Giriş ve Amaç: Preeklampsi; hipertansiyon, proteinüri ve/veya organ hasarı ile seyreden gebeliğe özgü bir hastalıktır. Bu çalışmada, gebelikte rutin takiplerin yapıldığı birinci basamakta bazı hematolojik parametrelere göre preeklampsinin öngörülebilir olup olmadığının araştırılması amaçlanmıştır.Gereç ve Yöntemler: Bu retrospektif çalışmaya 58 preeklampsili gebe ve 116 sağlıklı gebe dahil edildi. Preeklampsi grubunda tanı anına ve tanıdan 2-6 hafta öncesine ait iki farklı hemogram sonucu; kontrol grubu için doğum zamanı ve doğumdan 2-6 hafta öncesine ait iki farklı hemogram sonucu çalışmaya dahil edildi.Bulgular: Preeklampsi grubunun ikinci hemogram sonuçlarında trombosit sayısı, NLR ve PLR düzeyleri ilk hemogram sonuçlarına göre daha düşük, MPV düzeyleri daha yüksek olarak bulundu. ROC analizi sonucu, MPV’nin 8.95 (fL) kabul edildiği cut-off değerinde preeklampsiyi öngörmedeki duyarlılığı %75,9 ve özgüllüğü %33,3 olarak saptandı.Sonuç: Çalışmamızın sonuçlarını literatürle karşılaştırdığımızda MPV, NLR ve PLR verilerinin preeklampsi tanılı hastaları öngörmede kullanılabilir olduğu değerlendirildi. MPV’de yükselme, NLR ve PLR’de düşme eğiliminin preeklampsiyi öngörmede kullanılabilmesi için daha fazla hasta sayılı ve uzun süreli çalışmalara ihtiyaç duyulmaktadır., Objective: Preeclampsia is a pregnancy-specific disease with hypertension, proteinuria, and/or organ damage. This study aimed to investigate whether preeclampsia is predictable or not based on some hematological parameters at primary care units step where routine follow-ups are made during the pregnancy.Materials and methods: In this retrospective study, 58 pregnant women with preeclampsia and 116 healthy pregnant women were included. Two different hemogram results belonging to the time of diagnosis and 2-6 weeks before diagnosis in the preeclampsia group; and two different hemogram results belonging to the time of delivery and 2-6 weeks before delivery for the control group were included in the study. Results: It was determined that MPV levels were higher; platelet count, NLR, and PLR levels were lower in the second hemogram results compared with the first hemogram results of the preeclampsia group. The ROC analysis result, it was revealed that the sensitivity and specificity of MPV for predicting preeclampsia were 75,9% and 33,3%, respectively, when the cut-off value of MPV was accepted as 8,95 (fL).Conclusion: When we compared the results of our study with the literature, it was evaluated that MPV, NLR, and PLR data could be used for the prediction of preeclampsia patients. It has been required for a long duration of studies that need a large number of patients to use the tendency in an increase of MPV and the decrease of NLR and PLR for prediction of preeclampsia.

Introduction: Chronic kidney disease and frailty are two crucial clinical conditions increasing in prevalence globally. Both lead to severe complications that increase mortality and morbidity in patients. Conditions that may potentiate frailty in chronic kidney disease patients may complicate the follow-up of chronic disease and complicate long-term survival in this patient group. In this study, we aimed to evaluate frailty and related factors in chronic kidney disease patients over 65 years of age who were on dialysis and who were not. Materials and Methods: This cross-sectional study was carried out in geriatric chronic kidney disease patients followed in nephrology outpatient clinics or undergoing routine hemodialysis. Frailty was assessed using a scoring scale. Laboratory findings and their relationship with demographic and epidemiological data were investigated. Results: One hundred eighty-eight patients aged 65 and over were included in our study. Of the patients, 92 were female, and 96 were male. The mean age was 72.86 years. We found frailty in 82 patients (43.6%). Female gender, over 75 years old, under dialysis treatment, low-income status, and low education level were parameters significantly associated with frailty. In the regression analysis, we found that economic status and dialysis were variables that independently affected frailty in chronic kidney disease patients. Conclusions: In our study, the frequency of frailty was found to be high. Practical management and early assessment of frailty seem rational with the basic nephrological approach in patients with chronic kidney disease. Considering the high mortality rate among frail patients, we think these patients should be followed up more closely. Keywords: Frailty; Aged; Renal Insufficiency, Chronic.

