BackgroundEpstein-Barr virus (EBV) specific T-cell response measurement can help adjust immunosuppression in transplant patients with persistent infections. We aim to define T-cell responses against EBV in a cohort of pediatric liver-transplant patients.MethodsThirty-eight immunosuppressed pediatric liver-transplant patients (IP) and 25 EBV-seropositive healthy-adult controls (HC) were included in our cross-sectional study. Based on their EBV serological (S) and viral load (VL) status, patients were categorized into IP-SNEG, IP-SPOSVLNEG and IP-SPOSVLPOS groups. T-cell response was assessed at two timepoints by stimulating cells with EBV peptides (PepTivator®) and performing intracellular-cytokine and activation-induced marker staining. Background subtraction was used to determine EBV-specific T-lymphocyte frequency.ResultsPolyfunctional CD8+ T cells indicated previous EBV contact (IP-SNEG 0.00% vs IP-SPOS 0.04% and HC 0.02%; p=0.001 and p=0.01, respectively). Polyfunctional CD8+CD107a+IFNɣ+IL2-TNFα- profile was increased in serology-positive (IP-SNEG 0.01% vs IP-SPOS 0.13% and HC 0.03%; p=0.01 and p=0.50, respectively) and viral-load positive (IP-SPOSVLPOS 0.43% vs IP-SPOSVLNEG 0.07% and HC 0.03%; p=0.03 and p=0.001, respectively) patients. Central-memory cells were increased among serology-positive adults (IP-SNEG 0.00% vs IP-SPOS 0.13% and HC 4.33%; p=0.58 and p=0.002, respectively). At the second timepoint, IP-SNEG patients remained negative (first visit 0.01% vs second visit 0.00%, p=0.44). On the other hand, IP-SPOSVLPOS patients had cleared viral loads and, subsequently, decreased polyfunctional CD8+CD107a+IFNɣ+IL2-TNFα- cells (first visit 0.43% vs second visit 0.10%, p=0.81).ConclusionPolyfunctional CD8+ EBV-specific T-cell response allows detecting EBV previous contact in liver-transplant children. %CD8+CD107a+IFNɣ+IL2-TNFα- is increased in patients with positive viral loads. Central memory CD4+ T-cell population more effectively determines prior EBV-exposure in adults.

BackgroundWildland firefighters (WFFs) regularly face demanding physical and environmental conditions during their duties, such as high ambient temperatures, challenging terrains, heavy equipment and protective gear. These conditions can strain thermoregulatory responses, leading to increased fatigue and posing risks to their health and safety. This study examined the effectiveness of two cooling interventions during physical activity in hot environments.MethodsEight active male WFFs participated, comparing the effects of wearing a cooling vest (VEST) and personal protective equipment removal (PASSIVE) against a control condition (PPE). Participants walked on a treadmill at a speed of 6 km·h−1 for approximately 75-min under hot conditions (30°C and 30% relative humidity). Incremental slope increases were introduced every 15 min after the initial 20 min of activity, with 5-min passive recovery between each increment. 6. Occupational Injuries of Spanish Wildland Firefighters: A Descriptive Analysis

The work of wildland firefighters, especially of the so-called ‘Brigadas de Refuerzo contra Incendios Forestales’, is characterised by high physical demands and extreme operating conditions. These professionals face long workdays (12 h), walking with heavy loads (~25 kg), being exposed to high temperatures (>30 °C), and handling specialised tools in high-risk environments. This study aimed to describe the prevalence of occupational injuries among members of the ‘Brigadas de Refuerzo contra Incendios Forestales’ and its relationship to variables such as age and work experience. A total of 217 wildland firefighters (18 female and 199 male) correctly answered a questionnaire developed on an ad hoc basis to meet the study’s objectives. A high prevalence of occupational injuries was observed among them (~76%). Age and work experience were shown to be significantly associated with injuries. Individuals over 35 years of age with more than 10 years’ experience had a higher probability of injury (OR = 2.14, CI = 1.12–4.06 and OR = 2.46, CI = 1.30–4.67, respectively). Injuries occurred mainly during physical training (~46%), followed by preventive work (~33%) and forest fires (~20%). The most common injuries were tendonitis and muscle pain (~44% and ~21% respectively), followed by sprains (~21%). The results underline the need for physical activity programmes adapted to help wildland firefighters, especially older and more experienced individuals. The identification of risk factors such as age and work experience can contribute to the prevention and management of occupational injuries among this group of highly specialised forestry workers. Specific preventative measures during training are required to mitigate the risk of injury among these crews, who play a crucial role in protecting the environment and public safety.

