Titre : Connaissances, attitudes et pratiques de l’hygiène des mains par les infirmiers du service de pédiatrie du CHU Sylvanus OlympioObjectif : décrire les connaissances, attitudes et pratiques des infirmiers soignant les enfants sur l’hygiène des mains et les évaluer.Population et méthode : étude transversale a visée descriptive menée du 10 juin au 3 juillet 2018 par l’interview des infirmiers du service de pédiatre du CHU SO.Résultats : l’âge moyen des infirmiers était de 32 ± 8 ans avec des extrêmes de 19 et 53 ans. Le lavage des mains était plus connu (100%) que la friction hydro-alcoolique (75%) et plus pratiqué. La majorité des infirmiers pratiquaient l’hygiène des mains pour se protéger contre les infections (86,1%). L’eau et le savon liquide étaient insuffisamment connus (63,9%) comme matériel d’hygiène des mains, la solution hydro-alcoolique était moins connue (50%). Les étapes de l’hygiène des mains n’étaient pas connues par 55,6% des infirmiers. La durée de l’hygiène des mains et le volume d’antiseptique n’étaient pas connus. La connaissance du type d’hygiène des mains adapté à des situations de soins était mauvaise. La majorité des infirmiers (88,9%) avait entendu parler de l’hygiène des mains dans une sensibilisation.Conclusion : les connaissances, attitudes et pratiques des infirmiers souffrent d’insuffisance qui rendent leur pratique inadéquate alors qu’en milieu hospitalier ce geste simple est irremplaçable. Un programme rigoureux s’avère indispensable pour un changement de comportement. Hand washing was more widely known (100%) than hydro-alcoholic friction (75%) and more widely practiced. The majority of nurses practised hand hygiene to protect themselves against infection (86.1%). Water and liquid soap were insufficiently known (63.9%) as hand hygiene materials, hydro-alcoholic solution was less known (50%). The steps of hand hygiene were not known by 55.6% of the nurses. The duration of hand hygiene and the volume of antiseptic were not known. Knowledge of the type of hand hygiene adapted to care situations was poor. The majority of nurses (88.9%) had heard about hand hygiene in an awareness-raising event.Conclusion: Nurses' knowledge, attitudes and practices suffer from inadequacies that make their practice inappropriate, whereas in a hospital environment this simple gesture is irreplaceable. A rigorous programme is essential for a change in behaviour.

Abstract

Objectif : Évaluer le coût global moyen de la prise en charge de l’Ostéomyélite Aigüe (OA) chez un enfant drépanocytaire.Méthode : Étude rétrospective sur la période du 1er janvier 2008 au 30 novembre 2019, portant sur 23 dossiers de patients drépanocytaires pris en charge pour OA dans le service de pédiatrie du CHU Sylvanus Olympio.Résultats : L’âge moyen des patients était de 7 ans ± 4 ans avec des extrêmes de 15 mois et 17 ans. Le membre inférieur droit était le plus touché par l’OA (n=10). Dix-huit patients ont été hospitalisés et 5 patients traités en ambulatoire. Le coût global moyen de la prise en charge de l’OA était de 144 796,522 ± 48 785,034 francs CFA (84 310 - 250 855 francs CFA). Le coût global moyen était dominé par le coût du traitement antibiotique (26,8%), le coût du traitement de relai (23,8%), le coût des frais d’hospitalisation (17,90%) et le coût des examens paracliniques (14,80%).Conclusion : Le coût global moyen de la prise en charge de l’OA chez l’enfant drépanocytaire au CHU Sylvanus Olympio est très onéreux comparé au niveau de vie du togolais. Il a représenté 5 fois le SMIG au Togo. Objective: To assess the average overall cost of managing acute osteomyelitis in children with sickle cell disease.Method: Retrospective study of 23 sickle cell patients treated for acute osteomyelitis in the paediatric ward of the CHU Sylvanus Olympio from 1 January 2008 to 30 November 2019.Results: The mean age of the patients was 7 years ± 4 years with extremes of 15 months and 17 years. The right lower limb (n=10) was the most affected by acute osteomyelitis. Eighteen patients were hospitalized and 5 patients were treated as outpatients. The average overall cost of management of acute osteomyelitis was 144 796.522 ± 48 785.034 CFA francs (84 310 – 250 855 CFA francs). The average overall cost was dominated by the cost of antibiotic treatment (26.8%), the cost of follow-up treatment (23.8%), the cost of hospitalization (17.90%) and the cost of paraclinical examinations (14.80%).Conclusion: The average overall cost of treating acute osteomyelitis in children with sickle cell disease at the Sylvanus Olympio University Hospital is very expensive compared to the standard of living of Togolese people. It represented 5 times the minimum wage in Togo.