Purpose Spirituality contributes to the health and well-being of individuals. This study investigates the factors that predict the spirituality perceptions and the effect of death anxiety of nurses. Design and methods This was a cross-sectional study, 382 nurses working in four public hospitals intensive care and emergency departments setting in two different cities in Turkey. Findings Variables predicting the total and subscale scores of the Spirituality and Spiritual Care Rating Scale of nurses were determined by linear multiple regression analysis. Except for the religiousness subscale, the satisfaction level of nurses was found to be a significant predictor in total of Spirituality and Spiritual Care Scale. Practice implications Nurses working in high-risk units are unclear about their perception of spirituality and spiritual care.

Giriş ve Amaç: Aile hekimliği uygulaması bireylere ve ailelerine sürekli ve çok yönlü sağlık hizmeti sunmayı amaçlamaktadır. Bazı yaşlı bireyler için huzurevleri barınma ihtiyacı için bir seçenek olarak gündemdeki yerini korumaktadır. Bu araştırmada Manisa merkez ilçelerinde bulunan huzurevi sakinlerinin, ülkemizdeki aile hekimliği uygulaması hakkındaki farkındalıklarının ve bu uygulamadan ne kadar hizmet alabildiklerinin saptanması amaçlanmıştır.Gereç ve Yöntemler: Araştırma tanımlayıcı kesitsel tiptedir. Araştırma Manisa ili merkez ilçelerinde iki farklı huzurevinde yaşamakta olan sakinler arasında yapılmıştır. Çalışmaya 81 birey dahil edilmiştir. Demografik bilgiler ile katılımcıların aile hekimliği uygulaması ile ilgili farkındalıkları, Aile Hekimliğine kayıtlı olup olmadıkları ve aile hekimliği uygulamasından nasıl yararlandıkları gibi soruları içeren bir anket uygulanmıştır. Verilerin analizi için SPSS 15.0 programı kullanılmıştır.Bulgular: Katılımcıların yaş ortalaması 75,8 ± 9,20 (58 – 94)’dir. Katılımcıların %70,4’ünün aile hekimliği uygulamasından haberdar olduğu görülmüştür. Bununla birlikte katılımcılar, %64,2’sinin bir aile hekimine kayıtlı durumda olduklarını belirtmiştir. Katılımcıların %37,0’ı ilk olarak devlet hastanesine, %35,8 i üniversite hastanesine, %19,8 i aile hekimliğine ve %7,4 ü özel hastaneye başvuru yapmaktadır. Sonuç:Ülke çapında yaygın örgütlenmeye sahip aile sağlığı merkezlerinin yaşlı nüfus gruplarına vereceği sağlık hizmetlerinin niteliği, yaşlı nüfus topluluğu açısından büyük önem taşımaktadır. Kolay erişilebilir ve yaygın dağılımı olan aile hekimliği uygulamasının, başta huzurevlerinde yaşayan, ileri yaş nüfus grubuna tanıtılması sağlanmalıdır. Bu yönde atılacak adımların, en azından huzurevlerinde kalan yaşlı popülasyonun birinci basamak sağlık hizmetlerine erişimi açısından olumlu bir katkısı olacağı düşünülmektedir., Objective: The family medicine practice aims to provide continuous and versatile health services to individuals and their families. For some elderly individuals, nursing homes remain on the agenda as an option for their need for shelter. In this study, it was aimed to determine the awareness of nursing home residents in the central districts of Manisa about the family medicine practice in our country and how much service they can receive from this practice.Materials and Methods: The research is of descriptive cross-sectional type. The research was conducted among residents living in two different nursing homes in the central districts of Manisa. 81 individuals were included in the study. A questionnaire including demographic information and questions such as the awareness of the participants about the family medicine practice, whether they are registered in the practice and how they benefit from the family medicine practice was applied. SPSS 15.0 program was used for data analysis.Results: The mean age of the participants was 75.8 ± 9.2 (58 – 94). It was observed that 70.4% of the participants were aware of the family medicine practice. However, the participants stated that 64.2% of them were registered with a family doctor. 37.0% of the participants first applied to a state hospital, 35.8% to a university hospital, 19.8% to family medicine and 7.4% to a private hospital.Conclusion: The quality of health services to be provided to elderly population groups by family health centers with widespread organization throughout the country is of great importance for the elderly population. Family medicine practice, which is easily accessible and widely distributed, should be introduced to the elderly population, especially those living in nursing homes. It is thought that the steps to be taken in this direction will have a positive contribution to the access of the elderly population living in nursing homes to primary health care services

