PurposeThis study aimed to identify mental health, physical health, demographic and disease characteristics relating to work productivity in people with multiple sclerosis (MS).MethodsIn this cross-sectional study, 236 employed people with MS (median age = 42 years, 78.8% female) underwent neurological and neuropsychological assessments. Additionally, they completed questionnaires inquiring about work productivity (presenteeism: reduced productivity while working, and absenteeism: loss of productivity due to absence from work), mental and physical health, demographic and disease characteristics. Multiple linear and logistic regression analyses were performed with presenteeism and absenteeism as dependent variables, respectively.ResultsA model with mental and physical health factors significantly predicted presenteeism F(11,202) = 11.33, p ConclusionsIn people with MS, both cognitive and physical impact of fatigue are positively related to presenteeism, while symptoms of depression and cognitive impact of fatigue are positively related to absenteeism.Implications for rehabilitationMultiple sclerosis (MS) affects people of working age, significantly interfering with work productivity.Higher cognitive and physical impact of fatigue were associated with more presenteeism in workers with MS.A higher cognitive impact of fatigue and more depressive symptoms were associated with absenteeism in workers with MS.Occupational and healthcare professionals should be aware of the impact of both physical and mental health on work productivity in workers with MS.

Purpose In Europe, intravenous fosfomycin (IV) is used particularly in difficult-to- treat or complex infections, caused by both Gram-positive and Gram-negative pathogens including multidrug-resistant strains. Here, we investigated the efficacy and safety of intravenous fosfomycin under real-life conditions. Methods Prospective, multi-center, and non-interventional study in patients with bacterial infections from 20 intensive care units (ICU) in Germany and Austria (NCT01173575). Results Overall, 209 patients were included (77 females, 132 males, mean age: 59 +/- 16 years), 194 of which were treated in intensive care (APACHE II score at the beginning of fosfomycin therapy: 23 +/- 8). Main indications (+/- bacteremia or sepsis) were infections of the CNS (21.5%), community- (CAP) and hospital-acquired pneumonia (HAP)/ventilator-associated pneumonia (VAP, 15.3%), bone and joint infections (BJI, 11%), abdominal infections (11%), and bacteremia (10.5%). Most frequently identified pathogens were S. aureus (22.3%), S. epidermidis (14.2%), Enterococcus spp. (10.8%), E. coli (12.3%) and Klebsiella spp. (7.7%). At least one multidrug-resistant (MDR) pathogen was isolated from 51 patients (24.4%). Fosfomycin was administered with an average daily dose of 13.7 +/- 3.5 g over 12.4 +/- 8.6 days, almost exclusively (99%) in combination with other antibiotics. The overall clinical success was favorable in 81.3% (148/182) of cases, and in 84.8% (39/46) of patients with >= 1 MDR pathogen. Noteworthy, 16.3% (34/209) of patients developed at least one, in the majority of cases non-serious, adverse drug reaction during fosfomycin therapy. Conclusion Our data suggest that IV fosfomycin is an effective and safe combination partner for the treatment of a broad spectrum of severe bacterial infections in critically ill patients.

Background Patients surviving critical illnesses, such as sepsis, often suffer from long-term complications. After discharge from hospital, most patients are treated in primary care. Little is known how general practitioners (GPs) perform critical illness aftercare and how it can be improved. Within a randomised controlled trial, an outreach training programme has been developed and applied.Objectives The aim of this study is to describe GPs’ views and experiences of caring for postsepsis patients and of participating a specific outreach training.Design Semistructured qualitative interviews.Setting 14 primary care practices in the metropolitan area of Berlin, Germany.Participants 14 GPs who had participated in a structured sepsis aftercare programme in primary care.Results Themes identified in sepsis aftercare were: continuity of care and good relationship with patients, GP’s experiences during their patient’s critical illness and impact of persisting symptoms. An outreach education as part of the intervention was considered by the GPs to be acceptable, helpful to improve knowledge of the management of postintensive care complications and useful for sepsis aftercare in daily practice.Conclusions GPs provide continuity of care to patients surviving sepsis. Better communication at the intensive care unit–GP interface and training in management of long-term complications of sepsis may be helpful to improve sepsis aftercare.Trial registration number ISRCTN61744782.

