1. In-vivo blood pressure sensing with bi-filler nanocomposite

Abstract

Conductive elastomers present desirable qualities for sensing pressure in-vivo, such as high piezoresistance in tiny volumes, conformability and, biocompatibility. Many PDMS-based electrically conductive nanocomposites however, are susceptible to electrical drift following repeated stress cycles and chemical aging. Innovative approaches are needed to stabilize their percolation network against deformation to improve reproducibility and facilitate sensor calibration. One approach we propose here is to decouple the tunnelling-percolation network of HOPG nanoparticles from the incomplete viscoelastic recovery of the PDMS matrix by inserting minute amounts of insulating SiO$_2$ nanospheres. SiO$_2$ nanospheres effectively reduce the number of nearest neighbours at each percolation node switching off the parallel electrical pathways that might become activated by incomplete viscoelastic relaxation. We varied the size of SiO$_2$ nanospheres and their filling fraction to demonstrate nearly complete piezoresistance recovery when SiO$_2$ and HOPG nanoparticles have equal diameters ($\approx$400nm) and SiO$_2$ and HOPG volume fractions are 1% and 29.5% respectively. We demonstrate an in-vivo blood pressure sensor based on this bi-filler composite.

Abstract

Cardiovascular diseases represent a major cause of morbidity and mortality, necessitating research to improve diagnostics, and to discover and test novel preventive and curative therapies. All of which warrant experimental models that recapitulate human disease. The translation of basic science results to clinical practice is a challenging task. In particular for complex conditions such as cardiovascular diseases, which often result from multiple risk factors and co-morbidities. This difficulty might lead some individuals to question the value of animal research, citing the translational 'valley of death', which largely reflects the fact that studies in rodents are difficult to translate to humans. This is also influenced by the fact that new, human-derived in vitro models can recapitulate aspects of disease processes. However, it would be a mistake to think that animal models cannot provide a vital step in the translational pathway as they do provide important pathophysiological insights into disease mechanisms particularly on a organ and systemic level. While stem cell-derived human models have the potential to become key in testing toxicity and effectiveness of new drugs, we need to be realistic, and carefully validate all new human-like disease models. In this position paper, we highlight recent advances in trying to reduce the number of animals for cardiovascular research ranging from stem cell-derived models to in situ modelling of heart properties, bioinformatic models based on large datasets, and improved current animal models, which show clinically relevant characteristics observed in patients with a cardiovascular disease. We aim to provide a guide to help researchers in their experimental design to translate bench findings to clinical routine taking the replacement, reduction and refinement (3R) as a guiding concept. Abstract

The diagnostic characteristics of electromechanical mapping (EMM) were evaluated in porcine myocardial infarction (MI) models with the parallel application of cardiac magnetic resonance imaging (cMRI) from the aspect of different pathophysiology and localization. Balloon occlusion in the left anterior descending coronary artery (LAD balloon group) or coil deployment in the LAD (LAD coil group) or circumflex artery (Cx coil group) was applied percutaneously in 16 domestic pigs. Regional left ventricular viability data were captured via cMRI and EMM. The unipolar voltage (UV) value was significantly decreased in segments containing transmural and subendocardial late enhancement compared with viable segments in the LAD balloon, LAD coil, and Cx coil groups. Receiver operating characteristic analysis revealed area under the curve values of 0.809 and 0.691 in the LAD infarct territory, and 0.864 and 0.855 in the Cx infarct territory for the UV compared with cMRI viability results as transmural late enhancement or viable tissue and subendocardial late enhancement or viable tissue, respectively. In conclusion, the UV value detected the presence of scar tissue with differential transmural extent and which represented proper diagnostic features both in the reperfused and nonreperfused models. This data could provide additional benefit in the clinical use of EMM for diagnostic purposes. Les caracteristiques diagnostiques de la cartographie electromecanique (CEC) ont ete evaluees dans des modeles d'infarctus du myocarde (IM) chez le porc, parallelement a l'imagerie en resonance magnetique cardiaque (cIRM) afin d'evaluer l'aspect et la localisation de differentes pathophysiologies. L'occlusion par ballon de l'artere coronaire descendante anterieure gauche (DAG) (groupe DAG ballon) ou par deploiement d'un serpentin dans la DAG (groupe DAG serpentin) ou dans l'artere coronaire circonflexe (groupe Cx serpentin) a ete realisee de maniere sous-cutanee chez seize porcs domestiques. Les donnees de la viabilite regionale ventriculaire gauche ont ete captees par cRIM et par CEC. La valeur du voltage unipolaire (VU) etait significativement diminuee dans les segments comportant un rehaussement tardif transmural et subendocardique comparativement aux segments viables des groupes DAG ballon, DAG serpentin et Cx serpentin. L'analyse des caracteristiques receveur operant a revele une aire sous la courbe de 0,809 et 0,691 dans la zone d'infarctus de la DAG, et de 0,864 et 0,855 dans la zone d'infarctus de la Cx en VU comparativement aux resultats de la viabilite en cIRM correspondant au rehaussement tardif transmural, ou au tissu viable en presence de rehaussement tardif subendocardique, ou au tissu viable, respectivement. Abstract