Amaç: Hasta güvenliği kültürü, hata bildirimlerinde şeffaflık ve tıbbi hataların önlenmesine sistemli yaklaşımdan sağlık çalışanlarının bu kültür içinde nasıl davranacağına, değer yargılarının nasıl olması gerektiğine, inançlarının ve kurallarının düzenlenmesine kadar geniş bir perspektifte değerlendirilmektedir. Bu çalışma, aile hekimliği birimindeki sağlık çalışanlarının hasta güvenliği kültürü ile ilgili algılarını ölçmeyi amaçlamıştır. Yöntem: Tanımlayıcı nitelikte olan bu araştırma Manisa iline bağlı iki ilçe merkezinde bulunan aile sağlığı merkezlerindeki aile hekimliği birimlerinde görev yapan 108 sağlık çalışanı ile yürütülmüştür. Araştırma verileri hasta güvenliği ile ilgili literatür eşliğinde hazırlanmış soru formu ile elde edildi ve SPSS 15 paket programı kullanılarak istatistiksel olarak değerlendirildi. Bulgular: Katılımcıların genel yaş ortalaması ise 41,19±9,27’dir. Katılımcıların 48’i (%44,4) aile hekimi, 60’ı (%55,6) da aile sağlığı elemanıdır. Katılımcıların hasta güvenliği kültürü ile ilgili sorulara verdiği yanıtlar değerlendirildiğinde olumlu puan ortalaması genel olarak %81,47’dir. Tıbbi ofis işleyişi değerlendirmesinde “Bu ofiste birisi gerçekten meşgul ise, diğerleri ona yardım eder” (%90,7), “Bu ofiste gelişen süreçlere paralel olarak personel eğitim alır” (%89,8) soruları en yüksek iki olumlu yüzdeyi, “Bu ofiste iş akışı ile ilgili sorunlar var” (%62,9) sorusu ise en düşük olumlu yüzdeyi alan seçeneklerdir. Ofis içi iletişim ve hasta takibi ile ilgili başlıkta “Bu ofis hastalara koruyucu ya da rutin bakım için randevuları hatırlatıyor.” %94,3 ile en yüksek olumlu yanıta sahip iken, “Bu ofiste anlaşmazlıkları dile getirmek zordur.” %69,9 ile en düşük olumlu yanıt verilen seçenek olmuştur. Sonuç: Bu çalışmada hasta güvenliği kültürü ile ilgili olumlu yanıt oranları önceki çalışmalara göre görece daha yüksek olarak bulunmuştur. Ancak ofis içi iş akışı ve anlaşmazlıkların dile getirilmesi ile ilgili sorulara verilen yanıtlar en düşük olumlu yüzdelere sahiptir. Aile hekimliği sisteminde hasta güvenliği kültürünü değerlendirecek çalışmaların bu sonuçlar ışığında planlama yapması önerilebilir.

Newborn screening tests are an important preventive public health program that provides early detection of the treatable dise ases of infants. This study was carried out to investigate the knowledge levels of health workers and administrative staff on neonatal screening programs. Our research is a descriptive study. The sample wasn’t selected, and a questionnaire was conducted to 365 people in a period of about 2 months. The questionnaire used in the research consists of 20 questions prepared by searching the literature to measure the information of participants about neonatal screening programs. 89.04% and 10.95% of the employees participating in our study are health staff and administrative sta ff, respectively. 69% of the participants were female, and the average age was 33,13. 66% of the participants in the study had service years of 10 years o r less while 44% had service years of 11 years and longer. The largest group of the participants in the study is research assistants (28.4%) and the second largest group is nurses (22.1%). The total of General Administrative services, technical services and auxiliary servic es is 10,94%. It was seen that 98.4% of the participants were aware of the newborn screening tests. It can be concluded that both physicians and assistant health staff have high knowledge about subjects and applications related to newborn screening tests. However, it is noteworthy that some non-health staff members have inaccurate knowledge and thoughts about some applications related to newborn screening tests. © 2020, Yuzuncu Yil Universitesi Tip Fakultesi. Giris: Inflamasyonun kardiyovaskuler hastaliklardaki rolu hakkindaki kanitlarin gittikce artmasiyla, calismalar inflamatuvar markerlarin hastalik aktivitesi ve risk degerlendirmesindeki rolune odaklanmistir. Bu calismanin amaci inflamatuvar belirteclerin idiyopatik perikardiyal efuzyonla ile iliskisini ve hastalik aktivitesi ile olan korelasyonu olup olmadiginin incelenmesidir. Hastalar ve Yontem: Calisma populasyonu perikardiyal efuzyonu olan 107 hasta ile yas ve cinsiyet olarak benzer 35 saglikli gonullulerden olusmaktadir. Tum hastalar perikardiyal efuzyonun etyolojisine yonelik kapsamli bir klinik degerlendirmeye tabi tutulmustur. Bulgular: Plevral efuzyonu grubunda, yuksek sensitif C-reaktif protein (Hs-CRP), notrofil/lenfosit orani (NLR) ve platelet/lenfosit orani (PLR) (herbiri icin p< 0.001), saglikli gonullulerin grubuna gore anlamli olarak daha yuksekti. Perikardiyal efuzyonu olan hastalar efuzyon miktarina gore 3 gruba bolundu. Hs-CRP, NLR ve PLR perikardiyal efuzyon miktariyla korele bir sekilde artmaktadir. Coklu analizde, NLR (OR 1.363, 95% CI 1.026-1.809; p= 0.033) ve Hs-CRP (OR 1.090, %95 CI 1.030-1.152; p= 0.003), perikardiyal efuzyonu olan hastalarda ciddi perikardiyal efuzyonun bagimsiz bir prediktorleridir. Sonuc: Hs-CRP, NLR ve PLR perikardiyal efuzyonu olan hastalarda, hastalik aktivitesi, tedavi monitorizasyonunda ve risk siniflamasinda kullanilabilir.

