Abstract Female obesity is a worldwide health issue linked to chronic metabolic low-grade inflammation (metaflammation) causing multiple obesity-related co-morbid conditions. We aimed to assess the serum levels of wingless integration site family member 5 A (Wnt5a), leptin, and tumor necrosis factor-alpha (TNF-α) as markers of obesity-associated metaflammation and investigate the association with toll-like receptors2 (TLR2) gene (Arg753Gln) single nucleotide polymorphism (SNP) among Egyptian females. The study included 60 females with obesity and 30 matched controls. Serum levels of Wnt5a, leptin, and TNF-α were assessed by ELISA, while TLR2 (Arg753Gln) genotyping was done by PCR-RFLP. Serum Wnt5a, leptin, and TNF-α showed significantly higher levels in females with obesity than controls and a significant increase with higher classes of obesity. They showed significant positive correlations with each other. Only TNF-α and leptin were associated with metabolic syndrome (MetS) among the obesity group. According to TLR2 (Arg753Gln) SNP, the homozygous GG genotype was associated with elevated levels of Wnt5a, leptin, and TNF-α compared to the AA + GA model carriers. No significant differences were found in the distribution of TLR2 Arg753Gln (rs5743708) genotypes and alleles according to obesity or MetS, and the regression analysis showed no significant risk association. Serum Wnt5a, leptin, and TNF-α levels increase in women with obesity and the A allele of TLR2 (Arg753Gln) SNP could be protective against obesity-associated metaflammation.

Washingtonia is a palm tree that enriched with lignocellulosic biomass, which provided it with the potentiality for various applications. In this study, different biomass from Washingtonia plant, i.e. leaf stalk (W-LS), trunk core (W-TC), and trunk spine (W-TS) fibers, were collectively investigated by characterization. Under scanning electron microscopy (SEM) examination, the spine-shape structure of W-TS fiber along with its small particle size might facilitate it for fabrication reactivity. From chemical and elemental composition analysis, both W-TC and W-TS fibers were found with high cellulose content, while with the closely similar residual elements, suggesting them from the same source of trunk segment. However, the low cellulose content of W-LS fiber with its high ash content had somehow reduced its crystallinity and thermal stability properties. From X-ray diffraction (XRD) analysis, the crystallinity was estimated the highest for W-TS (68.2%), and while with the lowest for W-LS fiber (57.3%), which in line with the detected functional groups by Fourier Transform Infrared-ray (FTIR) spectrophotometry. In thermogravimetry (TGA) analysis, all fibers possessed great thermal resistance with Tonset in the range of 307.4–328.1°C and Tpeak in the range of 352.4–347.9°C. Washingtonia fiber had a high potential to be applied as a bio-material for versatile applications in the future.

Date palm fibre is considered as an agricultural waste suitable as reinforcement for polymeric composites has received interest as an interior component in the automotive industry. The aim of this work to investigate the physical and mechanical properties of date palm fibre (leaf stalk, tree trunk, fruit stalk and leaf sheath) reinforced epoxy composites and date palm/bamboo hybrid composites suitable for non-structural and semi-structural applications. The different composites were fabricated using a hand lay-up technique in combination with compression moulding method. Date palm fibre/bamboo hybrid composite, and pure composites were characterized by various mechanical testing such as tensile, flexural and low-velocity impact testing. Exposure to liquid water and environmental conditions affect the durability of biobased composites. The water absorption behaviour and its effects on the thickness swelling, changes in density and physical performance of different composites were further investigated. The experimental results of the date palm fibre/bamboo hybrid composite exhibited the highest mechanical properties compared to date palm fibre composite without hybridisation. The tensile strength, flexural strength and impact toughness values for the date palm/bamboo hybrid composite were recorded at 39.16 MPa, 61.10 MPa and 12.70 J/m, respectively. Interestingly, the physical test of thickness swelling and water absorption results has indicated that date palm fibre/bamboo hybrid composite displayed 27.68% and 15.39% less when compared with single date palm fibre composite. Therefore, the results have discovered that the date palm fibre/bamboo hybrid composite has the prospective to be applied as a non-structural and semi-structural components.