Glioblastoma (GB) stands out as the most prevalent and lethal form of brain cancer. Although great efforts have been made by clinicians and researchers, no significant improvement in survival has been achieved since the Stupp protocol became the standard of care (SOC) in 2005. Despite multimodality treatments, recurrence is almost universal with survival rates under 2 years after diagnosis. Here, we discuss the recent progress in our understanding of GB pathophysiology, in particular, the importance of glioma stem cells (GSCs), the tumor microenvironment conditions, and epigenetic mechanisms involved in GB growth, aggressiveness and recurrence. The discussion on therapeutic strategies first covers the SOC treatment and targeted therapies that have been shown to interfere with different signaling pathways (pRB/CDK4/RB1/P16ink4, TP53/MDM2/P14arf, PI3k/Akt-PTEN, RAS/RAF/MEK, PARP) involved in GB tumorigenesis, pathophysiology, and treatment resistance acquisition. Below, we analyze several immunotherapeutic approaches (i.e., checkpoint inhibitors, vaccines, CAR-modified NK or T cells, oncolytic virotherapy) that have been used in an attempt to enhance the immune response against GB, and thereby avoid recidivism or increase survival of GB patients. Finally, we present treatment attempts made using nanotherapies (nanometric structures having active anti-GB agents such as antibodies, chemotherapeutic/anti-angiogenic drugs or sensitizers, radionuclides, and molecules that target GB cellular receptors or open the blood–brain barrier) and non-ionizing energies (laser interstitial thermal therapy, high/low intensity focused ultrasounds, photodynamic/sonodynamic therapies and electroporation). The aim of this review is to discuss the advances and limitations of the current therapies and to present novel approaches that are under development or following clinical trials.

Actualmente nos encontramos en una situación de crisis sanitaria mundial (COVID-19) que afecta directamente múltiples áreas socioeconómicas, encontrándose entre ellas el sector laboral que se ha visto forzado a tomar medidas y acudir al teletrabajo, lo que tuvo como resultado una gran variedad de características positivas y negativas en la vida de las personas. Por ende, la presente investigación busca mostrar la relación que existe entre la satisfacción y las demandas laborales en un grupo de personas que realizan teletrabajo en call centers ubicados en Costa Rica, basado en la hipótesis de que, a mayor cantidad de horas de teletrabajo realizadas, mayores serán las demandas laborales y menor la satisfacción laboral de la persona. Para esto, se aplicaron dos escalas, una con el fin de medir las demandas laborales, y la otra para la satisfacción laboral, a una muestra de 100 personas. En cuanto a los resultados, estos arrojaron que, a mayor cantidad de horas diarias de teletrabajo, mayor percepción de demandas laborales y menor satisfacción laboral; caso contrario, quienes laboran menos horas al día perciben una menor demanda laboral y mayor satisfacción, por ende, es posible aprobar la hipótesis.

In August 2018, a fatal autochthonous case of Crimean-Congo hemorrhagic fever was confirmed in western Spain. The complete sequence of the viral genome revealed circulation of a new virus because the genotype differs from that of the virus responsible for another case in 2016. Practitioners should be alert to possible new cases.

BackgroundTwo automatable in-house protocols for high-troughput RNA extraction from nasopharyngeal swabs for SARS-CoV-2 detection have been evaluated.MethodsOne hundred forty one SARS-CoV-2 positive samples were collected during a period of 10-days. In-house protocols were based on extraction with magnetic beads and designed to be used with either the Opentrons OT-2 (OT-2in-house) liquid handling robot or the MagMAXTM Express-96 system (MMin-house). Both protocols were tested in parallel with a commercial kit that uses the MagMAXTM system (MMkit). Nucleic acid extraction efficiencies were calculated from a SARS-CoV-2 DNA positive control.ResultsNo significant differences were found between both in-house protocols and the commercial kit in their performance to detect positive samples. The MMkit was the most efficient although the MMin-house presented, in average, lower Cts than the other two. In-house protocols allowed to save between 350€ and 400€ for every 96 extracted samples compared to the commercial kit.ConclusionThe protocols described harness the use of easily available reagents and an open-source liquid handling system and are suitable for SARS-CoV-2 detection in high throughput facilities.

