BackgroundGuidelines advise for more than 20 years to use occlusive plastic wraps for temperature management during delivery room care but data on efficacy of different types of wrap are still scarce.MethodsA random sample of seven different types of plastic wrap was tested using prewarmed aluminium blocks.ResultsThe most effective wrap increased the time to cool by 2°C by one-third for the core and by 100% for the surface whereas the least effective wrap led to even faster heat loss compared with no wrap at all. The least effective wrap concerning all capacities tested was made from polyurethane that contains potentially toxic and narcotic monomers. Heat and water retention did not correlate to wrap thickness.DiscussionLarge differences in heat and water retention capacity warrant a careful choice of the type of wrap as some might be counterproductive. Wraps containing polyurethane should not be used.

Improving the quality of care in this challenging surrounding has an important impact on patient safety and on perinatal morbidity and mortality. Video recording (VR) offers the advantage to record and store work as done rather than work as recalled. It provides information about adherence to algorithms and guidelines, and technical, cognitive and behavioural skills. VR is feasible for education and training, improves team performance and results of research led to changes of international guidelines. However, studies thus far have not provided data regarding whether delivery room video recording affects long-term team performance or clinical outcomes. Privacy is a concern because data can be stored and individuals can be identified. We describe the current state of clinical practice in high- and low-resource settings, discuss ethical and medical-legal issues and give recommendations for implementation with the aim of improving the quality of care and outcome of vulnerable babies. IMPACT: VR improves performance by health caregivers providing neonatal resuscitation, teaching and research related to delivery room management, both in high as well low resource settings. VR enables information about adherence to guidelines, technical, behavioural and communication skills within the resuscitation team. VR has ethical and medical-legal implications for healthcare, especially recommendations for implementation of VR in routine clinical care in the delivery room. VR will increase the awareness that short- and long-term outcomes of babies depend on the quality of care in the delivery room.

Improving the quality of care in this challenging surrounding has an important impact on patient safety and on perinatal morbidity and mortality. Video recording (VR) offers the advantage to record and store work as done rather than work as recalled. It provides information about adherence to algorithms and guidelines, and technical, cognitive and behavioural skills. VR is feasible for education and training, improves team performance and results of research led to changes of international guidelines. However, studies thus far have not provided data regarding whether delivery room video recording affects long-term team performance or clinical outcomes. Privacy is a concern because data can be stored and individuals can be identified. Objective To assess delivery room management of infants born preterm at 4 Level III perinatal centers in 3 European countries.Study design This was a prospective, multicenter observational study. Management at birth was video-recorded and evaluated (Interact version 9.6.1; Mangold-International, Arnstorf, Germany). Data were analyzed and compared within and between centers.Results The infants (n = 138) differed significantly with respect to the median (25%, 75%) birth weight (grams) (Center A: 1200 [700, 1550]; Center B: 990 [719, 1240]; Center C: 1174 [835, 1435]; Center D: 1323 [971, 1515] [B vs A, C, D: P < .05]), gestational week (Center A: 28.4 [26.3, 30.0]; Center B: 27.9 [26.7, 29.6]; Center C: 29.3 [26.4, 31.0]; Center D: 30.3 [28.0, 31.9]), Apgar scores, rates of cesarean delivery, and time spent in the delivery room. Management differed significantly for frequency and drying time, rates of electrocardiographic monitoring, suctioning or stimulation, and for fundamental interventions such as time for achieving a reliable peripheral oxygen saturation signal (seconds) (Center A: 97.6 +/- 79.3; Center B: 65.1 +/- 116.2; Center C: 97.1 +/- 67.0; Center D: 114.4 +/- 140.5; B vs A, C, D: P < .001) and time for intubation (seconds) (Center A: 48.7 +/- 4.2; Center B: 49.0 +/- 30.7; Center C: 69.1 +/- 37.9; Center D: 65.1 +/- 23.8; B vs D, P < .025). Mean procedural times did not meet guideline recommendations. The sequence of interventions was similar at all centers.Conclusions The Video Apgar Study showed great variability in and between 4 neonatal centers in Europe. The study also showed it is difficult to adhere to published guidelines for recommended times for important, basic measures such as peripheral oxygen saturation measurements and intubation.

