This article gives an overview on the principles of reprocessing of rigid and flexible endoscopes used in ENT units including structural and spatial requirements based on general and ENT-specific risks of infection associated with diagnostic and therapeutic endoscopy. The underlying legal principles as well as recommendations from scientific societies will be exemplified in order to give a practical guidance to the otorhinolaryngologist. Preliminary results of a small nation-wide survey on infection control standards based on data of 29 ENT practices in Germany reveal current deficits of varying degree concerning infection control management including reprocessing of endoscopes. The presented review aims to give support to the establishment of a structured infection control management program including the evaluation of results by means of a prospective surveillance.

Aim: Clostridium difficile -associated diarrhea (CDAD) causes heavy financial burden on healthcare systems worldwide. As with all hospital-acquired infections, prolonged hospital stays are the main cost driver. Previous cost studies only include hospital billing data and compare the length of stay in contrast to non-infected patients. To date, a survey of actual cost has not yet been conducted.Method: A retrospective analysis of data for patients with nosocomial CDAD was carried out over a 1-year period at the University Hospital of Greifswald. Based on identification of CDAD related treatment processes, cost of hygienic measures, antibiotics and laboratory as well as revenue losses due to bed blockage and increased length of stay were calculated.Results: 19 patients were included in the analysis. On average, a CDAD patient causes additional costs of Euro 5,262.96. Revenue losses due to extended length of stay take the highest proportion with Euro 2,555.59 per case, followed by loss in revenue due to bed blockage during isolation with Euro 2,413.08 per case. Overall, these opportunity costs accounted for 94.41% of total costs. In contrast, costs for hygienic measures (Euro 253.98), pharmaceuticals (Euro 22.88) and laboratory (Euro 17.44) are quite low.Conclusion: CDAD results in significant additional costs for the hospital. This survey of actual costs confirms previous study results., Zielsetzung: Die Clostridium-difficile -assoziierte Diarrhö (CDAD) bedingt hohe finanzielle Belastungen für Gesundheitssysteme weltweit. Wie bei allen nosokomial erworbenen Infektionen ist ein verlängerter Krankenhausaufenthalt der wesentliche Kostentreiber. Bisherige Kostenstudien beziehen sich nur auf Krankenhausabrechnungsdaten vor Einführung des DRG-Entgeltsystems und den Vergleich von Verweildauer zu nicht infizierten Patienten. Eine Erhebung tatsächlicher Kosten steht bislang aus. Methode: Anhand einer retrospektiven Analyse wurden Daten der Universitätsmedizin Greifswald von Patienten mit einer stationär behandelten CDAD über einen 1-Jahres-Zeitraum ausgewertet. Über eine Identifizierung von CDAD-relevanten Behandlungsprozessen wurden die Kosten von Hygienemaßnahmen, Arzneimittel und Labor sowie Erlösausfälle bedingt durch Bettensperrungen und Verweildauerverlängerungen berechnet. Ergebnisse: 19 Patienten wurden in die Analyse eingeschlossen. Im Durchschnitt fallen pro CDADPatient zusätzliche Gesamtkosten in Höhe von 5.262,96 Euro an. Erlösausfälle aufgrund der verlängerten Verweildauer stellen mit 2.555,59 Euro pro Fall den höchsten Anteil dar, gefolgt von den Erlösausfällen aufgrund von Bettensperrungen während der Isolierung mit 2.413,08 Euro pro Fall. Insgesamt ergeben diese Opportunitätskosten einen Anteil von 94,41% an den Gesamtkosten. Die Kosten für Hygienemaßnahmen (253,98 Euro), Arzneimittel (22,88 Euro) und Labor (17,44 Euro) sind dem gegenüber gering.Schlussfolgerung: Die CDAD führt zu deutlichen Mehrkosten für das Krankenhaus. Aim: Information on the epidemiology of multiresistant bacteria (MRB) with zoonotic potential is growing but still remains quite incomplete. This narrative mini-review provides a general overview of the epidemiology of the most important zoonotic MRB in cattle, swine and poultry in Europe.Methods: A literature search was conducted mainly on the PubMed website including articles published until April 2012. Results: Livestock-associated methicillin-resistant Staphylococcus aureus (LA-MRSA) especially poses a zoonotic risk to people working in close contact with livestock. These people may become carriers themselves and the hazard of transmission into health-care facilities needs surveillance. Extended-spectrum beta-lactamases (ESBL) producing bacteria are widely spread in both humans and livestock, sharing