1. Successful response of intradermal hepatitis B vaccine in nonresponders of intramuscular hepatitis B vaccine in general and hemodialysis population

Abstract

Background: Hepatitis B infection is one of the most common infections worldwide, with its vaccination being an effective preventive measure. Nonresponse to hepatitis B vaccination increases population susceptibility to virus dissemination along with detrimental complications. Despite twice intramuscular vaccination series, 14.3% in the general population and 50% in hemodialysis patients fail to mount a response against hepatitis B. We aimed to evaluate the effectiveness of intradermal (ID) vaccination in the nonresponders amongst the general and hemodialysis population. Methods: A total of 5 doses of 10 μg of hepatitis B vaccine was given intradermally, 2 weeks apart, to both the study groups: patients who were on hemodialysis and the general population group who previously had failed to achieve satisfactory antibody titers with the IM administration of the vaccine. A hepatitis B surface antibody (HBsAb) titer of ≥10 IU/mL and ≥100 IU/mL were considered “responder” and “good responder,” respectively. Results: Out of a total of 95 participants, 49 (51.6%) were hemodialysis-dependent. Most of the participants were females 49 (51.6%). The mean age of all the participants was 39.02 ± 13.5 years (range: 18–70 years). Overall, 75.8% of the participants responded to the ID vaccination with a mean HBsAb titer of 263.5 ± 350.1 IU/L. Almost similar vaccination response was observed in both the hemodialysis and general population i.e., 75.5% and 76.1%, respectively (P = 1.00). In the hemodialysis group, the absence of hypertension (P = 0.04) and age ≥36 years (P = 0.016) were associated with an ID vaccination response. Conclusion: For those not responding to the conventional IM route of the hepatitis B vaccine, the ID route is an effective way of immunization in this group and this approach would lead to a decrease in infection rates in the vulnerable population such as those on hemodialysis.

Abstract

We report on gravitational wave discoveries from compact binary coalescences detected by Advanced LIGO and Advanced Virgo in the first half of the third observing run (O3a) between 1 April 2019 15:00 UTC and 1 October 2019 15:00 UTC. By imposing a false-alarm-rate threshold of two per year in each of the four search pipelines that constitute our search, we present 39 candidate gravitational wave events. At this threshold, we expect a contamination fraction of less than 10%. Of these, 26 candidate events were reported previously in near real-time through GCN Notices and Circulars; 13 are reported here for the first time. The catalog contains events whose sources are black hole binary mergers up to a redshift of ~ 0.8, as well as events whose components could not be unambiguously identified as black holes or neutron stars. For the latter group, we are unable to determine the nature based on estimates of the component masses and spins from gravitational wave data alone. The range of candidate event masses which are unambiguously identified as binary black holes (both objects ≥ 3 M⨀) is increased compared to GWTC-1, with total masses from ∼ 14M⨀ for GW190924 021846 to ∼ 150M⨀ for GW190521. For the first time, this catalog includes binary systems with significantly asymmetric mass ratios, which had not been observed in data taken before April 2019. We also find that 11 of the 39 events detected since April 2019 have positive effective inspiral spins under our default prior (at 90% credibility), while none exhibit negative effective inspiral spin. Given the increased sensitivity of Advanced LIGO and Advanced Virgo, the detection of 39 candidate events in ∼26 weeks of data (∼1.5 per week) is consistent with GWTC-1.

The novel coronavirus disease 2019 (COVID-19) pandemic, caused by severe acute respiratory syndrome coronavirus 2, has become a global challenge of unprecedented nature since December 2019. Although most patients with COVID-19 exhibit mild clinical manifestations and upper respiratory tract involvement, in approximately 5%-10% of patients, the disease is severe and involves multiple organs, leading to multi-organ dysfunction and failure. The liver and gastrointestinal tract are also frequently involved in COVID-19. In the context of liver involvement in patients with COVID-19, many key aspects need to be addressed in both native and transplanted organs. This review focuses on the clinical presentations and laboratory abnormalities of liver function tests in patients with COVID-19 with no prior liver disease, patients with pre-existing liver diseases and liver transplant recipients. A brief overview of the history of COVID-19 and etiopathogenesis of the liver injury will also be described as a prelude to better understanding the above aspects.