Background Head shape and head volume of preterm infants give important information on short- and long-term development. Three-dimensional (3D) assessment of a preterm infant's head would therefore provide more information than currently used two-dimensional methods. Aims To evaluate a contactless 3D imaging system to assess head shape and volume in preterm infants. Methods A protocol for 3D imaging and reconstruction of an infant's head with a portable stereophotogrammetric camera system was developed. It was validated on a manikin by comparison to an established stationary stereophotogrammetric device. Feasibility for clinical routine and 3D data analysis were assessed in six preterm infants. Results Ten 3D reconstructions from a manikin were done with ten images each taken from different angles. The accuracy of the 3D reconstruction was measured at the overlapping areas between two images. Comparing the portable to the stationary system, a high concordance was found for the 3D manikin head-reconstructions (mean difference 0.21 ± 0.03 mm). In preterm infants, digital evaluation of the head was proven to be feasible for head circumference (HC), cranial index and asymmetry indices. There was good concordance between manual and digital measurement of the HC (95% CI −0.85 to 0.38 mm). Conclusions The portable camera system allowed fast and contactless 3D image capture of a preterm infant's head without any risk or interference with neonatal care. Together with a new software, this technique would allow more precise evaluation of head growth even in very preterm infants and thereby may improve their care and long-term outcome.

Isolation precautions required for neonatal intensive care units are part of a bundle with the aim to prevent transmission, colonization, and infection with multidrug-resistant gram-negative pathogens as neonates face an increased risk of mortality and morbidity in case of infection. The following short report describes a transmission of 3MDRGN Klebsiella pneumoniae on a neonatal intensive care unit in a university hospital in Germany. This transmission occurred even though intensified infection control measures were in place, which impressively shows the importance of surveillance, outbreak management, and awareness of contributing factors regarding outbreak situations.

Introduction: Evidence concerning delivery room management in extremely low birth weight infants (ELBW) has grown substantially within the last 20 years, leading to several guidelines and recommendations. However, it is unknown in which extent local treatment strategies have changed and if they reflect current recommendations. Methods: A detailed questionnaire about treatment strategies for ELBW infants was sent to all German neonatal intensive care units (NICUs) treating ELBW infants in 1997. A follow-up survey was conducted in 2011 and sent to all NICUs in Germany, Austria and Switzerland. Results on delivery room management were compared to the first survey. Results: In 1997 and 2011, 63.6 and 66.2% of the approached hospitals responded. In 2011 similar results were observed between university and non-university hospitals as well as NICUs of different size. Differences between Germany, Austria and Switzerland were minimal. Changes over time were a lower initially applied fraction of inspired oxygen (FiO2) and peak inspiratory pressure (PiP) in 2011 compared to 1997. A longer time of apnea was tolerated before tracheal intubation is performed; the time of apnea was less frequently a sole criterion for intubation and surfactant was applied at lower FiO2 in 2011. The time of no thorax excursions and transport of the infant were considered an indication for intubation in 30.2 and 22.5%, and did not change in the observation period. Conclusion: Treatment strategies for delivery room management in ELBW infants changed significantly between 1997 and 2011 and largely reflect current recommendations.

The results of a measurement of the proton structure function F_2(x,Q^2)and the virtual photon-proton cross section are reported for momentum transfers squared Q^2 between 0.35 GeV^2 and 3.5 GeV^2 and for Bjorken-x values down to 6 10^{-6} using data collected by the HERA experiment H1 in 1995. The data represent an increase in kinematic reach to lower x and Q^2 values of about a factor of 5 compared to previous H1 measurements. Including measurements from fixed target experiments the rise of F_2 with decreasing x is found to be less steep for the lowest Q^2 values measured. This report discusses the physiological aspects of neonatal transition from breathing liquid to air. Further, we discuss reasonable medical interventions to actively assist a gentle transition, and focus on team aspects of preparing both the perinatal team and parents for the challenging situation of preterm labour and delivery. Our aim is to critically evaluate current concepts on the physiology of neonatal transition and the current assessment of the newborn infant, to present means to facilitate non-traumatic pulmonary aeration and ways to foster successful teamwork and professional parental guidance in the delivery room. The authors report on their own work and on that of other research groups, as recently published in peer reviewed medical journals. When born, the newborn needs to rapidly clear his/her lungs from fluid to establish breathing. Active fluid transport and passive resorption help to establish the pulmonary functional residual capacity (FRC). Prenatal administration of corticosteroids helps to form and maintain the FRC of the newborn. Many very low gestational age neonates (ELGAN) will breathe at birth but require medical assistance. This is best done by giving distending positive airway pressure at levels of 5 cmH(2)O, or greater. Monitoring of these infants should be by peripheral pulse oximetry. Some ELGANs may require non-invasive ventilation and/or exogenous Surfactant replacement, and even fewer may require intubation and mechanical ventilation. The obstetric and neonatal teams need to coordinate their joined efforts to secure a safe delivery for mother and child. Ways of communication between teams and parents are presented. Many neonatal teams use video recording as a tool to assess and improve their work. We give insights into the use of video as a means to improve teamwork and patient care alike.

