ABSTRACT OBJECTIVE To describe the methodological challenges and strategies of a web survey on the working conditions and health among delivery workers. METHODS The study population consisted of Brazilian delivery workers operating in the national territory. Procedures include building solid and ongoing collaboration with worker representatives and conducting a four-month data collection from February to May 2022, sharing the link to the online questionnaire on social media such as social networks (Facebook, Instagram) and messaging apps (WhatsApp, Telegram). RESULTS The recruitment of 41 leaders or influencers of delivery workers increased the dissemination of the study, some of whom participated in the consensual validation of the questionnaire; the production of content for social media for the dissemination of the questionnaire link on social networks and applications, and the in-person dissemination of the study at the delivery workers’ meeting points during the workday played a fundamental role, totaling around 132 hours in 45 shifts. The strategies adopted for data collection with a hybrid approach to dissemination made it possible to carry out the web survey. After four months of the web survey, 564 delivery workers, 543 men and 18 women, responded to the online questionnaire. CONCLUSION The web survey presented methodological strategies to overcome the challenge of reaching workers, including hybrid work, to increase the participation of workers, on whom epidemiological research is still scarce.

Abstract Background Indigenous people have historically suffered devastating impacts from epidemics and continue to have lower access to healthcare and be especially vulnerable to respiratory infections. We estimated the coverage and effectiveness of Covid-19 vaccines against laboratory-confirmed Covid-19 cases among indigenous people in Brazil. Methods We linked nationwide Covid-19 vaccination data with flu-like surveillance records and studied a cohort of vaccinated indigenous people aged ≥ 5 years between 18th January 2021 and 1st March 2022. We considered individuals unexposed from the date they received the first dose of vaccine until the 13th day of vaccination, partially vaccinated from the 14th day after the first dose until the 13th day after receiving the second dose, and fully vaccinated onwards. We estimated the Covid-19 vaccination coverage and used Poisson regression to calculate the relative risks (RR) and vaccine effectiveness (VE) of CoronaVac, ChAdOx1, and BNT162b2 against Covid-19 laboratory-confirmed cases incidence, mortality, hospitalisation, and hospital-progression to Intensive Care Unit (ICU) or death. VE was estimated as (1-RR)*100, comparing unexposed to partially or fully vaccinated. Results By 1st March 2022, 48.7% (35.0-62.3) of eligible indigenous people vs. 74.8% (57.9–91.8) overall Brazilians had been fully vaccinated for Covid-19. Among fully vaccinated indigenous people, we found a lower risk of symptomatic cases (RR: 0.47, 95%CI: 0.40–0.56) and mortality (RR: 0.47, 95%CI: 0.14–1.56) after the 14th day of the second dose. VE for the three Covid-19 vaccines combined was 53% (95%CI:44–60%) for symptomatic cases, 53% (95%CI:-56-86%) for mortality and 41% (95%CI:-35-75%) for hospitalisation. In our sample, we found that vaccination did not reduce Covid-19 related hospitalisation. However, among hospitalised patients, we found a lower risk of progression to ICU (RR: 0.14, 95%CI: 0.02–0.81; VE: 87%, 95%CI:27–98%) and Covid-19 death (RR: 0.04, 95%CI:0.01–0.10; VE: 96%, 95%CI: 90–99%) after the 14th day of the second dose. Conclusions Lower coverage but similar Covid-19 VE among indigenous people than overall Brazilians suggest the need to expand access, timely vaccination, and urgently offer booster doses to achieve a great level of protection among this group.

We analyzed epidemiologic characteristics and distribution of 492 deaths related to Chagas disease and coronavirus disease (COVID-19) co-infection in Brazil during March‒December 2020. Cumulative co-infected death rates were highest among advanced age groups, persons of Afro-Brazilian ethnicity and with low education levels, and geographically distributed mainly in major Chagas disease‒endemic areas.

There is limited evidence of the effectiveness of the CoronaVac vaccine for children against the Omicron SARS-CoV-2 variant. Here, the authors use data from Brazil for children aged 6–11 years and estimate effectiveness of 40% against infection and 59% against severe disease at least two weeks after the second dose.

Primary CoronaVac vaccination followed by a BNT162b2 booster dose confers protection against some SARS-CoV-2 variants but its effectiveness against Omicron is unknown. Here, the authors show that this combination confers a high level of protection against severe outcomes for up to 120 days, with evidence of waning for those aged 80 or older.

Dogs living in areas of Leishmania (Viannia) braziliensis transmission may present canine tegumentary leishmaniasis (CTL) characterized by cutaneous or muzzle ulcers as well as asymptomatic L. braziliensis infection. It is not clear if dogs participate in the transmission chain of L. braziliensis to humans. However, dogs may remain with chronic ulcers for a long time, and as there are no public policies about CTL, these animals die or are sacrificed. Here we compare the efficacy of intralesional meglumine antimoniate with intralesional 0.9% NaCl solution in CTL treatment. This randomized control study included 32 dogs with cutaneous or muzzle lesions who had L. braziliensis DNA detected by PCR in tissue biopsied. Group one received 5ml of intralesional Glucantime, and group two received 5ml 0.9% NaCl solution, both applied in the four cardinal points on days 0, 15, and 30. Cure was defined as complete healing of the ulcers in the absence of raised borders on day 90. There was no difference in animals’ demographic and clinical features in the two groups (p >.05). While at the endpoint, the cure rate was 87.5% in the group test, and in those who received 0.9 NaCl the cure rate was only 12.5%. As important as the high cure rate, the healing time was faster in dogs treated with antimony than in those treated with saline (p < .001). Intralesional meglumine antimoniate is effective in the treatment of dogs with L. braziliensis infection and accelerates the healing time of CTL. Author summary Dogs play an important role in the transmission of Leishmania infantum, the causal agent of visceral leishmaniasis. But very little is known about L. braziliensis infection in dogs. Canine tegumentary leishmaniasis (CTL) caused by L. braziliensis is documented in endemic areas of human cutaneous leishmaniasis. CTL occurs predominantly in males, and the scrotal sac and muzzle are the primary site of infection. As there are no policies regarding therapy for CTL, dogs die from the disease or are sacrificed. A few uncontrolled studies showed a cure of CTL after treatment with intralesional Glucantime (Sanofi-Aventis). Here, we compare the efficacy of intralesional Glucantime (N = 16) versus intralesional saline (N = 16) in the treatment of CTL. All dogs presented ulcerated lesions. The DNA of L. braziliensis was documented by PCR in biopsied tissue of all animals in the study. There were no differences among the two groups regarding age, gender, illness duration, lesion size, and lesion site. However, a higher cure rate (87.5%) was observed in the dogs treated with Glucantime compared to 12.5% in those treated with saline (p < .001). This study showed that intralesional Glucantime cures dogs infected with L. braziliensis and should be used to treat canine tegumentary leishmaniasis.