similar genotypes, especially of the CTX-M-group, which makes a zoonotic transfer very likely. Identical strains of vancomycin-resistant enterococci (VRE) were found both in humans and animals, after ingestion of animal strains transient colonization of the human gut may be possible. Only a few data are available on the transmission of methicillin-resistant coagulase-negative staphylococci (MR-CoNS) between humans and animals. Direct contact to colonized animals may be a risk factor as well as the exchange of resistance genes between human and animal staphylococci. Clostridium difficile (C. difficile ) ribotype 078 emerges in livestock and humans and a zoonotic transmission seems probable as genotypes and diseases resemble each other.Conclusion: All discussed MRB and C. difficile are important nosocomial agents which also occur in livestock and were found in foods of animal origin. Further analysis is needed to reveal the exact transmission routes and to perform a reliable risk assessment., Zielsetzung: Die Intention des vorliegenden narrativen Mini-Reviews ist es, dem Leser einen Überblick über die Prävalenz und Epidemiologie multiresistenter Bakterien in Europa bei Rindern, Schweinen und Geflügel zu geben und deren zoonotisches Potential zu beleuchten. Dental plaque critically affects the etiology of caries, periodontitis and periimplantitis. The mechanical removal of plaque can only be performed partially due to limited accessibility. Therefore, plaque still represents one of the major therapeutic challenges. Even though antiseptic mouth rinses reduce the extent of biofilm temporarily, plaque removal remains incomplete and continuous usage can even result in side effects. Here we tested argon plasma produced by kinpen09 as one option to inactivate microorganisms and to eliminate plaque. S. sanguinis biofilms cultivated in either the European Biofilm Reactor (EUREBI) or in 24 well plates were treated with argon plasma. In both test systems a homogeneous, good analyzable and stable biofilm was produced on the surface of titan plates within 72 h (>6,9 log10 CFU/ml). Despite the significantly more powerful biofilm production in EUREBI, the difference of 0.4 log10 CFU/ml between EUREBI and the 24 well plates was practically not relevant. For that reason both test models were equally qualified for the analysis of efficacy of cold atmospheric pressure plasma. We demonstrate a significant reduction of the biofilm compared to the control in both test models. After plasma application of 180 s the biofilm produced in EUREBI or in 24 well plates was decreased by 0.6 log10 CFU/ml or 0.5 log10 CFU/ml, respectively. In comparison to recently published studies analyzing the efficacy of kinpen09, S. sanguinis produces a hardly removable biofilm. Die Plaque stellt nach wie vor eine therapeutische Herausforderung dar, weil ihre Eliminierung durch mechanische Reinigung aufgrund der schweren Zugänglichkeit nur unvollständig gelingt. Antiseptische Mundspülungen können zwar über die Senkung der Bakterienzahl das Plaquewachstum hemmen, allerdings ohne dabei die Plaque selbst zu eliminieren. Darüber hinaus sind nur wenige antiseptische Wirkstoffe aufgrund möglicher Nebenwirkung für die Daueranwendung geeignet. Als mögliche Option zur Inaktivierung der Mikroorganismen in der Plaque bei gleichzeitiger Plaqueelimination wurde die Anwendung von Argonplasma, erzeugt mit dem kinpen09, an S. sanguinis Biofilmen, kultiviert im Europäischen Biofilmreaktor (EUREBI) bzw. in der 24-Well-Platte, untersucht. In beiden Modellen konnte auf Titanplättchen innerhalb von 72 h ein homogener, stabiler, gut analysierbarer Biofilm (>6,9 log10 KbE/ml) erzeugt werden. Trotz der signifikant stärkeren Biofilmbildung im EUREBI war die erzielte Differenz von 0,4 log10 KbE/ml ohne praktische Relevanz, so dass beide Prüfmodelle gleichermaßen zur Erprobung der Wirksamkeit von Plasmaquellen geeignet sind. Verglichen mit der Kontrolle war in beiden Biofilmmodellen nach der längsten gewählten Einwirkungszeit von 180 s eine signifikante Reduktion des Biofilms um 0,6 log10 KbE/ml bzw. 0,5 log10 KbE/ml erreichbar. Im Vergleich zur Wirksamkeit des kinpen09 in anderen Studien bildet S. sanguinis offensichtlich einen schwierig eliminierbaren Biofilm aus, wobei die Reifungszeit des Biofilms sowie andere Plasmazusammensetzungen und Behandlungseinstellungen die vergleichsweise geringe Effektivität beeinflusst haben dürften. Um die Wirksamkeit gegen S. sanguinis Biofilme zu verbessern, sind weitere Untersuchungen mit reduziertem Abstand zwischen Plasmaquelle und Biofilm, veränderter Zusammensetzung des Plasmas sowie mit anderen Plasmaquellen erforderlich.

