a 0,7 pmol/L. Le test au minirin permet une concentration urinaire qui passe de 163 mOsm/kg au debut du test a 216 mOsm/kg en fin de test ; osmolalite plasmatique de 305 a 298 mOsm ; natremie de 140 a 138 mmol/L et diurese nettement reduite. L’IRM hypophysaire ne retrouve pas d’anomalie hypophysaire. Le reste du bilan hypophysaire est normal ne mettant pas en evidence d’insuffisance ante-hypophysaire. Le traitement par minirinmelt permet la disparition du syndrome polyuro-polydipsique. Le cancer pulmonaire a petites cellules est la cause la plus frequente de secretion ectopique d’hormone anti-diuretique ou syndrome de Schwartz-Bartter. Dans notre observation, la symptomatologie est opposee puisque notre patient presente un diabete insipide. Par ailleurs, cancer pulmonaire et cancer du sein sont les cancers les plus frequemment responsables de metastases hypophysaires. Le diagnostic ante-mortem de metastase hypophysaire et de la tige pituitaire est difficile car la majorite des lesions sont cliniquement silencieuse et trop petites pour entrainer des modifications radiologiques.

Objective: This is a retrospective study accomplished to determine the efficiency of the first trimester ultrasound examination especially when performed on elective conditions. Materials and Method: The records of patients who had undergone early first trimester transvaginal ultrasound in Obstetrics and Gynecology Department of Cerrahpasa School of Medicine, Istanbul University between from January 1999 to December 2000 were reviewed. The records of 426 patients were eligible for the analysis. The age, parity, gestational week that the ultrasound performed, the complaint on appliance, the features visible on the ultrasound were noted. The patients were divided into two groups: patients without any complaint (Group 1) and patients applied with any complaint (Group 2). The prognosis of the pregnancy was determined through the follow-up. On the statistical analysis, χ-square test, student-t test, Pearson correlaton analysis and multiple linear regression analysis were utilized. Results: In group 1, 79% of examinations were normal whereas the rate of normal ultrasound was 50% in group 2. The difference between the groups was statistically significant (p=0.000). On group 1, 7 subsequent abortions, 7 missed abortions and one case of mole hydatiform were identified (7.6%). When the patient applied with a complaint, ultrasound examination revealed abnormal findings that enabled to reach an accurate diagnosis in 29.9% of these patients. 50 abortions (inevitable or incomplete), 13 missed abortions, 3 ectopic pregnancies and 2 mole hydatiform were diagnosed. The difference between the two groups according to ability to reach a diagnosis on the initial ultrasound was statistically significant (p=0.000). Conclusion: The value of ultrasound examination before the 10th weeks of pregnancy is not validated when the patient has no complaint even if it appears as the most powerful mean to assess the accurate gestational age and observing fetal cardiac activity is the best prognostic variable for the outcome of the fetus.

Objective: This is a retrospective study that is designed to investigate the prognosis of the patients with abnormal diabetes screening test and a negative 100 g oral glucose tolerance test that is accepted as the diagnostic test for gestational diabetes mellitus (GDM). Materials and Method: The records of 281 pregnant patients were reviewed. The data of the patients divided into groups with normal diabetes screening (1 h/50 g oral glucose tolerance test) and abnormal diabetes screening. The patients with abnormal diabetes screening were further divided into all values normal in 100 g Oral glucose tolerance test (OGTT), one value abnormal in 100 g OGTT and GDM subgroups. The prognosis of these patients was reanalyzed concerning the complications that may be attributed to glucose intolerance. Results: Abnormal diabetes screening test was detected in 89 patients (31.6%) and 14 cases were diagnosed as GDM (4.9%). The data of 9 patients were not suitable to analyze. In 31 cases out of 80 (38.75%), an obstetric complication was detected. The rate of similar complications was 27.0% (52/192) in the control group. The difference in the rate of complications was not statistically significant (p>0.05). When the patients with one value abnormal in 100 g OGTT, were further evaluated, the complication rates were 3/9 (33.3%) for the 1st hour abnormal patients, 1/1 for the 2nd hour abnormal patients and 1/1 for the 3rd hour abnormal patients. When the values of 100 g OGTT were compared in complicated and uncomplicated cases, the difference between the groups was not statistically significant (p>0.05). Conclusion: 100 g OGTT is the universal diagnostic test for GDM. There is a major subgroup of patients whose screening positive but diagnostic test (100 g OGTT) negative. These patients seem to be prone to develop obstetric complications related to glucose intolerance but there is not a proper means to detect these patients in clinical practice.