Abstract

Objective: Gastroesophageal reflux disease (GERD) prevalence ranges from 24% to 35% in Pakistani population. Studies have demonstrated GERD frequency is directly proportional to increasing weight. Thus, the frequency of obesity among gastro esophageal reflux disease patients was determined. Methodology: This cross-sectional study was conducted on outpatients with GERD visiting Department of Hepatogastroenterology, Sindh Institute of Urology and Transplantation, Karachi; from December 2017 to June 2018. Clinical and demographic parameters of study population were recorded. Body mass index (BMI) more than 27 was labeled as obese. Patients with history of ischemic heart disease, pregnancy, ascites and/or Patients on NSAIDS, bisphosphonates or steroids were excluded. Results: One hundred and fifty patients with mean age of 43.3±11.5 years were included in study. Eighty-seven patients (63.3%) had age more than 40 years. Majority of study population were males 95 (63.3%). Mean height and weight of study population were 163.09±9.56 cm and 72.92±15.13 kg, respectively. Mean BMI were 27.55±6.02 in which 71 (47.3%) were obese i.e., BMI >27. Majority were Urdu speaking i.e., 57 (38%) followed by Sindhi 31 (20%) Diabetes mellitus was documented in 22 (14.7%) and hypertension in 16 (10.7%) patients. Statistically significant association of GERD was seen in obese females (p-value: 0.018) Conclusion: Increase frequency of obesity is significantly associated with GERD. 5. A Tertiary Care Center's Experience with Clinicopathological Characteristics of Gallbladder Carcinoma in Our Population

Abstract

Gallbladder cancer (GBC) is the most common malignant biliary tract tumor with the shortest survival from the time of diagnosis. This poor prognosis is due to the destructive biologic behavior of GBC, lack of sensitive screening tests for early detection, and vague nature of first presentation. Here in this study, we will evaluate the baseline characteristics of the patients presenting with gallbladder carcinoma in our population.This retrospective study was conducted in the Department of Gastroenterology at Sindh Institute of Urology and Transplantation (SIUT), Karachi. Patient data were compiled and composed from the in-patient health records, radiology, and operational records. Those patients with suspicion of GBC, but negative at histology, or patients having inconclusive radiologic findings, were excluded. Baseline characteristics were recorded. Results were presented as means ± SD for quantitative data or as numbers with percentages for qualitative data. Continuous variables were analyzed using the Student'sA total of 162 patients were included in our study. Among them, 101 (62.3%) were females. Hypertension was the most common comorbid illness noted in 29 (17.9%) patients while 91 (56.2%) patients had no concurrent comorbidities. Most common risk factor for carcinoma of gallbladder was gallstones seen in 106 (65.1%) patients. The most common presenting complaint was combination of obstructive jaundice, weight loss with right hypochondrial pain seen in 66 (40.7%) patients. On CT abdomen, direct liver infiltration without lymphovascular invasion was noted in 77 (47.5%) patients followed by liver infiltration along with lymphovascular invasion in 26 (16%) patients and distant metastasis in 24 (14.8%) patients. On gallbladder (GB) mass biopsy, 58 (35.8%) patients had well-differentiated, 46 (28.4%) had moderately differentiated, while 33 (20.4%) had poorly differentiated adenocarcinoma. Of 162 patients, 103 (63.6%) patients underwent endoscopic retrograde cholangiopancreatography (ERCP). The most common finding on ERCP was proximal common bile duct (CBD) stricture with intrahepatic biliary system dilatation which was noted in 95 (58.6%) patients. Percutaneous transhepatic cholangiography (PTC) was performed only in 9 (5.6%) patients. Seventeen (10.5%) patients were managed by simple cholecystectomy, 39 (24.1%) patients underwent extended cholecystectomy, 14 (8.6%) patients underwent chemotherapy, while 102 (56.8%) patients were given palliative management. When followed for 1 year, 101 (62.3%) patients died within 6 months.The baseline characteristics, biopsy findings, modes of treatment, and rates of 1 year mortality were studied in patients with gallbladder carcinoma in our population. Advanced age, high white blood cell counts, and serum bilirubin at presentation with low lymphocyte count and presence of comorbid illnesses were the factors independently associated with increased mortality in patients with gallbladder carcinoma. Abstract

