Background Observational studies suggest that orally administered guaifenesin (GGE) may thin lower respiratory tract secretions but none have examined its effects on mucociliary and cough clearance (MCC/CC) during a respiratory tract infection (RTI). The current study was a randomized, parallel-group, double-blind, placebo-controlled study in non-smoking adults who suffered from an acute upper RTI. Methods We assessed the effects of a single dose of Mucinex ® 1200 mg (2 × 600 mg extended release tablets) (ER GGE) on 1) MCC/CC by assessing the rate of removal from the lung of inhaled radioactive tracer particles (Tc99m-sulfur colloid), 2) sputum dynamic rheology by stress/strain creep transformation over the linear part of the curve, 3) sessile drop interfacial tension by the deNouy ring technique, and 4) subjective symptom measures. MCC was measured during the morning (period 1) and compared to that in the afternoon 4 h later (period 2) immediately following either drug (n = 19) or placebo (n = 19). For both period 1 and 2 subjects performed 60 voluntary coughs from 60 to 90 min after inhalation of radio-labeled aerosol for a measure of CC. Sputum properties were measured from subjects who expectorated sputum during the cough period post treatment (n = 8–12 for each cohort). Results We found no effect of ER GGE on MCC or CC compared to placebo. MCC through 60 min for period 1 vs. 2 = 8.3 vs. 11.8% (placebo) and = 9.7 vs. 11.1% (drug) (NS) and CC for period 1 vs. 2 was 9.9 vs. 9.1% (placebo) and 10.8 vs. 5.6% (drug) (NS). There was no significant difference in sputum biophysical properties after administration of drug or placebo. Conclusions There was no significant effect of a single dose of ER GGE on MCC/CC or on sputum biophysical properties compared to placebo in this population of adult patients with an acute RTI. Suicides are a leading cause of death in psychiatric patients, and in society at large. Developing more quantitative and objective ways (biomarkers) for predicting and tracking suicidal states would have immediate practical applications and positive societal implications. We undertook such an endeavor. First, building on our previous blood biomarker work in mood disorders and psychosis, we decided to identify blood gene expression biomarkers for suicidality, looking at differential expression of genes in the blood of subjects with a major mood disorder (bipolar disorder), a high-risk population prone to suicidality. We compared no suicidal ideation (SI) states and high SI states using a powerful intrasubject design, as well as an intersubject case–case design, to generate a list of differentially expressed genes. Second, we used a comprehensive Convergent Functional Genomics (CFG) approach to identify and prioritize from the list of differentially expressed gene biomarkers of relevance to suicidality. CFG integrates multiple independent lines of evidence—genetic and functional genomic data—as a Bayesian strategy for identifying and prioritizing findings, reducing the false-positives and false-negatives inherent in each individual approach. Third, we examined whether expression levels of the blood biomarkers identified by us in the live bipolar subject cohort are actually altered in the blood in an age-matched cohort of suicide completers collected from the coroner’s office, and report that 13 out of the 41 top CFG scoring biomarkers (32%) show step-wise significant change from no SI to high SI states, and then to the suicide completers group. Six out of them (15%) remained significant after strict Bonferroni correction for multiple comparisons. Fourth, we show that the blood levels of SAT1 (spermidine/spermine N1–acetyltransferase 1), the top biomarker identified by us, at the time of testing for this study, differentiated future as well as past hospitalizations with suicidality, in a live cohort of bipolar disorder subjects, and exhibited a similar but weaker pattern in a live cohort of psychosis (schizophrenia/schizoaffective disorder) subjects. Three other (phosphatase and tensin homolog (PTEN), myristoylated alanine-rich protein kinase C substrate (MARCKS), and mitogen-activated protein kinase kinase kinase 3 (MAP3K3)) of the six biomarkers that survived Bonferroni correction showed similar but weaker effects. Taken together, the prospective and retrospective hospitalization data suggests SAT1, PTEN, MARCKS and MAP3K3 might be not only state biomarkers but trait biomarkers as well. Fifth, we show how a multi-dimensional approach using SAT1 blood expression levels and two simple visual-analog scales for anxiety and mood enhances predictions of future hospitalizations for suicidality in the bipolar cohort (receiver-operating characteristic curve with area under the curve of 0.813). Of note, this simple approach does not directly ask about SI, which some individuals may deny or choose not to share with clinicians. Lastly, we conducted bioinformatic analyses to identify biological pathways, mechanisms and medication targets. Overall, suicidality may be underlined, at least in part, by biological mechanisms related to stress, inflammation and apoptosis.