The increasing rates of preterm birth among twins implicate that solid data on associated risks and outcomes are required. Assessment of zygosity is often based on clinical criteria (evaluation of placenta; same gender, birth weight discordance as surrogate criteria for monochorionic/monozygotic twins). The aim of this study was to compare clinical versus genetic assessment of zygosity and to compare causes of preterm delivery as well as outcome data of very-low-birth-weight (VLBW; birth weight n = 176 sets of same gender twins and determined zygosity genetically. In a subgroup of 123 sets of twins, the attending physicians at the study centers were asked to document the parameter ‘zygosity’ (monozygotic/dizygotic) on the basis of their clinical judgment. Concordance between genetic and clinical assessment was 62.7% for monozygotic twins and 88.9% for dizygotic twins, respectively. Outcome parameters (death, BPD, ROP, NEC, IVH) were comparable in both groups. Genetically dizygotic twins were significantly more often born due to intrauterine infection (33% vs. 20% in monozygotic twins, p < .01) and antenatal antibiotics were more frequently given to mothers of dizygotic twins (62% vs. 47% in monozygotic twins, p < .01). Obstetric complications such as twin-twin-transfusion-syndrome were only seen in monozygotic twins as expected. The unexpected increase of antenatal antibiotic treatment and birth due to intrauterine infection in dizygotic twins should be confirmed in additional VLBW twin-cohorts.

Zusammenfassung Verschiedene Operationstechniken sind zur Korrektur des flexiblen kindlichen und adoleszenten Knicksenkfusses beschrieben. Sinus tarsi Implantate und die subtalare Schraubenarthrorise gewinnen zunehmend an Bedeutung. Sie sind minimalinvasiv anwendbar und berucksichtigen die individuelle Anatomie: sei es durch die verschiedenen Implantatgrosen, sei es durch die angepasste Schraubenblockierung des Ruckfusvalgus. In eine retrospektiven Untersuchung gingen die klinischen und radiologischen Daten von 20 Fusen bei 10 Patienten ein, die mit einem Sinus tarsi Implantat seit 2006 versorgt wurden. Der praoperative AOFAS Ruckfusscore besserte sich bei den 18 mit dem Kalix® Impalantat versorgten Fusen von 70 Punkten praoperativ auf 96 Punkte postoperativ. Radiologisch besserte sich der TMT I Index von durchschnittlich −36 auf −8. Der Winkel nach Costa-Bartani und der Winkel nach Kite, sowie der Talusneigungswinkel und der Calcaneusneigungswinkel erhielten durchschnittlich eine Korrektur hin zu Normwerten.

Objective: The Apgar score should be an objective method to assess the state of newborns; however, its applicability in preterm infants is hampered by large variations among different observers. The study tested whether physicians that give low scores to written case descriptions also apply lower scores to preterm infants. Patients and Methods: Descriptions (BMJ 2004; 329: 143–4) were sent to 14 neonatal units. Physicians were asked to evaluate the Apgar (case score). From seven units Apgar scores of all very low birth weight infants (VLBW) born between January 2004 and December 2006 were obtained from charts (clinical score). Results: In total, 121 physicians from 14 institutions (median 9, range 3–15) replied: 24 residents with 6-month neonatal experience, and 69 consultants. The assessment of the case scores was very heterogeneous with large variations in respiration, muscle tone and reflexes. Clinical scores were obtained from 1000 VLBW infants. The score depended on the gestational age, with a median of 4 at 24 and 7 at 27 weeks. With one exception, centres that assigned low case scores had also low clinical scores. Conclusion: There is considerable variation in assigning Apgar scores. Definitions are required to apply the Apgar score to infants under clinical conditions such as preterm delivery, resuscitation or artificial ventilation.