Nous presentons un homme de 70 ans ayant presente une dissociation atrioventriculaire en phase peroperatoire d’une exerese d’un pheochromocytome surrenalien gauche. Il s’agit d’une decouverte fortuite d’un pheochromocytome lors d’une exploration de prostatite. Un uroscanner avait mis en evidence une masse surrenalienne gauche de 91 × 86 mm. Les explorations biologiques mettaient en evidence une chromogranine A a 9 fois la normale (9N), metanephrine libre a 0,5N, normetanephrine libre a 3N, metanephrine urinaire a 0,7N et normetanephrine urinaire a 4N. Une scintigraphie MIBG ne mettait pas en evidence d’autre localisation. L’exerese a ete realisee par cœlioscopie. Lors de l’intervention et a trois reprise, une bradycardie a 30 battements par minute avec dissociation atrioventriculaire et echappement ventriculaire a QRS fins mais aussi larges est survenue. Le patient a ete transfere aux soins intensifs de cardiologie pour la pose d’un pacemaker. Les suites operatoires etaient simples. L’interet de la description de ce patient est d’exposer un cas de dissociation atrioventriculaire qui a pu etre masquee et « corrigee » par le pheochromocytome puis revele par son exerese. Les decharges de catecholamine liees aux manipulations de la glande surrenale peuvent egalement etre a l’origine d’un arret sinusal comme deja publie dans la litterature. Sur le plan anesthesique, des poussees hypertensives liees a la manipulation de la glande surrenales sont decrites en phase peroperatoire, ainsi que des hypokaliemies et hyperglycemies, liees a la secretion beta-mimetique. Classiquement, il n’y a pas de description de troubles conductifs lie a l’exerese d’un pheochromocytome ; cependant on retrouve quelques cas decrits.

Introduction Survenue d’une maladie de Basedow chez une patiente atteinte d’un syndrome de Turner et supplementee par levothyrox pour une hypothyroidie d’Hashimoto. Observation Patiente de 31 ans suivie pour un syndrome de Turner dans sa forme mosaique, decouvert a l’âge de 4 ans suite a un retard statural. Fin 2015, mise en place d’une supplementation par levothyrox devant l’apparition d’une hypothyroidie biologique avec anticorps anti-TPO positifs. En 2016, necessite de diminuer puis de stopper la supplementation par levothyrox devant l’apparition d’une hyperthyroidie frustre. Les anticorps anti-recepteur de la TSH sont fortement positifs a 35 en septembre 2016 traduisant donc une maladie de Basedow. L’hyperthyroidie n’est que biologiquement moderee devant la presence concomitante d’anticorps anti-TPO et anti-recepteur de la TSH. Demandeuse d’une solution radicale dans le cadre d’un projet de grossesse avec don d’ovocytes, elle a beneficie d’une thyroidectomie totale en octobre 2016 (TSI a 6 en fevrier 2017). Discussion La plus grande frequence des dysthyroidies auto-immunes chez les patientes turneriennes par rapport a la population generale est bien connue, avec une prevalence entre 30 et 50 % de l’hypothyroidie d’Hashimoto et de 0,5 a 5 % pour la maladie de Basedow selon les etudes, et serait due a une anomalie du chromosome X qui interfere avec l’auto-immunite. Si la recherche des anticorps anti-TPO semble etre realisee de facon courante chez ces patientes, il pourrait etre interessant de realiser dans le meme temps les anticorps anti-recepteur de la TSH notamment dans le cadre d’un projet de grossesse avec don d’ovocytes.

A new ligand, 2−(p−tolyl)−1,2,3,4−tetrahydroquinazoline−2 −carbaldehyde oxime, was synthesized from a condensation reaction of oxo−p−tolyl−acetaldehyde oxime with 2−aminobenzyl- amine and then amine−imine−oxime complexes of this compound that formed with Co(III) and Ni(II) ions were obtained. All structures were characterized by spectral methods, elemental analysis, magnetic susceptibility, molar conductivity, and thermal analyses. The results that were obtained show that the cyclic ligand, HL, undergoes a ring−opening reaction upon complexation with metal ions. The Co(III) and Ni(II) complexes have a metal−ligand ratio of 1:2 and the imine ligand coordinates through the amine, imine, and oxime groups nitrogen atoms with metal ions.