In this study, the authors address the question of whether firms may successfully pursue service recovery strategies after severe online privacy violations. The study treats online privacy violations as a service failure and uses justice theory to measure repurchasing intention after consumer complaints in three different scenarios. The three scenarios differ in the sense that the accountability and the outcome of the service failure are different. The results indicate that despite the different instances of online privacy violation in each scenario, the service recovery efforts consistently created satisfaction with service recovery, significantly increased consumer trust, decreased perceived risk and increased repurchase intentions. The study finds that that both distributive and procedural justice plays an important role in online service recovery while interactional justice did not have any impact. Finally, even in cases of severe online privacy violation service recovery plays an important role generating repurchase intentions.

Nosocomial pneumonia (HAP) is a frequent complication of hospital care. Most data are available on ventilator-associated pneumonia. However, infections on general wards are increasing. A central issue are infections with multidrug resistant (MDR) pathogens which are difficult to treat in the empirical setting potentially leading to inappropriate use of antimicrobial therapy.This guideline update was compiled by an interdisciplinary group on the basis of a systematic literature review. Recommendations are made according to GRADE giving guidance for the diagnosis and treatment of HAP on the basis of quality of evidence and benefit/risk ratio.This guideline has two parts. First an update on epidemiology, spectrum of pathogens and antimicrobials is provided. In the second part recommendations for the management of diagnosis and treatment are given. New recommendations with respect to imaging, diagnosis of nosocomial viral pneumonia and prolonged infusion of antibacterial drugs have been added. The statements to risk factors for infections with MDR pathogens and recommendations for monotherapy vs combination therapy have been actualised. Recommendations are made according to GRADE giving guidance for the diagnosis and therapy of HAP on the basis of quality of evidence and benefit/risk ratio. The guideline has two parts. First an update on epidemiology, spectrum of pathogens and antiinfectives is provided. In the second part recommendations for the management of diagnosis and treatment are given. Proper microbiologic work up is emphasized for knowledge of the local patterns of microbiology and drug susceptibility. Moreover this is the optimal basis for deescalation in the individual patient. The intensity of antimicrobial therapy is guided by the risk of infections with MDR. Structured deescalation concepts and strict limitation of treatment duration should lead to reduced selection pressure.

Dabigatran, ein direkter und reversibel bindender Faktor-IIa-Hemmer, ist in Europa zur Schlaganfallprophylaxe bei Vorhofflimmern und zur primaren Thromboseprophylaxe nach elektivem Huft- und Kniegelenkersatz zugelassen. Im Gegensatz zu Vitamin-K-Antagonisten ist eine routinemasige Kontrolle der gerinnungshemmenden Wirkung von Dabigatran nicht notwendig. Eine Abschatzung der antikoagulatorischen Wirkung von Dabigatran konnte fur den klinisch tatigen Arzt in spezifischen Situationen wie vor Notfalloperationen oder schweren Blutungen wichtig sein und kann durch die aktivierte partielle Thromboplastinzeit, Ecarin Clotting Time oder Thrombinzeit durchgefuhrt werden. Der spezifisch fur Dabigatran kalibrierte verdunnte Thrombinzeit Test (Hemoclot®) kann fur quantitative Aussagen zu Plasmakonzentrationen eingesetzt werden. Zur Normalisierung des Blutungsrisikos vor elektiven Eingriffen bei Patienten mit normaler Nierenfunktion reicht in der Regel ein Absetzen von Dabigatranetexilat 24 Stunden vor dem Eingriff aus. Bei eingeschrankter Nierenfunktion oder bei einem Eingriff mit erhohtem Blutungsrisiko verlangern sich die empfohlenen Absetzfristen dementsprechend. Beim Auftreten von Blutungen wahrend einer Therapie mit Dabigatran richtet sich die weiterfuhrende Behandlung nach dem Schweregrad und Lokalisation der Blutung. Eine Besonderheit besteht darin, dass Dabigatran mittels Dialyse aus dem Kreislaufsystem relativ rasch entfernt werden kann. Empfehlungen: Bei aus klinischer Sicht leichten Blutungen ist eine kurzfristige Verschiebung der Substanzeinnahme anzuraten. Der Zeitraum basiert hier auf dem individuellen Thromboserisiko des Patienten. Die Gabe von Faktorenkonzentraten ist bei leichten Blutungen nicht indiziert. Bei mittelschweren bis schweren Blutungen sollte nach Absetzen von Dabigatran insbesondere auf die Stabilisierung der Herz- und Kreislauffunktion durch Volumenersatz und Blutprodukte sowie auf die (lokale) Behandlung des Blutungsherdes geachtet werden. Falls die Option besteht, kann Dabigatran mittels Haemofiltration oder Dialyse schnell und effektiv aus dem Kreislaufsystem entfernt werden. Bei schweren oder lebensbedrohlichen Blutungen wird ein spezielles Hamostasemanagement erforderlich. Je nach Lokalisation der Blutung sollte nebst der hamodynamischen Stabilisierung des Kreislaufes der Einsatz von Faktorenkonzentraten in dieser Situation zeitnah erfolgen. Wichtig ist hierbei, dass die Standard- Laborparameter den Effekt der Faktorenkonzentrate nicht akkurat wiedergeben. Daher sollte der Effekt klinisch anhand der einsetzenden Blutgerinnung beurteilt werden.

