Abstract

Aim: This study aimed to define important clinical and laboratory features that may be useful in the differential diagnosis of pediatric patients who presented with temporary loss of consciousness and in whom cardiac causes had been excluded, especially in the differentiation of convulsive syncope and epileptic seizure. Methods: The records of patients presenting with temporary loss of consciousness and in whom cardiac causes had been excluded, were retrospectively evaluated. All patients were grouped according to their diagnosis and the data were evaluated comparatively. Results: Six-hundred-and-twelve patient files were evaluated, and 350 patient files were included in the study. 68.6% of the patients were diagnosed with vasovagal syncope, 13.1% were diagnosed with psychogenic pseudosyncope and 18.2% of the patients were diagnosed with epilepsy. 2. Severe Apnea in a Premature Infant after Accidental Vancomycin Overdose Responsive to Treatment with Exchange Transfusion

Abstract

Background: Mostly seen toxicities following vancomycin are ototoxicity and nephrotoxicity. We here report a very low birth weight preterm neonate who developed severe episodes of apnea after accidental iatrogenic vancomycin overdose, responsive to treatment with double volume exchange transfusion. Case report: A preterm neonate weighing 1380 grams received two doses of 10-fold of the normal dose of vancomycin per kg in this age group. She developed sudden onset of frequent and severe episodes of apnea, which required noninvasive ventilation. Using fluorescence polarization immunoassay, serum vancomycin level was found to be 84 μg/mL 10 hours after the last dose. The patient underwent exchange transfusion. Apnea episodes terminated 12 hours after exchange transfusion. The blood level of vancomycin decreased from 84 μg/mL before exchange to 67 μg/mL immediately post-exchange and eventually to less than 1 μg/mL in 36th hour after exchange. Discussion: Target peak concentration of vancomycin in neonates is between 20 and 40 μg/mL and trough concentration ranges from 5 to 10 μg/mL. Peak serum concentration of our patient can be back extrapolated to be about 336 μg/mL which was higher than the target level. This high plasma levels of vancomycin might be the cause of apnea in our patient as evidenced in similar reports. Conclusion: Apnea is a potential sign of vancomycin overdose in neonates and infants treated with this antibiotic. Exchange transfusion is a potential effective treatment to rapidly resolve this unwanted complication.

3. Postneonatal Epilepsy and Psychomotor Developmental Retardation Risk Factors in Term Neonatal Convulsions Without Hypoxic Ischemic Encephalopathy

