Rickettsial diseases, caused by a variety of obligate intracellular, gram-negative bacteria from the genera Rickettsia, Orientia, Ehrlichia, Neorickettsia, Neoehrlichia, and Anaplasma, belonging to the Alphaproteobacteria, are considered some of the most covert emerging and re-emerging diseases and are being increasingly recognized. Among the major groups of rickettsioses, commonly reported diseases in India are scrub typhus, murine flea-borne typhus, Indian tick typhus and Q fever. Rickettsial infections are generally incapacitating and difficult to diagnose; untreated cases have case fatality rates as high as 30-45 per cent with multiple organ dysfunction, if not promptly diagnosed and appropriately treated. The vast variability and non-specific presentation of this infection have often made it difficult to diagnose clinically. Prompt antibiotic therapy shortens the course of the disease, lowers the risk of complications and in turn reduces morbidity and mortality due to rickettsial diseases. There is a distinct need for physicians and health care workers at all levels of care in India to be aware of the clinical features, available diagnostic tests and their interpretation, and the therapy of these infections. Therefore, a Task Force was constituted by the Indian Council of Medical Research (ICMR) to formulate guidelines for diagnosis and management of rickettsial diseases. Background & objectives: Integrated Behavioral and Biological Assessment (IBBA) study is the first cross-sectional survey to study large number of covariates of HIV/STI (sexually transmitted infection) in India. Generally, districts are identified as of HIV high or low based on its prevalence. Instead, it would be optimal to label the districts based on several high-risk related covariates in the concurrent set up. The objectives of the present study were to obtain an index for each district, to discover 'natural' clusters and a map with Kriged estimates. Methods: The study population consisted of 10461 female sex workers (FSWs) from 29 sites spread over 24 districts from five HIV high prevalent States. Covariates based on demographic characteristics, sexual practices, knowledge of HIV/STI and biological variables were studied. The analyses were done on weighted estimates based on principal component analysis, cluster analysis and Kriging technique. Five factors were extracted and improved using varimax rotation and standardized factor scores obtained. Natural clusters in a multivariate setting were identified. Each district was expressed as geographic coordinates and using the standardized scores the Kriged estimates were obtained. Results: The proxy determinants were 'never used a condom', 'wanted to use a condom but did not use', 'experience of condom breakage' and 'current STI that needs a doctor'. Dimapur district stood first rank demanding the greatest attention. The cluster analysis branded Dimapur, Warangal, Prakasam, and Chittoor districts as a cluster, which required greatest attention and kriged estimates showed the high-risk concentrated regions as Andhra Pradesh, Maharashtra and northeast region. Interpretation & conclusions: The results of this study may help the programme officials and policy managers to concentrate on the key factors, and districts/regions, which need greater attention in the order of priority. The study aims to find out the knowledge and understanding of acquired immunodefi ciency syndrome (AIDS) in 1,200 randomly selected women in the age group of 15-45 years, of 16 villages belonging to Villupuram Health Unit District, Tamil Nadu, India, using a two-stage sampling design. Data collection was done in the period from August to November 2001, using both qualitative and quantitative methods. In all, 28% of the women had not heard of AIDS at all. More than one-third of the women who had heard of AIDS considered AIDS as a serious illness; and among them, 72% ranked AIDS as the number one killer disease. The main findings showed that the rural women's knowledge was poor in areas like cause, symptoms and prevention. Level of literacy of the women was signifi cantly associated with their knowledge of HIV/AIDS (P<0.05), showing that literates had better knowledge than illiterates. Also, there were misconceptions and false beliefs about cause and spread of the infection, which were found to be more prevalent among illiterates. Television was found to be the major source of knowledge on AIDS among both literates and illiterates. This study suggests a need for innovative, group-based repeated education on AIDS, particularly for rural women, in order to impart better knowledge and understanding on AIDS. [ABSTRACT FROM AUTHOR]