Background: Antibiotic resistance of bacterial pathogens is an emerging problem worldwide. To combat multidrug resistant organisms (MRDOs) networks of care providers have been established in all states in Germany. The HICARE-network, a project to combat MRDOs, founded by the Federal Ministry of Education and Research, has published data from 2010 of a voluntary, German-wide, multicenter point-prevalence survey in 2011 conducted in collaboration with the German Society of Hospital Hygiene. The aim of the present survey was the re-evaluation of the situation in 2012.Method: The survey was conducted as a voluntary, anonymous, point-prevalence in May 2012 using routine data of microbiological diagnostics of the hospitals. As in the former survey of 2010 it was differentiated between primary, secondary and tertiary care hospitals and only data from intensive care units, surgical and medical wards were collected. Based on the survey form used in 2010, an updated version was used including more pathogens and corrected issues observed in the former survey. Methicillin-resistant Staphylococcus aureus (MRSA) (total as well as separated in hospital-acquired (HA), community-acquired (CA) and lifestock-associated (LA) MRSA), vancomycin resistant Staphylococcus aureus (VRSA/GRSA), vancomycin resistant Enterococcus faecalis resp. Enterococcus faecium (VR-E. faecalis resp. Introduction: Physical plasma is a promising new technology regarding its antimicrobial effects. This is especially accounting for treatment of bacterial infection of chronic wounds. Plasma can be generated with different carrier gases causing various biological effects. Screening of different carrier gases and plasma generation setups is therefore needed to find suitable compositions for highly effective antimicrobial plasma treatments and other applications. Method: The plasma source used was a radio-frequency plasma jet which generates tissue tolerable plasma (TTP). The study compared the antimicrobial efficacy of air, argon, or helium plasma alone or admixed with 0.1%, 0.5%, and 1% oxygen against Pseudomonas aeruginosa (P. aeruginosa ) and Staphylococcus aureus (S. aureus ). Treatment took place in an environmentally open and delimited system. Therefore, bacteria were plated on agar and treated with plasma in a punctiform manner. The resulting inhibition zones were measured and the reduction factors were calculated by colony counting, respectively. Results: For S. aureus and P. aeruginosa , inhibition zones and overall reduction of colony forming units (CFU) on the agar plate were observed while an accumulative reduction of CFU dominated for S. aureus . The highest antimicrobial effect was shown in form of an inhibition zone for argon plasma with 0.1% oxygen admixture for both species. S. aureus was more sensitive for helium plasma with >0.1% oxygen admixture compared to P. aeruginosa which in turn was more sensitive for argon plasma with and without oxygen. The efficacy of air plasma was very low in comparison to the other gases. The treatment in a closed system predominantly enhanced the antimicrobial effect. The effect intensity varied for each treatment time and gas mixtures. Discussion: As expected, the antimicrobial effect mostly increased when increasing oxygen admixture to the carrier gases. Zur Wirksamkeitsbestimmung wurden Wachstums-Hemmhöfe ausgemessen und mittels Anzahl der koloniebildenden Einheiten (KbE) auf der Platte der Reduktionsfaktor bestimmt. Ergebnisse: Bei S. aureus und P. aeruginosa wurden sowohl Hemmhöfe als auch Gesamtreduktionen beobachtet, wobei es bei S. aureus verstärkt zu einer Gesamtreduktion kam. Am Merkmal der Hemmhöfe zeigte Argonplasma mit 0,1% Sauerstoff den stärksten antimikrobiellen Effekt an beiden Spezies. S. aureus war gegenüber Plasmabehandlungen mit Helium mit >0,1% Sauerstoff sensibler als P. aeruginosa , während P. aeruginosa sensibler nach Plasmabehandlungen mit Argon und Argon mit Sauerstoffbeimischungen reagierte. Aim: To assess trends in hygiene management in dental practices in comparison to an earlier survey in 2002/2003 and to point out key aspects for future efforts. Method: The infection prevention management of all dental practices in Greifswald (n=35) was determined by a questionnaire in a personal interview in 2008/2009.Results: 26% of the dentists did not use sufficient personal protective equipment during the general examination of the patient. In conservative and prosthetic dentistry, 15% still did not use adequate measures and 9% did not even in surgical interventions. Vaccination coverage was clearly too low, as only 35% of dentists were vaccinated against influenza and coverage with other vaccinations was also quite low. 11% of the dentists did not perform a documented anamnesis and in 29% of the dental practices no appointment system for risk patients existed.There were significant deficiencies in the reprocessing of medical devices and in the equipment needed for reprocessing. The opportunity to participate in further training in this field was rejected by 23% of the dentists.In 10 dental practices, the colony count in the dental unit water-conducting system was five times higher than the limit. A contamination with P. aeruginosa was discovered in 4 practices. All units were renovated.Discussion: Overall, both the hygiene management and hygiene equipment in the practices have improved considerably compared to the previous survey in 2002/2003. This demonstrates the positive effect of the KRINKO guidelines from 2006. However, the survey again showed relevant deficiences in the hygiene management of dental practices, which agrees with a Germany-wide online survey from 2009.Conclusion: While the study revealed persistent deficiencies in hygiene management, especially in reprocessing, it confirms that the KRINKO guidelines for dental practices from 2006 led to significant improvements in hygiene management. Objective: Many dental diseases are attributable