This paper presents results of an all-sky search for periodic gravitational waves in the frequency range [50, 1 190] Hzand and with frequency derivative range of approx. [-20,1.1] x 10(exp-10) H(sub 0)z s(exp-1) for the fifth LIGO science run (S5). The search uses a noncoherent Hough-transform method to combine the information from coherent searches on time scales of about one day. Because these searches are very computationally intensive, they have been carried out with the Einstein@Home volunteer distributed computing project. Postprocessing identifies eight candidate signals; deeper follow-up studies rule them out. Hence, since no gravitational wave signals have been found, we report upper limits on the intrinsic gravitational wave strain amplitude ho. For example, in the 0.5 Hz-wide band at 152.5 Hz, we can exclude the presence of signals with h(exp 0) greater than 7.6 x 10(exp-25) at a 90% confidence level. This search is about a factor 3 more sensitive than the previous Einstein@Home search of early S5 LIGO data.

Abstract

We present results of a search for continuously-emitted gravitational radiation, directed at the brightest low-mass X-ray binary, Scorpius X-1. Our semi-coherent analysis covers 10 days of LIGO S5 data ranging from 50-550 Hz, and performs an incoherent sum of coherent $\mathcal{F}$-statistic power distributed amongst frequency-modulated orbital sidebands. All candidates not removed at the veto stage were found to be consistent with noise at a 1% false alarm rate. We present Bayesian 95% confidence upper limits on gravitational-wave strain amplitude using two different prior distributions: a standard one, with no a priori assumptions about the orientation of Scorpius X-1; and an angle-restricted one, using a prior derived from electromagnetic observations. Median strain upper limits of 1.3e-24 and 8e-25 are reported at 150 Hz for the standard and angle-restricted searches respectively. This proof of principle analysis was limited to a short observation time by unknown effects of accretion on the intrinsic spin frequency of the neutron star, but improves upon previous upper limits by factors of ~1.4 for the standard, and 2.3 for the angle-restricted search at the sensitive region of the detector., 19 pages, 8 figures

This fact is the background for the great influence of the work in standardization bodies and testing institutes on the activities which are going on in research and development in this field. Nevertheless, there are other driving forces too behind these activities which have more or less technical andlor scientific reasons. In the following some of them are briefly outlined.

Abstract

Cathode spots on arc‐cleaned copper and molybdenum electrodes in vacuum were studied by fast image converter framing and streak camera photography with high temporal and spatial resolution. The frame exposure time was 20 ns and the interframe period was 200 ns; the streak sweep time was between 200 ns and 1 μs. Spatial structures with a resolution of 5 μm could be determined by observing the spot movement with a small slit at the streak camera and a high sweep speed. Strong fluctuations of the light emission of the spot were found with characteristic times of 50–100 ns. When the spot moved out of the slit field of view a stepwise decrease occurred in the measured light, indicating an internal substructure of the spot with distances between fragments less than 10 μm and even smaller fragment diameters. The current per fragment was estimated to be 20–40 A. The frames confirm the short time constants of the spot. From frame to frame a spot motion was observed in most cases, yielding spot residence times

Abstract

Whereas some experimental and theoretical work has been carried out on the basic understanding and damping of thermoacoustic oscillations, very little knowledge is available on technical uses. In part 1 of this paper, a complete model for the design of thermoacoustic engines was presented and experimentally verified with a semi-technical device. In this part, we discuss different possibilities of applications, including an electric generator, a cold compressor, thermoacoustic coolers and a drive for a pulse tube refrigerator. The discussions show that all of these applications are possible. They have some advantages and disadvantages in common, e.g. simplicity with few moving parts, moderate power density and delicate design. These engines have some potential to find applications for special uses, but for most cases, they are not likely to replace the classical devices.

Abstract

In cryogenics, geometric arrangements and operating conditions are often suitable for the excitement of thermoacoustic (or Taconis) oscillations. The basic geometry of a thermoacoustic oscillator, a half-open tube with high temperature at the closed end and low temperature at the open end, arises for example in the neck tubes of Dewar vessels or in connecting tubes between cold areas and warm pressure transducers. Oscillations may cause high heat flux into the vessel and errors in measurements. As shown in part 1 of this paper, a complete calculation of this effect is possible, but for the proper design of the piping, it is sufficient to determine the stability limit. This is demonstrated in a practical example. Very often, geometry or operating conditions cannot be selected freely, and systems capable of oscillating have to be built. For this case, or for existing systems, rules to design additional dampers are presented.