Abstract Background Increased intestinal permeability, either due to the exposure to antigens in asthmatic patients or due to a barrier defect, plays a critical role in susceptibility to environmental allergens. House dust mite allergy occurs more commonly than any other type of allergy among Egyptian asthmatic patients. Aim To assess the relation between serum zonulin level as a marker of increased intestinal permeability and the severity of house dust mite allergic asthma. Methods A case–control study which included 48 patients with house dust mite allergic asthma and 48 healthy control subjects attending the Allergy and Immunology Unit, Microbiology and Immunology Department, Faculty of Medicine, Zagazig University. Results A statistically significant difference was detected between the two studied groups with respect to serum IgE and serum zonulin levels (p ˂ 0.001 and ˂ 0.001, respectively). The mean serum zonulin was equal to 258.3 ± 153.01 ng/ml in the asthmatic group and 80 ± 13 ng/ml in the control group. Serum zonulin level significantly increased with the increase of asthma severity (p ˂ 0.001). The cut off value of serum zonulin was ≥ 198 ng/ml, and the area under the curve was 0.76. It displayed sensitivity equal to 80% and specificity equal to 71.4%. Its negative predictive value was equal to 83.3%. Conclusion Intestinal barrier dysfunction contributes to the pathogenesis of allergic asthma. Serum zonulin level reflects an increase in intestinal permeability. Zonulin acts as prognostic factor of severity in asthma. Correction of the gut barrier defect may have a potential positive prognostic effect in asthma.

Conocarpus is a buttonwood plant enriched with lignocellulosic biomass, which has high potential for various applications. In this study, different biomasses of the Conocarpus plant, such as, leaf fibers (CP–L), branches (CP–B) and trunk (CP–T), were examined collectively through characterization techniques. The chemical composition analysis showed that CP–T fibers have a higher cellulose content than CPL and CP–B fibers. Elementary analysis also detected versatile elements based on minerals such as Mg, Si, P, S, Cl, K and Ca in fiber with a high ash content. Based on the morphological analysis, the CP–T fiber with compact and striated characteristics can contribute more effectively to the reinforcement of composite materials. In addition, the particle size of the fibers increased with increasing crystallinity, which depends on the source of the fibers, being smaller in those obtained from leaves and greater in those obtained from the trunk. This finding showed that the size of the fiber is highly correlated with the stiffness of the fiber structure. In addition, all fibers regardless of their source have great thermal stability, with a high onset degradation temperature of 273.9–316.2 °C. In conclusion, Conocarpus biomass fibers, due to its advantageous properties, have a potential to be used as a bio-filler in polymer composites.

ABSTRACTBackground The WHO has declared COVID-19 as a global pandemic with a rapidly increasing number of patients at a rate exceeding the limits of health-care resources. Some people seem to be at higher risk of worse prognosis and increased mortality. Identifying these vulnerable groups is a necessity.Aim of study To identify the risk factors associated with ICU admission in COVID-19 patients.Methodology We present a retrospective study where the clinical data of patients with laboratory-confirmed COVID-19 infection were collected from Ain-Shams University Isolation-Hospital records on admission from 10 April 2020 to 30 July 2020 to identify risk factors in patients requiring ICU admission during hospital stay.Results Of 323 subjects diagnosed by the RT-PCR as positive COVID-19, 62 (19.2%) of which were admitted at the ICU. The mean age of the subjects was 46.6 ± 16 years. Significant morbidities were associated with higher age groups (p value 0.000), smokers (p value 0.004), Cairo-residents (p value 0.009), being a health-care provider (p value 0.001) and hypertensive patients (p value 0.000).Conclusions Elderly, smokers, diabetic and hypertensive need further attention during disease course. Our results call for further investigations of risk factors for COVID-19 severity; preferably on large prospective cohorts, to increase their validity.

Thermal expansion and viscoelastic properties of the date palm fibre (DPF)/epoxy composites were influenced by the type of filler from different parts of the date palm tree. The variation in properties is mainly due to the difference in fibre composition between the DPF from leaf stalk, leaf sheath, fruit bunch stalk, and tree trunk. Among the investigated composites, composite reinforced with the tree trunk fibre displayed higher thermal stability as visible from the lower weight loss, higher onset and inflection temperatures and maximum residue % from the thermogravimetric analysis (TGA). Lower coefficient of linear thermal expansion (CLTE) obtained from thermomechanical analysis (TMA) further provided supportive evidence for dimensional stability at elevated temperature. Based on the dynamic mechanical analysis (DMA) results, the DPF/epoxy composites with fruit bunch stalk fillers displayed a better property by showing higher storage modulus and lesser tan delta values. Hence, composite from the tree trunk and fruit bunch stalk fibres of date palm is recommended for applications requiring superior thermal resistance.