Background: Population aging is a global phenomenon with opportunities and challenges. The numbers and proportions of older people are increasing in almost every country. This study aimed to evaluate the perceptions of nursing homes among final-year medical students. Methods: This qualitative study used the word association test as a data collection tool to obtain the reflections regarding nursing homes among final-year students, and the concept of “nursing homes” was defined in face-to-face interviews of the volunteer students. Results: Analysis of data collected from 51 students regarding nursing homes resulted in five main categories (old age [frequency, f=74], positive affect emotions [f=66], negative affect emotions [f=165], loneliness [f=75], and abandonment [f=46]) based on the reflections that the term “nursing home” evoked among the students. In the present study, the final-year students primarily used more negative expressions. Conclusion: Qualitative studies with students, such as those included in the present study, can provide important clues for the development of educational curriculum and social health policies. © 2019 by The Korean Geriatrics Society.

Introduction The aim of this study was to assess psychometric properties of the Turkish version of the Dementia Attitudes Scale which is developed to determine attitudes towards dementia. Methods 326 volunteered students between the age of 20-44 years, studying in Manisa Celal Bayar University Faculty of Medicine and Faculty of Health Sciences are included in this methodological study. Data of the study were collected with "Demographic Data Form", "Dementia Attitudes Scale" and "UCLA-Geriatrics Attitudes Scale". For the analysis of the data, the SPSS and Lisrel software were used. Results The Cronbach's alpha coefficient of the Dementia Attitudes Scale is 0.84 which is quite high. Confirmatory factor analysis has supported three factor-structure of the scale: "Supporting attitude", "Accepting attitude" and "Exclusionary attitude". Confirmatory factor analysis revealed goodness of fit coefficients as 0.076 for RMSEA, 2.86 for the chi-square/sd, and 0.93 for CFI. The analysis showed that the adapted scale fits the model very well. The scale can discriminate between demographic characteristics and attitudes to dementia. The scale significantly correlates with UCLA-Geriatric Attitudes Scale measuring attitude towards the elderly. Conclusion Dementia Attitude Scale is a valid and reliable scale that can be used in studies evaluating the attitude towards dementia.

