Background: Motor and cognitive deficits and consequently mobility problems are common in geriatric patients. The currently available methods for diagnosis and for the evaluation of treatment in this vulnerable cohort are limited. The aims of the ComOn (COgnitive and Motor interactions in the Older populatioN) study are (i) to define quantitative markers with clinical relevance for motor and cognitive deficits, (ii) to investigate the interaction between both motor and cognitive deficits and (iii) to assess health status as well as treatment outcome of 1000 geriatric inpatients in hospitals of Kiel (Germany), Brescia (Italy), Porto (Portugal), Curitiba (Brazil) and Bochum (Germany). Methods: This is a prospective, explorative observational multi-center study. In addition to the comprehensive geriatric assessment, quantitative measures of reduced mobility and motor and cognitive deficits are performed before and after a a two week's inpatient stay. Components of the assessment are mobile technology-based assessments of gait, balance and transfer performance, neuropsychological tests, frailty, sarcopenia, autonomic dysfunction and sensation, and questionnaires to assess behavioral deficits, activities of daily living, quality of life, fear of falling and dysphagia. Structural MRI and an unsupervised 24/7 home assessment of mobility are performed in a subgroup of participants. The study will also investigate the minimal clinically relevant change of the investigated parameters. Diskussion: This study will help form a better understanding of symptoms and their complex interactions and treatment effects in a large geriatric cohort.

It is a disabling symptom of neuropathic pain following a lesion within the peripheral or central nervous system. In fact, two different pathophysiological mechanisms of cold allodynia (ie, hypersensitivity to innocuous cold) have been proposed. The peripheral sensitization of nociceptive neurons can produce cold allodynia, which can be induced experimentally by a topical application of menthol. An alternative mechanism involves reduced inhibition of central pain processing by innocuous cold stimuli. A model to induce the latter type of allodynia is the conduction block of peripheral A-fiber input.Patients and methods: In the presented study, functional MRI was used to analyze these two different experimental models of cold allodynia. In order to identify the underlying cerebral activation patterns of both mechanisms, the application of menthol and the induction of a mechanical A-fiber blockade were studied in healthy volunteers.Results: The block-induced cold allodynia caused significantly stronger activation of the medial polymodal pain processing pathway, including left medial thalamus, anterior cingulate cortex, and medial prefrontal cortex. In contrast, menthol-induced cold allodynia caused significantly stronger activity of the left lateral thalamus as well as the primary and secondary somatosensory cortices, key structures of the lateral discriminative pathway of pain processing. Mean pain intensity did not differ between both forms of cold allodynia.Conclusion: Experimental cold allodynia is mediated in different cerebral areas depending on the underlying pathophysiology. The activity pattern associated with block-induced allodynia confirms a fundamental integration between painful and non-painful temperature sensation, ie, the cold-induced inhibition of cold pain.Keywords: A-fiber block, menthol, cold allodynia, fMRI

On the contrary, the correct diagnostic classification of its symptoms can be challenging and most patients suffer from pain for years, until they are diagnosed correctly. The aim of this project was to characterize patients with unclassified extremity pain and to present a simple algorithm for a retrospective stratification approach.Patients and methods: The FabryScan includes a bedside-test and a questionnaire, consisting of 10 symptom-orientated and anamnestic questions. For the stratification of patients according to the likelihood for Fabry disease two different approaches were conducted. First, a prospective subgrouping based on the previously invented FabryScan evaluation system was conducted. The second retrospective approach consisted of a factor analysis and a subsequent two-way cluster analysis. Further on, 4 patients diagnosed with Fabry disease were stratified according to both approaches.Results: In total, 183 completed datasets were included in the statistical analysis. The first approach prospectively classified patients into 3 subgroups (n=40 [likely], n=96 [possible], n=47 [unlikely]) according to the FabryScan evaluation system. The second approach retrospectively stratified patients into 3 subgroups (n=47 [cluster 1], n=95 [cluster 2], n=41 [cluster 3]). Finally, the Fabry patients were sorted to the subgroups, indicative for the highest possibility of Fabry disease in both stratification approaches A and B.Conclusion: Both stratification approaches sorted patients with confirmed Fabry disease to the subgroups, indicative for the highest likelihood for Fabry. Phenotype stratification of patients with peripheral neuropathic pain can be conducted with a novel algorithm based on sensory profiles., In a recent cluster analysis, it has been shown that patients with peripheral neuropathic pain can be grouped into 3 sensory phenotypes based on quantitative sensory testing profiles, which are mainly characterized by either sensory loss, intact sensory function and mild thermal hyperalgesia and/or allodynia, or loss of thermal detection and mild mechanical hyperalgesia and/or allodynia. Here, we present an algorithm for allocation of individual patients to these subgroups. The algorithm is nondeterministic—ie, a patient can be sorted to more than one phenotype—and can separate patients with neuropathic pain from healthy subjects (sensitivity: 78%, specificity: 94%). We evaluated the frequency of each phenotype in a population of patients with painful diabetic polyneuropathy (n = 151), painful peripheral nerve injury (n = 335), and postherpetic neuralgia (n = 97) and propose sample sizes of study populations that need to be screened to reach a subpopulation large enough to conduct a phenotype-stratified study. The most common phenotype in diabetic polyneuropathy was sensory loss (83%), followed by mechanical hyperalgesia (75%) and thermal hyperalgesia (34%, note that percentages are overlapping and not additive). In peripheral nerve injury, frequencies were 37%, 59%, and 50%, and in postherpetic neuralgia, frequencies were 31%, 63%, and 46%. For parallel study design, either the estimated effect size of the treatment needs to be high (>0.7) or only phenotypes that are frequent in the clinical entity under study can realistically be performed. For crossover design, populations under 200 patients screened are sufficient for all phenotypes and clinical entities with a minimum estimated treatment effect size of 0.5.