The present study deals with the incorporation of different ratios of alkali-treated coir fibres (CF) and pineapple leaf fibres (PALF) in polylactic acid (PLA) hybrid composites. Developed hybrid composites are characterized in terms of mechanical, morphological, thermal and physical properties. Mechanical characterization revealed that alkali-treated C3P7 hybrid composites (CF:PALF = 3:7) showed highest tensile strength (30.29 MPa) and young’s modulus (5.16 GPa) among all hybrids composites, whereas C1P1 (CF:PALF = 1:1) showed highest impact properties. Scanning electron microscopy (SEM) justified the consequence of alkali treatment on fibre-matrix adhesion. Thermal analysis revealed that C1P1 and C7P3 (CF:PALF = 7:3) having a higher thermal stability and char content due to the higher content of lignin in CF. Remarkably, the coefficient of thermal expansion (CTE) of treated hybrid composites displayed lesser values than untreated hybrid composites. The physical tests revealed that C3P7 showed highest water absorption (WA) and thickness swelling (TS) though after treatment the WA and TS values get reduced. Overall results indicated that treated CF/PALF/PLA hybrid composites possess enhanced mechanical, thermal and physical properties with lowered CTE over untreated CF/PALF/PLA hybrid composites. The success of these findings results sustainable and degradable hybrid composites for different outdoor and food packaging-based applications.

Anatomically, oral cavity and central nervous system have a close relationship; the mouth and face are the location for 30-40% of the body's sensory and motor nerves. The identification of orofacial manifestations of neurological disorders is usually in direct relation with the responsibilities of a dental surgeon. Therefore, familiarizing dental surgeons with theses manifestations is essential to have better recognition, diagnosis, and correct decisions upon treating their associated Neurological Disorders. These manifestations should be efficiently analyzed using novel effective techniques since their related neurological disorders need to be early identified to avoid serious consequences. Furthermore, preventive dental care for patients with neurological disorders and all kind of rehabilitative treatments necessitates well-planned and effective novel approaches. The Internet of Medical Thing (IoMT) is a relatively new technology that allows the transfer of medical data over a secure network of medical sensors and wearable devices. The data transferred are of utmost importance in diseases diagnosis and treatment. In this paper, an IoMT-based Intelligent Guided Particle Local Search with Optimized Neural Networks (IGPLONN) approach is proposed. Firstly, dental data are collected from the International Collaboration on Cancer Reporting (ICCR) oral cavity and central nervous system. Secondly, features are extracted from data and IGPLONN algorithm is utilized to select the effective features by minimizing the feature dimension that helps improve the overall prediction rate. Finally, the obtained features are transferred to the central health application through the IoMT platform where they can be analyzed by dental practitioners for neurological disorders prediction. The hybrid optimized technique improves the overall oral-linked neurological diseases detection rate. Moreover, it efficiently manages the forecast parameters that are used to predict the dental metastasis with minimum computational complexity. The performance of the proposed system has been experimentally evaluated on MATLAB to verify its excellence. The results revealed that proposed IoMT-based IGPLONN method attains the maximum accuracy of 98.3% compared to other methods.

As one of the largest environmental offenders, the construction industry is a prime candidate for the application of the principles of sustainability. Sustainability could be achieved by using environment friendly materials and structural systems in construction industry. This paper aimed to study using geopolymer concrete as a structural board in constructing insulation sandwich panels under different types of loading. Four compression tests were performed on wall specimens in order to study the behavior of the panels under vertical in plane forces. Axial capacity prediction of wall panels was discussed using four published design equations. Eight one-way slab sandwich panels specimens were tested under flexure loading with different shear span ratios in order to compare the flexural behavior of conventional concrete and geopolymer concrete sandwich panels. The capacities of the slab panels were estimated using the conventional concrete codes provisions and compared to the experimental values. Geopolymer sandwich panels approved its ability to sustain different type of loading in the same manner as that of conventional concrete. A prototype was constructed to assess the constructability and the construction cost of the proposed system. The lack of production lines of geopolymer ingredients increases the material cost. On the other hand, the calculated total cost for different construction systems revealed that the labor cost is lower for the cast in place structural insulated panels CIP-SIP system.

The function of packaging is crucial in the maintenance of fresh meat product quality. This study aimed to assess the efficiency of six films added with coatings 2379L/220 and 2379L/221 (containing sage extracts) to inhibit Salmonella typhimurium, Staphylococcus aureus, and Escherichia coli, which showed that two of the six films had a significant effect. Additionally, the effects of the films on refrigerated skinless chicken breast meat were evaluated based on microbiological content, colour, weight loss, texture and pH. Four of the six films were examined could extend the storability of refrigerated chicken breast fillets for up to seven days. All six treated films improved the pH, colour stability, weight loss, and texture of the chicken fillets. Therefore, these findings suggested that the coatings containing sage extracts having different viscosities (2379L/220 and 2379L/221) were effective as antimicrobial adhesives in food packaging films and can be commercially applied in prolonging the storage of chicken breast meat without affecting their quality.

