Cardiopoietic cells, produced through cardiogenic conditioning of patients' mesenchymal stem cells, have shown preliminary efficacy. The Congestive Heart Failure Cardiopoietic Regenerative Therapy (CHART-1) trial aimed to validate cardiopoiesis-based biotherapy in a larger heart failure cohort. This multinational, randomized, double-blind, sham-controlled study was conducted in 39 hospitals. Patients with symptomatic ischaemic heart failure on guideline-directed therapy (n = 484) were screened; n = 348 underwent bone marrow harvest and mesenchymal stem cell expansion. Those achieving > 24 million mesenchymal stem cells (n = 315) were randomized to cardiopoietic cells delivered endomyocardially with a retention-enhanced catheter (n = 157) or sham procedure (n = 158). Procedures were performed as randomized in 271 patients (n = 120 cardiopoietic cells, n = 151 sham). The primary efficacy endpoint was a Finkelstein–Schoenfeld hierarchical composite (all-cause mortality, worsening heart failure, Minnesota Living with Heart Failure Questionnaire score, 6-min walk distance, left ventricular end-systolic volume, and ejection fraction) at 39 weeks. The primary outcome was neutral (Mann–Whitney estimator 0.54, 95% confidence interval [CI] 0.47–0.61 [value > 0.5 favours cell treatment], P = 0.27). Exploratory analyses suggested a benefit of cell treatment on the primary composite in patients with baseline left ventricular end-diastolic volume 200–370 mL (60% of patients) (Mann–Whitney estimator 0.61, 95% CI 0.52–0.70, P = 0.015). No difference was observed in serious adverse events. One (0.9%) cardiopoietic cell patient and 9 (5.4%) sham patients experienced aborted or sudden cardiac death. The primary endpoint was neutral, with safety demonstrated across the cohort. Further evaluation of cardiopoietic cell therapy in patients with elevated end-diastolic volume is warranted.

4. Modelling the Logistics Response to a General Infectious Disease

- Abstract
As an infectious disease has serious consequences and demands medical resources suddenly, an emergency management plan which can reduce the number of deaths should be studied. However, the papers in this area are still few and no paper proposes a model which can be adapted to general infectious diseases. This paper proposes a model which links the disease progression, the related medical intervention actions and the logistics deployment altogether to support the decision making process in case of the logistics response to an infectious disease from a strategic level. The number of the patients in different disease stages and the required medical resources for each period can be estimated by our model. The factors which have a great impact on the number of deaths can also be evaluated by this model. Numerical results take the H5N1 as an example to assess the potential contribution of our model.

- Abstract
The authors report a case of migration of a Palmaz stent used for treatment of a patient with a malignant superior vena caval syndrome. Transoesophageal echocardiography showed the metallic device in the tricuspid subvalvular apparatus. Several attempts to recover the stent percutaneously failed. This case illustrates a rare but serious complication of endovascular stenting: migration of the material. Although it is often possible to reposition the stent by radiological interventional techniques, when the stent is located in the right ventricle, the procedure is more difficult and usually fails. Transoesophageal echocardiography enables accurate localisation of the material with respect to the surrounding cardiac structures, especially the tricuspid valve.

- Abstract
Syncope, an eminently frequent syndrome, has multiple and varied causes. Its diagnosis is important in view of the poor prognosis of cardiac syncopes and also of the morbidity and discomfort associated with the other etiologies. Unfortunately, the cause eludes us in half the patients so that a clear prognosis and effective therapy can be given to only a few patients. Tilt-table testing, alone or with isoproterenol, is now considered as a noninvasive, sensitive and specific method for identifying patients predisposed to vaso-vagal syncope, a major cause of syncope of unknown origin. Tilt-table testing also helps in tracking many pathologies contributing to syncopes. In the absence of a universal treatment of vasovagal syncope, tilt-table testing has revealed itself as a contributory guide to the best possible treatment.