Objective.To identify variables contributing to interfacility differences in mortality among residents of long-term care facilities who have lower respiratory tract infection.Design.Multicenter, prospective, 1 -year observational study.Setting.Twenty-one long-term care facilities in 4 geographic areas of Canada.Participants.Residents of long-term care facilities prescribed antimicrobials for treatment of lower respiratory tract infection.Methods.Mortality rates were calculated for 3 definitions of lower respiratory tract infection: episodes with a clinical or radiographic diagnosis and treated with antimicrobials (definition 1); episodes with a physician diagnosis of pneumonia (definition 2); and episodes with chest radiography findings consistent with pneumonia (definition 3). Multilevel modeling was used to evaluate variables describing premorbid resident status, clinical presentation, management, and facility characteristics. Multivariable models were developed to identify independent predictors of mortality and determine whether facility-level variables remained independently associated with mortality rate after incorporation of individual-level variables.Results.Facility mortality rates varied from 0% to 17.8% for definition 1, from 0% to 47.1% for definition 2, and from 0% to 37.5% for definition 3. There were significant differences in mortality rate depending on which definition was used; for definitions 1 and 2, there were significant differences in mortality rate across facilities. Poorer premorbid resident status and a more severe presentation remained independent predictors of mortality in the multivariable analysis. There were also significantly increased mortality rates for episodes in which a fluoroquinolone was prescribed for initial treatment. For definitions 1 and 3, facility-level variables remained independently associated with mortality rate in the final multivariable model.Conclusions.Rates of mortality due to lower respiratory tract infection varied among long-term care facilities and differed within a facility, depending on the definition applied. Variables describing premorbid resident status, severity of presentation, and management did not fully explain the variation in mortality rate. Some facility-level variables remained independent predictors of mortality.

Factors in obtaining and interpreting tensile data on ceramic bodies are discussed, with particular attention given to the use of bending. A bend test can be used conveniently to obtain precise tensile data as long as stress is directly proportional to strain in both tension and compression. The moduli of elasticity in tension and in compression need not be equal. To account for any such inequality, strain in both bottom and top longitudinal fibers should be considered in computing the tensile stress. Extraneous stresses in bending can be essentially eliminated through proper specimen design and manner of loading. Details for the consideration of extraneous stresses in practice are presented, along with an equation for computing tensile stress precisely from bend-test data. Limited experimental work on oxide-base and silicate-base bodies supported the theoretical analysis of bending and indicated the suitability of a refined bend test for precise ceramic work.

Vibrio vulnificus is a gram-negative bacterium that contaminates filter-feeding shellfish such as oysters. After ingestion of contaminated oysters, predisposed people may experience highly lethal septicemia. Contamination of wounds with the bacteria can result in devastating necrotizing fasciitis, which can progress to septicemia. The extremely rapid progression of these diseases can render antibiotic treatment ineffective, and death is a frequent outcome. In this study, we examined the potential use of bacteriophages as therapeutic agents against V. vulnificus in an iron-dextran-treated mouse model of V. vulnificus infection. Mice were injected subcutaneously with 10 times the lethal dose of V. vulnificus and injected intravenously, either simultaneously or at various times after infection, with phages. Treatment of mice with phages could prevent death; systemic disease, as measured by CFU per gram of liver and body temperature; and local disease, as measured by CFU per gram of lesion material and histopathologic analysis. Two different phages were effective against three different V. vulnificus strains with various degrees of virulence, while a third phage that required the presence of seawater to lyse bacteria in vitro was ineffective at treating mice. Optimum protection required that the phages be administered within 3 h of bacterial inoculation at doses as high as 10 8 PFU. One of the protective phages had a half-life in blood of over 2 h. These results demonstrate that bacteriophages have therapeutic potential for both localized and systemic infections caused by V. vulnificus in animals. This model should be useful in answering basic questions regarding phage therapy.

Bacteriophages (phages) are viruses of bacteria that can kill and lyse the bacteria they infect. After their discovery early in the 20th century, phages were widely used to treat various bacterial diseases in people and animals. After this enthusiastic beginning to phage therapy, problems with inappropriate use and uncontrolled studies and ultimately the development of antibacterials caused a cessation of phage therapy research in the West. However, a few institutions in Eastern Europe continued to study and use phages as therapeutic agents for human infections. The alarming rise in antibacterial resistance among bacteria has led to a review of the Eastern European studies and to the initiation of controlled experiments in animal models. These recent studies have confirmed that phages can be highly effective in treating many different types of bacterial infections. The lethality and specificity of phages for particular bacteria, the ability of phages to replicate within infected animal hosts, and the safety of phages make them efficacious antibacterial agents. Although there are still several hurdles to be overcome, it appears likely that phage therapy will regain a role in both medical and veterinary treatment of infectious diseases, especially in the scenario of emerging antibacterial resistance.