Summary: Background: How the prefrontal cortex (PFC) recovers its functionality following lesions remains a conundrum. Recent work has uncovered the importance of transient low-frequency oscillatory activity (LFO; < 4 Hz) for the recovery of an injured brain. We aimed to determine whether persistent cortical oscillatory dynamics contribute to brain capability to support ‘normal life’ following injury. Methods: In this 9-year prospective longitudinal study (08/2012-2021), we collected data from the patient E.L., a modern-day Phineas Gage, who suffered from lesions, impacting 11% of his total brain mass, to his right PFC and supplementary motor area after his skull was transfixed by an iron rod. A systematic evaluation of clinical, electrophysiologic, brain imaging, neuropsychological and behavioural testing were used to clarify the clinical significance of relationship between LFO discharge and executive dysfunctions and compare E.L.´s disorders to that attributed to Gage (1848), a landmark in the history of neurology and neuroscience. Findings: Selective recruitment of the non-injured left hemisphere during execution of unimanual right-hand movements resulted in the emergence of robust LFO, an EEG-detected marker for disconnection of brain areas, in the damaged right hemisphere. In contrast, recruitment of the damaged right hemisphere during contralateral hand movement, resulted in the co-activation of the left hemisphere and decreased right hemisphere LFO to levels of controls enabling performance, suggesting a target for neuromodulation. Similarly, transcranial magnetic stimulation (TMS), used to create a temporary virtual-lesion over E.L.’s healthy hemisphere, disrupted the modulation of contralateral LFO, disturbing behaviour and impairing executive function tasks. In contrast to Gage, reasoning, planning, working memory, social, sexual and family behaviours eluded clinical inspection by decreasing LFO in the delta frequency range during motor and executive functioning. Interpretation: Our study suggests that modulation of LFO dynamics is an important mechanism by which PFC accommodates neurological injuries, supporting the reports of Gage´s recovery, and represents an attractive target for therapeutic interventions. Dogs play an important role in transmission of Leishmania infantum, but epidemiologic and clinical studies of canine tegumentary leishmaniasis (CTL) are scarce. In an endemic area of human American tegumentary leishmaniasis (ATL) caused by Leishmania braziliensis, we determine the prevalence and incidence of both CTL and subclinical (SC) L. braziliensis infection in dogs and evaluated if the presence of dogs with CTL or SC L. braziliensis infection is associated with the occurrence of human ATL. SC infection in healthy animals and CTL in animals with ulcers were determined by PCR on biopsied healthy skin or on ulcers or by detecting antibodies against soluble leishmania antigen. We compared the occurrence of human ATL in homes with dogs with CTL or SC infection with control homes without dogs or with dogs without CTL or SC infection. The prevalence of SC infection was 35% and of CTL 31%. The incidence of SC infection in dogs was 4.6% and of CTL 9.3%. The frequency of ATL in humans was 50% in homes with infected dogs and 13% in homes without L. braziliensis infection in dogs. CTL and SC infection is highly prevalent, and dogs may participate in the transmission chain of L. braziliensis.

Background: The timely identification of visceral leishmaniasis (VL) patients with a higher risk of death is essential for meeting the target of reducing case-fatality rates in the Americas. This study aimed to identify factors associated with death from VL in the State of Piaui, Brazil. Methods: Case-control study evaluating the following putative risk factors for death from VL: gender and age of the patient, local of residence, signs, and symptoms, laboratory data, comorbidities and days of evolution of the disease. The associations between risk factors and death were expressed as odds ratios and their respective 95% confidence intervals. Results: In the period analyzed a total of 2525 VL patients were admitted to the hospital, corresponding to 9,3% of all VL admissions in Brazil. Among them, 177 patients died (case-fatality rate of 7.0%). In the multivariate analysis the following variables showed a statistically significant association with death: ≥60 years, vomiting, edema, diarrhea, platelets

Compelling evidence supports the current international recommendation of at least 150 min/week of leisure-time physical activity (LTPA) during pregnancy. However, the potential relationship between LTPA and birth weight (BW) remains unclear in low- and middle-income countries. The purpose of this study was to examine the association between LTPA during pregnancy and offspring BW in an Amazonian population. Prospective cohort study was carried-out with 500 pregnant women and their offspring followed-up in the MINA-Brazil study, which was conducted in Cruzeiro do Sul, Western Brazilian Amazon. LTPA was assessed in the second (mean 19.6, SD 2.4 weeks) and third (mean 27.8, SD 1.6 weeks) gestational trimesters by a standardised interview and categorised according to the recommended cut-off of at least 150 min/week. We calculated offspring BW z-scores by sex and gestational age. We then explored the effect of LTPA during pregnancy on offspring BW, the association between LTPA and small-for-gestational-age (SGA) or large-for-gestational-age (LGA) births, and the mediating role of excessive and insufficient gestational weight gain (GWG). At least 150 min/week of LTPA during the third gestational trimester was associated with an offspring BW decrease of -0.35 z-score (95% CI: -0.65, -0.05) or -147.9 grams (95% CI: -260.9, -14.8), without increasing the frequency of SGA foetuses either in the second or third gestational trimester (p > 0.05). Excessive GWG mediated the effect of LTPA on the offspring BW (indirect effect = -0.05 z-score [95% CI: -0.10, -0.00] or -34.7 grams (95% CI: -66.1, -3.3]). This mediation effect was not observed for insufficient GWG. LTPA in the third, but not in the second, trimester of pregnancy was inversely associated with offspring BW without increasing the frequency of SGA, an effect that was partly mediated by excessive GWG.