to biofilms. The screening of antimicrobial substances, in particular, requires a high sample throughput and a realistic model, the evaluation must be as quick and as simple as possible. For this purpose, a colorimetric assay of the tetrazolium salt XTT (sodium 3'-[1-[(phenylamino)-carbony]-3,4-tetrazolium]-bis(4-methoxy-6-nitro)benzene-sulfonic acid hydrate) converted by saliva biofilms is recommended. Cleavage of XTT by dehydrogenase enzymes of metabolically active cells in biofilms yields a highly colored formazan product which is measured photometrically.Materials and method: The suitability of the XTT assay for detecting the vitality of ex vivo saliva biofilms was tested to determine the efficacy of chlorhexidine and ozone versus saliva biofilms grown on titanium discs.Results: The XTT method lends itself to testing the vitality of microorganisms in saliva biofilms. The sensitivity of the arrays requires a specific minimum number of pathogens, this number being different for planktonic bacteria and those occurring in biofilms. The antibacterial effect after treatment with chlorhexidine or ozone was measured by XTT conversion that was significantly reduced. The antimicrobial efficacy of 60 s 0.5% and 0.1% chlorhexidine treatment was equal and comparable with 60 s ozone treatment. Conclusion: The XTT assay is a suitable method to determine the vitality in saliva biofilms, permitting assessment of the efficacy of antimicrobial substances. Das Screening von antimikrobiellen Substanzen erfordert einen hohen Probendurchsatz mit einem realistischen Modell. Die Auswertung sollte hierbei so schnell und so einfach wie möglich durchführbar sein. Zu diesem Zweck wird ein kolorimetrischer Test mit dem Tetrazoliumsalz XTT (Natrium 3'-[1-[(phenylamino)-carbony]-3,4-Tetrazolium]-Bis(4-methoxy-6-nitro)benzol-sulfonsäurehydrat) empfohlen, das durch den Speichelbiofilm umgewandelt wird. Durch die Umsetzung von XTT durch Dehydrogenase-Enzyme von metabolisch aktiven Zellen in Biofilmen entsteht ein gefärbtes Formazanprodukt, das photometrisch nachgewiesen wird.Material und Methode: Die Eignung des XTT-Tests zum Nachweis der Vitalität von ex vivo Speichelbiofilmen wurde getestet, indem die Wirksamkeit von Chlorhexidin und Ozon auf Speichelbiofilme auf Titanplättchen bestimmen wurde.Ergebnisse: Die XTT-Methode eignet sich für die Testung der Vitalität der Mikroorganismen in Speichelbiofilmen. Die Empfindlichkeit des Tests erfordert eine bestimmte Mindestanzahl von Mikroorganismen, wobei sich diese Anzahl in planktonischen und Biofilmenkulturen unterscheidet. Die antibakterielle Wirkung nach der Behandlung mit Chlorhexidin oder Ozon wurde durch die Reduktion der XTT-Umsetzung nachgewiesen. Die antimikrobielle Wirksamkeit von 60 s Behandlung mit 0,05% und 0,1% Chlorhexidin war vergleichbar mit 60 s Ozonbehandlung.Fazit: Der XTT-Test ist eine geeignete Methode, um die Vitalität in Speichelbiofilmen zu bestimmen und erlaubt die Beurteilung der Wirksamkeit von antimikrobiellen Substanzen. Durch die schnelle und einfache Anwendbarkeit ist dieser Test besonders für Screenings geeignet.

Objective: Data on the prevalence of emerging bacterial pathogens like extended-spectrum-lactamase-building (ESBL) Gram negative organisms, multiresistant Pseudomonas and Acinetobacter species or toxin-building Clostridium difficile in German hospitals are sparse. To provide data for different regions in Germany, a one-day point prevalence study with five tertiary care hospitals and four secondary care hospitals was conducted on the 10th of February 2010.Method: For participating hospitals, the level of care (primary/secondary/tertiary), staffing with infection prevention personnel, availability of a MRSA-screening, microbiological support and the prevalence of five emerging bacterial pathogens in intensive care, surgical and medical wards was assessed by questionnaire.Results: Overall, 3411 patients were included. In tertiary hospitals, the following prevalences were given: MRSA 1.8%, ESBL E. coli 0.45%, ESBL Klebsiella spp. 0.41%, multiresistant Pseudomonas 0.53%, multiresistant Acinetobacter species 0.15%, VRE 0.49% und Clostridium difficile 1.01%. Recently, the HACCP (Hazard Analysis and Critical Control Points) concept was proposed as possible way to implement process-based hygiene concepts in clinical practice, but the extent to which this food safety concept can be transferred into the health care setting is unclear. We therefore discuss possible ways for a translation of the principles of the HACCP for health care settings. While a direct implementation of food processing concepts into health care is not very likely to be feasible and will probably not readily yield the intended results, the underlying principles of process-orientation, in-process safety control and hazard analysis based counter measures are transferable to clinical settings. In model projects the proposed concepts should be implemented, monitored, and evaluated under real world conditions., Das HACCP-Konzept (Hazard Analysis and Critical Control Points)-Konzept ist in jüngerer Zeit wiederholt als Möglichkeit für prozess-basierte Hygienekonzepte vorgeschlagen worden. Wie aber dieses, aus der Lebensmittelsicherheit stammende Konzept in der Gesundheitsfürsorge tatsächlich realisiert werden kann, ist unklar. Deshalb prüften wir Möglichkeiten einer Übertragung der Grundsätze des HACCP auf Anforderungen der Gesundheitsversorgung am Beispiel des Managements multiresistenter Erreger. Während eine direkte, unreflektierte Umsetzung der Lebensmittelkonzepte in die Gesundheitsversorgung weder machbar ist noch zu den gewünschten Ergebnissen führt, können die Grundprinzipien Prozessorientierung, Qualitätssicherung und Gefahrenanalysen-basierte Gegenmaßnahmen auch in der medizinischen Versorgung zur Anwendung kommen. In Modellimplementierungen sollten die vorgeschlagenen Konzepte unter realen Bedingungen erprobt, evaluiert und bewertet werden.

