2. The Prescription of Antibiotics in ENT Diseases in the ENT and Surgery Department Cervico-Facial at Sikasso Hospital

Abstract

Antibiotics are chemical compounds elaborated by a micro-organism or produced by synthesis and whose specific activity manifests itself in doses on the micro-organisms. They are commonly used in the management of infectious pathologies in ENT and Cervico-Facial Surgery. This study aims to evaluate the use of antibiotics in the therapeutic arsenal in the ENT-CCF department of the Sikasso hospital. This is a 4-month prospective study from November 1, 2016 to February 28, 2017 on the prescription of antibiotics in the ENT department of Sikasso hospital. Four hundred (400) patients were included in this study. The following variables were collected: gender, age, occupation, reason for consultation, history, prescription and evolution. During the study period 400 patients were retained. Among which 293 benefited from a prescription of antibiotics that is 73.25% of the cases and (107 cases) 26.75% without antibiotics. The female sex was predominant with 57% of cases, i.e. a sex ratio of 1.32. More than half of the work force were made up of learners and preschoolers. The reasons for consultation were dominated by otalgia with 35.8% followed by odynophagia in 15% of cases. 76.75% of our patients had no history. The prevalence of antibiotic therapy was 73.25%, including 49.83% amoxi+clavulanic acid, 10.92% cephalosporin, 10.60% macrolides, 3.75% sulfonamides, 1.70% amoxicillin and ear drops in 23.2% of cases. Apart from antibiotic therapy, antihistamines were the most prescribed with 32.75% followed by nasal corticosteroids in 23.36% of cases. Non-steroidal anti- inflammatory drugs were associated with the antibiotic in 33.78%, followed by the analgesic-nasal corticosteroid- antihistamine combination in 15.35% of cases. The evolution was favorable in 83.62% of cases, unfavorable in 10.92% of cases and 5.46% of patients did not show up for the check-up appointment on the 7th day. Abstract

The identification and dissection of the recurrent laryngeal nerve is essential to guarantee its anatomical and functional integrity. The superior approach of the recurrent nerve is a reliable surgical alternative. Various indications are recognized with a reliable landmark. This is the entry point into the larynx under the inferior horn of the thyroid cartilage. The limits of this technique, namely, the extralaryngeal divisions and the hemorrhages encountered at the point of entry of the larynx are a source of morbidity of the recurrent laryngeal nerve. A careful dissection, respect for the surgical steps, and the surgeon's experience are guarantees of a good result. We wanted through a review of the literature and our experience in the superior approach to discuss surgical indications, to identify landmarks at the point of entry of the larynx, to determine the limits of this approach, and to take precautions to mitigate the risk of recurrent laryngeal nerve injury.

Abstract

Introduction: Tongue papillomas are benign epithelial tumors of the tongue lining. It is a precancerous lesion of viral origin caused by the Human papilloma virus (HPV). Objective: To describe the clinical and histological manifestations of a particular type of oral papilloma. Observation: A young woman aged 28 years resident in Siguiri (Guinea), received in consultation for a painless mass on the tongue slow evolving for 6 months duration. There was no notion of mouth bleeding. She had no particular medical and surgical history and negative for retroviral test/HIV test. Examination of the oral cavity found a fungating mass in at Right or Left posterior third of the tongue, painful on contact, sessile and pedunculated and not bleeding on contact and of soft consistency. The excision of the tongue mass was performed under general anesthesia. Biopsy was taken and sent for HPE. The consequences were mild, allowing her to be released 3 days after surgery. Histological examination showed an acanthotic stratified para-keratinized squamous epithelium containing small parts of connective tissue and koilocytes containing connective tissue nuclei reminiscent of papilloma. There was no malignancy cell. Conclusion: The papilloma of the tongue is a benign proliferative lesion characterized by slow and painless growth. I would say early diagnosis and surgical excision should be done to avoid recurrence, contact bleeding and morbidity secondary to mass effect.

Abstract

We report an unusual observation of a child aged 07, who consulted us for a chronic odynophagia rebellious to multiple treatments. It dates back to one year followed by pediatrics under antibiotic therapy and anti-inflammatory in whom the examination found hypertrophy noninflammatory obstructive tonsillar. The last episode would go back a month. The rest of the general exam was unremarkable. An indication for tonsillectomy was made and was performed. We fortuitously find a large fishbone skewered in his right tonsil. The postoperative consequences were simple. With a follow-up of 12 months, the patient is doing well. [ABSTRACT FROM AUTHOR]

Abstract

Laryngeal stenosis is a permanent, usually acquired, narrowing of the laryngeal duct. Due to the pandemic of Acquired Immunodeficiency Syndrome (AIDS), the etiology of tuberculosis must be investigated. However, the signs of presumption are not specific. Management is controversial and remains difficult in our work context in an under-medicated country south of the Sahara. The purpose of the work is to report a case of laryngeal stenosis secondary to multifocal tuberculosis to discuss the difficulties associated with its management. This was a 35-year-old smoking patient with 25 packs / year. Admitted on 04/12/2017 to the ENT and Head and Neck Surgery Department for assessment of dysphonia associated with unencrypted slimming. The performed laryngoscopy showed disseminated ulceration of the laryngeal margin, and the histology showed granulomatous laryngitis. The intradermal reaction to tuberculin measured 10 mm. A chest X-ray performed on the face showed a chronic interstitial-bronchial syndrom. The diagnosis of laryngeal tuberculosis secondary to a pulmonary focus was retained. The course was marked by the sudden onset of laryngeal dyspnea requiring urgent tracheotomy. Postoperative examination of the larynx revealed synechia of the larynx. Anti-tuberculosis therapy for six months has resulted in favourable outcomes. However, it was noted a complication type laryngeal stenosis despite the attempted re-stabilization with impossibility to decant the patient. Laryngeal stenosis secondary to multifocal tuberculosis remains a rare and dreadful pathology. Therapeutic success depends on early and adequate management.

28. [Upper Airway Obstruction in a Type 4 Laryngeal Palmar in Infant].

Abstract

Introduction: Congenital malformations of the larynx in children are often manifested by laryngeal noise (stridor), dyspnea, dysphonia and sometimes swallowing disorders. Laryngomalacia is the most common anomaly, but it is necessary to know how to look for laryngeal paralysis, congenital subglottic stenosis, sometimes a subglottic angioma or a laryngeal diastema. Endoscopy is the master examination for confirming the diagnosis and guiding the management, which may be medical and/or surgical depending on the case [1]., Objective: Aim: The aim of our work is to study the diagnostic and therapeutic particularities of a congenital malformation in an infant in a context of insufficient materials., Observation: We report an observation of an infant aged 06 months, who was referred to us from pediatrics for chronic dyspnea with dysphonia dating back to birth without other congenital anomalies after multiple treatments without improvement based on nebulization, corticoids and antibiotics. Nasofibroscopy revealed a laryngeal web-like larynx connecting the two vocal cords on its anterior two-thirds leaving a small respiratory tract (Figure 1). The diagnosis of laryngeal palmaris was retained. Management consisted of resection during panendoscopy. Nasofibroscopy at regular intervals of up to twelve months were performed without particularity., Conclusion: Dyspnea in infants can be frequent and have many causes. Only a thorough clinical and paraclinical examination can help to diagnose laryngeal palmaris. They are confusing to all laryngeal malformations.