El objetivo de la investigación se sustenta en la caracterización del macizo rocoso, determinación de los parámetros de diseño y cuantificación de los costos de los explosivos de ambos métodos de explotación, que permitirá una operación segura y sostenible en el tiempo.

At present we are in a situation of global health crisis (COVID-19) that directly affects multiple socio-economic areas including the labor sector, being forced to take measures and resort to telework resulting in a variety of positive and negative characteristics in the lives of people. Therefore, the present research aims to show the relationship between satisfaction and labor demands in a group of people who perform telework in call centers in Costa Rica, based on the hypothesis that the more hours of teleworking performed, the greater the labor demands and lower job satisfaction of the person. For this, two scales were applied, one to measure labor demands, and the other work satisfaction, to a sample of 100 people. The results show that the people who work more hours of teleworking per day have a higher perception of labor demands and lower job satisfaction; on the contrary, those who work fewer hours per day perceive lower labor demands and higher job satisfaction, thus it is possible to support the hypothesis. Actualmente nos encontramos en una situación de crisis sanitaria mundial (COVID-19) que afecta directamente múltiples áreas socioeconómicas, encontrándose entre ellas el sector laboral que se ha visto forzado a tomar medidas y acudir al teletrabajo, lo que tuvo como resultado una gran variedad de características positivas y negativas en la vida de las personas. Por ende, la presente investigación busca mostrar la relación que existe entre la satisfacción y las demandas laborales en un grupo de personas que realizan teletrabajo en call centers ubicados en Costa Rica, basado en la hipótesis de que, a mayor cantidad de horas de teletrabajo realizadas, mayores serán las demandas laborales y menor la satisfacción laboral de la persona. Para esto, se aplicaron dos escalas, una con el fin de medir las demandas laborales, y la otra para la satisfacción laboral, a una muestra de 100 personas. En cuanto a los resultados, estos arrojaron que, a mayor cantidad de horas diarias de teletrabajo, mayor percepción de demandas laborales y menor satisfacción laboral; caso contrario, quienes laboran menos horas al día perciben una menor demanda laboral y mayor satisfacción, por ende, es posible aprobar la hipótesis.

- Abstract
The aim of this study was to analyze the type of physical selection tests used in different national calls for access to structural firefighters service and to describe the sex differences applied in the cut-score of the applicants. For this, 49 national calls between the years 2011-2020 were studied. The physical tests of the calls were classified according to their nature: generic (physical fitness assessment) or specific (work requirements assessment). In addition, the difference between the cut-score required of women and man in each of the tests was calculated. 85.7% of the calls made some type of differentiation by sex in the pass, 8.2% did not make a differentiation and 6.1% did not apply any differentiation in the pass but in the scale. In all calls there was some generic selection test, and in 57.1% only generic tests were performed. The mean difference between the marks required for men and women was 17.6 ± 7.9%. The mean value of the differences by sex in the specific tests was 18.3 ± 8.4%. In conclusion, the most used tests in the firefighters’ selection processes were the generic ones. There was a great heterogeneity both in the tests used and in the scales applied. Although in most of the calls some type of differentiation by sex was established, there does not seem to be a homogeneity of criteria for its establishment., El objetivo del presente trabajo fue analizar el tipo de pruebas físicas de selección utilizadas en diferentes convocatorias nacionales de acceso al servicio de Salvamento y Extinción de Incendios y describir las diferencias aplicadas en las marcas mínimas exigidas en función del sexo de los aspirantes. Para ello, se estudiaron 49 convocatorias de acceso a nivel nacional entre los años 2011-2020. Las pruebas físicas de las convocatorias fueron clasificadas en función de su carácter: genéricas (evaluación de las capacidades físicas básicas) o específicas (evaluación de los requerimientos laborales). Además, la diferencia entre las marcas exigidas a las mujeres y los hombres fue calculada en cada una de las pruebas. El 85.7% de las convocatorias realizaron algún tipo de diferenciación por sexo en el apto, el 8.2% no realizaron diferenciación y el 6.1% no aplicaron ninguna diferenciación en el apto, pero si en el baremo. En todas las convocatorias hubo alguna prueba de selección genérica, y en el 57.1% únicamente se realizaron pruebas genéricas. La diferencia media entre las marcas exigidas a los hombres y las mujeres fue del 17.6 ± 7.9%. El valor medio de las diferencias por sexo en las pruebas específicas fue del 18.3 ± 8.4%. En conclusión, las pruebas de selección más utilizadas fueron las genéricas, existiendo una gran heterogeneidad tanto en las pruebas utilizadas como en los baremos aplicados. Aunque en la mayoría de las convocatorias se estableció algún tipo de diferenciación por sexo, no parece existir una homogeneidad de criterios para su establecimiento.

