- Abstract
The efficiency of aero-engines combustion chambers with thermal barrier coating (TBC) is improved when numerous cooling holes are laser drilled with inclined angles. However, during the laser drilling process, especially in the percussion mode, a detrimental crack can be generated at the TBC interface. Thus, each hole could be edged with a non-visible delaminated area underneath the ceramic top-coat. The present work is focused on the thorough study of the delamination induced by laser percussion drilling when interrupted drilling conditions are presented. Shallow angle drilling was applied on separated holes with 1 to 4 laser pulses respectively and various acute incident angles. Crack length was assessed by conventional metallographic preparation. A special experimental method was carried out in order to inspect the delaminated interface and the lateral edge of a semi-hole. This non-destructive assessment of the delamination of laser drilled TBC was complemented by a 3D imaging of a semi-hole using X-Ray microscopy. Results are presented with attention on both crack initiation and propagation during the laser percussion drilling of plasma-sprayed TBC.

- Abstract
UTERINE FIBROIDS. Uterine fibroids are the most common tumors of the female genital tract. They can be asymptomatic or associated with various symptoms like abnormal uterine bleeding, pelvic pain, pelvic or extra pelvic compressive signs, or anaemia. They can also be associated with infertility. The main further examination for fibroids diagnosis is pelvic ultrasound. Pelvic MRI (Magnetic Resonance Imaging) is also useful before surgery or interventional radiology for fibroid mapping. All fibroids must be located according to the FIGO classification. No treatment is necessary in the absence of clinical symptoms. The management of uterine fibroids depends on the clinical signs, the mapping, and the preservation of the uterus as well as fertility. - Abstract
Introduction: Mnemonic discrimination (MD), the ability to discriminate new stimuli from similar memories, putatively involves dentate gyrus pattern separation. Since lithium may normalize dentate gyrus functioning in lithium-responsive bipolar disorder (BD), we hypothesized that lithium treatment would be associated with better MD in lithium-responsive BD patients., Methods: BD patients (N = 69; N
Responders = 16 [23 %]) performed the Continuous Visual Memory Test (CVMT), which requires discriminating between novel and previously seen images. Before testing, all patients had prophylactic lithium responsiveness assessed over ≥1 year of therapy (with the Alda Score), although only thirty-eight patients were actively prescribed lithium at time of testing (55 %; 12/16 responders, 26/53 nonresponders). We then used computational modelling to extract patient-specific MD indices. Linear models were used to test how (A) lithium treatment, (B) lithium responsiveness via the continuous Alda score, and (C) their interaction, affected MD., Results: Superior MD performance was associated with lithium treatment exclusively in lithium-responsive patients (Lithium x AldaScore β = 0.257 [SE 0.078], p = 0.002). Consistent with prior literature, increased age was associated with worse MD (β = -0.03 [SE 0.01], p = 0.005)., Limitations: Secondary pilot analysis of retrospectively collected data in a cross-sectional design limits generalizability., Conclusion: Our study is the first to examine MD performance in BD. Lithium is associated with better MD performance only in lithium responders, potentially due to lithium's effects on dentate gyrus granule cell excitability. - Abstract
Background: The Leave No One Behind (LNOB) agenda compels sexual and reproductive health and rights (SRHR) implementers to focus on the multiple and intersecting forms of discrimination and inequalities. One strategy to address these is Payment by Results (PbR). Using the Women's Integrated Sexual Health (WISH) programme as a case study, this paper examines if and how PbR can ensure equitable reach and impact., Methods: Given the complexity of PbR mechanisms, a theory-based approach was used in the design and analysis of this evaluation, drawing on four case studies. These were conducted by reviewing global and national programme data and by interviewing 50 WISH partner staff at national level and WISH programme staff at global and regional levels., Results: The case studies found that inclusion of equity-based indicators in the PbR mechanism had demonstrable effects on people's incentives, on how systems work, and on modes of working. The WISH programme was successful in achieving its desired programme indicators. The use of Key Performance Indicators (KPIs) clearly incentivised several strategies for service providers to innovate and reach adolescents and people living in poverty. However, there were trade-offs between performance indicators that increased coverage and others that increased equitable access, as well as several systemic challenges that limited the possible incentive effects., Conclusions: The use of PbR KPIs incentivised several strategies to reach adolescents and people living in poverty. 14. COVID-19 outbreak among temporary foreign workers in British Columbia, March to May 2020.