Amaç: Meme kanseri kadınlarda en sıkgörülen kanser olması nedeniyle önemlidir. Literatürde meme kanserinin aileninhastalığı olarak değerlendirilmesi gerektiği vurgulanmaktadır. Bu çalışmada,meme kanserli hasta yakınlarının sosyodemografik özellikleri ile bakım yüklerive yaşam kaliteleri arasındaki ilişkinin incelenmesi amaçlanmıştır.Materyalve Metot: ÇalışmamızaManisa Celal Bayar Üniversitesi Hafsa Sultan Hastanesi Tıbbi Onkoloji veRadyasyon Onkolojisi polikliniklerine Kasım 2016-Mart 2017 tarihleri arasındabaşvuran 100 meme kanserli hastanın öncelikli yakını dâhil edilmiştir. Veritoplama aracı olarak sosyodemografik form, Zarit Bakıcı Yükü Ölçeği, WHOQOLBREF-TR (Dünya Sağlık Örgütü Yaşam Kalitesi Ölçeği – Türkçe Ulusal Kısa Sürümü)kullanılmıştır. Anketler yüz yüze görüşme tekniği ile araştırmacı tarafındanuygulanmıştır.Bulgular: Meme kanserli hasta yakınlarının %68,00 oranında erkek ve %54,00oranında hastanın eşi olduğu, %56,00 oranında ilköğretim eğitim seviyesinde,%53,00 oranında gelir düzeylerinin orta düzeyde olduğu sonucuna ulaşılmıştır.Yaşam kalitesi sosyal alan puanının erkek cinsiyette ve hastaya yakınlığa göreeşlerde daha düşük olduğu; bakım verenin yaş ortalaması arttıkça sosyal alandayaşam kalitesinin azaldığı; gelir seviyesi arttıkça çevresel, ulusal çevreselve sosyal alanda yaşam kalitesinin arttığı; eğitim seviyesi arttıkça çevreselve ulusal çevresel alanda yaşam kalitesinin arttığı; bakım verme süresiaçısından ilk 6 ay sosyal alan yaşam kalitesi puanlarının en yüksek düzeydeolduğu ve bakım yükü arttıkça çevresel, ulusal çevresel ve sosyal alanda yaşamkalitesinin azaldığı, bakım yükünün ise gelir seviyesi azaldıkça arttığı,cerrahi, kemoterapi, radyoterapi süreçlerini tamamladıktan sonraki süreçte dahafazla olduğu sonucuna ulaşılmıştır.Sonuç: Meme kanserli hastayakınlarının bakım yükleri ve yaşam kaliteleri periyodik aralıklarladeğerlendirilmeli, meme kanserli hastalar kadar onlara bakım veren hastayakınları; sosyoekonomik durumları göz önünde bulundurularak sosyal açıdan,özellikle hastanın eşi eğitim açısından desteklenmelidir. Bakım verenlerineğitim alanında, sosyal ve ekonomik alanda gereksinimlerinin belirlenmesi vekarşılanması, sorunlarının göz ardı edilmemesi gerekmektedir. Böylece bakımverenlerin bakım yüklerinin azaltılması ve yaşam kalitelerinin iyileşmesi, memekanseri tedavisi alan hastaların tedavi süreçlerini ve hastalıkla başa çıkabilmeçabalarını da olumlu etkileyecektir., Objectives: Breast cancer is vital because it’s the most frequent type of cancer observed in women. It’s stressed in the literature that breast cancer needs to be considered as a family disease. In this study, the aim is to examine the relationship between the sociodemographic characteristics as well as the burden of caregiving on the patient’s relatives and their life quality. Materials and Methods: The close relatives of 100 patients with breast cancer admitted to the Medical Oncology and Radiation Oncology Polyclinics of Hafsa Sultan Hospital in Manisa Celal Bayar University between November 2016 and March 2017, are included in our study. As data collection tool, sociodemographic form, the Zarit caregiver burden scale, WHOQOL BREF-TR (World Health Organization Quality of Life Scale – Turkish national short version) were used. Surveys were implemented by the researcher with face-to-face interview technique.Results: The results show that 68.00% and 54.00% of patient relatives with breast cancer are male and are the patient's spouses, and 56.00% and 53.00% are at the primary education level and at the middle-income level, respectively. The results are as follows: Life quality score in social domain is lower in males and spouses when considered in terms of kinship. As age average of caregivers increases, their life quality decreases in social domain. As income level goes up, life quality in environmental, national environmental and social domain increases. When education level arises, life quality increases in environmental and national environmental domain. In terms of caregiving period, the first six months have the highest scores of life quality in social domain and as caregiving burden rises, life quality in environmental, national environmental and social domain reduces. Moreover, when income level decreases, caregiving burden increases and it rises dramatically in the period after surgical, chemotherapy, radiotherapy processes are completed. Conclusion: Caregiving burdens and life quality of patient’s relatives should be evaluated at periodic intervals. Both patients with breast cancer and patient relatives, especially spouses, should be supported in terms of education by their socio-economic conditions being taken into consideration. The educational, social and economic needs of caregivers should be determined and fulfilled and their problems should not be ignored. In that way, burdens of the caregivers would be reduced and their life quality would improve and this would have a positive impact on the treatment process of the patients and on their efforts to cope with the cancer.