OBJETIVO: Avaliar a associação entre eventos de vida produtores de estresse (EVPE) e queixas de insônia (QI). MÉTODOS: Foram analisados dados seccionais de 695 auxiliares de enfermagem de um hospital universitário, participantes do Estudo Pró-Saúde - coorte de funcionários de uma universidade no Rio de Janeiro. As informações foram obtidas através de um questionário multidimensional e autopreenchido, que avaliou a ocorrência de EVPE nos últimos 12 meses, variáveis socioeconômicas e demográficas e QI. As QI foram analisadas como desfecho politômico (frequente, ocasional, e ausente). Odds ratios brutos e ajustados foram calculados através de regressão logística multinomial. RESULTADOS: A prevalência total de QI foi de 45,8% (16,7% frequentes e 29,1% ocasionais). Após ajuste por sexo, idade, estado civil, renda familiar per capita e regime de trabalho, os EVPE associados com QI frequentes foram: "rompimento de relação amorosa" (OR = 3,32; IC95% 1,90 - 5,78), "ter tido problemas graves de saúde" (OR = 2,82; IC95% 1,73 - 4,58); "dificuldades financeiras graves" (OR = 2,38; IC95% 1,46 - 3,88), e "mudança forçada de moradia" (OR = 1,97; IC95% 1,02 - 3,79). Com relação às QI ocasionais, houve associação apenas com rompimento de relação amorosa (OR = 2,30; IC95% 1,42 - 3,74) e dificuldades financeiras graves (OR = 1,87; IC95% 1,27 - 2,75). CONCLUSÕES: Dada a responsabilidade com vidas humanas que os auxiliares de enfermagem assumem durante seu horário de trabalho, nossos achados podem contribuir para ações mais efetivas, por parte dos serviços de saúde ao trabalhador, para lidar com o estresse nessa categoria.OBJECTIVE: To evaluate the association between stressful life events (SLE) and insomnia complaints (IC). METHODS: Cross-sectional data from 695 certified nursing assistants from a university hospital, participants of the Pro-Saude Study - a cohort of university employees in Rio de Janeiro, were analyzed. Information was obtained through a self-administered multidimensional questionnaire, which evaluated the occurrence of SLE in the previous 12 months, socioeconomic and demographic variables, and IC. Insomnia complaints were analyzed as a polythomic outcome (frequent, occasional, and absent). Crude and adjusted odds ratios were calculated through multinomial logistic regression. RESULTS: Total prevalence of IC was 45.8% (frequent 16.7% and occasional 29.1%). After adjusting for sex, age, marital status, per capita family income, and work hours, the SLE associated with frequent IC were: "breakup of a love relationship" (OR = 3.32; 95%CI 1.90-5.78), "having had severe health problems" (OR=2.82; 95%CI 1.73-4.58), "severe financial problems" (OR=2.38; 95%CI 1.46-3.88), and "forced change of residence" (OR=1.97; 95%CI 1.02-3.79). Occasional IC was associated only with "breakup of a love relationship" (OR=2.30; 95%CI 1.42-3.74) and "severe financial problems" (OR=1.87; 95%CI 1.27-2.75). CONCLUSIONS: Given the responsibility over human lives taken on by nursing assistants during their work hours, our findings may contribute to more effective actions to be taken by health services toward workers in this category, helping them better deal with stress.

This paper focuses on recent trends in risky sexual practices for HIV/AIDS among men who have sex with men (MSM) in Fortaleza, Ceará State, Brazil. Four cross-sectional surveys were conducted (1995, 1998, 2002, and 2005) among MSM 14 years or older who reported oral or anal sex in the previous 12 months. Sexual practices were considered risky whenever the respondent reported unprotected receptive or insertive anal intercourse in the six months preceding the interview. Different selection techniques were used to recruit the study population: snowball (1995, 1998, 2002 - 32%); time-space sampling (2002 - 68%); and respondent-driven sampling (2005). Analyses were based on the comparison between proportions. High rates of risky sexual practices were reported in 1995 (49.9%), decreasing in 1998 (32.6%), increasing again in 2002 (51.3%), and showing the lowest level in 2005 (31.4%). Participants with more schooling increased their risky practices from 1998 to 2002, decreasing in 2005. Among individuals with medium or low schooling, risky behavior declined from 2002 to 2005. The article highlights the need for behavioral surveillance to properly address STD/HIV prevention.Estudaram-se as tendências recentes das práticas sexuais de risco para DST/AIDS entre homens que fazem sexo com homens (HSH) em Fortaleza, Ceará, Brasil. Realizaram-se quatro estudos seccionais (1995, 1998, 2002 e 2005) em HSH, com 14 anos ou mais, que tenham referido prática sexual oral ou anal com homens nos últimos 12 meses. Definiu-se prática sexual de risco uma relação anal insertiva ou receptiva sem uso de preservativo nos seis meses que antecederam a entrevista. Utilizaram-se técnicas de recrutamento do tipo snow ball (1995, 1998, 2002 - 32%); time space sampling (2002 - 68%) e respondent driven sampling (2005). Análises basearam-se nas comparações entre proporções. Elevados percentuais de práticas sexuais de risco foram referidos em 1995 (49,9%), decrescendo em 1998 (32,6%), crescendo em 2002 (51,3%) e apresentando os menores percentuais em 2005 (31,4%). Indivíduos com escolaridade mais elevada aumentaram as práticas de risco entre 1998 e 2002, decrescendo no último período. Nos de estrato de escolaridade baixa ou média observou-se declínio no comportamento de risco entre 2002 e 2005. Os resultados deste estudo salientam a importância da vigilância comportamental para adequação dos programas de prevenção das DST/AIDS nessa população.

INTRODUCTION: Leisure-Time Physical Activity (LTPA) is a health behavior that is considered relatively stable over the course of life; this life-long habit seems to be shaped during youth and early adulthood. LTPA is one of the few healthy behaviors more prevalent among men than among women. METHODS:Data from 3,199 participants of the Pro-Saude Study were analyzed - a cohort of employees of a university in Rio de Janeiro, collected with a self-reported multidimensional questionnaire. The association between LTPA in early adulthood and later was investigated using multinomial logistic regression, with estimated odds ratios (OR), considering three outcome categories: inactive, insufficiently active, and active. Past LTPA was grouped into three categories: never/rarely (reference category), sometimes and frequently/always. Estimates were adjusted for: age; participants' schooling, their father's and mother's schooling, and color/race. RESULTS:Analysis shows men who engaged in LTPA sometimes and almost always/always in early adulthood had an OR= 1.42 (95% CI: 0.70-2.89) and OR= 3.33 (95% CI: 1.82-6.10), respectively, of being physically active in later adulthood than those who did not engage in LTPA. Among women, the corresponding odds ratios were lower: OR = 1.19 (95% CI: 0.79-1.79) and OR =1.42 (95% CI: 1.00-2.04). CONCLUSION:LTPA during early adulthood is associated with physical activity later in adulthood, and this association is stronger in men than in women. Public policies that encourage LTPA among youth, with specific efforts directed at women, could increase the proportion of physically active adults.INTRODUÇÃO: Atividade física é um comportamento de saúde relativamente estável, em geral determinado no início da vida adulta. Atividade física de lazer (AFL) é um dos poucos comportamentos benéficos à saúde mais prevalente entre homens que entre mulheres. MÉTODOS: Foram analisados dados de 3.199 participantes do Estudo Pró-Saúde - coorte de funcionários de universidade no Rio de Janeiro, coletados por meio de questionário multidimensional e autopreenchível. A associação entre AFL no início da vida adulta e posteriormente foi analisada com uso de regressão logística multinomial, com estimativas de razões de chance (odds ratio) e seus respectivos intervalos de 95% de confiança considerando três categorias de desfecho: sedentários, insuficientemente ativos e ativos. AFL pregressa foi agrupada em 3 categorias: nunca/raramente (categoria de referência), às vezes, quase sempre/sempre. Estimativas foram ajustadas por idade, escolaridade, escolaridade dos pais e cor/raça. RESULTADOS:Homens que se engajavam em AFL às vezes e quase sempre/sempre no início da vida adulta tiveram, respectivamente, OR = 1,42 (IC 95%: 0,70-2,89) e OR = 3,33 (IC 95%: 1,82-6,10) de ser fisicamente ativos em idade posterior do que os sedentários. Entre as mulheres, os odds ratios foram menores: OR = 1,19 (IC 95%: 0,79-1,79) e OR =1,42 (IC 95%: 1,00-2,04). CONCLUSÃO:A prática de AFL durante o início da vida adulta apresentou associação com atividade física posterior, e essa associação foi mais expressiva em homens do que em mulheres. Políticas públicas que encorajem a AFL entre os jovens, com esforços específicos direcionados às mulheres, poderiam aumentar a proporção de adultos fisicamente ativos.