Introduction: As part of the global strategic plan to reduce the number of measles deaths in India, the state of Tamilnadu aims at ≥95% measles vaccination coverage. A study was conducted to measure overall coverage levels for the Poondi Primary Health Center (PPHC), a rural health-care facility in Tiruvallur District, and to determine whether any of the PPHC's six health subcenters had coverage levels <95%. Methods: The Lot Quality Assurance Sampling (LQAS) method was used to identify health subcenters in the PPHC area with measles vaccination coverage levels <95% among children aged 12-23 months. Lemeshow and Taber sampling plans were used to determine that the measles vaccination status of 73 children aged 12-23 months had to be assessed in each health subcenter coverage area, with a 5% level of significance and a decision value of two. If more than two children were unvaccinated, the null hypothesis (i.e., that coverage in the health subcenter was low [<95%]) was not rejected. If the number of unvaccinated children was two or fewer, the null hypothesis was rejected, and coverage in the subcenter was considered to be good (i.e., ≥95%). All data were pooled in a stratified sample to estimate overall total coverage in the PPHC area. Results: For two (33.3%) of the six health subcenters, more than two children were unvaccinated (i.e., coverage was <95%). Combining results from all six health subcenters generated a coverage estimate of 97.7% (95% confidence interval = 95.7-98.8) on the basis of 428 (97.7%) of 438 children identified as vaccinated. Conclusion: LQAS techniques proved useful in identifying small health areas with lower vaccination coverage, which helps to target interventions. Monthly review of vaccination coverage by subcenter and village is recommended to identify pockets of unvaccinated children and to maintain uniform high coverage in the PPHC area. An explosive common-source epidemic of typhoid fever, probably the world's biggest, occurred in Sangli Town (Maharashtra State), India, between December 1975 and February 1976 when, with the incidence rate of 6.59% over this 12-week period, probably more than 9000 cases occurred in a population of about 135 000. Faecal contamination of municipal water supply, which was receiving unsatisfactory and inadequate chlorine treatment, was responsible for the epidemic. Contamination at suitable intervals resulted in a heavy build-up of in the population. Massive central contamination of the ill-maintained municipal water-supply system with faecally-contaminated waste-water from a population of about 37 000, in which there were thought to be over 250 typhoid cases, ultimately resulted in the explosive epidemic. One hundred fifty wild strains of Xanthomonas were isolated on five selective and nonselective media. The comparative effectivity of the five media has been discussed. Fifteen polysaccharide-producing strains shortlisted were identified up to species level. These were studied for polysaccharide production in shake flasks using YM media. The highest final yield of 21.3 g/L of the amount of carbon source supplied has been obtained in the optimum medium in shake-flask experiments from Xanthomonas campestris ICa-125 strain isolated from cabbage. Rheological properties of the exocellular polysaccharide (EPS) have been compared with standard xanthan. EPS from X. campestris ICa-125 was found to be superior with respect to heat stability and effect of electrolytes as compared to standard xanthan. [ABSTRACT FROM AUTHOR]

Abstract Background During 2001–2007, the National Institute of Epidemiology (NIE), Chennai, Tamil Nadu, India admitted 80 trainees in its two-year Field Epidemiology Training Programme (FETP). We evaluated the first seven years of the programme to identify strengths and weaknesses. Methods We identified core components of the programme and broke them down into input, process, output and outcome. We developed critical indicators to reflect the logic model. We reviewed documents including fieldwork reports, abstracts listed in proceedings and papers published in Medline-indexed journals. We conducted an anonymous online survey of the graduates to collect information on self-perceived competencies, learning activities, field assignments, supervision, curriculum, relevance to career goals, strengths and weaknesses. Results Of the 80 students recruited during 2001–2007, 69 (86%) acquired seven core competencies (epidemiology, surveillance, outbreaks, research, human subjects protection, communication and management) and graduated through completion of at least six field assignments. The faculty-to-student ratio ranged between 0.4 and 0.12 (expected: 0.25). The curriculum was continuously adapted with all resources available on-line. Fieldwork led to the production of 158 scientific communications presented at international meetings and to 29 manuscripts accepted in indexed, peer-reviewed journals. The online survey showed that while most graduates acquired competencies, unmet needs persisted in laboratory sciences, data analysis tools and faculty-to-student ratio. Conclusions NIE adapted the international FETP model to India. However, further efforts are required to scale up the programme and to develop career tracks for field epidemiologists in the country.