In a prospective 2-year study, serological responses to selected pathogens were analyzed in 224 episodes of fever attributable to respiratory tract infection (51.8%) or of unknown source (48.2%) in 131 residents of two long-term-care facilities. A serological response was identified in 45 episodes (20.1%): Chlamydia pneumoniae (14 episodes), Haemophilus influenzae type b (1), influenza virus type A (14), respiratory syncytial virus (RSV;2), parainfluenza virus type 3 (7), C. pneumoniae and H. influenzae (3), C. pneumoniae and influenza virus type A (2), C. pneumoniae and RSV (1), and C. pneumoniae and parainfluenza virus type 3 (1). No serological responses to Chlamydia psittaci, Chlamydia trachomatis, parainfluenza virus types 1 and 2, influenza virus type B, or Mycoplasma pneumoniae were seen. Vaccination did not affect the duration of fever in those residents with serologically confirmed influenza A. Serologically confirmed C. pneumoniae infection was detected in 9.4% of all febrile episodes. Serological responses to a second agent were detected in 33% of the patients with C. pneumoniae infections, and these dual infections were associated with an underlying malignancy (P = .02). C. pneumoniae should be recognized as a potential pathogen when choosing empirical antimicrobial therapy for respiratory tract infection in residents of long-term-care facilities.

purpose: To describe the epidemiology and characteristics of gross hematuria in elderly residents of nursing homes and to identify the associations of gross hematuria with urinary infection and the potential contribution of urinary infection to morbidity. patients and methods: This was a prospective, descriptive study of episodes of gross hematuria identified by the nursing staffs at two long-term-care facilities over 2 years. Episodes were characterized with respect to patient variables, presence of bacteriuria, duration of hematuria, therapeutic interventions, and genitourinary investigations. Clinical and serologic criteria were used to identify invasive infection. results: The incidence of gross hematuria was 31/100,000 resident days. Bacteriuria was present in 58 (74%) of 78 episodes with evaluable cultures. Fifty-two (61%) episodes lasted more than 24 hours, 25 (29%) were temporally associated with fever, and antimicrobials were given for 53 (61%) episodes. Gross hematuria occurred more frequently in men than in women and was more frequently associated with fever in men. Twenty-four (28%) episodes occurred in subjects with indwelling catheters, 30 (34%) in subjects with known genitourinary abnormalities, 26 (30%) in subjects with no genitourinary investigations, and 4 (4.6%) in subjects with genitourinary investigations but no abnormalities identified. No advene clinical outcomes were identified in patients in whom antimicrobial therapy was not initiated. The maximal estimated incidence of invasive urinary infection associated with hematuria was 5.8/100,000 resident days, and of bacterial hemorrhagic cystitis, 6.3/100,000 resident days. conclusions: These data suggest that underlying genitourinary abnormalities are present in most elderly institutionalized subjects with gross hematuria when genitourinary investigations are performed. Although bacteriuria is usually present, urinary infection, by itself, k an infrequent cause of gross hematuria. Afebrile hematuria without irritative symptoms probably does not require antimicrobial therapy. A standard approach to this clinical problem in the institutionalized elderly should be developed to optimize patient management and appropriate use of antimicrobial therapy.

A novel approach, utilizing a two-dimensional (2D) statistical correlation of mid- and near-infrared spectra, is presented as a means to assist with qualitative spectral interpretation. The method utilizes cross-correlation by least-squares to assess changes in both regions that result from changes in sample composition. The technique has been applied to complex agricultural samples that differ in wax (cuticle), carbohydrate, protein, and lignin content. Dispersive near-infrared (NIR) and interferometric mid-infrared (FT-IR) diffuse reflectance spectra were obtained on each of the samples, and point-for-point 2D cross-correlation was obtained. The technique permits the correlation of the combination and overtone region of the NIR to the fundamental vibrations in the mid-infrared (MIR) region. This allows the determination of the most probable source of NIR signals and verification of the “real” information content of the purely statistically derived signals whose intensities currently are used for quantitative analysis in this spectral region.

Over the past ten years, increasing attention has been given by both employers and researchers to the psychological problems which can be experienced by first-line disaster workers. These developments are briefly charted. Following a discussion of the nature of psychological trauma, the role of the professional counsellor is explored in connection with (a) preparing rescue and emergency staff for working in a disaster situation, (b) supporting them at the scene of a disaster, and (c) providing psychological assistance in the days and weeks after a disaster. Attention is then turned to the kind of preparation and self-management that might be called for in the counsellors who offer such support.