Objective: Tobacco use is the most important public health concern worldwide, particularly in developing countries. It is estimated that 1.7 billion people worldwide will be smokers by 2025 if any intervention do not made. Turkey took an important step in the fight against smoking with the introduction of the project ‘smokeless airspace’ in May 2009. The aim of this study is to evaluate the prevalence of smoking and characteristics of smokers among undergraduates attending Celal Bayar University, Manisa, Turkey. Methods: This cross-sectional study was realized among Manisa Celal Bayar University students between March 2015 and June 2015. A self-administered questionnaire was used to obtain sociodemographic data and determine smoking behaviour and nicotine dependence among university students. Descriptive statistical analyses were used to measure the prevalence of smoking and determine correlations between dependent and independent variables. Chi-square test was used to compare categorical variables Results: Of the 2170 respondents, 518 (23.9%) were current smokers, comprising 312 (60.2%) males and 206 (39.8%) females. Cigarette smoking was significantly associated with having friends who smoked, male gender, and parental smoking. Based on the Fagerstrom Tolerance Test, 8.9% of the students were identified as being addicts. According to Fagerström Tolerance of Nicotine Dependence. 193 out of 518 (37.2%) students were reported as nicotine dependent. Conclusion: Deleterious effects of smoking on human health should be incessantly emphasized in the university curriculum, policies of ‘smokeless airspace, should be supported by stronger, and more effective programs in order to encourage university students to quit smoking. Amaç: Tütün kullanımı, özellikle gelişmekte olan ülkelerde dünyadaki en önemli halk sağlığı olarak kabul edilmektedir. Herhangi bir müdahale yapılmazsa, 2025 yılına kadar dünyadaki 1,7 milyar insanın sigara içeceği sanılmaktadır. Türkiye, Mayıs 2009’da “dumansız hava sahası” projesinin başlatılmasıyla sigara içilmesine karşı mücadelede önemli bir adım attı. Bu çalışmanın amacı, Celal Bayar Üniversitesi, Manisa, Türkiye’ye gelen lisans öğrencileri arasında sigara içme prevalansını ve sigara içenlerin özelliklerini değerlendirmektir. Yöntem: Kesitsel tipteki bu araştırma Mart 2015 - Haziran 2015 tarihleri arasında Manisa, Celal Bayar Üniversitesi öğrencileri arasında gerçekleştirildi. Sosyodemografik veriler ile üniversite öğrencileri arasında sigara içme davranışını ve nikotin bağımlılığını belirlemek için anket kullanıldı. Bağımlı ve bağımsız değişkenler arasındaki ilişkileri belirlemek için tanımlayıcı istatistikler kullanıldı. Kategorik değişkenleri karşılaştırmak için ki-kare testi uygulandı. Bulgular: Ankete katılan 2170 katılımcının 518’i (%23,9) [312 (%60,2) erkek, 206 (%39,8) kadın] mevcut sigara içiciydi. Sigara içimi erkek cinsiyet, anne-babanın ve arkadaşların sigara içme durumları ile anlamlı şekilde ilişkiliydi. Fagerstrom Tolerans Testine dayanarak, öğrencilerin %8,9’u bağımlı olarak tanımlandı. Fagerström Nikotin Bağımlılığı Toleransına göre sigara içen 518 öğrencinin 193’ü (%37,2) nikotin bağımlısı olarak rapor edilmiştir. Sonuç: Yasal düzenlemelerin yanı sıra üniversite öğrencileri arasında sigarayı önlemeyi teşvik etmek için sigaranın insan sağlığı üzerindeki olumsuz etkilerinin eğitim müfredatında sürekli olarak işlenmesi ve “dumansız hava sahası” gibi sigara içiciliğini azaltmaya yönelik politikaların daha güçlü ve etkili programlar ile desteklenmesi gerekmektedir.