The geographic spread and urbanization of visceral leishmaniasis in Brazil has been described since the early 1980s. However, the putative factors associated with this process, its full characterization, and the implications for disease control still challenge researchers and Public Health professionals. Although the available data show that the disease occurs mainly in urban areas, current knowledge is insufficient to claim specificity in urban transmission as compared to rural niches. Transmission scenarios in urban settings appear to be highly heterogeneous, each showing some degree of similarity to the rural epidemiological pattern. The understanding of a relatively recent and complex problem like the introduction, spread, and maintenance of visceral leishmaniasis in urban areas requires new analytical approaches that consider the network of relevant variables and more elaborate methods capable of capturing the dynamics of the environmental and demographic transformations taking place in transmission areas.A expansão geográfica e urbanização da leishmaniose visceral no Brasil é um fenômeno notável desde a década de 1980. Entretanto, o detalhamento dos motivos relacionados a este processo, assim como sua própria caracterização e as implicações em termos de ações de controle da doença vêm desafiando pesquisadores e profissionais da área de Saúde Coletiva. Ainda que os dados disponíveis demonstrem que a doença ocorre majoritariamente em áreas urbanas, o conhecimento acumulado não permite afirmar que há suficiente particularidade na transmissão urbana, em contraposição àquela que ocorre nos tradicionais nichos rurais. O que parece existir é uma heterogeneidade nos cenários de transmissão em meio urbano, cada um deles com nuanças mais ou menos distintas, mostrando maior ou menor similaridade em relação ao padrão epidemiológico rural. A compreensão de um problema relativamente recente e complexo, como é a introdução, disseminação e manutenção da leishmaniose visceral em meio urbano, necessita de abordagens analíticas que considerem a estrutura intricada das variáveis envolvidas, além de métodos mais elaborados para captação da dinâmica das transformações demográficas e ambientais em curso nas áreas de transmissão.

O objetivo deste estudo foi estimar a prevalência de hipertensão arterial segundo estratos de circunferência abdominal (CA) e índice de massa corporal (IMC, em kg/m²) em uma população de funcionárias públicas de universidade localizada no Rio de Janeiro, Brasil. Foram analisados dados seccionais de 1.743 mulheres não grávidas de 24 a 69 anos participantes, em 2001, do Estudo Pró-Saúde. Mulheres com baixo peso ou apresentando IMC > 35kg/m² foram excluídas. Mulheres que apresentavam pressão sangüínea sistólica > 140mmHg ou diastólica > 90mmHg ou faziam uso de medicação anti-hipertensiva foram consideradas hipertensas. A análise foi conduzida segundo dois estratos de CA (normal: < 88cm; elevado: > 88cm) e três de IMC (eutrofia: 18,5-24,9kg/m²; sobrepeso: 25,0-29,9kg/m²; e obesidade I: 30,0-34,9kg/m²). Entre mulheres eutróficas, participantes com valores elevados de CA apresentaram o dobro da prevalência de hipertensão arterial do que aquelas com CA < 88cm (18% vs. 8%; p < 0,05). Adicionalmente à mensuração do IMC, a aferição da CA na rotina dos serviços de saúde pode contribuir para a identificação precoce ou suspeição de hipertensão arterial.This study aimed to estimate the prevalence of hypertension according to levels of abdominal circumference (AC) and body mass index (BMI, expressed as kg/m²) among public employees at a university in Rio de Janeiro, Brazil. Cross-sectional data were obtained for 1,743 non-pregnant women aged 24-69 years participating in the Pró-Saúde Study. Underweight women and those with BMI > 35kg/m² were excluded. Hypertension was defined as systolic blood pressure >140mmHg or diastolic > 90mmHg or the use of anti-hypertensive drugs. Age-adjusted prevalence rates for hypertension were calculated considering two strata of AC (normal: < 88cm; high: > 88cm) and three levels of BMI (normal: 18.5-24.9kg/m²; overweight: 25.0-29.9kg/m²; and obesity I: 30.0-34.9kg/m²). Among normal-weight women, participants with high AC showed a two-fold higher prevalence of hypertension as compared to those with AC < 88cm (18% vs. 8%, p < 0.05). Routine measurement of AC, in addition to BMI, could contribute to early identification of hypertension in women.

Queixas relativas à saúde têm sido mais freqüentemente associadas a problemas no trabalho do que a qualquer outro aspecto da vida, incluindo problemas financeiros ou familiares. Nesse estudo, investigamos a associação entre estresse no trabalho e a interrupção das atividades habituais por problemas de saúde. Esta investigação, do tipo transversal, se insere em um estudo prospectivo de 2.343 funcionários técnico-administrativos de uma universidade no Estado do Rio de Janeiro - Estudo Pró-Saúde. Procederam-se as análises multivariadas valendo-se dos modelos lineares generalizados e assumindo-se uma distribuição binomial e função de ligação logarítmica. Os homens em atividades com alta exigência apresentaram prevalência da interrupção das atividades habituais duas vezes maior do que aqueles cujas atividades não foram classificadas nessa categoria (baixa exigência; ativo; passivo), ajustando-se por idade (RP = 2,06; IC95%: 1,54-2,76). Entre as mulheres, a prevalência do desfecho foi 45% maior (RP = 1,45; IC95%: 1,17-1,79).Health-related complaints have been associated with work problems more frequently than with any other aspect of life, including financial and family problems. This study focuses on the association between stress at work and interruption of routine activities due to health problems. The study adopted a cross-sectional approach and is part of a prospective study of 2,343 technical and administrative employees at a public university in the State of Rio de Janeiro, Brazil, known as the Pró-Saúde Study. Multivariate analyses were conducted using the generalized linear models and assuming a binomial distribution and log-link function. Men in jobs with heavy demands showed twice the prevalence of interruption of routine activities as compared to those not classified in this category (low demand; active; passive), after adjusting for age (PR = 2.06; 95%CI: 1.54-2.76). Among women, the outcome prevalence was 45% greater (PR = 1.45; 95%CI: 1.17-1.79).