Objective: Successful treatment of chronic low back pain (LBP) is difficult in clinical practice and hard to measure in trials. One reason might be the use of insufficient outcome parameters. The aim was to investigate the importance of typical clinical characteristics of chronic LBP on QoL and functionality. Methods: A total of 51 patients with chronic LBP (19 with, 32 without radiculopathy) were investigated with different questionnaires. Results: Burdening symptoms differed in frequency, intensity and impairment of QoL and functionality between patients with and without radiculopathy and between the area of pain within the same patient, i.e. between back and leg. Symptoms of nerve affection such as prickling pain and numbness were rated higher in the area of radiating pain than on the back in radiculopathy and typical neuropathic pain symptoms such as burning pain, prickling, spontaneous pain and feeling of deep pressure and pain at the beginning of movement were rated with a higher impairment of QoL and functionality in patients with compared to those without radiculopathy. Furthermore, intensity, impairment of QoL and functionality were not necessarily reported in association with one another: some patients were highly impaired in QoL or functionality despite a moderate to low pain intensity, whereas others suffered from severe pain but were less impaired in QoL or functionality. Conclusion: Results suggest the consideration of impairment of QoL and functionality in addition to symptom intensity for treatment evaluation of chronic LBP. This can help to improve overall well-being of the patients and enhance efficacy in clinical pain trials and patient-centered treatment.

Background and aims:Laser-evoked potentials (LEPs) are used for the assesment of human nociceptive fibres (A-delta and C-afferents) and are therefore a useful and well-established method to appraise central pain procession. Repetitively applied painful LEPs lead to habituation, measurable by a decrement of the N2/P2 amplitude in healthy subjects. In contrast, neuropathic pain patients e.g. with painful radiculopathy present reduced or abolished LEP-habituation, which makes the LEP-habituation paradigm an interesting tool to detect central sensitization. Nevertheless, the currently used paradigm lasts too long for a clinical application. The aim of this investigation is to determine whether a shorter interstimulus intervall (ISI) still leads to a significant habituation.Methods: In this study, 21 healthy subjects have been tested by applying 100 painful laser stimuli at the back of the right hand using an ISI of 5-7s in comparison to an ISI of 8-12s in previous studies. Subjects had to rate the perceived laser stimuli on a numerical rating scale (0= no pain, 10= worst pain imaginable) after hearing a ping tone. Results: The results show significant LEP-habituation during the time-course of repetitively applied laser stimuli. The comparison of the two ISIs (i.e. 8-12s and 5-7s) showed no significant difference regarding the habituation of LEPs and pain, i.e. there was a comparable N2/P2 amplitude and pain rating decrement.Conclusion: These results indicate an equivalence of the shorter ISI, which only lasts 10 minutes (compared to 25), with the previously applied LEP habituation paradigm. Thus, in future studies it might possible to save precious time by resorting to this improved LEP habituation paradigm.