Zusammenfassung Gegenstand und Ziel: Nach einer Übersicht über Dermatophytosen beim Pferd wird über die Ergebnisse von zwei Studien zur therapeutischen Wirksamkeit und Verträglichkeit von HippoTrichon®, einer Lebendvakzine gegen Trichophyton equinum beim Pferd, unter Feldbedingungen berichtet. Material und Methoden: In einer Wirksamkeitsstudie wurden 13 Pferde, die unter Dermatophytose litten und zum Teil vorbehandelt waren, zweimal im Abstand von 14 Tagen mit einer Injektion von je 5 ml HippoTrichon® behandelt. In einer zweiten Studie zur Unschädlichkeit erfolgte bei 29 Pferden im Abstand von 14 Tagen eine prophylaktische Immunisierung mit je 5 ml HippoTrichon®. In beiden Gruppen wurden die Pferde am Tag vor der Immunisierung und an den nachfolgenden 10 Tagen klinisch untersucht. Ergebnisse: Bei 7 von 13 Pferden waren die dermatomykosetypischen Hautläsionen 10 Tage nach der zweiten Impfung vollständig abgeheilt. Eine deutliche Abheilungstendenz war bei weiteren 5 Pferden zu erkennen. Bei einem Tier bildeten sich nach zunächst deutlicher Abheilungstendenz neue Hautläsionen. Insgesamt 40 von 42 Pferden vertrugen die Injektion von HippoTrichon ® gut. Schmerzhafte Schwellungen bei 2 Pferden waren innerhalb von 4 Tagen abgeklungen. Schlussfolgerung: HippoTrichon® eignet sich aufgrund guter Verträglichkeit sowohl zur Prophylaxe als auch zur Therapie von Trichophytosen beim Pferd. Ein Behandlungserfolg bei Trichophytie ist aufgrund der guten Wirksamkeit nicht nur bei Erstbehandlungen, sondern auch bei vorbehandelten und chronischen Fällen zu beobachten.

Das Cinderella-Schuh-Syndrom beschreibt ein haufig beobachtetes, bisher nicht als Entitat aufgefuhrtes Missverhaltnis von Patientenanspruch bezuglich der Schuhwahl und seinen morphologischen Gegebenheiten. Dysproportionierte Schuhe und Strumpfe werden aus personlichen Grunden, die oft unbewusst bleiben, aufgrund von Moderichtungen, Fehlinformation und falscher Beratung getragen und konnen Fuserkrankungen induzieren. Im Rahmen der Primarentstehung, der Behandlung und der Rezidivprophylaxe von Fuserkrankungen, sowie bei der Beurteilung von Rezidivursachen nach korrekt ausgefuhrten Fusoperationen hat das Cinderella-Schuh-Syndrom einen wichtigen Stellenwert. Es zu erkennen, individuell mentale Ursachen wirksam zu therapieren, passende Fusbekleidung zu beurteilen und uber gesellschaftliche Ursachen aufzuklaren ist Aufgabe des fuschirurgisch tatigen Arztes.

BACKGROUND. Recent investigations have reported an influence of thrombophilic mutations and antithrombotic risk factors with development of intraventricular hemorrhage. It was our objective for this study to investigate the impact of genetic polymorphisms of hemostasis genes on the primary outcome measures of sepsis, bronchopulmonary dysplasia, intraventricular hemorrhage, and periventricular leukomalacia in a large cohort of very low birth weight infants. METHODS. There were 586 very low birth weight infants enrolled prospectively in a multicenter trial between September 2003 and July 2005, and an additional 595 very low birth weight infants, who had been recruited in a previous prospective trial, were studied. DNA samples were taken by buccal swab, and genotypes of factor V Leiden mutation, prothrombin G20210A mutation, the factor VII-323 del/ins polymorphism, and the factor XIII-Val34Leu polymorphisms were determined by polymerase chain reaction and restriction enzyme digestion. RESULTS. In contrast to data published previously, the frequency of intraventricular hemorrhage or periventricular leukomalacia was not significantly influenced by any of the genetic variants tested. Carriers of the factor XIII-Val34Leu polymorphism, however, had a higher sepsis rate and a longer period of hospital care compared with noncarriers. The factor VII-323 del/ins polymorphism was found to be a potential protective factor against bronchopulmonary dysplasia. CONCLUSIONS. We could not confirm previously reported associations of hemostasis gene variants and development of intraventricular hemorrhage in very low birth weight infants. To better understand gene-disease associations in very low birth weight infants, the prospective development of large-scale cohorts with well-defined phenotypes and corresponding DNA samples is essential.

