4. Limits of liberal plurality : from political identity to strong recognition

Abstract

Through the analysis of liberal theories of plurality and diversity –multiculturalism, interculturalism, nationalisms, cosmopolitanism, communitarianism-, I make a series of arguments regarding the positive recognition of difference. I acknowledge the merits and benefits of these theories in providing political recognition to minority groups. However, I introduce the notions of strong identity and recognition that could open the door for solutions to some puzzles left unsolved by the theories that promote political recognition, including claims of essentialism, issues on the categorisation of minorities and majorities, problems related to social cohesion, integration and liberalisation of minorities, and of course issues on recognition of cultural value and identity. Constant with the analysis. I portray the concept of identity from an ontological perspective. I argue that identity mainly refers to what we are. In the strict sense, it is not a property or something we can instrumentally use to deal with the issues of everyday life. Identity also can be understood as membership, belonging, something socially constructed, and the social glue in modern societies while nevertheless being more than that. We do not have multiple identities, but a complex unified identity that includes everything that we are. To develop this part of the argument, I draw on Charles Taylor’s theory of the modern self. Finally, I suggest that strong recognition is pre-dialogical, and does not lead to the direct integration of minorities into the main culture. Further, it fosters internal social change, supports a stronger idea of collective autonomy, and is not mediated by institutions. These notions of strong identity and recognition allow us to move beyond some limitations of the liberal theories, while at the same time valuing the benefits of political recognition. I argue that strong recognition does not contravene political recognition but, in fact, enables it.

Abstract

El objetivo de este estudio fue el de determinar los supuestamente beneficiosos efectos de añadir ácido hialurónico como coadyuvante a la artroscopia de la ATM (articulación temporomandibular), en comparación a la realización de esta técnica artroscópica de forma estándar. Para ello se realizó un ensayo clínico aleatorizado (RCT) a doble ciego sobre 51 pacientes en estadios III y IV de Wilkes. Se analizaron variables clínicas y radiológicas diversas, como el dolor, apertura máxima bucal, posición discal en resonancia magnética que fueron analizados en periodos de 3, 6 y 9 meses en ambos grupos de estudio. Abstract

Liraglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist used for the management of type 2 diabetes and obesity. It was the first GLP-1 receptor agonist to be approved by the US Food and Drug Administration and the European Medicines Agency for the treatment of obesity. To date, numerous skin adverse reactions to liraglutide have been reported, but data regarding hypersensitivity reactions are scarce, raising concerns about its safety and clinical management. We present the case of a 56-year-old female patient with class 3 obesity who was started on subcutaneous liraglutide (Saxenda) by her endocrinologist. One month after starting the aforementioned treatment, the patient presented well-defined, round, erythematous pruriginous plaques surrounding the injection site, around 24 hours after the drug administration. A liraglutide-induced, delayed-type hypersensitivity reaction was suspected, which could be subsequently confirmed by allergy testing and histopathological study. This paper explores the clinical use of liraglutide, the occurrence of hypersensitivity reactions, diagnosis, management, and implications for future research. Abstract

RESUMEN El granuloma de células gigantes (GCG) es una patología de etiología no esclarecida que aparece tras traumatismos o procesos inflamatorios. Puede ser asintomático, debutar como una masa de comportamiento inflamatorio o comportarse de forma localmente agresiva. El diagnóstico resulta de la combinación de datos clínicos, radiológicos e histológicos. El tratamiento puede ser quirúrgico, mediante cirugías más o menos extensas, así como médico con diferentes fármacos. Se presenta el caso de un paciente intervenido de un GCG mandibular que acudió a nuestra consulta con una recidiva de la lesión, manejada quirúrgicamente mediante resección segmentaria y reconstrucción con colgajo libre de peroné, añadiendo al tratamiento la rehabilitación dental para realizar un manejo global del caso. ABSTRACT Giant cell granuloma (GCG) is a pathology of unclear aetiology that appears after trauma or inflammatory processes. It may be asymptomatic, have an early presentation as a mass with inflammatory reaction, or behave in a locally aggressive manner. Diagnosis results from a combination of clinical, radiological and histological data. Treatment can be surgical, with more or less extensive surgery, as well as medical with different drugs. We present the case of a patient who underwent surgery for a mandibular GCG who came to our hospital with a recurrence of the lesion, which was managed surgically by segmental resection and reconstruction with a fibula free flap, adding dental rehabilitation to the treatment for a global management of the case.