A novel 1,2,3,4-tetrahydroquinazoline oxime was synthesised from a condensation reaction of 2-(naphthalen-2-yl)-2-oxoacetaldehyde oxime with 2-aminobenzylamine. Subsequently, a-imine oxime complexes of this compound that formed with Co(III) and Ni(II) metal ions were obtained. All structures were characterised by spectral methods (FT-IR, nmr, AAS, mass), elemental analysis, thermal analyses, magnetic susceptibility and molar conductivity. The analyses confirmed the following molecular formulae: [Co(L)2]Cl • C2H5OH • 0,5H2O and [Ni(HL)2]Cl2. The Co(III) complex is diamagnetic. Magnetic susceptibility measurements revealed octahedral geometry for the Co(III) and Ni(II) complexes. In the complexes, the chloride ion was found to be noncoordinated to the metal ions as confirmed by conductivity measurements. The NMR spectra of the Co(III) complex confirmed the presence ofethanol in the Co(III) complex. The experimental results of TG-DTA showed that the Co(III) complex contained ethanol and crystal water in the first two stages. The end of the thermal decomposition of the complexes yielded a final product of a metal oxide.

The structural properties of alkali metals are studied with the use of the linear muffin-tin orbital--atomic sphere approximation method to calculate the total energies of these elements as a function of the number of k points within the local-density approximation. The role of zero-point correction and the k-point sampling on the stable structure determinations are emphasized. \textcopyright{} 1996 The American Physical Society.

We have carried out low-field ac measurements of harmonic susceptibilities and ac magnetization of a single-phase partially melt-textured high-temperature superconductor over a wide range of temperatures, field amplitudes and measuring frequencies. Both the fundamental and third-harmonic susceptibility versus temperature and ac magnetization at fixed temperatures close to exhibit strong ac field amplitude and small frequency dependences. In the third-harmonic susceptibility, we observed a double-peak feature in the vicinity of as a function of the applied ac field. This strong field dependence is associated with depinning of vortices at some B and T in different superconducting matrices, which involves different ac loss mechanisms. Experimental results are compared with predictions of some existing theoretical models.

The linear-muffin-tin-orbital method within the atomic-sphere approximation has been used to calculate the structural energy differences and specific-heat coefficients of the alkaline-earth metals. It has been shown that while non-local exchange corrections to the local-density approximation play almost no role for the structural energy differences, specific-heat coefficient values, especially for Sr, are correctly predicted only when these corrections are taken into account.

Calismada Siyah Alaca inekler VKS’lerine gore 3 gruba ayrilmistir. Birinci Grup VKS≤3 olan (VKS = 3 dahil) 9 inekten, 2. Grup VKS 3 ile 4 (VKS =3 ve VKS = 4 dahil degil) arasinda degisen 29 inekten ve 3. Grup VKS≥4 (VKS = 4 dahil) olan 27 inekten olusmustur. Calismada VKS’nin transfer edilebilir kalitedeki embriyo sayi ortalamasi, transfer edilemez kalitedeki embriyo sayi ortalamasi, toplam embriyo sayi ortalamasi, transfer edilebilir embriyo orani ile transfer edilemez embriyo oranina olan etkisinin istatistiksel olarak onemsiz oldugu tespit edilmistir (P>0.05). Denemede ineklerde VKS ile transfer edilebilir kalite embriyolar arasinda (r = -0.156, P = 0.215), transfer edilemez kalite embriyo sayisi arasinda (r = 0.186, P=0.137), toplam embriyo sayisi arasinda (r = 0.007, P = 0.958), transfer edilebilir embriyo orani arasinda (r = -0.205, P = 0.101) ve transfer edilemez kalitedeki embriyo orani arasinda (r = 0.205, P = 0.101) dusuk ve onemsiz duzeyde bir iliski saptanmistir.

Summary The relationship between donor age in donor cows (n=165) and quality of embryos was examined in this study. According to their donor age, the Holstein donors were divided into three groups. In the first group there were 61 donors between 2-3 years old; in the second group, 28 donors between 4-5 years old; and in the third group, 76 donors between ≥6 years old. In the study, donor cow age were aff ected transferable embryos and total embryo numbers statistically significant (P 0.05). The transferable embryo rate in groups was found between 53.46-63.24% in the experiment. A significant correlation (r=0.142) was recorded, i.e. between donor age and transferable embryos; a significant correlation was recorded (r=-0.138), i.e. between donor age and total embryos.