Abstract

The ignition and arc phases of vacuum arcs were investigated using differential dye laser absorption photography with simultaneous high spatial (micrometer) and temporal (nanosecond) resolution. The discharge duration was 800 ns, the current 50-150 A, the electrode material copper, and the cathode-anode distance less than 50 mu m. A 0.4 ns laser pulse (tunable, gamma =480-530 nm) was used to obtain momentary absorption photographs of the cathode region. During ignition, an optically thick anode plasma expanded toward the cathode, decaying within 25 ns after bridging the electrode gap. In the arc phase, a fragmentary structure of the cathode spots was observed in situ for the first time. The microspots have a characteristic size of 5-10 mu m. They appear and disappear on a nanosecond time scale. The plasma density of the microspots was estimated to be greater than (3-6)*10/sup 26/ m/sup -3/. >

Abstract

Thermoacoustic oscillations are well known in cryogenics; in most cases they occur spontaneously and are undesired. Much work has been done to calculate this effect; with the final breakthrough achieved by Rott. In this paper, we present an extension to Rott's theory, which allows its application to more complicated geometries. Frequency, pressure amplitudes, power and heat flow can be determined with a moderate amount of computer calculations. Experimental verification has been made for a thermoacoustic heat engine, and good agreement has been found. Based on this theory and on experience with the experimental setup, possible applications of thermoacoustic oscillations as heat engines and as refrigerators will be discussed in part 2 of this paper. In part 3, guidelines for the correct design of cryogenic piping and of different dampers are given.

Abstract

Ninety-four cases of limb vascular traumas were observed in children during a 25 year-period. Treatment was done less than 12 hours after the injury in 83 cases, later than 24 hours in 7 cases. Superior limbs were more often concerned than inferior limbs: 65 vs 29 cases. Bone lesions were found in 34% of cases, articular lesion in 12% and neurological lesions in 40%. Vascular repair was necessary in 63 cases, mostly by suture (41 cases) or venous graft (16 cases). Two patients died because of associated polytrauma. In 8 cases, anatomical and functional results are good, but follow-up is short. In 65 cases, results were initially satisfactory; 5 patients had later complications leading to sequelae in 3 cases. In 19 cases complications occurred and led to amputation in 5 cases; among the remaining 14 patients, results were finally satisfactory in 9 cases and sequelae were observed in 4 cases. Early diagnosis problems and treatment are discussed. Associated lesions have to be taken into account to assess the results.

Abstract

Imaging differential laser absorption spectroscopy was used to map the electron density distribution of vacuum arc cathode spots. The temporal resolution was determined by the sequence of laser pulses (pulses of 0.4-ns duration, with 3-ns separation). The spatial resolution was limited by diffraction (better than 5 /spl mu/m). Fragments of cathode spots have been detected, and the following parameters could be derived: current density on the order of 10/sup 12/ A/m/sup 2/, plasma density greater than 3-6/spl times/10/sup 26/ m/sup -3/, fragment size 5-10 /spl mu/m and smaller, current per fragment 20-40 A (for copper electrodes and are current 50-150 A), and the lifetime of dense plasma on the order of a few nanoseconds.