Abstract An outbreak of malaria in Naxalbari, West Bengal, India, in 2005 was investigated to understand determinants and propose control measures. Malaria cases were slide-confirmed. Methods included calculation of annual blood examination rates (ABER, number of slides examined/population), collection of water specimens from potential vector-breeding sites, sorting of villages in categories depending on the number of abandoned wells within two kilometers radius and review of the DDT spray coverage. Cases were compared with matched neighbourhood controls in terms of personal protection using matched odds ratios (MOR). 7,303 cases and 17 deaths were reported between April 2005 and March 2006 with a peak during October rains (Attack rate: 50 per 1,000, case fatality: 0.2%). The attack rate increased according to the number of abandoned wells within 2 kilometres radius (P < 0.0001, Chi-square for trend). Abandoned wells were Anopheles breeding sites. Compared with controls, cases were more likely to sleep outdoors (MOR: 3.8) and less likely to use of mosquito nets and repellents (MOR: 0.3 and 0.1, respectively). DDT spray coverage and ABER were 39% and 3.5%, below the recommended 85% and 10%, respectively. Overall, this outbreak resulted from weaknesses in malaria control measures and a combination of factors, including vector breeding, low implementation of personal protection and weak case detection.

Abstract Background In leprosy endemic areas, patients are usually spatially clustered and not randomly distributed. Classical statistical techniques fail to address the problem of spatial clustering in the regression model. Bayesian method is one which allows itself to incorporate spatial dependence in the model. However little is explored in the field of leprosy. The Bayesian approach may improve our understanding about the variation of the disease prevalence of leprosy over space and time. Methods Data from an endemic area of leprosy, covering 148 panchayats from two taluks in South India for four time points between January 1991 and March 2003 was used. Four Bayesian models, namely, space-cohort and space-period models with and without interactions were compared using the Deviance Information Criterion. Cohort effect, period effect over four time points and spatial effect (smoothed) were obtained using WinBUGS. The spatial or panchayat effect thus estimated was compared with the raw standardized morbidity (leprosy prevalence) rate (SMR) using a choropleth map. The possible factors that might have influenced the variations of prevalence of leprosy were explored. Results Bayesian models with the interaction term were found to be the best fitted model. Leprosy prevalence was higher than average in the older cohorts. The last two cohorts 1987–1996 and 1992–2001 showed a notable decline in leprosy prevalence. Period effect over 4 time points varied from a high of 3.2% to a low of 1.8%. Spatial effect varied between 0.59 and 2. Twenty-six panchayats showed significantly higher prevalence of leprosy than the average when Bayesian method was used and it was 40 panchayats with the raw SMR. Conclusion Reduction of prevalence of leprosy was 92% for persons born after 1996, which could be attributed to various intervention and treatment programmes like vaccine trial and MDT. The estimated period effects showed a gradual decline in the risk of leprosy which could be due to better nutrition, hygiene and increased awareness about the disease. Comparison of the maps of the relative risk using the Bayesian smoothing and the raw SMR showed the variation of the geographical distribution of the leprosy prevalence in the study area. Panchayat or spatial effects using Bayesian showed clustersing of leprosy cases towards the northeastern end of the study area which was overcrowded and population belonging to poor economic status.

Abstract Background On 26 December 2004, a tsunami struck the coast of the state of Tamil Nadu, India, where one-dose measles coverage exceeded 95%. On 29 December, supplemental measles immunization activities targeted children 6 to 60 months of age in affected villages. On 30 December, Cuddalore, a tsunami-affected district in Tamil Nadu reported a cluster of measles cases. We investigated this cluster to estimate the magnitude of the problem and to propose recommendations for control. Methods We received notification of WHO-defined measles cases through stimulated passive surveillance. We collected information regarding date of onset, age, sex, vaccination status and residence. We collected samples for IgM antibodies and genotype studies. We modeled the accumulation of susceptible individuals over the time on the basis of vaccination coverage, vaccine efficacy and birth rate. Results We identified 101 measles cases and detected IgM antibodies against measles virus in eight of 11 sera. Cases were reported from tsunami-affected (n = 71) and unaffected villages (n = 30) with attack rates of 1.3 and 1.7 per 1000, respectively. 42% of cases in tsunami-affected villages had an onset date within 14 days of the tsunami. The median ages of case-patients in tsunami-affected and un-affected areas were 54 months and 60 months respectively (p = 0.471). 36% of cases from tsunami-affected areas were above 60 months of age. Phylogenetic analyses indicated that the sequences of virus belonged to genotype D8 that circulated in Tamil Nadu. Conclusion Measles virus circulated in Cuddalore district following the tsunami, although there was no association between the two events. Transmission despite high one-dose vaccination coverage pointed to the limitations of this vaccination strategy. A second opportunity for measles immunization may help reducing measles mortality and morbidity in such areas. Children from 6 month to 14 years of age must be targeted for supplemental immunization during complex emergencies.