Over the past 10 years, researchers and employers have shown an increased interest in the psychological problems that can be experienced by first-line disaster workers. This article briefly reviews available research in the area, and then illustrates the increased management concern over these problems by describing recent developments in the UK Police Service. Disaster contingency planning in this Service has come to place particular emphasis upon the provision of crisis intervention after potentially traumatic disaster work experiences: early, brief, problem-focused treatment, which aims to restore distressed individuals to a precrisis level of functioning. The possible benefits of such early intervention are considered, and then attention is turned to the types of information required for guiding intervention in specific cases. Beyond basic information relating to current symptoms and coping activities, it will be useful to determine what experiences from before, during and after the disaster work are implicated in the person's current disturbance, and how the person is modelling these experiences. It will also be important to identify those contemporaneous, psychological appraisal processes that are sustaining the disturbance: individuals who are left to adjust ‘naturally’ typically pay little or no attention to these powerfully disturbing, but in principle modifiable appraisal processes.

Psychologically traumatic experiences can arise out of apparently normal, everyday situations, can affect perfectly ordinary working people, and may only involve one or two individuals. Four case illustrations are given of unsuspecting employees who found their lives thrown into extreme turmoil by the experience of a traumatic event. These included a railway suicide, an acid‐throwing attack, an explosion in an apartment block, and a rape by a work colleague. The nature of psychological trauma is examined, and then the benefits of early intervention in such cases, and the need for employers to provide a counselling service which is sophisticated enough to cater for problems of this kind is considered.

This pilot study was undertaken to characterise the pharmacokinetics, pharmacodynamics and potential clinical efficacy of levofloxacin 750 mg once daily for 5 days for treatment of women with acute uncomplicated pyelonephritis. Four women diagnosed with acute pyelonephritis were enrolled. Following pre-therapy specimen collection, an initial oral dose of 750 mg levofloxacin was administered. The mean pharmacokinetic parameters for the first dose were: maximum serum concentration (C(max)) 12.5+/-4.7 mg/L (range 5.6-16.0mg/L) (fC(max) 8.8+/-3.3, where f indicates the levofloxacin free or non-protein-bound fraction), area under the serum concentration-time curve (AUC) 85.4+/-14.1 mgh/L (range 66.2-96.8 mgh/L) (fAUC 59.8+/-9.9) and serum half-life (t(1/2)) 6.7+/-0.5h. Mean urine concentrations were 88.0+/-100mg/L at the 0-3 h collection, 307+/-143 mg/L at 3-6 h, 170+/-107 mg/L at 6-12 h and 85+/-8 mg/L at 12-24 h. Mean levofloxacin serum pharmacodynamics for infecting Escherichia coli were: C(max)/minimum inhibitory concentration (MIC) 323+/-185(fC(max)/MIC 226+/-129); and AUC/MIC 2339+/-830(fAUC/MIC 1647+/-579). Mean urine levofloxacin concentration/MIC ratios were: 900+/-1389 for 0-3 h, 12100+/-4950 for 3-6 h, 5922+/-3912 for 6-12 h and 2233+/-1037 for 12-24 h. Levofloxacin eradicated E. coli from the urine by 3-6 h after the first dose. Levofloxacin 750 mg once daily for 5 days has pharmacodynamics that support further evaluation of this regimen for treatment of women with acute uncomplicated pyelonephritis.

Single, high dose regimens of gentamicin plus metronidazole for colorectal surgical prophylaxis have not been adequately studied. Patients received single high dose gentamicin (4.5 mg/kg) plus metroni-dazole (500 mg) preoperatively or multiple standard dose gentamicin (1.5 mg/kg) plus metronidazole (500 mg) preoperatively and every 8h for 24h postoperatively. The deep surgical site infection (SSI) rates were 8.1% (6/74) and 6.9% (5/72) in the single high dose and multiple standard dose groups, respectively (P= 0.94). There was a trend towards fewer superficial SSIs in the single high dose group with infection rates of 18.9% (14/74) vs. 30.6% (22/72) (P= 0.05). Diabetes mellitus (odds ratio = 7.04) and surgery duration of longer than 3h (odds ratio = 5.46) were independent risk factors for the development of SSIs. A subset analysis of prolonged operations found significantly fewer superficial SSIs in the single high dose group than in the multiple standard dose group with rates of 22.2% (6/27) vs. 55% (11/20), respectively (P= 0.021). Single high dose gentamicin plus metronidazole preoperatively was at least as effective as the multiple standard dose regimen and may be more effective for prolonged operations.

Particularly since the Bradford football stadium fire of May 1985, the UK Police Service has been developing increasingly sophisticated procedures for managing post-traumatic stress reactions in its officers. Coupled with the growth of dedicated occupational health units within the Police Service, this suggests that physicians working within police forces will have an increasingly important role to play in the management of such problems. A brief account is given of the confidential screening and counselling service for police officers that was instituted after the Bradford fire, and a corresponding description is given of the more elaborate procedures implemented after the Hillsborough football stadium crush disaster of April 1989. In conclusion, the possible scope for preventive management of Post-traumatic stress disorder (PTSD) is highlighted.