Selective MSCs differentiation protocol into pancreatic beta cells was conducted in the present study using exendin-4 and TGF-beta. Differentiated and undifferentiated MSCs were assessed in experimental type I diabetes in rats. Ninety female white albino rats were included in the study and divided equally (n=15/group) into 6 groups: healthy control, healthy control rats received acellular tissue culture medium, diabetic rats, diabetic rats received acellular tissue culture medium, diabetic rats received undifferentiated MSCs and diabetic rats received differentiated MSCs. Therapeutic efficacy of undifferentiated versus differentiated MSCs was evaluated via assessment of quantitative gene expressions of insulin1, insulin 2, Smad-2, Smad-3, PDX-1, PAX-4, neuroD. Blood glucose and insulin hormone levels were also assessed. Results showed that quantitative gene expressions of all studied genes showed significant decrease in diabetic rat groups. Use of undifferentiated and differentiated MSCs led to a significant elevation of expression levels of all genes with more superior effect with differentiated MSCs except smad-2 gene. As regards insulin hormone levels, use of either undifferentiated or differentiated MSCs led to a significant elevation of its levels with more therapeutic effect with differentiated MSCs. Blood glucose levels were significantly decreased with both undifferentiated and differentiated MSCs in comparison to diabetic groups but its levels were normalized 2 months after injection of differentiated MSCs. In conclusion, use of undifferentiated or differentiated MSCs exhibited significant therapeutic potentials in experimental type I diabetes in rats with more significant therapeutic effect with the use of differentiated MSCs.

An innovative, biomimetic, green synthesis approach was exploited for the synthesis of humane and environmental friendly nanomaterials for biomedical applications. Ultrafine bonelike apatite (BAp) 2D plate-like structures were prepared using fenugreek seed extract during the biosynthesis wet-chemical precipitation route. The chemical analysis, morphology and structure of the prepared 2D nanoplates were characterized by inductively coupled plasma atomic emission spectroscopy (ICP-OES), electron microscopy (SEM and TEM), X-ray diffraction (XRD) and Fourier transform infrared (FTIR) spectroscopy. A 2D plate-like nanostructure of BAp with an average width (length) of 12.67 ± 2 nm and thickness of 3.8 ± 1.2 nm was obtained. BAp 2D crystals were tuned by interaction with the fenugreek organic molecules during the fabrication process. In addition to Ca and P ions, bone mineral sources such as K, Mg, Na, SO4 and CO3 ions were incorporated into BAp nanoplates using fenugreek seed extract. The overall organic molecule concentration in the reaction process increased the effectiveness of hydroxyl groups as nucleation sites for BAp crystals. Accordingly, the size of the biosynthesized BAp plate-like structure was reduced to its lowest value. Biosynthesis BAp 2D plate-like nanocrystals showed good viability and higher growth of MC3T3 osteoblast-like structures than that of the control sample. BAp 2D nanoplates prepared by a facile, ecofriendly and cost-effective approach could be considered a favorable osteoconductive inorganic biomaterial for bone regeneration applications.

Purpose: To compare the efficacy and tolerability of brinzolamide/timolol (BT) and dorzolamide/timolol (DT) fixed combinations on intraocular pressure (IOP) reduction. Methods: Patients with primary open angle glaucoma or normal tension glaucoma were randomized to receive either BT or DT. IOPs were measured at baseline, 2 weeks, and 1, 2, and 3 months. The primary outcome measures were the mean change in IOP from baseline at each visit. Secondary outcome measures included the tolerability of each fixed combination. Results: Seventy-three patients (73 eyes) were included; 37 eyes in BT group and 36 eyes in DT group. Baseline mean IOP were 24.14 ± 4.5 and 29.53 ± 6 mmHg for BT and DT, respectively (P < 0.001). Both BT and DT provided statistically significant mean IOP reductions from baseline values within each group at all study visits (P < 0.001). DT provided greater mean IOP reductions from baseline than BT at each visit which was statistically significant at 2 weeks (P = 0.037). Mean percentage of IOP reduction was 24.35% and 46.33% at 2 weeks (P < 0.001), and 24.65% and 47% at 3 months (P < 0.001) for BT and DT, respectively. Patients' tolerability appeared to be better for DT than for BT with complete ocular comfort without any ocular adverse effects in 31 patients (81.1%) in DT group and 11 patients (29.7%) in BT group (P < 0.001). Conclusion: Both drops provide effective IOP reduction which was greater, and patients were more likely to achieve lower target pressures with DT than with BT.