Cardioprotection; Drug development; Infarction Cardioprotección; Desarrollo de fármacos; Infarto Cardioprotecció; Desenvolupament de fàrmacs; Infart Acute myocardial infarction (AMI) and the heart failure (HF) which may follow are among the leading causes of death and disability worldwide. As such, new therapeutic interventions are still needed to protect the heart against acute ischemia/reperfusion injury to reduce myocardial infarct size and prevent the onset of HF in patients presenting with AMI. However, the clinical translation of cardioprotective interventions that have proven to be beneficial in preclinical animal studies, has been challenging. One likely major reason for this failure to translate cardioprotection into patient benefit is the lack of rigorous and systematic in vivo preclinical assessment of the efficacy of promising cardioprotective interventions prior to their clinical evaluation. To address this, we propose an in vivo set of step-by-step criteria for IMproving Preclinical Assessment of Cardioprotective Therapies (‘IMPACT’), for investigators to consider adopting before embarking on clinical studies, the aim of which is to improve the likelihood of translating novel cardioprotective interventions into the clinical setting for patient benefit. To examine the relationship between plasma cytokine levels and cardiac and hemodynamic function. Design. Measurement of cytokines and the systolic (left ventricular dimensions, heart rate, and cardiac output) and diastolic (early and late transmitral peak flow velocity: E and A-waves and their ratio) functions of the left ventricle (assessed by echocardiography) in rabbits. Setting and interventions. Laboratory and echocardiographic analyses were performed at baseline and at 1, 3, 6, 12, and 24 h after acute necrotizing pancreatitis induction (Group ANP), in rabbits after somatostatin pretreatment (Group S-ANP) and in sham-operated controls (Group C). Measurements and results. Left ventricular dilatation occurred at 6 h and cardiac output was increased 3 h after induction of acute necrotizing pancreatitis. Somatostatin pretreatment mitigated the left ventricular enlargement and filling abnormalities. Plasma level of IL-6 was increased significantly 3 h after pancreatitis induction, but to a lesser extent in Group S-ANP, while somatostatin prevented TNF[alpha] release (IL-6: Group ANP: 0, 0, 518+-139, 956+-125, 373+-48, and 122+-37 pg/ml Group S-ANP: 0, 0, 191+-68, 261+-49, 23+-13, and 0 pg/ml TNF[alpha]: Group ANP: 88+-42, 371+-40, 2963+-291, 276+-30, 197+-106, and 23+-14 U/l Group S-ANP: 91+-34, 41+-25, 68+-42, 25+-9, 0, and 0 U/ml). The increase in plasma level of IL-6 correlated significantly with left ventricular end-diastolic diameter and volume, cardiac output, and diastolic dysfunction. Conclusions Plasma levels of IL-6, but not TNF[alpha] correlate with cardiac output and left ventricular filling characteristics in acute pancreatitis. Abstract

Ischemic heart disease including myocardial infarction develops on the basis of several risk-factors and comorbidities such as obesity, diabetes, hypertension, and hypercholesterolemia. Ischemic heart disease is the leading cause of mortality worldwide, therefore, identification of novel drug targets for cardioprotection is of great importance. Ischemic preconditioning, postconditioning, and remote conditioning trigger endogenous cardioprotective mechanisms that render the heart more resistant to lethal ischemic-reperfusion injury. However, major cardiovascular co-morbidities such as hyperlipidemia, diabetes, and their co-medications interfere with these cardioprotective mechanisms thereby limiting the efficacy of cardioprotective ischemic conditioning maneuvers. Ischemia reperfusion injury and cardioprotection by conditioning have been shown to affect global myocardial gene expression profile at the transcript level. Further understanding and the comprehensive analysis of the cardioprotective gene expression fingerprint in normal, protected, and in comorbid conditions may lead to identification of novel molecular targets for cardioprotection.
