Despite considerable advances in laryngological research, there is still a plethora of (benign) vocal fold pathologies that cannot be treated causally. This is due to the limited accessibility and sensitive microarchitecture of the vocal folds, which cannot be investigated at a cellular level. Consequently, current pathophysiological knowledge is frequently based on macroscopic findings. The impact of interventions is mainly evaluated endoscopically or via indirect diagnostic methods.The aim of this article is to discuss state-of-the-art biotechnological methods used in laryngological research, illustrated by practical examples.In recent years, animal and in vitro experiments have significantly contributed to a continuous expansion of knowledge in this field, particularly regarding vocal fold inflammation and scar formation. Vocal fold fibroblasts, the most important cellular component of the lamina propria, can be accredited a central role in these processes.Our knowledge regarding macroanatomy and macropathophysiology of several pathologies has increased considerably in recent years. In vitro trials have shown, e.g., that vocal fold fibroblasts in an inflammatory setting secrete less profibrotic and proinflammatory cytokines when exposed to vibration. Early vocal exercises after surgical interventions on the vocal folds may therefore promote better wound healing and consequently improved phonation. Research in molecular laryngology should create a solid basis of knowledge for subsequent clinical studies.HINTERGRUND: Trotz erheblicher Fortschritte in der laryngologischen Forschung gibt es eine Reihe von (benignen) Stimmlippenerkrankungen, die kausal nicht therapierbar sind. Das liegt an der eingeschränkten Zugänglichkeit sowie der sensiblen Mikroarchitektur der Stimmlippen, die nicht auf zellulärer Ebene erforscht werden können. Das pathophysiologische Verständnis endet dadurch häufig in der makroskopischen Ebene, die Folgen von Interventionen werden großteils endoskopisch oder mit indirekten Methoden evaluiert.Im nachfolgenden Beitrag stellen die Autor(inn)en biotechnologische State-of-the-Art-Methoden vor, die in der laryngologischen Forschung Anwendung finden, verbunden mit praktischen Beispielen.Tierversuche und Zellkulturexperimente haben in den letzten Jahren zu einer signifikanten Wissenserweiterung beigetragen, dies insbesondere in den Bereichen Stimmlippeninflammation und -narbenbildung. Dem Stimmlippenfibroblasten, als wichtigstem zellulärem Bestandteil der Lamina propria, kommt dabei eine zentrale Rolle zu.Mittlerweile besteht bei einigen Krankheitsbildern ein tieferes Verständnis von Makroanatomie und Makropathophysiologie als je zuvor. In-vitro-Versuche zeigten beispielsweise, dass Stimmlippenfibroblasten in einem inflammatorischen Setting weniger profibrotische und proinflammatorische Zytokine sezernierten, wenn sie Vibrationen ausgesetzt sind. Umgesetzt auf die Klinik könnte das bedeuten, dass eine frühe Stimmaktivierung nach operativen Eingriffen an den Stimmlippen zu besserer Heilung und besseren stimmlichen Ergebnissen führt. Unsere Vision lautet, dass die molekulare Laryngologie ein gesichertes Fundament an Wissen bereitstellen soll, auf das in weiterer Folge klinische Studien aufgebaut werden können.

Objectives/Hypothesis To reverse sarcopenia and increase the volumes of atrophied laryngeal muscles by functional electrical stimulation (FES) using a minimal invasive surgical procedure in an aged ovine model. Study Design Prospective animal study. Methods A stimulation electrode was placed unilaterally near the terminal adduction branch of the recurrent laryngeal nerve (RLN) adjacent to the right cricothyroid joint. The electrode was connected to an implant located subcutaneously at the neck region. Predesigned training patterns were automatically delivered by a bidirectional radio frequency link using a programming device and were repeated automatically by the implant every other day over 11 weeks in the awake animal. Outcome parameters comprised volumetric measurements based on three‐dimensional reconstructions of the entire thyroarytenoid muscle (TAM), as well as gene expression analyses. Results We found significant increases of the volumes of the stimulated TAM of 11% and the TAM diameter at the midmembranous parts of the vocal folds of nearly 40%. Based on gene expression, we did not detect a shift of muscle fiber composition. Conclusions FES of the terminal branches of the RLN is a secure and effective way to reverse the effects of age‐related TAM atrophy and to increase volumes of atrophied muscles. Objective: To examine the diversity in paediatric vision and hearing screening programmes in Europe. Methods: Themes for comparison of screening programmes derived from literature were used to compile three questionnaires on vision, hearing, and public health screening. Tests used, professions involved, age, and frequency of testing seem to influence sensitivity, specificity, and costs most. Questionnaires were sent to ophthalmologists, orthoptists, otolaryngologists, and audiologists involved in paediatric screening in all EU full-member, candidate, and associate states. Answers were cross-checked. Results: Thirty-nine countries participated; 35 have a vision screening programme, 33 a nation-wide neonatal hearing screening programme. Visual acuity (VA) is measured in 35 countries, in 71% of these more than once. First measurement of VA varies from three to seven years of age, but is usually before age five. At age three and four, picture charts, including Lea Hyvarinen, are used most; in children over four, Tumbling-E and Snellen. As first hearing screening test, otoacoustic emission is used most in healthy neonates, and auditory brainstem response in premature newborns. The majority of hearing testing programmes are staged; children are referred after 1–4 abnormal tests. Vision screening is performed mostly by paediatricians, ophthalmologists, or nurses. Funding is mostly by health insurance or state. Coverage was reported as >95% in half of countries, but reporting was often not first-hand. Scarring of the vocal folds leads to a deterioration of the highly complex micro-structure with consecutively impaired vibratory pattern and glottic insufficiency. The resulting dysphonia is predominantly characterized by a reduced vocal capacity. Despite the considerable progress in understanding of the underlying pathophysiology, the treatment of scarred vocal folds is still an unresolved chapter in laryngology and phonosurgery. Essential for a successful treatment is an individual, multi-dimensional concept that comprises the whole armamentarium of surgical and non-surgical (i.p. voice therapy) modalities. An ideal approach would be to soften the scar, because the reduced pliability and consequently the increased vibratory rigidity impede the easiness of vibration. The chosen phonosurgical method is determined by the main clinical feature: Medialization techniques for the treatment of glottic gap, or epithelium freeing techniques for improvement of vibration characteristics often combined with injection augmentation or implantation. In severe cases, buccal mucosa grafting can be an option. New developments, include treatment with anxiolytic lasers, laser technology with ultrafine excision/ablation properties avoiding coagulation (Picosecond infrared laser, PIRL), or techniques of tissue engineering. However, despite the promising results by in vitro experiments, animal studies and first clinical trials, the step into clinical routine application has yet to be taken.