A porous surface of Ti–HA composite was successfully fabricated by pulsed current activated sintering (PCAS), followed by leaching using diluted H3PO4. The Ti and HA powders were mixed at different contents of the HA, Ti-5, 10, 30 and 40 wt% HA powders. The mixed powders were pressed in a coated graphite die using pulsed current activated sintering (PCAS) under pressure of 60 MPa at temperature of 1000 °C for 5 min. The sintered Ti–HA specimens were immersed in the eight kinds of leaching solutions at room temperature for 24 h. The leached specimen’s surfaces were characterized using XRD, SEM, EDX and Rockwell hardness. The XRD patterns after sintering show that many phases were detected at the sintered specimen surfaces such as; Ti2O, CaO, CaTiO3, TixPy in addition to the remaining Ti and HA. Furthermore, the high concentration H3PO4 leaching solution is more efficient than the low concentration. Also the produced porous surfaces of Ti–HA materials containing more than 30% HA have a low relative density and hardness than the commercial Ti–6Al–4V ELI alloy. In a word, the presence of porous surface coated by HA will promote the nucleation of the biological apatite created with the human tissue and increase the bonding between them. So, the produced porous materials are considered so easy for the muscle cells to permeate after transplanted with high coherence.

According to the survey on various health centres, smart log-based multi access physical monitoring system determines the health conditions of humans and their associated problems present in their lifestyle. At present, deficiency in significant nutrients leads to deterioration of organs, which creates various health problems, particularly for infants, children, and adults. Due to the importance of a multi access physical monitoring system, children and adolescents’ physical activities should be continuously monitored for eliminating difficulties in their life using a smart environment system. Nowadays, in real-time necessity on multi access physical monitoring systems, information requirements and the effective diagnosis of health condition is the challenging task in practice. In this research, wearable smart-log patch with Internet of Things (IoT) sensors has been designed and developed with multimedia technology. Further, the data computation in that smart-log patch has been analysed using edge computing on Bayesian deep learning network (EC-BDLN), which helps to infer and identify various physical data collected from the humans in an accurate manner to monitor their physical activities. Then, the efficiency of this wearable IoT system with multimedia technology is evaluated using experimental results and discussed in terms of accuracy, efficiency, mean residual error, delay, and less energy consumption. This state-of-the-art smart-log patch is considered as one of evolutionary research in health checking of multi access physical monitoring systems with multimedia technology.

The gene expression profile p16, c-erbB-3 and bcl2 in hepatocellular carcinoma (HCC) patients with and without associated HCV infection, was assessed. Forty-eight subjects were included in the study and divided equally into two groups: HCC with and without HCV associated infection. Adjacent paracancerous tissues were assessed as control samples. Correlations with various clinico-pathological parameters of the tumour were assessed: stage, grade, and tumour size. The c-erbB-3 oncogene was expressed in 83.33% (40/48) of the total HCC sample and in 31.25% (15/48) of the noncancerous lesions. C-erbB-3 was expressed in 87.5% (21/24) of the HCC cases with associated HCV infection and in 79.16% (19/24) of the HCC cases without associated HCV infection. Gene expression of c-erbB-3 was significantly correlated with the clinico-pathological parameters of the tumour. P16 gene expression was found in 12.5% (6/48) of the total HCC sample and in 25% (12/48) of the para-cancerous lesions. P16 was expressed in 12.5% (3/24) of HCC cases with and without associated HCV infection. Gene expression of p16 exhibited significant negative correlation with clinico-pathological parameters of the tumour. Bcl2 gene expression was found in 20.8% (10/48) of the total HCC sample and in the para-cancerous lesions. Bcl2 was expressed in 20.8% (5/24) of the HCC cases with and without HCV associated infection. Gene expression of bcl2 did not show significant correlations with the clinico-pathological parameters of the tumour. In conclusion, gene expression profiles of p16 and c-erbB-3 could be used as prognostic molecular markers in HCC.

The fruits of four wild plants, namely, Capparis decidua, Ficus carica, Syzygium cumini, and Ziziphus jujuba, are separately used as traditional dietary and remedial agents in remote areas of Khyber Pakhtunkhwa, Pakistan. The results of our study on these four plants revealed that the examined fruits were a valuable source of nutraceuticals and exhibited good level of antimicrobial activity. The fruits of these four investigated plants are promising source of polyphenols, flavonoids, alkaloids, terpenoids, and saponins. These four plants' fruits are good sources of iron, zinc, copper, manganese, selenium, and chromium. It was also observed that these fruits are potential source of antioxidant agent and the possible reason could be that these samples had good amount of phytochemicals. Hence, the proper propagation, conservation, and chemical investigation are recommended so that these fruits should be incorporated for the eradication of food and health related problems.