Background: Prevention of post-endoscopy infections is an important objective to assure patient safety. Endowashers, or high throughput irrigation water pumps, are a frequently used device on endoscopes. Recommendations published by professional bodies and regulatory health agencies cover not only adequate reprocessing of fiber-endoscopes but also state accepted methods of regular microbial sampling. Although major instruments like endoscopes are covered by these recommendations, other devices used as optional add-ons for endoscopes are not included. Objective: The aim of this prospective study was to investigate the potential for endowashers to act as a possible source of infection.Method: 24 endowashers were sampled. Sterile rinse-water was pumped through the endowasher and tested microbiologically according to standardised tests. Sampling was performed in 18 hospitals, including 2 university teaching hospitals, in northern Germany. If endowashers were contaminated, devices were reprocessed and re-tested.Results: Of 44 samples, 6 (14%) were contaminated with pathogens of up to >20,000 cfu/ml. Pseudomonas aeruginosa and other Gram-negative non-fermenters such as Stenotrophomonas spp. (18x) and Acinetobacter spp. (2x), followed by Staphylococcus aureus (1x), Enterobacter cloacae (1x), Candida albicans (1x), Serratia spp. (1x), Streptococcus spp . (1x) and others (2x).Conclusion: Endowashers can be a potential source of infection. Despite their common use, they are not routinely sampled microbiologically. Endowashers should be clearly mentioned in respective guidelines and routine quality control sampling of endowashers should be part of such recommendations. If endowashers are not monitored regularly, devices with single-use hoses should be used., Hintergrund: Die Prävention von Infektionen nach endoskopischen Eingriffen ist Bestandteil der Gewährleistung der Patientensicherheit. Endowasher oder Hochdruckirrigationswasser-pumpen sind ein häufig benutztes Device in der Endoskopie. Empfehlungen von Fachgremien und Institutionen betreffen nicht nur die adäquate Aufbereitung von flexiblen Endoskopen, sondern auch die richtige mikrobiologische Probenahme. Obwohl die hauptsächlichen Instrumente wie Endoskope in den Empfehlungen berücksichtigt sind, trifft das nicht für Devices als optionaler Zusatz zu Endoskopen zu.Zielsetzung: Mittels einer prospektiven Studie sollte das Risiko für Endowasher als mögliche Infektionsquelle untersucht werden.Methode: Zur Untersuchung der Endowasher (n=24) wurde steriles Wasser durch den Endowasher gepumpt und mikrobiologisch gemäß Standardmethode untersucht. Die Proben wurden in 18 Krankenhäusern einschließlich zwei Universitätsklinika in Nordostdeutschland entnommen. Bei positivem bakteriologischem Befund wurden die Endowasher aufbereitet, erneut mikrobiologisch überprüft und erst bei negativem Befund erneut eingesetzt.Ergebnisse: Von 44 Proben waren 6 (14%) mit Krankheitserregern in einer Menge von bis zu >20.000 KbE/ml kontaminiert. Es dominierten Pseudomonas aeruginosa und andere Gram-negative Non-Fermenter wie Stenotrophomonas spp . Dermatitis of hands is a problem among nurses. The aim of this prospective questionnaire based survey was to analyze practice and knowledge of skin care of medical and surgical nurses. 250 questionnaires were distributed. 49% of respondent stated that they perform skin care at least 1-2 times/day. After hand-wash 15% of participants perform skin protection, after hand-disinfection only 2%. 40% give skin care products less than 3 minutes to be applied. It was shown that this knowledge is lacking in many individuals, leading to wrong behavior at work and insufficient use of skin protection and skin care products., Die sog. Abnutzungsdermatose ist ein Problem bei Pflegekräften. Mit Hilfe einer prospektiven Fragebogenerhebung sollten das Wissen zum Hautschutz und zur Hautpflege und die praktische Umsetzung bei in der Intensivpflege tätigen internistischen und chirurgischen Pflegekräften analysiert werden. Background: An in-vitro study was conducted investigating the antim icrob ial efficacy of polihexanide and triclosan against clinical isola tes and reference laboratory strains of Staphylococcus aureus and E scherich