OBJECTIVE: In a previous study, elevated urine antibody levels in institutionalized subjects with asymptomatic bacteriuria were associated with decreased survival. This study was undertaken to confirm the previous observation in a prospective study in a larger cohort and to explore selected other variables that may be associated with survival. DESIGN: A prospective, 24-month, observational cohort study. SETTING: Three large nursing homes in Winnipeg, Manitoba. PARTICIPANTS: Permanent residents were identified by initial screening urine cultures and subjects with bacteriuria were enrolled. The median age of subjects was 76 years, 51% were women, and the median duration of residence before enrollment was 26 months. Subjects were highly functionally impaired. MEASUREMENTS: Monthly urine specimens were collected for culture, leukocyte count, and urine antibody. Serum specimens for antibody to uropathogens and IL6 were obtained at enrollment and every 6 months. Anthroporphometric tests of nutritional status and functional and mental status were also measured every 6 months. Residents were stratified as having elevated or not elevated urine antibody, based on the initial urine specimen. The mean urine antibody for all of each subject's specimens was also calculated, and subjects were stratified as low, intermediate, or elevated urine antibody. Outcomes measured included mortality, infection and antibiotic use, and functional, mental, and nutritional decline. RESULTS: Ninety-eight bacteriuric subjects were enrolled in the study; 34 (35%) had elevated urine antibody and 64 (65%) had not elevated urine antibody. This chapter discusses spectroscopic quantitative analysis. Chemometrics means performing calculations on measurements of chemical data. This can be anything for calculating pH from a measurement of hydrogen ion activity to computing a Fourier transform interpolation of a spectrum. The chapter shows some of the basic tenets of chemometric spectral analysis methods. The chapter provides a basic understanding of mathematics to apply them for solving analytical problems. Classical methods of quantitative analysis are relatively straightforward in that they use well-understood relationships between the independent data (spectral absorbances) and the dependent data (concentrations). Some methods are univariate—that is, they solve only one equation based on one measured value and have one calibration value per sample, whereas others are multivariate—that is, they solve a series of equations using many measurements per sample for one calibration value. The calculated eigenvectors came from the original calibration data, they must somehow relate to the concentrations of the constituents that make up the samples.

purpose: Bacteriuria is common among institutionalized elderly populations, but the contribution of urinary infection to febrile morbidity is unknown because of difficulties in clinical ascertainment. This study was undertaken to measure the contribution of urinary infection to febrile morbidity using both clinical and serologic criteria. methods: Episodes of fever in residents of two long-term care institutions were identified prospectively for 2 years. Serum and urine specimens were obtained initially and at 4 weeks. The proportion of episodes attributable to urinary infection was determined by both standard clinical criteria proposed for use in these populations and serum antibody response to uropathogens. results: For 372 fever episodes, 211 met clinical criteria for infection: 147 (40%) of the respiratory tract; 26 (7%) of the genitourinary tract; 25 (6%) of the gastrointestinal tract; and 13 (3%) of skin and soft tissue. Of the remaining 161 fever episodes, 2 (1%) were noninfectious and 159 (43%) were of unknown origin. The prevalence of bacteriuria for residents with nongenitourinary sources of fever varied from 32% to 75%. An antibody response meeting serologic criteria for urinary infection occurred in 26 (8.3%) of 314 episodes with paired sera obtained; 10 (43%) of 23 identified clinically as genitourinary infection, 14 (11%) of 132 unknown, 1 (4%) of 25 gastrointestinal, and 1 (0.8%) of 122 respiratory. The positive predictive value of bacteriuria for febrile urinary infection identified by clinical criteria was 11% (95% confidence interval [CI] 4%, 18%) and identified by serologic criteria was 12% (95% CI 7%, 17%). conclusions: Urinary infection contributes to less than 10% of episodes of clinically significant fever in this high-prevalence bacteriuric population. A restrictive clinical definition for genitourinary infection has poor sensitivity and specificity compared with serologic criteria for identification of fever of urinary source, and bacteriuria has a low predictive value for identifying febrile urinary infection.

The incidence and prevalence of decubitus ulcers, and their complications and microbiology were studied prospectively in two large long term care facilities in Winnipeg, Manitoba between January 1, 1989 and December 30, 1990. The initial prevalence of decubitus ulcers was 2.6 and 1.6% at the two institutions, with an incidence of 3.4 and 4.8 per 100,000 resident days, respectively. The incidence of decubitus ulcer infection was 1.4 per 1000 ulcer days. The only other complication identified was in one resident who required an indwelling catheter to permit ulcer healing. An average of 2.4 organisms grew from surface swabs of ulcers; anaerobes were isolated from 14% of cultures. Aspirates from clinically noninfected ulcers had bacteria isolated in 30% of specimens. Two-thirds of organisms isolated were considered potentially pathogenic. Concurrent bacteriuria was present for 75% of sampling episodes. Organisms present in the urine were simultaneously isolated from decubiti in only 5% of specimens. Decubitus ulcers are uncommon in long term care institutions. The urinary tract of the bacteriuric elderly appears to be an infrequent source of organisms colonizing decubiti.