SummaryPatients with severe renal insufficiency (sRI) have been suggested to be at an increased risk of bleeding with low-molecular-weight heparins (LMWH). We aimed at assessing the benefits and risks of certoparin in comparison to unfractionated heparin (UFH) in these patients. In this subgroup analysis of the CERTIFY trial, acutely ill, non-surgical patients ≥70 years received certoparin 3,000U aXa o.d. or UFH 5,000 IU t.i.d. One hundred eighty-nine patients had a glomerular filtration rate (GFR) ≤30 ml/min/1.73 m2, 3,050 patients served as controls. Patients with sRI had a mean age of 85.9 ± 6.6 years (controls 78.4 ± 6.0) and were treated for a mean of 9.3 ± 3.7 days (9.9 ± 4.3). Thromboembolic event rates were comparable (4.55 vs. 4.21%; OR1.08; 95%CI 0.5–2.37) but bleeding was increased in sRI (9.52 vs. 3.54%; OR2.87; 95%CI 1.70–4.83). The incidence of the combined end-point of proximal DVT, symptomatic non-fatal PE and VTE related death was 6.49% with certoparin and 2.60% with UFH (OR2.60; 95%CI 0.49–13.85). There was a decrease in total bleeding with certoparin (OR0.33; 95%CI 0.11–0.97), which was non-significant in patients with GFR >30 ml/min/1.73 m2. In two multivariable regression models certoparin and immobilisation 30 ml/ min/1.73 m2). In conclusion, certoparin 3,000U anti Xa o.d. was as efficacious as 5,000 IU UFH t.i.d. in patients with sRI but had a reduced risk of bleeding.

Hintergrund Die Implementierung neuer Anasthetika kann sich laut zahlreicher pharmakookonomischer Studien gunstig auf die Ausleitungszeiten und damit auf die Leistungsfahigkeit einer Anasthesieabteilung auswirken. Aufgrund der artifiziellen Studienprotokolle ist unklar, ob sich diese Ergebnisse auch auf den klinischen Routinebetrieb deutscher Krankenhauser ubertragen lassen.

Bei der Krampfadererkrankung (Krampfaderleiden, primare Varikose) handelt es sich um eine degenerative Erkrankung der Venenwand im oberflachlichen Venensystem der Beine, bei der sich unter dem Einfluss verschiedener Realisationsfaktoren (z. B. Schwangerschaften, Orthostasebelastung) im Laufe des Lebens in unterschiedlicher Auspragung und Schweregrad Krampfadern (Varizen) entwickeln [243]. Von der primaren Varikose ist die sekundare Varikose zu unterscheiden, die auf dem Boden obliterierender Prozesse im tiefen Venensystem entstehen kann. Nach topographischen bzw. morphologischen Kriterien werden im Wesentlichen folgende Krampfadertypen unterschieden: Stammvarizen, Seitenastvarizen, Perforansvarizen [25], Retikulare Varizen, Besenreiservarizen. Zur anatomischen und topographischen Bezeichnung der Venen sollte die, in einem transatlantischen Konsensusdokument erarbeitete, Nomenklatur nach VEIN-TERM verwendet werden [50, 99]. Die hamodynamisch bedeutsame Stammveneninsuffizienz lasst sich nach Hach (1981) in Stadien einteilen [450]. Bei Insuffizienz ab der Mundungsregion wird die Refluxstrecke durch den distalen Insuffizienzpunkt bestimmt (Tab. 1). Tab. 1 Stadieneinteilung (Refluxstrecke) der Stammvenen. B. pudendale, gluteale, pelvine Varikose [2, 122].