ia coli .Methods: The minimal inhibitory concentration (MIC) and the minimal microbicidal concentration (MMC) were determined following DIN 58940-81 using a micro-dilution assay and a quantitative suspension test following EN 1040. Polihexanide was tested in polyethylene glycol 4000, triclosan in aqueous solutions.Results: Against all tested strains the MIC of polihexanide ranged between 1-2 µg/mL. For triclosan the MICs varied depending on strains ranging between 0.5 µg/mL for the reference strains and 64 µg/mL for two clinical isolates. A logRF >5 without and logRF >3 with 0.2% album in burden was achieved at 0.6 µg/mL triclosan. One exception was S. aureus strain H-5-24, where a triclosan concentration of 0.6 µg/mL required 1 minute without and 10 minutes with albumin burden to achieve the same logRFs. Polihexanide achieved a logRF >5 without and logRF >3 with albumin burden at a concentration of 0.6 µg/mL within 30 sec. The exception was the North-German epidemic MRSA strain, were an application time of 5 minutes was required. Conclusion: The clinical isolates of E. coli generally showed higher MICs against triclosan, both in the micro-dilution assay as well in the quantit ativ e suspension test than comparable reference laboratory strains. Polihexanid wurde in einer Polyethylene-Glykol 4000 Lösung, Triclosan in wässriger Lösung angesetzt. Ergebnisse: Die MHK von Polihexanid gegen alle getesteten Stämme lag zwischen 1-2 µg/mL. Die MHK von Triclosan war stammabhängig und lag bei 0,5 µg/mL für die getesteten Referenzstämme und bei 64 µg/mL für zwei klinischen Isolate. Gegen alle Stämme erreichte Triclosan bei einer Konzentration von 0,6 µg/mL einen logRF >5 ohne und logRF >3 mit 0,2% Albuminbelastung. Als Ausnahme erwies sich S. aureus -Stamm H-5-24, für den eine Triclosan-Konzentration von 0,6 µg/mL bei einer Einwirkungszeit von 1 min ohne bzw. 10 min mit Albuminbelastung erforderlich war, um dieselben logRF zu erreichen. Polihexanid erreichte bei einer Konzentration von 0,6 µg/mL einen logRF >5 ohne und logRF >3 mit Belastung bei einer Einwirkungszeit von 30 s . Hier erwies sich als Ausnahme der Norddeutsche MRSA Epidemiestamm, gegen den bei selber Konzentration eine Einwirkungszeit von 5 min erforderlich war. Schlussfolgerungen: Die klinischen E. coli -Isolate erforderten höhere MHKs bei Triclosan als die untersuchten Referenzstämme. Für Polih exan id und Triclosan konnte eine stammabhängige Empfindlichkeit gezeigt werden. Beide Antiseptika werden im klinischen Einsatz jedoch regelhaft bei weit höheren Konzentrationen eingesetzt, womit die unterschiedlichen Empfindlichkeiten klinisch keine Rolle spielen sollten.

In the recent past, the antimicrobial applicability of tissue tolerable plasma (TTP) has been examined, and is increasing in importance. Many different plasma sources can be used medically. In order to obtain reliable test results by using plasma sources, it is essential not only to assess physical parameters but also to screen a use-oriented application of plasma sources. It is necessary to both determine the biological efficacy and establish uniform coverage of the treated surfaces. This article shows a simple screening method for this. We used newly inoculated agar plates with bacterial test organisms and treated the agar plates with two surface barrier discharged (SDB) plasmas. The carrier gas was argon and air., In jüngster Zeit gewinnt die Anwendbarkeit von gewebeverträglichem Plasma (Tissue Tolerable Plasma - TTP) auf der Körperoberfläche u.a. zur Wundbehandlung an Bedeutung. Hierfür kommt eine Vielzahl verschiedener Plasmaquellen in Frage. Um verlässliche Untersuchungsergebnisse beim Einsatz von Plasmaquellen zu gewährleisten, muss neben der Bestimmung der physikalischen Parameter zuvor ein anwendungsbezogenes Screening der Plasmaquellen erfolgen, um nicht nur deren Effekte auf biologische Systeme, sondern auch um eine homogene Plasmawirkung auf den zu behandelnden Oberflächen zu bestimmen. In dieser Arbeit wird hierfür als einfache Screeningmöglichkeit die Exposition frisch beimpfter Agarplatten mit Testbakterien anhand von zwei Oberflächen-Barriere-Entladungs (surface barrier discharged - SBD)-Plasmen mit Argon und Luft als Trägergas vorgestellt.