20. Bilirubin Measurement for Neonates: Comparison of 9 Frequently Used Methods

OBJECTIVE. High blood concentrations of bilirubin are toxic to the brain and may cause kernicterus. Therefore, determination of bilirubin levels is performed for many newborns, and several different methods are available. We compared 9 frequently used methods for bilirubin determination among newborns under routine conditions, to define their sequence of use. METHODS. In a prospective study, bilirubin concentrations were determined with 9 different methods, ie, 3 skin test devices, 3 nonchemical photometric devices (including 2 blood gas analyzers), and 3 laboratory analyzers. RESULTS. A total of 124 samples were obtained. All 3 laboratory methods showed very strong correlations with each other, and their means were used as comparison values. To these comparison values, the skin test devices had correlation coefficients between 0.961 and 0.966, and the nonchemical photometric devices between 0.980 and 0.994. Bland-Altman plots demonstrated good agreement with the comparison values for all nonchemical photometric devices. All skin test devices and 1 nonchemical photometric device underestimated bilirubin levels, particularly at high concentrations. CONCLUSIONS. In the routine care of newborns, the first method for bilirubin testing should be a skin test. If the skin test result exceeds 200 μmol/L and other analytes are to be determined with a nonchemical photometric device, then bilirubin can be included in this analysis and the result trusted up to 250 μmol/L. If the skin test result exceeds 200 μmol/L and only bilirubin concentrations are needed, then a standard laboratory method is the first choice, to avoid repeated blood sampling. Bilirubin concentrations from nonchemical photometric devices that exceed 250 μmol/L should be confirmed with standard laboratory methods.

We investigated the association between the interleukin 6 (IL-6)-174-genotype and unfavorable outcomes in preterm infants since it has been reported that the IL-6-174GG-genotype is associated with increased susceptibility to sepsis, and the IL-6-174CC-genotype is more common in preterm infants with severe intraventricular hemorrhage (IVH). We studied 1206 preterm infants with a birth weight below 1500 g. In contrast to previously published data, the frequency of IVH grade IV, periventricular leukomalacia, ventricular-peritoneal-shunting or death was not different between infants with different IL-6-genotypes: IL-6-174GG (n = 430) 8%, IL-6-174GC (n = 605) 9% and IL-6-174CC (n = 167) 12% (P = 0.2 for IL-6-174CC vs GG + GC). Furthermore, we were not able to confirm previously reported association between sepsis and the IL-6-174GG-genotype. Blood-culture-proven sepsis occurred in 19% of IL-6-174GG-carriers (n = 157), 26% of IL-6-174GC-carriers (n = 193) and 27% of infants carrying the IL-6-174CC-genotype (n = 67). We were not able to confirm previously reported associations between sepsis, cerebral injury and the IL-6-174-genotype in VLBW-infants.

Background: Basophils of some individuals do not release histamine upon activation of their high-affinity immunoglobulin E (IgE) receptor (FcRI), but do so if this receptor is circumvented for cell activation. This so-called nonresponder phenomenon is clinically relevant, because in various studies atopy was less frequent or absent in nonresponder individuals. So far, it is unknown if this phenomenon is acquired during adulthood or exists from birth on. Methods: Histamine release was determined from isolated leucocytes stimulated with anti-IgE or calciumionophor. Also, random primed cDNA was synthesized and the open reading frame (ORF) of the FcRI β-subunit amplified and sequenced. Results: In the first part of our study, we examined the role of atopic status, type of atopy, and age in a random population of 95 children of whom we found 22% to be nonresponder. None of these parameters correlated with the nonresponder status. Except for food allergy, no specific type of atopy correlated with histamine release. The mechanism underlying the nonresponder phenomenon is assumed to occur early in the signalling cascade. We hypothesized that mutations in the FcRI β-chain may be associated with the nonresponder status, and in the second part of our study sequenced the β-subunit in 20 responders and 20 nonresponders. Two conservative and two nonconservative heterozygous one base mutations (Thr179Thr, Asp216Asp, Ile147Leu and Glu237Gly) were found in two nonresponders and one responder. Three of these mutations have not been described so far. Conclusion: The nonresponder phenomenon is present from birth on and genetically determined. In our population, it was not associated with age or the presence of atopy, and appeared not to be caused by mutations in the FcRI β-chain.