Die Osteoarthrose ist eine der häufigsten chronischen, degenerativen Erkrankungen des Bewegungsapparates. Eine konservative Therapie kann bislang keine langfristige Symptomlinderung erreichen und den Krankheitsprozess nicht aufhalten. Bislang wurde angenommen, dass die Schmerzentstehung der Arthrose rein nozizeptiv ist. Eine genaue Analyse des somatosensorischen Phänotyps insichtlich Funktionsverlust oder Sensitivierung ermöglicht die Quantitativ Sensorische Testung (QST) und erlaubt Rückschlüsse auf die zugrunde liegende Pathophysiologie. Ziel dieser Arbeit war die Charakterisierung des sensorischen Phänotyps und Analyse hinsichtlich Schmerzintensität, demographischen Charakteristika, Funktionalität, Lebensqualität und psychischer Gesundheit von Patienten mit Arthrose. Weiterhin sollten Unterschiede zwischen Patienten mit und ohne zentrale Schmerzsensibilisierungsprozesse sowie die konservativ und operativ behandelten Patienten, einerseits innerhalb der Gruppen longitudinal und andererseits im Gruppenvergleich, analysiert werden. Die Studie bestand aus zwei Untersuchungszeitpunkten (Visite 1 und Visite 2), an denen nach Erhebung der Anamnese, demographischen Daten und Schmerzintensität, eine QST im Areal des übertragenden Schmerzes am Oberschenkel durchgeführt wurde. Weiterhin wurden validierte Fragebögen eingesetzt zur Erfassung der Funktionalität, gesundheitsbezogenen Lebensqualität, psychischen Gesundheit, Qualität des Schlafs und Schmerzcharakteristika. 49 Patienten mit Coxoder Gonarthrose haben an dieser Studie teilgenommen. 31 der Patienten haben auch an Visite 2, teilgenommen; 13 dieser Patienten erhielten zwischenzeitlich eine Gelenkersatzoperation, deren Indikation unabhängig von dieser Studie gestellt wurde. Das somatosensorische Profil der Arthrosepatienten war charakterisiert durch eine Sensibilisierung im Sinne einer thermischen und/oder mechanischen Hyperalgesie. Fast die Hälfte der untersuchten Patienten wiesen Zeichen einer zentralen Schmerzsensibilisierung auf. Diese Patienten hatten stärkere Schmerzen als Patienten ohne zentrale Sensibilisierung. Hinsichtlich Erkrankungsdauer, Häufigkeiten der charakteristisch neuropathischen Symptome, Funktionseinschränkung, Schlafstörungen, Psychische Gesundheit sowie Lebensqualität fanden sich keine Unterschiede. Die Patienten, die im Verlauf eine Gelenkersatzoperation erhielten, hatten bei Visite 1 mehr Schmerzen und stärkere Funktionseinschränkungen als die Patienten mit konservativer Therapie; Unterschiede der Lebensqualität oder psychischen Gesundheit sowie im sensorischen Profil der Patientengruppen konnten nicht gefunden werden. Nach Gelenkersatzoperation nahmen die Schmerzintensität und die zentrale Sensibilisierung ab. Anhand dieses Ergebnisses und dem Vergleich der Patienten mit und ohne zentrale Sensibilisierung ist anzunehmen, dass die Schmerzintensität ein Hauptfaktor in der Generierung dieser zentralen Sensibilisierungsprozesse ist und analog die Verminderung der Schmerzintensität eine zentrale Rolle bei der Reduktion zentraler Sensibilisierung spielt. Für die Patienten mit konservativer Therapie konnte, abgesehen von einer verminderten Kälteschmerzschwelle und einer verminderten taktilen Wahrnehmung, keine Veränderung im Verlauf in den Ergebnissen der QST und den Fragebogenergebnissen gefunden werden. Bemerkenswert ist, dass sich ein Zusammenhang mit der subjektiven Zufriedenheit mit dem Gelenkersatz und dem Ausmaß der Funktionsverbesserung nach der Behandlung fand, aber nicht mit der Schmerzreduktion oder den Schmerzen nach der Behandlung. Das Vorhandensein einer depressiven Störung bei Visite 1 war mit einer geringeren Schmerzreduktion nach operativer Behandlung und somit einem geringeren Behandlungsansprechen verbunden. Zusammenfassend fand sich bei Arthrosepatienten hinsichtlich der sensorischen Auffälligkeiten insbesondere eine Überempfindlichkeit gegen Hitze und mechanische Reize, wobei ein substanzieller Prozentsatz der Patienten Zeichen einer zentralen Sensibilisierung aufwies und dies mit der angegebenen Schmerzintensität korrelierte.