Glutamine has been shown to be an important metabolic substrate of enterocytes in many animals, including cats, dogs, hamsters, human beings, monkeys, rabbits, rats, and sheep. To determine whether glutamine is important in the metabolism of cells of the equine gastrointestinal tract, we examined transintestinal differences in glutamine concentrations in the arterial and venous circulation, and measured activity of the major glutamine catabolizing enzyme, glutaminase. Arteriovenous differences provide an index of the amount of a given substrate removed by the tissue across which the measurements are made, and commonly are expressed as a percentage of substrate removed, or percent extraction. Arteriovenous differences for glutamine were determined in 7 anesthetized adult horses (weight, 450 to 500 kg) before and after an i.v. glutamine infusion. The mean baseline arterial glutamine concentration (+/- SEM) was 572 +/- 24 microM; this concentration quadrupled (to 2,167 +/- 135 microM, P less than 0.01) 1 minute after i.v. bolus infusion of a 17.5-g glutamine load. Baseline extraction by the portal-drained viscera was 7.5 +/- 1.5%; this value increased to 18 +/- 2% at 1 minute (P less than 0.01) and had returned to baseline values 60 minutes later. Arteriovenous differences were greatest across the jejunum (11.8 +/- 1.8% in the baseline period vs 33.1 +/- 3.1% at 1 minute, P less than 0.001), with smaller differences across the colon, suggesting that the jejunum was the more avid utilizer of glutamine. Glutaminase activity was 4.38 +/- 0.16 and 4.00 +/- 0.60 mumol/mg of protein/h under standard conditions in jejunal and ileal mucosa, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

An improved test fixture for biaxial-tension strength testing of ceramics featuring uniform pressure loading of disks was developed and qualified. Biaxial data were obtained for an alumina ceramic, along with comparable uniaxial data from three- and four-point flexure tests. Weibull statistics provided a good description of the size effect on data from the two uniaxial tests, but underestimated the effect of stress biaxiality. The biaxial strengthening observed in the alumina ceramic is consistent with that observed previously in a glass-ceramic.

Bend-test curves were calculated for materials obeying three very different damage laws. One result of the calculations is that the neutral axis migrates towards the compression face as the load-point displacement increases. For the three damage laws investigated, there is a simple relationship between the location of the neutral axis and the secant modulus derived from the bend-test record; this relationship is quite insensitive to the exact form of the damage law. The stress at a point 20 per cent of the beam depth from the tensile outer fibre is also fairly insensitive to the form of the damage law. Combining these two observations, it is possible to use bend-test data to make a good estimate of the tensile stress-strain curve of a material subject to damage.

Summary: The structures of the colicin Ib plasmid (ColIb), the delta transfer factor and a plasmid determining kanamycin resistance and colicin Ib production called KColIb, were compared. Radiolabelled mini-ColIb plasmids and isolated DNA fragments of ColIb were used as probes for nitrocellulose blots of digests of the other two large plasmids. The structure of delta was consistent with it having one large deletion of about 10 MDa in the SB fragment and two insertions of approximately 6 MDa and 12 MDa in the SB and SA fragments of the ColIb plasmid. It was hypothesized that KColIb had six small insertions in SA, SB, SE and near the junction of the SB and SD fragments. However, ColIb, KColIb and delta were homologous for at least 70% of their lengths. The highly conserved regions in the three plasmids were the regions that corresponded to fragments SA, SC and SD of ColIb. In addition, delta and KColIb differed from ColIb at similar sites. The possible evolution of these plasmids is discussed.

After infection of Escherichia coli with bacteriophage T5, a major new protein species, as indicated by polyacrylamide-gel electrophoresis, appears in the whole membrane fraction of the infected cells. Ultraviolet-irradiated phage do not induce the synthesis of this protein. The time course of synthesis of the protein as well as its appearance in cells infected in the presence of chloramphenicol and then blended and in infected cells containing the Col Ib factor are consistent with it being a pre-early protein; i.e., a protein induced by the spontaneously injected 8% piece of T5-DNA, the transcription and translation of which is necessary for the entry of the whole DNA molecule into the cell. The protein is found in both inner and outer membrane fractions of E. coli but appears as only one band in two gel systems. Some evidence indicates that the protein exists in a soluble form in the cytoplasm prior to incorporation into the cell membrane and that it can be incorporated into a membrane fraction in an in vitro system.