Abstract

Im Ergebnis von Diffusionsuntersuchungen am System Cu–Ti bei Temperaturen zwischen 750°C und 850°C konnten die intermetallischen Phasen Ti2Cu, TiCu, Ti3Cu4, Ti2Cu3 und Ti2Cu7 mit Hilfe der quantitativen Elektronenstrahl-Mikroanalyse identifiziert werden. Im Zustandsschaubild dieses Systems nach Zwicker war bisher die Phase Ti3Cu4 als instabil ausgewiesen und fur Ti2Cu7 als mogliche Phase noch TiCu3 angegeben worden. Aus dem Gesetz fur das Wachstum der gesamten Diffusionszone sowie fur die einzelnen Phasen (d ∼ √t) folgt, das in diesem System bei Verwendung von Kompaktmaterial Volumendiffusion und nicht die Phasengrenzreaktion geschwindigkeitsbestimmend ist und somit die Diffusionskoeffizienten nach dem Matano-Verfahren bestimmt werden konnen. – Es wurden die chemischen Diffusionskoeffizienten fur die einzelnen Phasen bei 750°C, 800°C und 850°C sowie die partiellen Diffusionskoeffizienten DCu und DTi bei 850°C in der Ti2Cu7-Phase ermittelt. Die chemischen Diffusionskoeffizienten gehorchen Arrhenius-Gleichungen, aus denen die Diffusionsparameter Q (Aktivierungsenergie) und D0 (Frequenzkonstante) fur die identifizierten Phasen berechnet werden konnten. As a result of diffusion studies in the binary system of Cu–Ti within a temperature range of 750°C and 850°C the intermetallic phases Ti2Cu, TiCu, Ti3Cu4, Ti2Cu3 and Ti2Cu7 have been detected by means of electron microprobe analysis. Up to now according to the equilibrium diagram of Zwicker the intermetallic phase Ti3Cu4 was reported to be instable only. By the present investigation a time dependence of the width of the total diffusion zone as well as of each of the phases, d∼ √t, has been verified, i.e. volume diffusion is dominating. The chemical diffusion coefficients of all phases have been measured for 750°C, 800°C and 850°C. The partial diffusion coefficients for Cu and Ti in Ti2Cu7 for 850°C have also been determined. The chemical diffusion coefficients follow the Arrhenius law, the activation energy Q and the frequence constant D0 could be calculated.

Abstract

Lanthanum, cerium, and europium in calcites from carbonate-sulfide banded ores were determined by standard addition using inductively coupled plasma mass spectrometry (ICP-MS). The concentration ranges in solution were for La 700-36 ng/g, Ce 2000-149 ng/g, and for Eu 140-2.4 ng/g. Only Ce was determined by both ICP methods, and the results agree within 3 to 4%. In a second step, a scan over the mass range of the rare earth elements (REE) was performed. The solutions were analyzed directly without applying preconcentration or separation procedures. ICP atomic emission spectrometry (AES) was used to determine Ca, Mn, and Fe. The matrix Ca is present in a concentration range from 1600 to 1300 μg/g and the major impurities Mn and Fe are 152-32 and 100-28 μg/g, respectively. The detection limits of ICP-MS for REE are found to be better by two orders of magnitude than for ICP-AES. A commercially available SCIEX Elan ICP-MS with additional software was used to make mathematical corrections for isobaric interferences and molecular ions.

Abstract

In this article the writer suggests an explanation for the germicide effect of compressed oxygen. The production of poisonous products of metabolism can be ignored, because the bacterial development is normal after removal of the pressure. Lack of nutritive material through inhibition of metabolism and permanent damage to the enzymes is also not a satisfactory explanation. The possibility is discussed that oxygen as an acceptor of electrones creates an ionisation of important parts of the cell and resulting in the death of the bacteria. The effect seems to be similar but weaker than that of H2O2.

Abstract

Most individuals with autism spectrum disorder (ASD)-a complex, life-long developmental disorder-do not have access to the care required to address their diverse health needs. Here, we review: (1) common barriers to healthcare access (shortage/cost of services; physician awareness; stigma); (2) barriers encountered primarily during childhood (limited screening/diagnosis; unclear referral pathways), transition to adulthood (insufficient healthcare transition services; suboptimal physician awareness of healthcare needs) and adulthood (shortage of services/limited insurance; communication difficulties with physicians; limited awareness of healthcare needs of aging adults); and (3) advances in research/program development for better healthcare access. A robust understanding of barriers to accessing healthcare across the lifespan of autistic individuals is critical to ensuring the best use of healthcare resources to improve social, physical, and mental health outcomes. Stakeholders must strengthen healthcare service provision by coming together to: better understand healthcare needs of underserved populations; strengthen medical training on care of autistic individuals; increase public awareness of ASD; promote research into/uptake of tools for ASD screening, diagnosis, and treatment; understand specific healthcare needs of autistic individuals in lower resource countries; and conduct longitudinal studies to understand the lifetime health, social, and economic impacts of ASD and enable the evaluation of novel approaches to increasing healthcare access. IMPACT: Despite the growing body of evidence, our understanding of barriers to healthcare encountered by individuals with ASD remains limited, particularly beyond childhood and in lower resource countries. We describe current and emerging barriers to healthcare access encountered by individuals with ASD across the lifespan. We recommend that stakeholders develop evidence-informed policies, programs, and technologies that address barriers to healthcare access for individuals with ASD and consider broad, equitable implementation to maximize impact., (© 2021. 48. Engendered Expressions of Anxiety: Men's Emotional Communications With Women and Other Men.