Introduction: With the growing evidence about the role of inflammation on cardiovascular disorders, studies have been focused on inflammatory markers for risk evaluation and disease activity monitoring. The aim of this study was to investigate whether inflammation has a role in idiopathic pericardial effusion (PE) and to reveal its correlation with the disease activity. Patients and Methods: The study population consisted of 107 patients with PE and 35 age-matched and sex-matched healthy volunteers. Each patient underwent a comprehensive clinical evaluation to identify the probable underlying systemic diseases or other potential causes for PE. Results: High-sensitivity C-reactive protein levels (hs-CRP), neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) (p< 0.001 for each) were significantly higher in the PE group than in healthy volunteers. The patients with PE were divided into three subgroups according to the amount of PE. hs-CRP, NLR and PLR significantly correlated with the amount of PE. In the multivariate logistic regression model NLR (OR 1.363, 95% CI 1.026-1.809; p= 0.033) and hs-CRP (OR 1.090, 95% CI 1.030-1.152; p= 0.003) remained as independent predictors of severe PE in patients who have PE. Conclusion: hs-CRP, NLR and PLR may be used for disease activity monitoring, treatment response evaluation and risk stratification of patients with PE. Giriş: İnflamasyonun kardiyovasküler hastalıklardaki rolü hakkındaki kanıtların gittikçe artmasıyla, çalışma- lar inflamatuvar markerların hastalık aktivitesi ve risk değerlendirmesindeki rolüne odaklanmıştır. Bu çalış- manın amacı inflamatuvar belirteçlerin idiyopatik perikardiyal efüzyonla ile ilişkisini ve hastalık aktivitesi ile olan korelasyonu olup olmadığının incelenmesidir. Hastalar ve Yöntem: Çalışma popülasyonu perikardiyal efüzyonu olan 107 hasta ile yaş ve cinsiyet olarak benzer 35 sağlıklı gönüllülerden oluşmaktadır. Tüm hastalar perikardiyal efüzyonun etyolojisine yönelik kap- samlı bir klinik değerlendirmeye tabi tutulmuştur. Bulgular: Plevral efüzyonu grubunda, yüksek sensitif C-reaktif protein (Hs-CRP), nötrofil/lenfosit oranı (NLR) ve platelet/lenfosit oranı (PLR) (herbiri için p< 0.001), sağlıklı gönüllülerin grubuna göre anlamlı olarak daha yüksekti. Perikardiyal efüzyonu olan hastalar efüzyon miktarına göre 3 gruba bölündü. Hs-CRP, NLR ve PLR perikardiyal efüzyon miktarıyla korele bir şekilde artmaktadır. Çoklu analizde, NLR (OR 1.363, 95% CI 1.026-1.809; p= 0.033) ve Hs-CRP (OR 1.090, %95 CI 1.030-1.152; p= 0.003), perikardiyal efüzyo- nu olan hastalarda ciddi perikardiyal efüzyonun bağımsız bir prediktörleridir. Sonuç: Hs-CRP, NLR ve PLR perikardiyal efüzyonu olan hastalarda, hastalık aktivitesi, tedavi monitörizas- yonunda ve risk sınıflamasında kullanılabilir.

A 66-year-old patient with idiopathic dilated cardiomyopathy underwent transvenous extraction of an implantable cardioverter-defibrillator. The distal part of the electrode was broken during manual traction through the left subclavian vein. In the present case, we showed a rare complication of transvenous lead extraction and its management.

Early and accurate risk prediction is an important clinical demand in patients with infective endocarditis (IE). The platelet-to-lymphocyte ratio (PLR) is an independent predictor of worse prognosis in various cardiovascular diseases. The aim of this study was to determine the value of PLR in the prediction of in-hospital mortality among IE patients. We retrospectively analyzed the clinical, laboratory, and echocardiographic data of 59 adult patients with definite IE and in 40 adult controls. In-hospital mortality occurred in 16 (27%) patients. Vegetation size, levels of high-sensitive C-reactive protein and procalcitonin, neutrophil-to-lymphocyte ratio, and PLR were significantly higher in the in-hospital-mortality-positive group than in the in-hospital-mortality-negative group (p = 0.004, p = 0.009, p = 0.030, p = 0.001, and p = 0.008, respectively). Lymphocyte count was, however, significantly lower in the in-hospital-mortality-positive group (p = 0.004). In the receiver-operating characteristic analysis, PLRs over 191.01 predicted in-hospital mortality with 56.3% sensitivity and 81.4% specificity [area under the curve 0.725, 95% confidence interval (CI) 0.594–0.833; p = 0.0027]. In the multivariate analysis, PLR was found to be an independent predictor of in-hospital mortality in patients with IE (odds ratio 1.022, 95% CI 1.003–1.042; p = 0.021). In conclusion, higher PLR may predict in-hospital mortality in patients with IE.