- Abstract
To evaluate molecular assays for Mpox diagnosis available in various clinical microbiology services in Spain through a quality control (QC) approach. A total of 14 centers from across Spain participated in the study. The Reference Laboratory dispatched eight serum samples and eight nucleic acid extracts to each participating center. Some samples were spiked with Mpox or Vaccinia virus to mimic positive samples for Mpox or other orthopox viruses. Participating centers provided information on the results obtained, as well as the laboratory methods used. Among the 14 participating centers seven different commercial assays were employed, with the most commonly used kit being LightMix Modular Orthopox/Monkeypox (Mpox) Virus (Roche®). Of the 12 centers conducting Mpox determinations, concordance ranged from 62.5% (n = 1) to 100% (n = 11) for eluates and from 75.0% (n = 1) to 100% (n = 10) for serum. Among the 10 centers performing Orthopoxvirus determinations, a 100% concordance was observed for eluates, while for serum, concordance ranged from 87.5% (n = 6) to 100% (n = 4). Repeatedly, 6 different centers reported a false negative in serum samples for Orthopoxvirus diagnosis, particularly in a sample with borderline Ct = 39. Conversely, one center, using the TaqMan™ Mpox Virus Microbe Detection Assay (Thermo Fisher), reported false positives in Mpox diagnosis for samples spiked with vaccinia virus due to cross-reactions. We observed a positive correlation of various diagnostic assays for Mpox used by the participating centers with the reference values. Our results highlight the significance of standardization, validation, and ongoing QC in the microbiological diagnosis of infectious diseases, which might be particularly relevant for emerging viruses.

Low birth weight and rapid postnatal weight gain are independent predictors of obesity and diabetes in adult life, yet the molecular events involved in this process remain unknown. In inbred and outbred mice, this study examines natural intrauterine growth restriction (IUGR) in relation to body weight, telomere length (TL), glucose tolerance, and growth factor gene (Igf1, Igf2, Insr, Igf1r, and Igf2r) mRNA expression levels in the brain, liver, and muscle at 2- and 10 days of age and then at 3- and 9 months of age. At birth, ~15% of the animals showed IUGR, but by 3 and 9 months, half of these animals had regained the same weight as controls without IUGR (recuperated group). At 10 days, there was no difference in TL between animals undergoing IUGR and controls. However, by 3 and 9 months of age, the recuperated animals had shorter TL than the control and IUGR-non recuperated animals and also showed glucose intolerance. Further, compared to controls, Igf1 and Igf2 growth factor mRNA expression was lower in Day 2-IUGR mice, while Igf2r and Insr mRNA expression was higher in D10-IUGR animals. Moreover, at 3 months of age, only in the recuperated group were brain and liver Igf1, Igf2, Insr, and Igf2r expression levels higher than in the control and IUGR-non-recuperated groups. These data indicate that catch-up growth but not IUGR per se affects TL and glucose tolerance, and suggest a role in this latter process of insulin/insulin-like growth signaling pathway gene expression during early development.

This article discusses a case of intraventricular neurocysticercosis in a 22-year-old female migrant from Honduras who arrived in Spain. The patient presented with symptoms such as headache, diplopia, and retroocular pain, and was diagnosed with neurocysticercosis based on radiological and microbiological criteria. The patient underwent surgery for the placement of a ventriculoperitoneal shunt and received pharmacological treatment. The article highlights the risk factors and prevalence of neurocysticercosis in rural or low-income areas, particularly in Honduras. [Extracted from the article]