O objetivo deste trabalho foi avaliar a efetividade do controle vetorial e da eliminação de cães infectados na incidência de infecção por Leishmania chagasi. Um estudo de intervenção comunitário foi realizado em Teresina entre 1995 e 1996. A área foi dividida em 34 lotes alocados aleatoriamente a 4 tipos de intervenção: 1) borrifação intradomiciliar e de anexos residenciais; 2) borrifação intradomiciliar e eliminação de cães infectados; 3) combinação de borrifação intradomiciliar e de anexos e eliminação canina; 4) apenas borrifação intradomiciliar. Em comparação com lotes que receberam apenas borrifação intradomiciliar, a eliminação canina diminui em 80% a incidência de infecção. A borrifação de anexos, associada ou não à eliminação canina, não apresentou efeito significativo. A proteção oferecida pela remoção de cães infectados sugere que esta estratégia pode reduzir o pool de fontes de infecção para flebotomíneos.The objective of this study was to evaluate the effect of vector control and elimination of infected dogs on the incidence of infection with Leishmania chagasi. A randomized community intervention trial was carried out in Teresina between 1995 and 1996. The area was divided in 34 blocks randomically allocated to 4 types of intervention: 1) spraying houses and animal pens with insecticide; 2) spraying houses and eliminating infected dogs; 3) combination of spraying houses and animal pens, and eliminating infected dogs, and 4) only spraying houses. In comparison to blocks receiving only household spraying, culling dogs decreased in 80% the incidence of infection. Spraying animal pens, associated or not with culling dogs, showed no significant effect. The protection offered by culling dogs suggests that this strategy might reduce the source of infection for the vector.

No Brasil, o controle do vetor Lutzomyia longipalpis é uma das principais estratégias utilizadas para limitar a expansão da leishmaniose visceral americana. Entretanto, poucos são os estudos sobre a infecção natural destes flebotomíneos por espécies de Leishmania. Um estudo sobre a infecção natural de Lu. longipalpis por Leishmania sp. foi realizado no bairro Bela Vista, Teresina, Piauí, um dos principais focos urbanos da leishmaniose visceral americana no Brasil. Entre fevereiro de 2004 e janeiro de 2005 realizaram-se 180 capturas usando-se armadilhas luminosas do tipo CDC. Foram dissecadas e examinadas, em média, dez fêmeas por captura para detecção de formas evolutivas de Leishmania sp., identificando-se 1.832 exemplares de Lu. longipalpis e seis de Lu. whitmani. Vinte (1,1%) espécimes, todas de Lu. longipalpis, estavam infectadas com as formas procíclica e nectomonada de Leishmania sp., localizadas, principalmente, na porção posterior do trato digestivo. O maior percentual de insetos infectados foi encontrado quatro meses após o período de maior precipitação pluviométrica, sugerindo que variáveis climáticas podem contribuir para a predição não apenas da abundância destes dípteros, mas também do seu grau de infecção natural.In Brazil, control of the vector Lutzomyia longipalpis is one of the main strategies used to limit the expansion of American visceral leishmaniasis. However, studies on the ecology of this sand fly are rare, especially regarding its natural infection with species of Leishmania. A study of the natural infection of Lu. longipalpis by Leishmania sp. was carried out in the Bela Vista neighborhood in the city of Teresina, Piauí State, Brazil, an important area of American visceral leishmaniasis transmission. From February 2004 to January 2005, sand flies were captured with CDC light traps. Approximately 10 female sand flies in each capture were dissected and examined for the presence of evolutionary forms of Leishmania sp. Two sand fly species were identified: 1,832 were Lu. longipalpis and six Lu. whitmani. Twenty female sand flies (1.1%), all Lu. longipalpis, were infected with procyclic and nectomonad forms of Leishmania sp., found mostly in the hindgut. Higher proportions of infected sand flies were found four months after the rainy season, suggesting that environmental factors may predict not only vector abundance (as already known) but also their level of infection.

Geospatial analysis was used to study the epidemiology of Schistosoma mansoni, intestinal parasites and co-infections in an area (Ilha das Flores) in Sergipe, Brazil. We collected individually georeferenced sociodemographic, behavioral and parasitological data from 500 subjects, analyzed them by conventional statistics, and produced risk maps by Kernel estimation. The prevalence rates found were: S. mansoni (24.0%), Trichuris trichiura (54.8%), Ascaris lumbricoides (49.2%), Hookworm (17.6%) and Entamoeba histolytica (7.0%). Only 59/500 (11.8%) individuals did not present any of these infections, whereas 279/500 (55.8%) were simultaneously infected by three or more parasites. We observed associations between S. mansoni infection and various variables such as male gender, being rice farmer or fisherman, low educational level, low income, water contact and drinking untreated water. The Kernel estimator indicated that high-risk areas coincide with the poorest regions of the villages as well as with the part of the villages without an adequate sewage system. We also noted associations between both A. lumbricoides and hookworm infections with low education and low income. A. lumbricoides infection and T. trichiura infection were both associated with drinking untreated water and residential open-air sewage. These findings call for an integrated approach to effectively control multiple parasitic infections.

Neste artigo, relatam-se propriedades psicométricas do Instrumento Abreviado de Avaliação de Qualidade de Vida da Organização Mundial da Saúde (WHOQOL-Bref), composto por 24 questões distribuídas em quatro domínios (físico, psicológico, relações sociais e meio ambiente) e duas questões globais sobre qualidade de vida e saúde global, quando aplicado no Estudo Pró-Saúde (estudo de coorte de trabalhadores de uma universidade no Estado do Rio de Janeiro, Brasil), em seu estudo piloto e entre os 3.574 participantes da fase 2 (2001) do estudo. Foi estimada a confiabilidade teste-reteste dos escores, com coeficiente de correlação intraclasse variando entre 0,76 e 0,91 nos diversos domínios. Níveis de consistência interna, avaliados pelo coeficiente alfa de Cronbach, variaram entre 0,69 e 0,79. Na análise de fatores, pelo método de fatores principais iterados e rotação varimax, não foi replicada exatamente a estrutura do instrumento original, verificada em seus testes de campo; uma das possíveis razões para as discrepâncias observadas refere-se à natureza da população de estudo, em idade laboral ativa e relativamente saudável.This paper reports on the psychometric properties of the World Health Organization Abbreviated Instrument for Quality of Life Assessment (WHOQOL-Bref), comprising 26 items which measure the following broad domains: physical health, psychological health, social relationships, and environment. The instrument was used in the Pró-Saúde Study, a cohort study of public employees at university campuses in Rio de Janeiro, Brazil, during the pilot phase, and by 3,574 subjects in study phase 2 (2001). The estimated test-retest reliability of the responses, measured by intraclass correlation coefficients, ranged from 0.76 to 0.91 across domains. The internal consistency of the items, measured by Cronbach's alpha coefficients, was estimated between 0.69 and 0.79. The factor structure, using iterated principal factor methodology and varimax rotation, did not exactly replicate the structure of the original instrument as assessed during its field tests; among other reasons, the observed discrepancies may be due to the study population's characteristics (active working-age and relatively healthy).