An investigation was made of strength degradation caused by moisture-assisted slow crack growth of surface flaws in two dense polycrystalline ceramics. Specimen sizes were varied. The observed strength degradation of each ceramic was predictable usingK IC as a material property in fracture-mechanics relations, suggesting that the only material variable involved was critical flaw size. The strength of one of the ceramics in water decreased significantly with increased specimen size, but its Weibull modulus was essentially unaffected by specimen size and slow crack growth.

Strains of Escherichia coli K-12 containing the colicin Ib (Col Ib) factor did not produce progeny phage when infected by T5 bacteriophage. The cells were killed but did not lyse. If sodium dodecyl sulfate (SDS) was added to T5-infected E. coli (Col Ib), lysis occurred prematurely, but no phage were produced. SDS had no effect on infected cells that did not contain the Col Ib factor or on uninfected cells with or without the Col Ib factor. Cells that contained a mutant Col Ib factor that allowed phage production were not prematurely lysed after infection in the presence of SDS. When the Col Ib-containing cells were infected, protein and RNA synthesis stopped at about 10 min postinfection, and the cells released abnormal amounts of 32P-containing material, ATP, and beta-galactosidase into the medium. They also became inhibited in their ability to accumulate thiomethyl-beta-D-galactopyranoside and to utilize glycerol. Two alternative hypotheses are presented to explain these results.

A case study is presented of a problem-solving exercise with UK police officers who experienced psychological problems after their duties in connection with the Bradford fire disaster of May 11 1985. It is argued that they typify the ‘forgotten victims’ of disasters: official helpers of one kind or another whose experiences may lead to significant psychological disturbance. Out of the 399 officers who had been involved with the fire, 234 (59 per cent) completed a screening questionnaire and 34 attended a series of confidential counselling sessions. Their specific problems included performance guilt, reconstruction anxiety, generalized irritability, focused resentment and motivational changes. Treatment interventions were tailored to these problems. A second administration of the screening questionnaire three weeks after counselling, and a follow-up approximately nine months after the disaster, indicated a satisfactory level of recovery in the officers.

Bend strengths of two sizes of specimens of an alumina ceramic were determined under conditions that either enhanced or restricted sub critical crack growth. The specimen sizes differed in each linear dimension by a factor of five and the effective size of small specimens was varied by the use of both 3- and 4-point bending. Strength was dependent on specimen size under both crack-growth conditions, and the dependence was associated with variations in flaws at fracture origins. The experimental data were analyzed for applicability of Weibull's statistical relations and in terms of fractographic examinations of strength-controlling flaws.

Mixed-mode fracture of soda-lime glass was studied using a diametral-compression test that features disk specimens with symmetric through-cracks. The test enables one to study fracture under pure mode I loading, pure mode II loading, or any combination of mode I and mode II loading by a simple alignment of the crack relative to the diameter of compression loading. The disk specimens were precracked with the aid of both chevron notches and water-assisted subcritical crack growth. The directions of noncoplanar crack extensions and the relative magnitudes of mode I and mode II stress-intensity factors for mixed-mode fracture under inert conditions were compared to the predictions of three different mixed-mode fracture theories. None of the theories was completely adequate to explain the experimental observations, but a maximum hoop stress criterion modified to include second order, nonsingular term in the series solution for the crack-tip region stress gave reasonable agreement with the experimental results.

When Escherichia coli is infected with bacteriophage BF23, two new proteins with molecular weights greater than 10,000, as indicated by polyacrylamide gel electrophoresis, are found associated with the cells' membranes. One of these, found associated with both the inner and outer membrane, has a molecular weight of about 55,000 and is regulated by the A1 gene of this phage, a gene found on the spontaneously injected 8% piece of BF23 DNA, DNA that codes for the synthesis of proteins necessary for the injection of the whole phage genome. The other protein, often undetected in whole membrane preparations, is found exclusively associated with the inner membrane. Evidence indicates that this protein is also regulated by the initially injected 8% piece of the DNA.

Uniaxial and equibiaxial tensile strengths are reported for glass-ceramic specimens exhibiting strength isotropy. Uniaxial strengths were determined in both 3- and 4-point bend tests. Stressing rate was a controlled variable and the tests were conducted in both dry N2 and water environments, to provide data for different conditions of slow crack growth. There was little or no difference between strengths in the 3- and 4-point bend tests, indicating the absence of a size effect. In comparable tests, the strength under equibiaxial tension was greater than under uniaxial tension. The biaxial strengthening became less pronounced with increased slow crack growth during testing. A maximum observed biaxial-to-uniaxial strength ratio of 1.21 resulted from ball-on-ring and uniaxial bend tests in dry N2. These results are attributed to differences in flaw severity between biaxial and uniaxial stressing. The decreasing strength ratios can be explained by a change in configuration of critical flaws due to slow crack growth. Numerical calculations, using a simplified two-dimensional model of the strength-controlling flaw, supported this hypothesis. The calculations also indicate that the slow crack growth exponent is smaller for the biaxial tension mode of stressing than for the uniaxial mode.