To prevent persistent neurodevelopment and physical growth deficits in neonatal care, it is mandatory to determine blood glucose levels as quickly and precisely as possible, preferably using micro-methods. However, most commercially available instruments have not been validated and approved for this purpose. The aim of this study was to validate the Glucometer Elite XL, a newly developed device for point-of-care testing (POCT). In samples from 869 newborn infants, glucose levels were simultaneously measured by the Glucometer Elite XL in whole blood and by an accepted clinical laboratory method in haemolysed blood using the ECA 2000 device. An acceptable method agreement was found between the POCT and the ECA 2000 method (mean difference 0.013 mmol/l, SD 0.69). Anaemia of prematurity (AOP) is a common sequela of preterm birth.1 It remains largely unknown how individual neonatal intensive care units (NICUs) manage AOP and whether treatment strategies have changed over time. We present the results of a standardised multicentre survey to assess the alterations in the treatment of AOP in NICUs caring for extremely low birthweight (ELBW) infants comparing Germany in 1997 (n=125 NICUs) to German-speaking countries in 2011 (n=129): Germany (n=110), Austria (n=10) and Switzerland (n=9).2 Preparation strategies for red blood cells (RBCs), transfusion volume, and rate and protocols for erythropoietin administration were analysed. Sind Civinini als anatomischer Erstbeschreiber der Neuropathie und Durlacher mit seiner Publikation des klinischen Bildes der Neuralgie wegweisend in der Erforschung dieser Vorfuserkrankung gewesen, so wurde die Benennung des Krankheitsbildes T. G. Morton zugedacht. Eine kurze Ubersicht soll das historische Umfeld charakterisieren.

Die Erstbeschreibung der intermetatarsalen Neuralgie mit dem Aspekt einer Neurombildung wird historisch und klinisch begrundet den Herren Civinini und Durlacher zugerechnet. Definitionsgemas versteht man bisher unter der „Morton“ Metatarsalgie eine gutartige Vergroserung des plantaren Nerven distal der Metatarsalkopfchen, alternativ kommt beim Entrapment eine Neuropathie mit sanduhrformiger Einengung des Nerven in Betracht. Die klinischen Daten von 71 intermetatarsalen Neuropathien bei 53 Patienten mit sich anschliesenden Operationen, einschlieslich der histologischen und radiologischen Befunde aus dem Sankt Elisabeth-Hospital in Gutersloh werden evaluiert. Hierbei bestatigt sich die mogliche Differenzierung in Verdickung und Ausdunnung des perineuralen Gewebes der resezierten Nerven. Eine radiologisch signifikante Unterscheidung ergab die Messung der relativen Metatarsalekopfchenbreite, je breiter die Kopfchen, um so eher korrelierte der Befund mit einer Taillierung des Gesamtdurchmessers. Die differenzierte klinische Untersuchung bleibt die Basis der Diagnosestellung bei der Civinini-Durlacher Neuropathie oder dem Entrapment des Plantarnerven. Die Resektion ist nach erfolgloser konservativer und schuhtechnisch adaquater Behandlung die Therapie der Wahl.

A measurement of the inclusive cross section for the deep-inelastic scattering of positrons off protons at HERA is presented at momentum transfers $8.5 \leq Q^2 \leq 35 GeV^2$ and large inelasticity $y = 0.7$, i.e. for the Bjorken-x range $0.00013 \leq x \leq 0.00055$. Using a next-to-leading order QCD fit to the structure function F_2 at lower y values, the contribution of F_2 to the measured cross section at high y is calculated and, by subtraction, the longitudinal structure function F_{L} is determined for the first time with an average value of $F_L=0.52 \pm 0.03 (stat)$^ {+0.25}_{-0.22}$ (syst) at $Q^2=15.4 GeV^2$ and $x=0.000243$., 17 pages, latex, 4 Figures