Abstract

Objective: Neonatal convulsions may be an early sign of brain injury and the presence of convulsions in the neonatal period has been associated with long-term sequelaes such as mental retardation, postnatal epilepsy and death. We aimed to determine associations of etiological factors with neurodevelopment and postneonatal epilepsy and evaluate the risk factors in newborns with neonatal convulsions that were not related to hypoxic-ischemic encephalopathy. Material and Methods: This study included full-term infants who were born between January 2010 and December 2014 and had neonatal convulsion history, had no history of hypoxic-ischemic encephalopathy and were followed for at least 1 year at our neurology clinic. Results: Forty-nine patients were included to the study. Among the identified etiologies on first clinical visit, hypoglycemia was the most common cause which was presented in 11 (40.74%; 11/27 patients) patients. During follow-up, 22.4% (n=11) of patients developed postneonatal epilepsy. In 4 of 7 patients with abnormal Bayley II test results, epilepsy developed in the follow-up. The risk for development of postneonatal epilepsy was significantly associated with abnormal neurological findings, such as cerebral palsy or significant delays in developmental stages; being not benefited from acute treatment and follow-up abnormal EEG findings of the patients. Conclusion: Hypoglycemia should be primarily investigated and treated in term neonatal seizures without hypoxia. Abnormal neurological findings, being not benefited from the acute treatment and follow-up EEG findings were associated with developing epilepsy. In the literature, most of the studies were limited due to short follow-up periods. More information about prognostic factors in neonatal convulsions and the occurrence of postneonatal epilepsy is needed., Amaç: Yenidoğan konvulziyonları, yenidoğan döneminde akut nörolojik hastalıkların en yaygın semptomlarından biridir. Hastalarda ölüm, mental retardasyon ve postneonatal epilepsi riskini arttırdığı bilinmektedir. Bu çalışmada sebebi doğumsal hipoksik iskemik ensefalopati olmayan ve hastanemizde yenidoğan konvulziyonu sebebiyle takip edilmiş hastalarda etyolojik faktörler ile nörogelişim ve epilepsi ilişkisi ve risk faktörlerinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntemler: Bu çalışmaya hastanemiz nöroloji polikliniğinde takipli, Ocak 2010 - Aralık 2014 tarihleri arasında miadında doğan, doğumsal hipoksik iskemik ensefalopati öyküsü olmayıp, yenidoğan döneminde nöbet öyküsü olan, en az 1 yıl takibimizde kalan hastalar dahil edildi. Bulgular: Çalışmaya 49 hasta dahil edildi. İlk başvuru sırasında saptanabilen en sık etyolojik faktör, 11 (%40.74) hastada saptanan hipoglisemiydi. İzlemde hastaların %22.4’ünde (n=11) postneonatal epilepsi geliştiği görüldü. Bayley II testleri anormal saptanan 7 hastanın 4’ünde epilepsi geliştiği belirlendi. Yenidoğan döneminde başlanan tedaviye yanıtın olmaması, anormal nörolojik muayene bulgularının varlığı ve izlemde anormal EEG bulgularının olması, hastalarda postneonatal epilepsi gelişimi için risk faktörleri arasındaydı. Sonuç: Hipoglisemi, yenidoğan nöbetlerinde saptanabilen ve tedavi edilebilen sebeplerden bir tanesidir. Anormal nörolojik muayene bulguları, başlanan tedaviye alınan yanıtlar ve izlem EEG bulguları epilepsi gelişimi ile ilişkili görüldü. Bu konuda yapılmış olan çalışmaların çoğu, kısa gözlem süreleri sebebiyle sınırlıdır. Yenidoğan konvulziyonları ve postneonatal epilepsi gelişimi açısından daha fazla prognostik faktör belirlemek için daha fazla çalışmaya ihtiyaç vardır.

4. Evaluation of Children with Migraine: Three Years Experience

Abstract

Background: Headache in pediatric age group is one of the common causes of admission to both general pediatrics and pediatric neurology outpatient clinics. The aim in this study was to retrospectively evaluate the clinical and laboratory findings of our cases diagnosed with migraine. Material and methods: Between 2013 and 2016, patients who applied to the Pediatric Neurology Outpatient Clinic of Dr. Sami Ulus Pediatrics Training and Research Hospital and were diagnosed with migraine were included. Statistical comparison was made between subgroups, which were migraine with aura and migraine without aura. Results: In present study, 11.6% of the patients who applied with the complaint of headache consisted of patients diagnosed with migraine. It was determined that 18.8% (n:27) of migraine patients were under the age of 10, and 81.3% (n:117) were over the age of 10. Forty-four (48.4%) female patients and 13 (24.5%) male patients had migraine with aura, and there was a statistically significant difference in terms of gender (p=0.008). Also there was a statistically significant difference between the mean age and gender of migraine patients with aura and migraine without aura (p=0.017). Conclusion: Since migraine is a chronic disease which is observed more frequently than estimated during childhood. We shared the results of the findings that may make us think of the diagnosis of migraine at the first admission, and may also guide us for migraine with aura in a patient followed up with a diagnosis of migraine.

Abstract

Neuropathy is the most common adverse effect. CYP3A4 and CYP3A5 enzymes of cytochrome p450 enzyme system are responsible in vincristine metabolism. Genetic polymorphism may alter the vincristine metabolism and the neurotoxicity rate. In this study, distribution of CYP3A5 alleles among Turkish children with malignancies, relation between CYP3A5 genotype and neurotoxicity rates, as well as severity and duration of neuropathy and total vincristine doses were investigated. Patient group consisted of 115 patients (age, 1 to 17 y) with acute lymphoblastic leukemia and solid tumors, who were treated with vincristine consisting chemotherapy protocols. Control group consisted of 50 children without any neurological symptom or disorders. All patient files were reviewed for presence and severeness of neurotoxicity symptoms. Blood samples were obtained and CYP3A5 genotypes were analyzed. Neurotoxicity occurred in 20.8% of patients. Although it was found to occur more frequently after 4 doses of vincristine, and rates were higher in the low-dose vincristine group suggesting other contributing factors.