Mechanical tests of most ceramics are subject to stringent requirements, more so than tests of materials which possess some ability to undergo plastic deformation. In this paper, sources of stress concentrations and spurious displacements which may be imposed during four-point bend tests of ceramic materials are identified and discussed. Particular areas of concern involve unequal moments at the loading points, twisting due to skewness of the contact lines, wedging stresses at point contacts, and counter moments produced by friction at the loading point-specimen interface. A bend fixture incorporating design features to significantly reduce these effects is described and results of measurements of the surface stresses in calibration specimens under load in the fixture are presented. A cleavage site map of the colicin Ib plasmid (ColIb) has been determined for the enzymes Sall, XhoI, and HindIII by analysis of partial digests, double digests, DNA-DNA hybridization, and Tn5-induced insertion mutants. The site of the colicin gene has been determined by probing with cloned DNA coding for colicin production, as well as by analysis of a colicin negative ColIb:Tn5.

A programme was developed for training people to apply generic problem-solving techniques to their personal problems. These are basic problem-solving operations which, in principle, should be applicable to any specific problem. Sixty-four male student volunteers were divided into sets of internals and externals, on the basis of pre-test scores on the Rotter Internal-External (I-E) scale. They were assigned at random to a training group and a no-training control group, and subjects in the training group attended a five-week programme. Dependent measures included the standard of bachelor's degrees gained by subjects at graduation, the I-E scale, and the neuroticism scale of the Eysenck Personality Inventory. The two scales were administered one week and eight weeks after the programme. Results demonstrated that the training group achieved a higher standard in their bachelor's degrees, developed an increased belief in internal control and became more emotionally stable. There was no evidence of an interaction between locus of control and the effects of the programme.

After infection of Escherichia coli B by bacteriophage T5, a major new protein species, as indicated by polyacrylamide gel electrophoresis, appears in the cells' membranes. Phage mutants with amber mutations in the first-step-transfer portion of their DNA have been tested for their ability to induce membrane protein synthesis after they infect E. coli B. We have found that phage with mutations in the Al gene of T5 do not induce the synthesis of the T5-specific major membrane protein, whereas phage that are mutant in the A2 gene do induce its synthesis. We conclude that gene Al must function normally for T5-specific membrane protein biosynthesis to occur and that only the first 8% (first-step-transfer piece) of the DNA need be present in the cell for synthesis to occur.

When T5 bacteriophage infects Escherichia coli B, 42 K + is immediately released from cells that have been preloaded with this ion. The rate of ion release and the total amount released are dependent on the multiplicity of infection and are not diminished by the use of mutants which can only inject 8% of their DNA. Normally, the ion release stops at about 6 min postinfection. If the host cells contain the colicinogenic factor, Col Ib, so that the infection is abortive, K + release continues. Evidence is presented to show that this continued ion release cannot be explained by a “damage and repair” hypothesis. The results are, however, consistent with the interpretation of membrane depolarization due to ion pore formation as the cause of the abortive infection.

Fractographically observed critical-flaw boundaries in strength-tested specimens of 2 polycrystalline ceramics were used in calculating critical stress-intensity factors (KIC)- Each ceramic exhibited a KIC which had little or no dependence on critical-flaw characteristics and which agreed with the value obtained from independent determinations on artificially precracked specimens. Analyses both of fracture-mirror sizes and of water-enhanced subcritical crack growth data supported the evidence that K1C is a material constant. The fracture-mirror analysis further indicated that the parameter A/KIC, where A is the fracture-mirror constant, is a dimensionless, material-independent constant.

An investigation of the fundamentals of the adherence of porcelain enamels to metals indicated that good adherence is the result of metal-to-metal bonds between the atoms in the base metal and the proper metallic ions in the enamel. To accomplish this type of bond, the enamel must meet certain chemical and thermodynamic requirements: (1) The enamel at the interface must be saturated with an oxide of the metal and (2) this oxide must be one which, when in solution in the glass, will not be reduced by the metal. In the case of iron, the oxide is FeO. Many of the phenomena observed in commercial enameling were investigated and found to be related to adherence, but not essential for its development. An example is the precipitation of metallic particles in the enamel. Much of the complexity in commercial enameling arises from the limitations imposed by practical considerations. For example, because enamels usually are fired in air, the heavy scale developed during the early stages of firing must be removed before adherence can be developed. Likewise, as the conditions of the enamel-metal interface change rapidly during firing, “adherence-promoting oxides” are used to help maintain the necessary conditions for the time required in commercial enameling. Surface roughness, although not necessary for excellent adherence, was found to improve the apparent adherence when the bond between the enamel and the metal was relatively weak.