Summary Bevacizumab is a monoclonal antibody against vascular endothelial growth factor (VEGF), which is used to treat several cancers. Currently, experience with anti-VEGF treatment for psoriasis is limited, and no published reports on this use exist. We describe a patient with metastatic colon cancer and psoriasis who experienced complete remission of psoriasis during treatment with bevacizumab and combination chemotherapy without any other treatment for psoriasis. These data suggest that bevacizumab may be a promising therapeutic agent for the treatment of psoriasis.

We report the magnetic resonance (MR) angiographic findings of an asymptomatic case with abnormal branching of aortic arch and Kommerell's diverticulum, which to our knowledge has not been described previously.

Rhombencephalosynapsis is a rare cerebellar malformation characterized by absence of the vermis and midline fusion of hemispheres. It is easily recognized by MRI. No specific syndrome has been described in association with this cerebellar anomaly. Early diagnosis is possible especially when coexisting brain anomalies are present. We report a symptomatic case of rhombencephalosynapsis with moderate hydrocephalus.

To determine the incidence of late-onset congenital adrenal hyperplasia (LOCAH) due to 21-hydroxylase deficiency among hirsute women and to evaluate the results of the ACTH stimulation test with the clinical characteristics.Prospective, controlled study. One hundred women with hirsutism and 14 normally cycling women without hirsutism were included in this study at the Division of Reproductive Endocrinology, Department of Obstetrics and Gynecology, Cerrahpasa School of Medicine, Istanbul University. After basal serum progesterone (P) and 17 hydroxyprogesterone (17OHP) levels were determined, an ACTH stimulation test was performed on cycle day 3-5. The same parameters were checked 30 minutes later. We estimated the 21 hydroxylase activity by calculating the change in 17OHP (17OHP 30-0) and the summed rate of the change in P and 17OHP ([P30-0] + [17OHP30-01/30 minutes). The 95th percentile for these estimates in normal women were calculated, and values above three times the 95th percentile were considered to distinguish women with LOCAH due to 21-hydroxylase deficiency.The 95th percentile for 17OHP 30-0 and (P30-0) + (17OHP30-0)/30 minutes in normal women was 1.6 and 8.9 ng/dL/min, respectively. Regarding 17OHP 30-0 values, three women with hirsutism had levels above three times the 95th percentile of these estimates, and 28 women had estimates of more than the 95th percentile but less than threefold. Seventeen of 28 women had oligomenorrhea, and all had severe hirsutism. The women with severe hirsutism and oligomenorrhea had significantly higher ACTH-stimulated serum 17OHP levels and values for 17OHP 30-0 and (P30-0 + (17OHP30-0)/30 min) than did normally cycling women.The incidence of LOCAH due to 21-hydroxylase deficiency and mild 21-hydroxylase deficiency is 3% and 28%, respectively, in women with hirsutism. Clinical characteristics are not helpful in determining 21-hydroxylase deficiency. However, the incidence of 21-hydroxylase deficiency is more common among women with severe hirsutism and oligomenorrhea. Conclusions: Proteins which can be analyzed with proteomics display tumor load and response to treatment. These proteins can be used for diognosis, treatment modalities and prognostic analysis. In the literature there are very few studies for proteomics analysis of pericardial effusions. Advance in proteomics analyses is apperant but present studies do not produce novel, specific biomarkers. Developments increase in proteomics field and data analyse programs are improved.

High temperature, thin ceramic insulation coatings are necessary for the application of High Temperature Superconductors (HTS) in high field magnet technology. Especially in the so called “wind and react” technology, insulation coatings have to withstand the final heat treatment at about 900°C, otherwise turn to turn insulation will be broken. We have developed a sol-gel coating method for HTS tape conductors at NHMFL. Excellent adhesion at the interface due to the significant interfacial reactions, indicates that this sol-gel route is a feasible method for applying zirconia coatings. Sol-gel technology is a low temperature chemical route of preparing inorganic material. The stability and the coating characteristics of a organozirconium compound have been investigated to coat ZrO2 films on silver sheathed HTS tape conductors. This method was used to coat stainless steel with ZrO2.The ZrO2 coatings were obtained by dipping the silver sheathed HTS tape in a solution of zirconiumtetrabutoxide, isopropanol and acetylacetone.