Human monkeypox has become increasingly frequent worldwide since the outbreak was first reported in May 2022.As cidofovir is effective against vaccinia and other Orthopoxvirus diseases, we hypothesize that its topical use could be an effective treatment for monkeypox skin lesions, avoiding the adverse effects of systemic administration.We conducted a prospective study to collect data on the clinical and virologic course of patients with monkeypox. All patients were offered symptomatic treatment. They were also offered treatment with topical cidofovir on a compassionate use basis. 12 patients received treatment with topical cidofovir 1%, while the other received only symptomatic treatment. Prospective visits were scheduled for the collection of clinical and virological data.Lesions cleared quicker in the cidofovir-treated group (HR 4.572; p = 0.0039). Median time to resolution was 12 (11.5-15) and 18 (16-21) days, respectively. On day 14, PCR-positive skin lesions were detected in 10% of the cidofovir sample, compared with 62.5% of the non-treated group (p:0.019). Local adverse effects were frequent (50%), especially in the anogenital region. No systemic adverse effects were reported.The study is not a clinical trial and lacks a placebo-controlled arm.Topical cidofovir is a potentially relevant therapy in patients with skin lesions but mild systemic involvement. Reducing time to resolution could shorten isolation time and improve the cosmetic impact in areas such as the face.

In August 2018, a fatal autochthonous case of Crimean-Congo hemorrhagic fever was confirmed in western Spain. The complete sequence of the viral genome revealed circulation of a new virus because the genotype differs from that of the virus responsible for another case in 2016. Practitioners should be alert to possible new cases. Con la presente investigación se busca principalmente establecer un perfil de ansiedad, estrés y estados de ánimo del Personal Especialista en Extinción de Incendios Forestales (PEEIF) o bomberos forestales tanto a nivel general como en situaciones de emergencia. La muestra se compuso de 24 sujetos a los que se administró una batería de cuestionarios en tres ocasiones (sin emergencia, primer a emergencia y última emergencia), compuesta por un cuestionario sociodemográfico adhoc, el Perfil de Estados de Ánimo (POMS); y el Inventario de ansiedad estado-rasgo (STAI- E/R), realizando un análisis descriptivo y comparativo a través de la prueba t-student y comparaciones múltiples con Anova de medidas repetidas. Se observó que los miembros PEEIF, a nivel general, tenían valores bajos de ansiedad estado, ansiedad rasgo, tensión, depresión, cólera, fatiga y confusión, mientras que el vigor fue elevado. En situaciones de emergencia, los valores se vieron modificados en su totalidad, debido a la fuente de estrés que supone. Se demuestra que el trabajo de extinción de incendios forestales se caracteriza por tener efectos negativos a nivel emocional sobre sus profesionales, mediados por la experiencia, cargo y condiciones familiares. Abstract: The present research mainly seeks to establish a profile of anxiety, stress and moods of the Forest Fire Extinction Specialist Personnel (PEEIF) or wildland firefighters both at a general level and in emergency situations. The sample was made up of 24 subjects who were administered a battery of questionnaires on three occasions (no emergency, first emergency and last emergency), composed of an ad-hoc sociodemographic questionnaire, the Profile of Mood States (POMS); and the State-Trait Anxiety Inventory (STAI-E / R), carrying out a descriptive and comparative analysis through the bat-student test and multiple comparisons with repeated measures Anova. It was observed that the PEEIF members, at a general level, had low values of state anxiety, trait anxiety, tension, depression, anger, fatigue and confusion, while vigor was high. In emergency situations, the values were modified in their entirety, due to the source of stress involved. It is shown that forest fire extinguishing work is characterized by having negative emotional effects on its professionals, mediated by experience, position and family conditions.

At present we are in a situation of global health crisis (COVID-19) that directly affects multiple socio-economic areas including the labor sector, being forced to take measures and resort to telework resulting in a variety of positive and negative characteristics in the lives of people. Therefore, the present research aims to show the relationship between satisfaction and labor demands in a group of people who perform telework in call centers in Costa Rica, based on the hypothesis that the more hours of teleworking performed, the greater the labor demands and lower job satisfaction of the person. For this, two scales were applied, one to measure labor demands, and the other work satisfaction, to a sample of 100 people. The results show that the people who work more hours of teleworking per day have a higher perception of labor demands and lower job satisfaction; on the contrary, those who work fewer hours per day perceive lower labor demands and higher job satisfaction, thus it is possible to support the hypothesis., Actualmente nos encontramos en una situación de crisis sanitaria mundial (COVID-19) que afecta directamente múltiples áreas socioeconómicas, encontrándose entre ellas el sector laboral que se ha visto forzado a tomar medidas y acudir al teletrabajo, lo que tuvo como resultado una gran variedad de características positivas y negativas en la vida de las personas. Por ende, la presente investigación busca mostrar la relación que existe entre la satisfacción y las demandas laborales en un grupo de personas que realizan teletrabajo en call centers ubicados en Costa Rica, basado en la hipótesis de que, a mayor cantidad de horas de teletrabajo realizadas, mayores serán las demandas laborales y menor la satisfacción laboral de la persona. Para esto, se aplicaron dos escalas, una con el fin de medir las demandas laborales, y la otra para la satisfacción laboral, a una muestra de 100 personas. En cuanto a los resultados, estos arrojaron que, a mayor cantidad de horas diarias de teletrabajo, mayor percepción de demandas laborales y menor satisfacción laboral; caso contrario, quienes laboran menos horas al día perciben una menor demanda laboral y mayor satisfacción, por ende, es posible aprobar la hipótesis.