Aim: The biofilm formation plays a crucial role in the pathogenesis of many acute and subacute microbial infections as well as of chronic wounds. For the stimulation of wound healing tissue tolerable plasma (TTP) is an alternative to antiseptic or chemotherapeutic therapies. The mechanical influence of argon plasma on the penetration of bacteria into the tissue related to the antimicrobial effect of the plasma is unknown. Hence the effects of argon plasma versus argon gas were examined for bacterial reduction as well as for the penetration depth of P. aeruginosa in a 3-D human epidermis model, the so-called combi-model. Method: 3-D NHEK cell cultures were inoculated with P. aeruginosa SG81 and treated after 20 min incubation with TTP. The distribution and penetration depth of the bacteria in the cell culture were compared by confocal laser scan microscope (CLSM) between control and TTP effect. Result: The penetration of bacteria into the epidermis tissue and a significant reduction of the bacteria could be proven by means of TTP. Therefore justified chances result for treatment of wounds with TTP., Problemstellung: Die Biofilmbildung spielt eine entscheidende Rolle in der Pathogenese vieler akuter und subakuter mikrobieller Infektionen sowie bei chronischen Wunden. Unbekannt ist dabei der mechanische Einfluss von Argon-Plasma auf das Eindringen der Bakterien in das Gewebe in Verbindung mit der anmikrobiellen Wirkung des Plasmas. Dazu wurden die Effekte von Argon-Plasma versus reinem Argongas auf die bakterielle Reduktion und die Eindringtiefe im Kombimodell von P. aeruginosa auf 3-D-Epidermismodellen untersucht. Methode: Es wurden 3-D-NHEK-Zellkulturen mit P. aeruginosa SG81 beimpft und nach 20 min Inkubation mit TTP behandelt. Die Verteilung und Eindringtiefe der Bakterien in der Zellkultur wurde mittels konfokalem Laserscanmikroskop (CLSM) zwischen Kontrolle (Argongas) und TTP-Einwirkung verglichen. Die Ermittlung der antimikrobiellen Wirkung von TPP erfolgte anhand der Bestimmung der Koloniebildenden Einheiten (KbE). Ergebnis: Es konnten die Penetration der Bakterien in das Epidermisgewebe und eine signifikante Erregerreduktion mittels TTP nachgewiesen werden. Damit ergeben sich begründete Chancen für eine Wundbehandlung mit TTP.

MRSA-infected or -colonized staff can be a vector for transmission between patients. Therefore, reliable detection and rapid decolonisation of carriers is important. Literature on the effectiveness of MRSA decolonization regimes is very heterogeneous. Existing studies differ distinct in design, endpoints and patient populations studied and are therefore hard to compare. This paper summarizes routine data from decolonization treatment of MRSA-positive staff and relatives from March 2007-March 2009 at the University Hospital of Greifswald. From a total of 107 persons rated, 98.1% were successfully decolonized. In 73 cases (68%) decolonization was achieved in the first cycle. Background: Procedures for prevention of surgical site infections are clearly regulated in evidence-based guidelines. However, realisation of guidelines is still crucial. Several tools for implementation are available. Studies about compliance in terms of implementation of guidelines and recommendations confirm the necessity to realize consequently evid enc e-based measures. Material and method: To support the implementation of the guideline "prevention of surgical site infections" of the commission for hospital hygiene and infection prevention at the Robert Koch Institute in Berlin an educational film was produced. Results: The film represents also the start of a practice-oriented training concept. Material und Methode: Zur Unterstützung der Umsetzung der Richtlinie "Prävention postoperativer Infektionen im Wundgebiet" der Kommission für Krankenhaushygiene und Infektionsprävention beim Robert Koch-Institut wurde ein Lehrfilm zur Prävention postoperativer Wundinfektionen hergestellt, der zugleich der Auftakt für ein praxisorientiertes Fortbildungskonzepts ist. Ergebnis: Dargestellt ist der Operationsablauf für einen Patienten, beginnend mit der Aufklärung im Arzt-Patient-Gespräch, fortgesetzt über die präoperative Vorbereitung sowie intra- und postoperative Präventionsmaßnahmen.Schlussfolgerungen: Mit dem Lehrfilm wird ein Beitrag zur Umsetzung der World Alliance for Patient Safety von 2004 geleistet. The special importance of the surgeon's hand in the multi-barrier concept to prevent surgical site infections is based on two facts: the resident flora on the hands cannot be totally eradicated and the surgical glove is no reliable barrier for micro-organisms. The aim of the pre-surgical disinfection is therefore to eliminate the transient flora and reduce the resident flora to the greatest possible amount for the duration of the surgery. New experimental evidence has led to changes in the pre-surgical disinfection. The article