A história pregressa de tentativa de suicídio é um importante preditor do suicídio e de novas tentativas. Este artigo apresenta o perfil dos casos de tentativas de suicídio detectados por meio de um sistema de monitoramento para esses agravos em um hospital geral no Rio de Janeiro, Brasil. Entre abril de 2001 e março de 2002 foram registrados 160 tentativas de suicídio, sendo 68% entre mulheres e 26% entre adolescentes. A ingestão de pesticidas e o abuso de medicamentos foram os principais métodos utilizados. Mulheres utilizaram os dois métodos na mesma proporção, enquanto 2/3 dos homens empregaram pesticidas. Em relação à prevalência dos fatores de risco para tentativas de suicídio, identificou-se que 21% dos pacientes haviam procurado serviços de saúde nos trinta dias anteriores ao evento, 28% referiram tentativas anteriores, 23% fizeram referência a casos de tentativas ou suicídio na família. Ainda que os dados populacionais disponíveis sejam escassos, as tentativas de suicídio parecem ser um evento importante de morbidade, particularmente entre adolescentes e adultos jovens. Sistemas de vigilância para esse agravo podem ser úteis para um melhor conhecimento do problema.Previous suicide attempts are an important predictor of both repeated attempts and suicide. This paper presents the profile of patients who had attempted suicide and were admitted to a general hospital in Rio de Janeiro, Brazil. From April 2001 to March 2002, 160 suicide attempts were recorded (68% women; 26% adolescents). Ingestion of pesticides and prescription drugs were the two most common methods used. The two methods had been used by similar numbers of women, while two-thirds of men had used pesticides. As for prevalence of factors associated with attempted suicide, 21% of patients had been in contact with health services within 30 days prior to the event, 28% mentioned previous suicide attempts, and 23% reported other cases of suicide or attempts in the family. Despite the limited data in Brazil, suicide attempts appear to be an important health problem, particularly among youth. Surveillance systems for suicide attempts could help expand knowledge on this problem.

A adequação das condutas terapêuticas e a validade das inferências epidemiológicas sobre a hipertensão arterial dependem crucialmente da acurácia dos procedimentos para sua aferição. Este artigo relata a experiência dos procedimentos adotados para o treinamento e controle de qualidade da aferição da pressão arterial na Fase 2 do Estudo Pró-Saúde, conduzido entre 3.574 funcionários de uma universidade no Rio de Janeiro, Brasil, em 2001. Foram utilizados aparelhos de coluna de mercúrios e técnicas padronizadas de aferição, baseada em protocolos internacionais. Foram monitoradas a adesão dos aferidores às técnicas preconizadas, a preferência por dígitos terminais, a diferença entre aferições consecutivas e a proporção de dados faltantes. O conjunto de procedimentos utilizados contribuiu para minimizar possíveis erros sistemáticos associados à técnica auscultatória de aferição da pressão arterial, possibilitando análises válidas acerca de fatores associados à ocorrência da hipertensão arterial.

Este artigo verifica a validade de constructo da escala de apoio social utilizada em um estudo de coorte de trabalhadores de uma universidade no Rio de Janeiro, Brasil (Estudo Pró-Saúde). Os dados foram obtidos na primeira etapa do estudo, realizada em 1999, na qual 4.030 funcionários técnico-administrativos completaram, no ambiente de trabalho, um questionário multidimensional e autopreenchível. Associações entre as dimensões de apoio social e características sócio-demográficas, relacionadas à saúde e ao bem-estar, foram avaliadas. Por meio da análise fatorial, foi possível discriminar três dimensões de apoio: interação social positiva/apoio afetivo; apoio emocional/de informação; e apoio material. Foram confirmadas as hipóteses de que indivíduos menos solitários, com melhor estado de saúde auto-referido, com relato de participação mais freqüente em atividades sociais em grupo e sem suspeição para transtornos mentais comuns teriam maior percepção de apoio social. Conclui-se que há evidências de que o instrumento apresenta validade de constructo, indicando-se sua utilização em análises futuras, no âmbito do Estudo Pró-Saúde e em populações similares.

Este estudo teve o objetivo de avaliar a associação de fatores sócio-demográficos com a prática de atividade física de lazer (AFL). O estudo de base, do tipo seccional, foi realizado entre 4.030 funcionários de uma universidade do Estado do Rio de Janeiro, através de questionário preenchível pelos próprios indivíduos. AFL foi obtida através da avaliação das atividades físicas praticadas nas duas semanas anteriores ao preenchimento do questionário, avaliada através da estimativa do Equivalente Metabólico semanal (EMS). Os valores de razão de chances entre fatores sócio-demográficos e AFL, e foram estimados através de modelos de regressão logística multinomial. As perguntas elaboradas para avaliar a prática de AFL apresentaram reprodutibilidade moderada, com o coeficiente kappa variando entre 0,45 a 0,88 entre as atividades avaliadas. A prevalência de praticantes de AFL foi maior entre os homens (52,2% homens; 40,8% mulheres). Tanto para homens como para as mulheres, quanto maior a escolaridade e a renda familiar per capita, maior a prática de AFL, quando se comparou o terço extremo de EMS com inatividade física de lazer. A prática de AFL foi maior entre os homens, sendo observado associação entre AFL e variáveis sócio-demográficas em ambos os sexos.This study evaluated the relationship between socio-demographic variables and leisure-time physical activity (LTPA). A baseline survey questionnaire was completed by 4,030 employees. LPTA was assessed as engagement in physical activities during the previous two weeks. The metabolic cost of LTPA was calculated based on frequency and amount of time participating in physical activities, assigning weekly metabolic equivalents (METs) to each activity. Odds ratios (OR) to express the relationship between socio-demographic factors and LTPA were estimated from multinomial regression models. The questions used to evaluate LTPA presented moderate reproducibility, with kappa correlation coefficients ranging from 0.45 to 0.88. Prevalence of LTPA was higher among men (52.2% men; 40.8% women). For both men and women, higher educational level and higher income were both associated with engaging in LPTA, this relationship being stronger for the highest tercile of METs as compared to physical inactivity. Men were more engaged in LTPA, and there was a strong association between LTPA and socio-demographic variables for both genders.