Lateral flow immunoassays (LFIA) for rapid detection of specific antibodies (IgM and IgG) against SARS-CoV-2 in different human specimens have been developed in response to the pandemic. The aim of this study is to evaluate three immunocromathographic assays (Sienna®, Wondfo® and Prometheus®) for detection of antibodies against SARS-CoV-2 in serum samples, considering RT-qPCR as a reference. A total of 145 serum samples from 145 patients with clinical suspicion of COVID-19 were collected: all of the samples were tested with Sienna®, 117 with Wondfo® and 89 with Prometheus®. The overall results of sensitivity, specificity, positive predictive value and negative predictive value obtained were as follows: 64.4%, 75%, 85.5% and 47.8% with Sienna®; 45.2%, 81.8%, 80.5% and 47.4% with Wondfo® and 75.5%, 12.5%, 51.4% and 29.4% with Prometheus®. The accuracy of the test for Sienna®, Wondfo® and Prometheus® was 67.6%, 59% and 47.2%, with a prevalence of COVID-19 of 69.7%, 62.4% and 55.1% respectively. Sensitivity of the three tests (Sienna®, Wondfo® and Prometheus® respectively) along the three different stages was 36.6%, 18.8% and 68.6% in the early stage (first week); 81.3%, 74.1% and 90.9% in the intermediate stage (second week) and 100%, 83.3% and 100% in the late stage (third week). The results demonstrate that even though Prometheus® presented a high sensitivity, the specificity was notably lower than the other two tests. Sienna® showed the greatest contrast between sensitivity and specificity, achieving the best accuracy, followed by Wondfo®. The sensitivity of the three ICT assays was higher in late stages of the disease. The work performed by wildland firefighters (WFFs) is very demanding owing to the conditions in which they have to operate. It has been reported that these professionals walk long distances over unstable and steep terrain carrying heavy loads, handle tools manually and repeatedly and are subject to a high level of thermal stress. Under such conditions, the risk of developing chronic pain (CP) is high, although despite this, there are no available data pertaining to CP among WFFs, to the best of our knowledge. As such, the aim of this study is to describe CP in Spanish helitack crews, for which purpose 221 WFFs (203 men and 18 women) completed an online self-report questionnaire. Approximately 60% of WFFs reported suffering from CP, of which 45.5% had CP in more than one body region at the same time. Age and length of service were associated with the probability of suffering from CP. Likewise, the age and height of WFFs and weight of the protective equipment used increased the prevalence of CP. Lastly, gender and job position affected CP location, with women and forepersons reporting greater prevalence of CP in the lower limbs. To conclude, the results obtained suggest the major prevalence of CP among Spanish WFFs, with current data suggesting in turn the importance that age, stature, gender, length of service, weight of protective equipment and job position have on the prevalence and location of CP.