Die Empfehlungen fur die Antikoagulation bei uber 80-jahrigen Patienten orientieren sich an der Abwagung des Thromboembolie- und des Blutungsrisikos. Sie erganzen die bisher vorliegenden Empfehlungen fur die betroffenen Indikationen. Die postoperative Thromboembolieprophylaxe erfolgt bei dieser Patientengruppe mit niedermolekularem Heparin (NMH). Bei sehr hohem Blutungsrisiko werden angepasste Kompressionsstrumpfe oder die intermittierende pneumatische Kompression eingesetzt. Bei sehr hohem Thromboembolierisiko wird ab dem 2. postoperativen Tag die Dosis gesteigert. Blutungs- und Thromboembolierisiko sind taglich neu festzulegen und die Therapie entsprechend anzupassen. Bei akuten Erkrankungen mit Bettlagerigkeit erfolgt die Thromboembolieprophylaxe wie postoperativ. Die Therapie der venosen Thromboembolie soll mit zwei Drittel der therapeutischen Dosis erfolgen. PurposeFew marketing studies look at service classifications for self‐service technologies (SSTs) and none directly compare consumer‐based perceptions of traditional services to SSTs. To fill this gap, this study aims to examine how customers perceived traditional services and SSTs on service classifications criteria proposed by Lovelock, Bowen and Bell.Design/methodology/approachIn two separate studies consumer ratings for each classification method on each service were obtained. Using multi‐dimensional scaling (MDS), 13 traditional services and 12 SSTs were separately mapped onto a perceptual space of service classifications.FindingsThe comparison of the two perceptual spaces reveals that consumers viewed the classifications of convenience, person/object, and delivery for SSTs differently than that for traditional services. The classifications of traditional services were represented by two dimensions of customization/standardization and person/object. In contrast, the classifications of SSTs were represented by two dimensions of customization/standardization and separability/inseparability. Thus the description of the underlying dimensions of services varied by traditional services or SSTs.Research limitations/implicationsIt is possible that the results of the MDS were influenced by the use of preset classifications. Results may also be influenced by the authors' choice of MDS method. Further research is needed regarding the classification of SSTs and the use of these classifications for SST design.Originality/valueThis research extends previous consumer‐based classification research by including SSTs. The findings identified separate typologies for SSTs and traditional services. The typologies should be of interest to both researchers and managers who are interested in how SSTs are perceived by consumers.

The peptide hormone uroguanylin stimulates chloride secretion via activation of intestinal guanylyl cyclase C (GC-C). It is characterized by two disulfide bonds in a 1-3/2-4 pattern that causes the existence of two topological stereoisomers of which only one induces intracellular cGMP elevation. To obtain an unambiguous structure-function relationship of the isomers, we determined the solution structure of the separated uroguanylin isoforms using NMR spectroscopy. Both isomers adopt well-defined structures that correspond to those of the isomers of the related peptide guanylin. Furthermore, the structure of the GC-C-activating uroguanylin isomer A closely resembles the structure of the agonistic Escherichia coli heat-stable enterotoxin. Compared with guanylin isomers, the conformational interconversion of uroguanylin isomers is retarded significantly. As judged from chromatography and NMR spectroscopy, both uroguanylin isoforms are stable at low temperatures, but are subject to a slow pH-dependent mutual isomerization at 37 degrees C with an equilibrium isomer ratio of approximately 1:1. The conformational exchange is most likely under the sterical control of the carboxy-terminal leucine. These results imply that GC-C is activated by ligands exhibiting the molecular framework corresponding to the structure of uroguanylin isomer A.