summarises the actual knowledge and practice of the pre-surgical disinfection with special focus on pre-conditions for hand disinfection, requirements on the efficacy and the influence on a previous hand wash, duration of disinfection and practical implementation. Because some countries still prefer scrubs to alcoholic rubs, we compare both methods based on efficacy, compliance and dermal tolerance with the conclusion that rubs are superior to scrubs in all evaluated categories., Im Multibarrierenkonzept der Prävention postoperativer Wundinfektionen besitzt die Hand des Chirurgen einen besonderen Stellenwert, weil sich ihre residente Flora durch keine Maßnahme vollständig entfernen lässt und der OP-Handschuh als Erregerbarriere keine absolute Sicherheit gewährleistet. Da sich auf Grund neuer experimenteller Befunde Veränderungen für die Praxis der chirurgischen Händedesinfektion ergeben haben, liegt der Schwerpunkt der Übersicht auf der zusammenfassenden Darstellung des aktuellen Wissenstandes mit folgenden Schwerpunkten: Voraussetzungen für die effektive Händedesinfektion, Erläuterung der Anforderungen an die Wirksamkeit des Desinfektionsverfahrens und dessen Wirkungsverminderung durch vorausgehende Händewaschung, erforderliche Dauer der chirurgischen Händedesinfektion und praktische Durchführung. Da in einigen Ländern anstelle der alkoholischen Händedesinfektion (rub) noch die chirurgische Händewaschung (scrub) durchgeführt wird, werden beide Verfahren einer kritischen vergleichenden Analyse mit dem Ergebnis unterzogen, dass rubs bezüglich Effektivität, Verträglichkeit und Compliance durch die Chirurgen als Mittel der Wahl scrubs deutlich überlegen sind.

This paper presents the development of and justification for a different procedure in surgical hand disinfection based on results of our own studies and data from the literature. An overview of the importance of hand disinfection, outstanding persons who substantially contributed to its development, and fundamentals of its implementation are also presented. Focussing on the resident flora as the target of surgical hand disinfection, the microflora of the hand is addressed. Consequences for disinfection are discussed as a result of this analysis. Results of our own studies on reducing spore load through social handwashing, on alcohol concentration and bacterial count in worn-glove juice, and on the comparison of the effectivity of standard handwashing methods vs. alcoholic hand disinfections vs. disinfection alone without handwashing vs. disinfection alone plus additional brushing-in of the tested (European Standard DIN prEN12791) disinfectant ethanol 79% v/v, 60% v/v propan-1-ol or 60% v/v propan-2-ol around the fingernails are also discussed. To summarize, disinfection alone and disinfection in conjunction with brushing-in of alcohol tend to be superior to the standard method with a previous washing with soap. From a dermatological and microbiological hygienic point of view, it is recommended that handwashing with soap should be performed to remove bacterial spores at least 10 minutes prior to hand disinfection, in order to optimize skin tolerance and to prevent reducing the efficacy of the disinfection., Inhalt der Arbeit ist die Entwicklung und Begründung eines veränderten Vorgehens bei der chirurgischen Händedesinfektion aufgrund der Ergebnisse eigener Untersuchungen und der Datenlage aus der Literatur. Dazu wird zunächst ein Überblick über die Bedeutung der Händedesinfektion, wichtige Persönlichkeiten, die sich um ihre Entwicklung verdient gemacht haben und Grundfragen ihrer Durchführung gegeben. Die Mikroflora der Hand wird mit dem Schwerpunkt der residenten Flora als Angriffspunkt der chirurgischen Händedesinfektion behandelt. Im Ergebnis dieser Analyse werden die sich daraus ergebenden Konsequenzen für die Desinfektion diskutiert. Die Ergebnisse eigener Untersuchungen zur Sporenlastreduktion durch die soziale Händewaschung, zur Alkoholkonzentration und zur Bakterienzahl im Handschuhsaft sowie zum Vergleich der Effektivität der Standardmethode mit Händewaschung vor der alkoholischen Händedesinfektion, der alleinigen Desinfektion ohne Händewaschung und der alleinigen Desinfektion mit zusätzlichem Einbürsten des geprüften Desinfektionsmittels Ethanol 79% v/v, 60% v/v Propan-1-ol bzw. 60% v/v Propan-2-ol in den Nagelfalz, geprüft nach der Euronorm DIN prEN12791, werden diskutiert. Zusammenfassend ergibt sich, dass die alleinige Desinfektion und die Desinfektion in Verbindung mit Einbürsten des Alkohols der Standardmethode mit vorausgehender Seifenwaschung tendenziell überlegen sind. Aus dermatologischer und hygienisch-mikrobiologischer Sicht wird empfohlen, die Seifenwaschung zur Entfernung von Bakteriensporen mindestens 10 min vor der Händedesinfektion durchzuführen, um die Hautverträglichkeit zu optimieren und die Wirksamkeit der Desinfektion nicht zu reduzieren.