Giriş: Bu çalışmada aile hekimlerinin tamamlayıcı ve alternatif tedavilere karşı tutumlarının ortaya koyulması amaçlanmıştır. Yöntem: Çalışmaya Afyonkarahisar Halk Sağlığı Müdürlüğü Aile Hekimliği Birimine bağlı olarak çalışan 78 aile hekimi katılmıştır. Katılan aile hekimlerine tamamlayıcı ve alternatif tedavi uygulamaları hakkındaki tutumlarını değerlendirmek amacıyla oluşturulan bir anket formu uygulanmıştır. Bulgular: Çalışmaya katılan aile hekimlerinde tamamlayıcı ve alternatif tedavi yöntemlerinin etkinliğine inanma %83,3 (n=65), tamamlayıcı ve alternatif tedavi yöntemlerinden herhangi birini kullanma %75,,6 (n=59) olarak izlenmiştir. Aile hekimleri %80,,8 oranında tamamlayıcı ve alternatif tedavi yöntemlerini hastalarına tavsiye etmektedir. En çok tavsiye ettikleri yöntem bitkisel ilaçlar ve vitaminlerdir (%79,,5). Giriş: Sağlık çalışanları toplam nüfus içinde yaşlı popülasyon oranının giderek artmasından kaynaklanacak demografik değişikliklerin doğuracağı değişim ve gelişmelere hazırlıklı olmalıdır. Biz bu çalışmada Celal Bayar Üniversitesi Tip Fakültesi öğrencilerinin yaşlılığa karşı genel tutumlarının belirlenmesi ve tıp eğitimi sürecinde tutum konusunda sınışar arasındaki farklılıkları tespit etmeyi amaçladık. Gereç ve Yöntem: Bu çalışma, Ekim-Aralık 2014 tarihleri arasında Celal Bayar Üniversitesi Tip Fakültesinde okumakta olan 406 öğrenci üzerinde yürütülmüştür. Verilerin toplanmasında kullanılan anket formu üçbölümden oluşmaktadır. Birinci bölümde öğrencilerinin sosyodemografik özellikleri sorulmuş, ikinci bölümde KOGAN Yaşlılara Karşı Tutum Ölçeği Türkçe formu ve üçüncü bölümde UCLA Geriatrik Tutum Ölçeği Türkçe formu kullanılmıştır. Bulgular: Çalışmaya katılan öğrencilerin yaşaralığı18-27 olup (ortalama yaş=21.04), %60.8'i kız, %91.4'ünün ise çekirdek aile yapısına sahip olduğu görülmüştür. Sınışara göre UCLA toplam puanları değerlendirildiğinde 1. sınıf öğrencilerinin ortalama puanlarının diğer sınışara göre daha yüksek olduğu saptanmıştır. UCLA'ya göre yaşlılara karşı tutumda kadınlarda erkeklere göre anlamlı pozitif tutum saptanmıştır. KOGAN da ise cinsiyet açısından yaşlılara yönelik tutumda herhangi anlamlı bir fark saptanmamıştır. Sonuç: Çalışmada öğrencilerin yaşlılara karşı tutumlarının sınıf ilerledikçe biraz daha olumsuz yönde değiştiği söylenebilir. Ayrıca kız öğrencilerin tutumlarının da daha olumlu olduğu görülmektedir. Normal bireylere nazaran görece incinebilir yapıda olan yaşlı popülasyonun sağlık personelinden beklentileri göz önüne alındığında, hekim sorumluluğunun da artmış olacağı öngörülmek durumundadır. Introduction: Health workers must be prepared for the changes and developments due tothe ever-increasing proportion of elderly individuals within the general population. This study aimed to identify the general attitudes of students of the Faculty of Medicine of Celal Bayar University (CBU) toward senility and evaluate differences in the attitudes of students at different stages of medical education. Materials and Method: This study was conducted between October and December 2014and included a total of 406 students attending the Faculty of Medicine of the CBU. A questionnaire form composed of three sections was used to collect data. Results: The age range of study participants was 18-27 years (average age, 21.04 years),60.8% were females, and 91.4% had a nuclear family structure. When assessing overall UCLAscores according to the stage of medical education, the average scores of first-year students were found to be higher than other year groups. The results of the UCLA questionnaire demonstrated significantly higher overall scores concerning attitudes toward older persons in female students compared with male students. No significant differences in KOGAN questionnaire scoresconcerning attitudes toward older people were observed between male and female students.Conclusion: The findings of the present study indicate that attitudes toward the elderly change negatively as students progress through medical education. In addition, female students hadmore positive attitudes toward elderly patients than male students. Considering the expectationsfrom the healthcare providers of the elderly population, who are more fragile compared to normal individuals, it should be envisaged that the responsibilities of physicians have increased.

Aim In this study, it was aimed to detect the family physicians’ attitudes to the complementary and alternative treatment (CAT). Methods Seventyeight family physicians who are working for Afyonkarahisar Public Health Directorate, Department of Family Medicine had attended to the study. A questionnaire prepared to assess the attitudes to of the family physicians about complementary and alternative treatment was applied. Results For the family physicians in the study; the ratio of believing efficiency of CAT was 83.3% (n:65) and using any of the CAT methods was 75.6%. Family physicians, as a ratio of 80.8% were expressed that they recommend their patients any of the CAT methods. The most commonly advised method is herbal treatment and vitamins (79.5%).