Neuropathische Schmerzen sind Folge einer Läsion oder Ausdruck einer Erkrankung, welche hauptsächlich das somatosensorische Nervensystem betrifft. Aktuell übliche Testverfahren zur Prüfung des somatosensorischen Systems umfassen dabei nur selten die Kältesensation. Für den Nachweis von Pathologien dünner Nervenfasern und des spinothalamischen Trakts gelten Methoden wie Laser-evozierte Potentiale (LEPs) als zuverlässige Verfahren. Jedoch kann die kälteleitende A-delta-Faser Subklasse nicht mittels Laser-, Hitze- oder Schmerzstimuli detektiert werden. Die Überprüfung der kälteleitenden A-delta-Faser Subklasse und damit des spinothalamischen Trakts ist durch Kälte-evozierte Potentiale möglich. Bei dem in dieser Studie getesteten Verfahren werden kurze Kältestimuli über eine Thermode auf die Haut appliziert und die mittels EEG erhobenen Kurvenausschläge ausgewertet. Während der Studie konnten bei 16 gesunden Probanden Kälte-evozierte Potentiale zuverlässig an Hand, Gesicht, Fußrücken und Fußsohle mit diesem kommerziell erhältlichen Stimulator abgeleitet werden. Sowohl reversible A-Faser-Funktionsverluste während eines A-Faserblocks (hier waren die Kälte-evozierten Potentiale unterdrückt) und die Wiederherstellung der A-Faser-Funktion kurze Zeit nach Lösung des A-Faserblocks konnten objektiv mit Hilfe von Kälte-evozierten Potentialen nachgewiesen werden. Die Studie dient als Grundlage für weitere Untersuchungen bezüglich Kälte-evozierter Potentiale, welche sich mit Small-Fiber-Neuropathien und der Detektion von A-delta-Faser-Störungen befassen. Der nächste Schritt für einen diagnostischen Gebrauch von Kälte-evozierten Potentialen wird der Versuch sein, reproduzierbare Ergebnisse in einer Patientenstudie mit nachgewiesenen A-Faser-Funktionsstörungen zu erzeugen.

Laser-evozierte Potenziale dienen als nicht invasives Verfahren zur selektiven Beurteilung der Nervenfaserfunktion von dünn myelinisierten Aδ- und unmyelinisierten C-Fasern. Vorangegangene Studien zu Laser-evozierten Potenzialen bei Radikulopathie-Patienten zeigten eine reduzierte Amplitude und eine Latenzverlängerung im abgeleiteten Elektroenzephalogramm der betroffenen Seite im Vergleich zur gesunden Seite. Die vorliegende Studie untersuchte Laser-evozierte Potenziale von 38 Patienten mit schmerzhafter Radikulopathie der Nervenwurzeln L5 oder S1, sowie 20 gesunde, alters- und gewichtsangepasste Probanden in den Dermatomen L5 und S1 beidseits. Für die Nervenfasermessung erfolgte die Abgabe von Infrarotlaser-Reizen in jedes der vier Testareale, die über eine Stimulation oberflächlicher Nozizeptoren zur Aktivierung von Aδ- und C-Fasern führen. Der subjektiv empfundene Schmerz wurde anhand der Numerischen Rating-Skala vom Patienten bewertet. Es wurde eine Quantitativsensorische Testung durchgeführt und krankheitsspezifische Daten mittels Fragebögen und Funktionsanalyse erfasst. In ihrer Gesamtheit unterschieden sich die Laser-evozierten Potenziale der betroffenen Dermatome in Latenz und Amplitude nicht signifikant von den Laser-evozierten Potenzialen der kontralateralen gesunden Kontrollseite, den ipsilateralen benachbarten Dermatomen, sowie den Dermatomen der gesunden Probanden. Bei einer Subgruppen-Analyse von Patienten mit L5 links-Radikulopathie, die vermehrt unter Paresen litten und klinisch schwerer betroffen waren, zeigte sich eine reduzierte Amplitude und eine signifikant verlängerte N2-Latenz. Latenzveränderungen Laser-evozierter Potenziale wiesen auf eine ausgeprägte Nervenwurzelschädigung hin, die mit einer Verschlechterung der körperlichen Funktionsfähigkeit, mit einer schweren klinischen Betroffenheit und Schmerzen einherging. Hingegen sind Veränderungen der N2/P2-Amplitude nur dann von klinischer Relevanz, wenn die Amplitude komplett oder nahezu vollkommen verschwunden ist. Die Ergebnisse lassen darauf schließen, dass Laser-evozierte Potenziale nicht geeignet sind Nervenfaserschäden oder Schmerzen bei Patienten mit milder klinischer Symptomatik zu detektieren. Bei Patienten mit ausgeprägten radikulären Symptomen, wie Paresen, können Laser-evozierte Potenziale zwischen einer moderaten und einer schweren Nervenwurzelkompression differenzieren.