RESUMEN Introducción: Presentamos una paciente sometida a reconstrucción de articulación temporomandibular (ATM) con una prótesis de stock tipo Walter-Lorenz (Zimmer Biomet Inc., Biomet Microfixation, Jacksonville, FL) con el apoyo de un sistema de planificación virtual quirúrgica 3D y guías de corte elaboradas gracias a tecnología computer-aided-manufacturing (CAD/CAM). Caso clínico: Paciente mujer de 66 años con historia de múltiples cirugías de ATM bilaterales que acude de nuevo a consulta por empeoramiento funcional y dolor a nivel de ATM izquierda, portadora de una prótesis de Walter-Lorenz en la ATM derecha. Se interviene para colocación de otra prótesis de stock tipo Walter-Lorenz a nivel de ATM izquierda con planificación virtual quirúrgica 3D de las osteotomías de la fosa y la rama, y fabricación de las guías de corte con tecnología CAD/CAM. La cirugía se ajusta de manera precisa a la planificación previa. En las revisiones posteriores, la paciente presenta apertura oral como la previa y control del dolor. Conclusión: La reconstrucción de ATM con planificación quirúrgica virtual 3D de prótesis de stock Walter-Lorenz permite aumentar la precisión en su colocación y simplificar la intervención quirúrgica. ABSTRACT Introduction: We present the reconstruction of the temporomandibular joint (TMJ) with Walter-Lorenz prosthesis (Zimmer Biomet Inc., Biomet Microfixation, Jacksonville, FL) performing 3D virtual surgical planning and cutting guides with computer-aided-manufacturing technology (CAD / CAM). Clinical case: A 66-year-old female patient who had a history of multiple TMJ surgeries complained of functional worsening and pain in the left TMJ, with Walter-Lorenz prosthesis in the right TMJ. Intervention was performed to place another Walter-Lorenz prosthesis in the left TMJ with virtual 3D surgical planning of the osteotomies of the fossa and the ramus, and surgical guides fabricated with CAD/CAM technology. Surgery was carried out according to surgical planning. In the subsequent revisions, the mouth opening was stable and the pain was reduced. Conclusion: TMJ reconstruction with 3D virtual surgical planning of Walter-Lorenz stock prosthesis allows to increase the precision in its placement and simplify the surgical intervention.

42. A Statewide University System (16 Campuses) Creates Collaborative Learning Communities in Second Life

For the first time in history, an entire statewide public institution of higher education is extending all of its 16 campuses into the online virtual world of Second Life and is creating one of the largest virtual collaborative learning communities in the world. There are three levels of assessment and research for this initial-entry project: (1) at the System level; (2) at the individual campus level; and (3) at the course level. Questions to be addressed include: How can educators use virtual worlds to create large-scale learning communities? How do virtual worlds enhance collaborative and interdisciplinary learning? How are 16 geo-spatially separated campuses collaborating in virtual worlds? What have been major obstacles? How will this project continue to grow and evolve? The creation of the System's virtual collaborative learning community of students, faculty, researchers, and administrators has begun to allow participants to learn, share, collaborate, and grow alongside one another. Abstract

Introduction: Admission hyponatremia, frequent in patients hospitalized for COVID-19, has been associated with increased mortality. However, although euvolemic hyponatremia secondary to the Syndrome of Inappropriate Antidiuresis (SIAD) is the single most common cause of hyponatremia in community-acquired pneumonia (CAP), a thorough and rigorous assessment of the volemia of hyponatremic COVID-19 subjects has yet to be described. We sought to identify factors contributing to mortality and hospital length-of-stay (LOS) in hospitalized COVID-19 patients admitted with hyponatremia, taking volemia into account. Method: Retrospective study of 247 patients admitted with COVID-19 to a tertiary hospital in Madrid, Spain from March 1st through March 30th, 2020, with a glycemia-corrected serum sodium level (SNa) < 135 mmol/L. Variables were collected at admission, at 2nd-3rd day of hospitalization, and ensuing days when hyponatremia persisted. Admission volemia (based on both physical and analytical parameters), therapy, and its adequacy as a function of volemia, were determined. Results: Age: 68 years [56-81]; 39.9% were female. Median admission SNa was 133 mmol/L [131- 134]. Hyponatremia was mild (SNa 131-134 mmol/L) in 188/247 (76%). Volemia was available in 208/247 patients; 57.2% were euvolemic and the rest (42.8%) hypovolemic. Hyponatremia was left untreated in 154/247 (62.3%) patients. Admission therapy was not concordant with volemia in 43/84 (51.2%). In fact, the majority of treated euvolemic patients received incorrect therapy with isotonic saline (37/41, 90.2%), whereas hypovolemics did not (p=0.001). The latter showed higher mortality rates than those receiving adequate or no therapy (36.7% vs. 19% respectively, p=0.023). The administration of isotonic saline to euvolemic hyponatremic subjects was independently associated with an elevation of in-hospital mortality (Odds Ratio: 3.877, 95%; Confidence Interval: 1.25-12.03). Conclusion: Hyponatremia in COVID-19 is predominantly euvolemic. Isotonic saline infusion therapy in euvolemic hyponatremic COVID-19 patients can lead to an increased mortality rate. Thus, an exhaustive and precise volemic assessment of the hyponatremic patient with CAP, particularly when due to COVID-19, is mandatory before instauration of therapy, even when hyponatremia is mild.