Most ecologic studies use geographical areas as units of observation. Because data from areas close to one another tend to be more alike than those from distant areas, estimation of effect size and confidence intervals should consider spatial autocorrelation of measurements. In this report we demonstrate a method for modeling spatial autocorrelation within a mixed model framework, using data on environmental and socioeconomic determinants of the incidence of visceral leishmaniasis (VL) in the city of Teresina, Piauí, Brazil. A model with a spherical covariance structure indicated significant spatial autocorrelation in the data and yielded a better fit than one assuming independent observations. While both models showed a positive association between VL incidence and residence in a favela (slum) or in areas with green vegetation, values for the fixed effects and standard errors differed substantially between the models. Exploration of the data's spatial correlation structure through the semivariogram should precede the use of these models. Our findings support the hypothesis of spatial dependence of VL rates and indicate that it might be useful to model spatial correlation in order to obtain more accurate point and standard error estimates.A maioria dos estudos ecológicos utiliza áreas geográficas como unidades de observação. Uma vez que as áreas geograficamente próximas tendem a ser mais semelhantes do que as distantes, as estimativas da magnitude do efeito e dos intervalos de confiança devem levar em conta a auto-correlação espacial das medidas. Neste estudo demonstramos um método para modelar a auto-correlação espacial dentro de um referencial de modelo misto, utilizando dados sobre determinantes ambientais e sócio-econômicos da incidência de leishmaniose visceral (LV) na cidade de Teresina, Estado do Piauí. Um modelo com uma estrutura de covariância esférica indicou uma auto-correlação espacial significativa nos dados e produziu melhor ajuste quando comparado com outro modelo que pressupunha observações independentes. Embora ambos modelos tenham demonstrado associações positivas entre incidência de LV e residência em favelas ou áreas com vegetação verde, os valores para os efeitos fixos e erros-padrão diferiram substancialmente entre os modelos. A estrutura da correlação espacial dos dados deve ser explorada através do semivariograma, antes da utilização destes modelos. Nossos achados favorecem a hipótese da dependência espacial dos coeficientes de incidência de LV e sugerem que a modelagem da correção espacial poderia ser útil para obter estimativas pontuais e de erros-padrão mais acuradas.

Os estudos de agregados espaço-temporais têm sido freqüentemente utilizados em epidemiologia e saúde pública nas últimas décadas, muitas vezes sem a adequada compreensão de seus limites e potencialidades. O objetivo geral deste artigo é rever alguns tópicos essenciais para aprimorar a interpretação e utilidade de estudos neste contexto. Inicia-se com uma discussão sobre o significado epidemiológico do conceito de agregação espaço-temporal. A seguir, descrevem-se cinco métodos analíticos, assim como os procedimentos necessários para sua utilização. Apresenta-se também uma proposta de generalização conceitual e estatística das técnicas analisadas. Por fim, discute-se uma gama de tópicos relevantes para a otimização do uso destes estudos em epidemiologia e saúde pública.Methods for the analysis of space-time clusters have been used frequently in epidemiology and public health in recent decades. However, they have generally been applied without an appropriate critique of their limits and usefulness. This article reviews some of the most important issues involved in improving the interpretability and utility of such methods in this context. We begin with a brief discussion about the epidemiological meaning of the concept of space-time clustering. We then describe five commonly used methods, including the procedures for their use. We also present a conceptual and statistical generalization of the techniques. Finally, we summarize some important issues for use of these methods in epidemiology and public health.

This study evaluates the association between postpartum depression and interruption of exclusive breastfeeding in the first two months of life. Cohort study of 429 infants < 20 days of age to four primary health care units in Rio de Janeiro, Brazil. Interruption of exclusive breastfeeding (outcome) was defined as the introduction of water, other types of liquids, milk, or formulas or any food. Postpartum depression was assessed using the Edinburgh Post-Natal Depression Scale. Associations between variables were expressed as prevalence ratios (baseline) and risk ratios (follow-up), with their respective 95% confidence intervals, estimated by Poisson regression with robust variance. Children of mothers with postpartum depressive symptoms were at higher risk of early interruption of exclusive breastfeeding in the first and second months of follow-up (RR = 1.46; 95%CI: 0.98-2.17 and RR = 1.21; 95%CI: 1.02-1.45, respectively). Considering mothers that were exclusively breastfeeding at the first month, postpartum depression was not associated with interruption of exclusive breastfeeding in the second month (RR = 1.44; 95%CI: 0.68-3.06). The results indicate the importance of maternal mental health for the success of exclusive breastfeeding.Avaliou-se a associação entre depressão pós-parto e interrupção precoce do aleitamento materno exclusivo nos dois primeiros meses de vida. Estudo de coorte com 429 crianças < 20 dias de idade em quatro unidades de saúde no Rio de Janeiro, Brasil. Considerou-se como interrupção precoce do aleitamento materno exclusivo a introdução de chá, água, suco, leite artificial ou qualquer outro alimento. Na avaliação da depressão pós-parto utilizou-se a Edinburgh Post-natal Depression Scale. Associações foram expressas como razões de prevalências (linha de base) e riscos relativos (primeiro e segundo meses de vida) e respectivos intervalos de 95% de confiança estimados via regressão de Poisson com variância robusta. Filhos de mulheres com sintomas de depressão pós-parto apresentam maior risco de interrupção precoce do aleitamento materno exclusivo nos dois meses de seguimento (RR = 1,46; IC95%: 0,98-2,17 e RR = 1,21; IC95%: 1,02-1,45, respectivamente). Entre mães que amamentam exclusivamente até o primeiro mês de vida, depressão pós-parto não se associou à interrupção precoce do aleitamento materno exclusivo (RR = 1,44; IC95%: 0,68-3,06). Esses achados apontam para a importância da saúde mental materna no sucesso do aleitamento materno exclusivo.

Neste artigo faz-se uma análise da magnitude das causas de morte no estado e no município do Rio de Janeiro em 1990, com base no cálculo dos "anos potenciais de vida perdidos" (APVPs) entre as idades de 1 e 70 anos. Esta análise leva a uma substancial modificação na importância relativa das causas de óbito, pois este método imputa um maior valor às mortes que ocorrem nas fases precoces da vida. De acordo com este método, a análise pelos grandes grupos de causas de óbito mostra que as duas regiões apresentam o grupo das "causas externas" como principal causa de APVP, sendo as mesmas responsáveis por mais de 30,0% dos APVPs entre 1 e 70 anos. Esta estimativa está em contraposição à predominância das doenças cardiovasculares, indicada pelos perfis de mortalidade que utilizam os coeficientes específicos de mortalidade por grandes grupos de causas. A análise por causas específicas de morte aponta os homicídios como principais responsáveis pelos APVPs nas duas regiões, seguidas pelos neoplasmas malignos, no município do Rio de Janeiro, e por "outros tipos de violência" (suicídios e violências em que ignora se de causa acidental ou intencional), no estado do Rio de Janeiro. Exceção feita às doenças neoplásicas, as taxas de APVP por grandes grupos de causas, em ambos os sexos, são sempre menores no município do Rio de Janeiro. O total de APVP e as taxas de APVP são marcadamente mais altas no sexo masculino, em decorrência, principalmente, das causas externas. Os autores salientam a importância da utilização deste indicador de mortalidade prematura para a definição de prioridades e chamam a atenção para o caráter devastador dos traumas e violências como causa de mortes em idades jovens.This article is concerned with the magnitude of causes of death in the State and Municipality of Rio de Janeiro, in 1990, according to the "Years of Potential Life Lost" (YPLL) from one to 70 years of age. This indicator provides a more precise evaluation of the greater relevance of early deaths. An analysis of YPLL by main causes of death shows that in both the State and Municipality, external causes (traumas and injuries) are the leading causes with over 30% of YPLL. This finding goes against the accepted mortality-rate-based view of chronic heart disease as the most important cause of death. Analysis of specific causes of death for both the State and Municipality puts homicides in first place, followed by cancers in the Municipality of Rio de Janeiro and "other types of violence" (suicides and violence of unknown type, whether accidental or intentional) in the State of Rio de Janeiro. With the exception of cancers, YPLL rates by main death groups are consistently lower in the Municipality of Rio de Janeiro than the State as a whole, irrespective of gender. Absolute figures and rates for YPLL are strikingly higher in males, due mainly to "external causes". The authors stress the importance of using this indicator of premature deaths for defining priorities in health actions. The devastating effect of traumas and injuries as causes of deaths at early ages is also emphasized.