AIM To compare treatment strategies for respiratory failure in extremely low birth weight (ELBW) infants in Germany in 1997 to Germany, Austria and Switzerland in 2011. METHODS A detailed questionnaire about treatment strategies for ELBW infants was sent to all German centres treating ELBW infants in 1997. A follow-up survey was conducted in 2011 in Germany, Austria and Switzerland. RESULTS In 1997 and 2011, 63.6% and 66.2% of the hospitals responded. In 2011 the response rate was higher in Switzerland than in Germany, and in university hospitals versus non-university hospitals. Treatment strategies did not differ between university and non-university hospitals as well as NICUs of different sizes in 2011. Differences between Germany, Austria and Switzerland were minimal. At the electron-proton collider HERA the inclusive D-*+/- meson photoproduction cross section has been measured with the H1 detector in two different, but partly overlapping, kinematical regions. For the first, where [W-gamma p] approximate to 200 GeV and Q(2) < 0.01 GeV2, the result is sigma(gamma p --> c(c) over barX$) = (13.2 +/- 2.2(-1.7-4.8)(+2.1+9.9)) mu b. The second measurement for Q(2) < 4 GeV2 yields sigma(gamma p) --> c(c) over barX$) = (9.3 +/- 2.1(-1.8-3.2)(+1.9+6.9)) mu b at [W-gamma p] approximate to 142 GeV and sigma(gamma p --> c(c) over barX$) = (20.6 +/- 5.5(-3.9-7.2)(+4.3+15.4)) mu b at [W-gamma p] approximate to 230 GeV, respectively. The third error accounts for an additional uncertainty due to the proton and photon parton density parametrizations. Differential cross sections are presented as a function of the D-*+/- transverse momentum and rapidity. The results compare reasonably well with next-to-leading order QCD calculations. Evidence for diffractive photoproduction of charm quarks is presented.

The total cross sections for the elastic electroproduction of $\rh0$ and $J/\Psi$ mesons for $Q^2$ $>$ 8 GeV$^2$ and $ \simeq 90$ GeV/c$^2$ are measured at HERA with the H1 detector. The measurements are for an integrated electron$-$proton luminosity of $\simeq$~3~pb$^{-1}$. The dependences of the total virtual photon$-$proton ($\gamma^* p$) cross sections on $Q^2$, $W$ and the momentum transfer squared to the proton ($t$), and, for the $\rho$, the dependence on the polar decay angle ($\cos \theta^*$), are presented. The $J/\Psi$ : $\rh0$ cross section ratio is determined. The results are discussed in the light of theoretical models and of the interplay of hard and soft physics processes.

We present a new measurement of the total photoproduction cross section performed with the H1 detector at HERA. For an average centre of mass energy of 200 GeV a value of σtotλp=165±2±11µb has been obtained. A detailed analysis of the data in adequate kinematic regions enabled a decomposition of the total cross section in its elastic, single diffractive dissociation and remaining non-diffractive parts, based on safe assumptions on the double diffractive dissociation contribution.

A search for direct production of new leptons in the mass range from 10 GeV up to 225 GeV is presented by the H1 experiment at HERA. The data were obtained during 1993 and correspond to an integrated luminosity of 528 nb −1 . The search includes heavy lepton decays to final states e ( ν ) γ and e ( ν ) W , e ( ν ) Z with the subsequent decay of the W and Z bosons into jets or lepton pairs. No evidence was found for the production of new massive electrons or neutrinos in any of the decay channels. Rejection limits for excited electrons and neutrinos are derived.

Cross sections are presented for the inclusive production of charged particles measured in electron-proton collisions at low Q 2 with the H1 detector at HERA. The transverse momentum distribution extends up to 8 GeV/ c . Its shape is found to be harder than that observed in p p collisions at comparable centre-of-mass energies √S γp ≈ √S p p ≈ 200 GeV , and also harder than in γp collisions at lower energies √ S γp ≈ 18 GeV. Results from quantum chromodynamics (QCD) calculations agree with the measured transverse momentum and pseudorapidity cross sections.

The performances of the H1 liquid argon calorimeter modules for the energy measurements and identification of electrons are studied with test data taken at CERN in the energy range 5 to 166 GeV. Various electron identification estimators exploiting global or detailed shower characteristics are studied and compared. The usage of impact position measurements is also discussed. A best combination of robust shower estimators leads typically to π-misidentification probabilities in the range 1.5 to 5.0 × 10−3 at 30 GeV for 95% electron detection efficiency. This further reduces to ∼ 10−4 for these pions to be misidentified as electrons below 25 GeV.