[EN] CD160 is a member of the immunoglobulin superfamily with a pattern of expression mainly restricted to cytotoxic cells. To assess the functional relevance of the HVEM/CD160 signaling pathway in allogeneic cytotoxic responses, exon 2 of the CD160 gene was targeted by CRISPR/Cas9 to generate CD160 deficient mice. Next, we evaluated the impact of CD160 deficiency in the course of an alloreactive response. To that aim, parental donor WT (wild-type) or CD160 KO (knock-out) T cells were adoptively transferred into non-irradiated semiallogeneic F1 recipients, in which donor alloreactive CD160 KO CD4 T cells and CD8 T cells clonally expanded less vigorously than in WT T cell counterparts. This differential proliferative response rate at the early phase of T cell expansion influenced the course of CD8 T cell differentiation and the composition of the effector T cell pool that led to a significant decreased of the memory precursor effector cells (MPECs) / short-lived effector cells (SLECs) ratio in CD160 KO CD8 T cells compared to WT CD8 T cells. Despite these differences in T cell proliferation and differentiation, allogeneic MHC class I mismatched (bm1) skin allograft survival in CD160 KO recipients was comparable to that of WT recipients. However, the administration of CTLA-4.Ig showed an enhanced survival trend of bm1 skin allografts in CD160 KO with respect to WT recipients. Finally, CD160 deficient NK cells were as proficient as CD160 WT NK cells in rejecting allogeneic cellular allografts or MHC class I deficient tumor cells. CD160 may represent a CD28 alternative costimulatory molecule for the modulation of allogeneic CD8 T cell responses either in combination with costimulation blockade or by direct targeting of alloreactive CD8 T cells that upregulate CD160 expression in response to alloantigen stimulation. 48. Effect of a High-Intensity Circuit Training Program on the Physical Fitness of Wildland Firefighters

Wildland firefighting implies high physical and psychological demands for the personnel involved. Therefore, good physical fitness can help increase the work efficiency of wildland firefighters (WFFs) and safeguard their health. High-intensity circuit training (HICT) could be a good alternative to improve the physical condition of WFFs since it stands out for its functionality and economy. Therefore, the objective of this study was to analyze the effects of HICT on the WFFs’ physical fitness. The study involved 9 WFFs (8 males and 1 female; 29.8 ± 2.8 years; 175.6 ± 6.7 cm) who completed a training program and 9 WFF candidates (8 males and 1 female; 24.7 ± 6.6 yr, 176.5 ± 7.0 cm) as a control group. WFFs performed an 8-week HICT program (two weekly training sessions). The training sessions lasted approximately 45 min and were performed at an intensity >80% of maximal heart rate and RPE values >7.5. At the beginning and the end of the intervention, subjects’ physical fitness (i.e., aerobic capacity, cardiorespiratory endurance, muscular resistance and explosive strength) was assessed through different tests. After completing the training program, WFFs had significantly increased (p < 0.05) the speed at which the ventilatory thresholds were determined (12.4 ± 13.9 and 5.7 ± 7.3% for the ventilatory and respiratory compensation thresholds, respectively) and also their abdominal (31.2 ± 17.2%), lumbar (34.1 ± 13.5%) and upper limb (13.3 ± 16.0%) strength. Moreover, improvements (p < 0.05) in the explosive strength of legs (7.1 ± 5.8%) and performance in a specific physical employment test (12.2 ± 6.1%) were observed. In conclusion, the results of this study show that an 8-week high-intensity circuit training program could be an effective and safe method to improve WFFs’ physical fitness and performance.

The Pack Test (PT) is a widely used test to establish readiness for work in wildland firefighting. It is common to perform this test dressed in regular exercise clothing. However, wildland firefighters (WFF) have to wear personal protective equipment (PPE) during their deployments, which increases the physiological strain and reduces their work capacity. This study aimed to analyse the impact of full PPE on PT performance. Nine male professional WFF performed in random order a PT walking at the fastest possible self-pace wearing two different clothing configurations: (i) traditional short sports gear (SG) and (ii) the PPE currently used by Spanish WFF. Heart rate (HR), rating of perceived exertion and lap time were recorded during the PT. In addition, oxygen uptake (VO2) was estimated through the individual VO2–HR relationship previously obtained during a graded exercise test. There was a significant decrease in the PT performance (i.e., completion time) (~12%, p <, 0.05) in PPE. The physiological demands with this configuration were significantly higher (~10%, p <, 0.05). WFF spent ~13 min above the anaerobic threshold in PPE vs. ~4 min in SG. A multiple stepwise regression analysis revealed that PT performance variation might be explained by the maximal aerobic velocity (84.5%) in PPE and the VO2max (75.9%) in SG. In conclusion, wearing complete PPE increases WFF’s physiological strain, which translates into a significant PT performance reduction. Performing the test walking at the fastest possible self-pace wearing the PPE would better reflect the high-intensity effort periods reported in real scenarios.