Abstract

While the contemporary therapeutic discourse inveigles us to talk about our personal problems, a countervailing neo-liberal healthist discourse, aligning with conventional masculinity norms, presumes that we will manage any issues independently. This discursive tension can be difficult to navigate, especially for men confronted with still powerful traditional expectations around masculinity (e.g., self-reliance; personal control; restricted emotionality). Although qualitative research has examined how men negotiate masculinities with respect to depression, to date there has been scant attention focused on men experiencing anxiety. This article reports on an interview study with men, some with anxiety diagnoses and some without ( N = 17). Thematic analysis highlights that participants can and do talk about their anxieties, most readily with significant women in their lives (e.g., partners; mothers)-although this is not always straightforward. Talking to other men was more fraught, and while participants were wary of sharing problems with male friends, or signaled issues indirectly, they also highlighted situations where they would open up e.g., workspaces where they felt safe; with best friends. Those who had gone through a therapeutic process over many years tended to me more comfortable talking to others, male or female, about their mental health-and were also keen to other support to others where they could. Our analysis suggests that despite stereotypical notions of silent, self-contained men, there are many contexts where men may feel comfortable sharing their stories of pain and suffering. Abstract

The intestinal immune system is an important modulator of glucose homeostasis and obesity-associated insulin resistance. Dietary factors, the intestinal microbiota and their metabolites shape intestinal immunity during obesity. The intestinal immune system in turn affects processes such as intestinal permeability, immune cell trafficking, and intestinal hormone availability, impacting systemic insulin resistance. Understanding these pathways might identify mechanisms underlying treatments for insulin resistance, such as metformin and bariatric surgery, or aid in developing new therapies and vaccination approaches. Here, we highlight evolving concepts centered on intestinal immunity, diet, and the microbiota to provide a working model of obesity-related metabolic disease.

Abstract

Background/objectives: Low-grade chronic inflammation in visceral adipose tissue and the intestines are important drivers of obesity associated insulin resistance. Bioactive compounds derived from plants are an important source of potential novel therapies for the treatment of chronic diseases. In search for new immune based treatments of obesity associated insulin resistance, we screened for tissue relevant anti-inflammatory properties in 20 plant-based extracts., Methods: We screened 20 plant-based extracts to assess for preferential production of IL-10 compared to TNFα, specifically targetting metabolic tissues, including the visceral adipose tissue. We assessed the therapeutic potential of the strongest anti-inflammatory compound, indigo, in the C57BL/6J diet-induced obesity mouse model with supplementation for up to 16 weeks by measuring changes in body weight, glucose and insulin tolerance, and gut barrier function. We also utilized flow cytometry, quantitative PCR, enzyme-linked immunosorbent assay (ELISA), and histology to measure changes to immune cells populations and cytokine profiles in the intestine, visceral adipose tissue (VAT), and liver. 16SrRNA sequencing was performed to examine gut microbial differences induced by indigo supplementation., Results: We identifed indigo, an aryl hydrocarbon receptor (AhR) ligand agonist, as a potent inducer of IL-10 and IL-22, which protects against high-fat diet (HFD)-induced insulin resistance and fatty liver disease in the diet-induced obesity model. Therapeutic actions were mechanistically linked to decreased inflammatory immune cell tone in the intestine, VAT and liver. Specifically, indigo increased Lactobacillus bacteria and elicited IL-22 production in the gut, which improved intestinal barrier permeability and reduced endotoxemia. These changes were associated with increased IL-10 production by immune cells residing in liver and VAT., Conclusions: Indigo is a naturally occurring AhR ligand with anti-inflammatory properties that effectively protects against HFD-induced glucose dysregulation. Compounds derived from indigo or those with similar properties could represent novel therapies for diseases associated with obesity-related metabolic tissue inflammation.