- Abstract
Background: During the coronavirus disease 2019 (COVID-19) pandemic, temporary foreign workers (TFWs) provided a critical role to maintaining the food supply in Canada, yet workers faced a number of challenges that made them particularly vulnerable to COVID-19. The objective of this study was to describe the epidemiological investigation and public health response to a COVID-19 outbreak among TFWs in an agricultural setting in British Columbia from March to May 2020., Methods: An outbreak was declared on March 28, 2020 following detection of two cases of COVID-19 among a group of 63 TFWs employed by a nursery and garden centre. Outbreak control measures included immediate isolation of cases, case finding via outreach screening and testing, cohorting of asymptomatic workers and enhanced cleaning and disinfection. The outbreak was declared over on May 10, 2020., Results: A total of 26 COVID-19 cases were identified among the group of TFWs; no cases were identified among local workers. Cases were primarily male (77%) with a median age of 41 years. Symptom onsets ranged from March 8 to April 9, 2020. One case required overnight hospitalization for pneumonia., Conclusion: This was the first COVID-19 community outbreak identified in British Columbia and the first COVID-19 outbreak identified among TFWs in Canada. This outbreak began prior to implementation of provincial and federal quarantine orders for international travellers. A provincial policy was later developed that requires TFWs to quarantine in government-funded accommodation prior to deployment to agricultural settings., Competing Interests: Competing interests: None.

- Abstract
Plant cell walls are composed of cellulose, hemicellulose, and lignin, collectively known as lignocellulose. Microorganisms degrade lignocellulose to liberate sugars to meet metabolic demands. Using a metagenomic sequencing approach, we previously demonstrated that the microbiome of the North American porcupine (Erethizon dorsatum) is replete with genes that could encode lignocellulose-degrading enzymes. Here, we report the identification, synthesis and partial characterization of four novel genes from the porcupine microbiome encoding putative lignocellulose-degrading enzymes: β-glucosidase, α-L-arabinofuranosidase, β-xylosidase, and endo-1,4-β-xylanase. These genes were identified via conserved catalytic domains associated with cellulose- and hemicellulose-degradation. Phylogenetic trees were created for each of these putative enzymes to depict genetic relatedness to known enzymes. Candidate genes were synthesized and cloned into plasmid expression vectors for inducible protein expression and secretion. The putative β-glucosidase fusion protein was efficiently secreted but did not permit Escherichia coli (E. coli) to use cellobiose as a sole carbon source, nor did the affinity purified enzyme cleave p-Nitrophenyl β-D-glucopyranoside (p-NPG) substrate in vitro over a range of physiological pH levels (pH 5-7). The putative hemicellulose-degrading β-xylosidase and α-L-arabinofuranosidase enzymes also lacked in vitro enzyme activity, but the affinity purified endo-1,4-β-xylanase protein cleaved a 6-chloro-4-methylumbelliferyl xylobioside substrate in acidic and neutral conditions, with maximal activity at pH 7. At this optimal pH, KM, Vmax, and kcat were determined to be 32.005 ± 4.72 μM, 1.16x10-5 ± 3.55x10-7 M/s, and 94.72 s-1, respectively. Thus, our pipeline enabled successful identification and characterization of a novel hemicellulose-degrading enzyme from the porcupine microbiome. Progress towards the goal of introducing a complete lignocellulose-degradation pathway into E. coli will be accelerated by combining synthetic metagenomic approaches with functional metagenomic library screening, which can identify novel enzymes unrelated to those found in available databases., Competing Interests: The authors have declared that no competing interests exist.

- Abstract
In the light of political, economic and cultural factors the authors give a historical overview of psychiatric practice in the public sector in Martinique (French West Indies) which was for a long time concentrated at the Colson Specialized Medical Center. The evolution of mental health problems is described in parallel with socio-economic changes. Broad opportunities for daily application of psychiatric practice in problematic social situations call forth the notion of "extended psychiatry". A number of changes are now shaping with the creation of diversified decentralized facilities, e.g. in penitentiaries and in some local hospitals. Other projects now being discussed are less certain and there is a risk of a two-speed system between the Specialized Hospital Center and the Regional University Hospital Center. Their authors place hopes in a project covering the whole island in which public sector psychiatry would represent a metacultural model going beyond ideologic handicaps and opening the way to new solidarity.

- Abstract
50 neonates were selected in a high perinatal risk population and examined by means of cerebral ultrasound (US) and auditory brainstem responses (ABR). They were allocated in 5 groups according to their US diagnosis (normal scans, isolated peri-intraventricular haemorrhage, large haemorrhage with associated lesions, hypoxic-ischaemic lesions in term neonates, miscellaneous disorders). ABR abnormalities were found in 40% of the infants. They were more frequent in neonates with cerebral lesions. Furthermore, the more extensive the cerebral lesions, the more severe the ABR abnormalities. Most of the ABR changes were of audiological type (severe: threshold greater than 80 dB [14%]; moderate: threshold less than or equal to 80 dB [20%]), whereas neurological abnormalities were rare (6%). The prognostic value of ABR appears in this study to depend on the possible recovery in the neonatal period. Severe hearing loss was associated either with an early death or with major handicaps. ABR represent the method of first choice for the detection of hearing loss in the Neonatal Unit, while they seemed to be of limited value in assessing brainstem function.