An analysis is presented of scaling violations of the proton structure function F 2 ( x , Q 2 ) measured with the H1 detector at HERA in the range of Bjorken x values between x = 3 × 10 −4 and 10 −2 for four-momentum transfers Q > 2 larger than 8.7 GeV 2 . The structure function F 2 ( x , Q 2 ) is observed to rise linearly with ln Q 2 . Under the assumption that the observed scaling violations at small x ⩽ 0.01 are described correctly by perturbative QCD, an estimate is obtained of the gluon distribution function G ( x , Q 0 2 ) at Q 2 2 = 20 GeV 2 .

The liquid argon calorimeter of the H 1 detector presently taking data at the HERA ep collider at DESY, Hamburg, is described. The main physics requirements and the most salient design features relevant to this calorimeter are given. The aim to have smooth and hermetic calorimetric coverage over the polar angular range 4° ≤ θ ≤ 154° is achieved by a single liquid argon cryostat containing calorimeter stacks structured in wheels and octants for easy handling. The absorber materials used are lead in the electromagnetic part and stainless steel in the hadronic part. The read-out system is pipelined to reduce the dead time induced by the high trigger rate expected at the HERA collider where consecutive bunches are separated in time by 96 ns. The main elements of the calorimeter, such as the cryostat, with its associated cryogenics, the stack modules, the read-out, calibration and trigger electronics as well as the data acquisition system are described. Performance results from data taken in calibration runs with full size H 1 calorimeter stacks at a CERN test beam, as well as results from data collected with the complete H 1 detector using cosmic rays during the initial phase of ep operations are presented. The observed energy resolutions and linearities are well in agreement with the requirements.

A measurement of the proton structure function F2 (x, Q2) is presented with about 1000 neutral current deep inelastic scattering events for Bjorken x in the range x ⋍ 10−2 – 10−4 and Q2 > 5 GeV2. The measurement is based on an integrated luminosity of 22.5 nb−1 recorded by the H1 detector in the first year of HERA operation. The structure function F2 (x, Q2) shows a significant rise with decreasing x.

Inclusive jet cross sections are measured in photoproduction at HERA using the H1 detector. The data sample of e+ p -> e+ + jet + X events in the kinematic range of photon virtualities Q^2 < 1 GeV^2 and photon-proton centre-of-mass energies 95 < W_gammap < 285 GeV represents an integrated luminosity of 24.1 pb^-1. Jets are defined using the inclusive k_T algorithm. Single- and multi-differential cross sections are measured as functions of jet transverse energy E_T^jet and pseudorapidity ��^jet in the domain 5 < E_T^jet < 75 GeV and -1 < ��^jet < 2.5. The cross sections are found to be in good agreement with next-to-leading order perturbative QCD calculations corrected for fragmentation and underlying event effects. The cross section differential in E_T^jet, which varies by six orders of magnitude over the measured range, is compared with similar distributions from p pbar colliders at equal and higher energies., 34 pages, 9 figures and 7 tables

A direct search for new particles in the H1 experiment at HERA is presented for masses ranging from 35 GeV up to ∼ 250 GeV. The data sample of 24 nb −1 accumulated during the first year of operation was analysed for signatures of scalar and vector leptoquarks, leptogluons, excited electrons and excited neutrinos with flavours of the first generation. No evidence for the production of such particles was found in various possible decay channels. Rejection limits are derived.

Measurements of the scattered electron energy spectrum and the differential cross sections dσ/d log(x) and dσ/dQ2 for inclusive neutral current deep inelastic electron-proton scattering are presented. The data were obtained with the H1 detector at HERA during its first running period in which 26.7 GeV electrons collided with 820 GeV protons. The data correspond to an integrated luminosity of 1.3 nb−1 and allow the first studies of the structure of the proton at values of x down to 10−4 for Q2 > 5 GeV2.

We present first results on the total photoproduction cross section measurement with the H1 detector at HERA. The data were extracted from low Q2 collisions of 26.7 GeV electrons with 820 GeV protons. The γp total cross section has been measured by two independent methods in the γp center of mass energy range from 90 to 290 GeV. For an average center of mass energy of 195 GeV a value of σtot (γp) = 159 ± 7 (stat.) ± 20 (syst.) μb was obtained.