Tanto para homens como para as mulheres, quanto maior a escolaridade e a renda familiar per capita, maior a prática de AFL, quando se comparou o terço extremo de EMS com inatividade física de lazer. A prática de AFL foi maior entre os homens, sendo observado associação entre AFL e variáveis sócio-demográficas em ambos os sexos.

Most ecologic studies use geographical areas as units of observation. Because data from areas close to one another tend to be more alike than those from distant areas, estimation of effect size and confidence intervals should consider spatial autocorrelation of measurements. In this report we demonstrate a method for modeling spatial autocorrelation within a mixed model framework, using data on environmental and socioeconomic determinants of the incidence of visceral leishmaniasis (VL) in the city of Teresina, Piauí, Brazil. A model with a spherical covariance structure indicated significant spatial autocorrelation in the data and yielded a better fit than one assuming independent observations. While both models showed a positive association between VL incidence and residence in a favela (slum) or in areas with green vegetation, values for the fixed effects and standard errors differed substantially between the models. Exploration of the data's spatial correlation structure through the semivariogram should precede the use of these models. Our findings support the hypothesis of spatial dependence of VL rates and indicate that it might be useful to model spatial correlation in order to obtain more accurate point and standard error estimates.

Os estudos de agregados espaço-temporais têm sido freqüentemente utilizados em epidemiologia e saúde pública nas últimas décadas, muitas vezes sem a adequada compreensão de seus limites e potencialidades. O objetivo geral deste artigo é rever alguns tópicos essenciais para aprimorar a interpretação e utilidade de estudos neste contexto. Inicia-se com uma discussão sobre o significado epidemiológico do conceito de agregação espaço-temporal. A seguir, descrevem-se cinco métodos analíticos, assim como os procedimentos necessários para sua utilização. Apresenta-se também uma proposta de generalização conceitual e estatística das técnicas analisadas. Por fim, discute-se uma gama de tópicos relevantes para a otimização do uso destes estudos em epidemiologia e saúde pública.

INTRODUCTION: Leisure-Time Physical Activity (LTPA) is a health behavior that is considered relatively stable over the course of life; this life-long habit seems to be shaped during youth and early adulthood. LTPA is one of the few healthy behaviors more prevalent among men than among women. METHODS:Data from 3,199 participants of the Pro-Saude Study were analyzed - a cohort of employees of a university in Rio de Janeiro, collected with a self-reported multidimensional questionnaire. The association between LTPA in early adulthood and later was investigated using multinomial logistic regression, with estimated odds ratios (OR), considering three outcome categories: inactive, insufficiently active, and active. Past LTPA was grouped into three categories: never/rarely (reference category), sometimes and frequently/always. Estimates were adjusted for: age; participants' schooling, their father's and mother's schooling, and color/race. RESULTS:Analysis shows men who engaged in LTPA sometimes and almost always/always in early adulthood had an OR= 1.42 (95% CI: 0.70-2.89) and OR= 3.33 (95% CI: 1.82-6.10), respectively, of being physically active in later adulthood than those who did not engage in LTPA. Among women, the corresponding odds ratios were lower: OR = 1.19 (95% CI: 0.79-1.79) and OR =1.42 (95% CI: 1.00-2.04). CONCLUSION:LTPA during early adulthood is associated with physical activity later in adulthood, and this association is stronger in men than in women. Public policies that encourage LTPA among youth, with specific efforts directed at women, could increase the proportion of physically active adults.

Abstract: The aim of this study was to investigate the role of intimate partner violence in the early interruption of exclusive breastfeeding in the first three months of life. We used data from a prospective cohort of 564 children attending four primary health clinics in Rio de Janeiro, Brazil. Interruption of exclusive breastfeeding was defined as a child receiving any kind of liquid or solid, regardless of breast milk intake, measured by a 24 hour recall. The Portuguese version of the Conflict Tactics Scales (CTS-1) was used to assess intimate partner violence. Associations were expressed as prevalence ratios and relative risks and their respective 95% confidence intervals. Children of mothers who experienced severe violence had 30% greater likelihood of early interruption of exclusive breastfeeding in the second month of life as compared to those who did not experience this type of violence. Strategies in health services for promoting exclusive breastfeeding should consider identifying and addressing family violence.

Neste artigo faz-se uma análise da magnitude das causas de morte no estado e no município do Rio de Janeiro em 1990, com base no cálculo dos "anos potenciais de vida perdidos" (APVPs) entre as idades de 1 e 70 anos. Esta análise leva a uma substancial modificação na importância relativa das causas de óbito, pois este método imputa um maior valor às mortes que ocorrem nas fases precoces da vida. De acordo com este método, a análise pelos grandes grupos de causas de óbito mostra que as duas regiões apresentam o grupo das "causas externas" como principal causa de APVP, sendo as mesmas responsáveis por mais de 30,0% dos APVPs entre 1 e 70 anos. Esta estimativa está em contraposição à predominância das doenças cardiovasculares, indicada pelos perfis de mortalidade que utilizam os coeficientes específicos de mortalidade por grandes grupos de causas. A análise por causas específicas de morte aponta os homicídios como principais responsáveis pelos APVPs nas duas regiões, seguidas pelos neoplasmas malignos, no município do Rio de Janeiro, e por "outros tipos de violência" (suicídios e violências em que ignora se de causa acidental ou intencional), no estado do Rio de Janeiro. Exceção feita às doenças neoplásicas, as taxas de APVP por grandes grupos de causas, em ambos os sexos, são sempre menores no município do Rio de Janeiro. O total de APVP e as taxas de APVP são marcadamente mais altas no sexo masculino, em decorrência, principalmente, das causas externas. Os autores salientam a importância da utilização deste indicador de mortalidade prematura para a definição de prioridades e chamam a atenção para o caráter devastador dos traumas e violências como causa de mortes em idades jovens.