Abstract

BACKGROUND: Retinoblastoma is the most common intraocular cancer worldwide. There is some evidence to suggest that major differences exist in treatment outcomes for children with retinoblastoma from different regions, but these differences have not been assessed on a global scale. We aimed to report 3-year outcomes for children with retinoblastoma globally and to investigate factors associated with survival. METHODS: We did a prospective cluster-based analysis of treatment-naive patients with retinoblastoma who were diagnosed between Jan 1, 2017, and Dec 31, 2017, then treated and followed up for 3 years. Patients were recruited from 260 specialised treatment centres worldwide. Data were obtained from participating centres on primary and additional treatments, duration of follow-up, metastasis, eye globe salvage, and survival outcome. We analysed time to death and time to enucleation with Cox regression models. FINDINGS: The cohort included 4064 children from 149 countries. The median age at diagnosis was 23·2 months (IQR 11·0-36·5). Extraocular tumour spread (cT4 of the cTNMH classification) at diagnosis was reported in five (0·8%) of 636 children from high-income countries, 55 (5·4%) of 1027 children from upper-middle-income countries, 342 (19·7%) of 1738 children from lower-middle-income countries, and 196 (42·9%) of 457 children from low-income countries. Enucleation surgery was available for all children and intravenous chemotherapy was available for 4014 (98·8%) of 4064 children. The 3-year survival rate was 99·5% (95% CI 98·8-100·0) for children from high-income countries, 91·2% (89·5-93·0) for children from upper-middle-income countries, 80·3% (78·3-82·3) for children from lower-middle-income countries, and 57·3% (52·1-63·0) for children from low-income countries. Abstract

Introduction : La voie d’accouchement dont la césarienne peut avoir une incidence sur la survenue de l’asphyxie néonatale. L’objectif de cette étude était de déterminer la fréquence de l’asphyxie néonatale et de décrire ses caractéristiques selon la voie d’accouchement (césarienne versus accouchement voie basse).Matériel et méthodes : Une étude rétrospective descriptive et analytique sur une période de 18 mois (1er janvier 2018 à 30 juin 2019) a été menée dans les services de la maternité et de pédiatrie de l’hôpital de Bè à Lomé. Ont été inclus, les dossiers des nouveau-nés ayant fait une asphyxie néonatale et ayant été transférés de la maternité de l’hôpital de Bè à la réanimation pédiatrique de l’hôpital de Bè. L’analyse statistique a été réalisée sur le logiciel R 3.3.4. Le seuil de significativité choisi était 0,05.Résultats : Nous avons enrôlé 244 (4,95%) cas d’asphyxie néonatale sur 4929 accouchements. Le nombre de cas d’asphyxie néonatale issu d’un accouchement par voie basse était de 126 (4,45%) sur 2831 accouchements par voie basse. Le nombre de cas d’asphyxie issu d’un accouchement par césarienne était de 118 (5,62%) sur 2098 césariennes. La césarienne était associée à l’asphyxie néonatale (p = 0,03, OR = 1,3 [0,98-1,65]). La césarienne d’urgence représentait 80,50% (N=95) des cas d’asphyxie par césarienne et la césarienne programmée 19,50% (N=23) des cas. La plupart (41,80%) des nouveau-nés asphyxiés étaient accouchés à un terme compris entre 39 Semaines d’Aménorrhées (SA) et 40 SA+6 jours par césarienne d’urgence (48,42%) et par voie basse (42,06%).Conclusion : La fréquence de l’asphyxie néonatale était relativement élevée et la césarienne en était plus pourvoyeuse que l’accouchement par voie basse. Most (41.80%) of the asphyxiated newborns were delivered at a term between 39 weeks of amenorrhea and 40 weeks of amenorrhea and 6 days by emergency cesarean section (48.42%) and vaginal delivery (42.06%).Conclusion: The frequency of neonatal asphyxia was relatively high and caesarean section was more likely to cause it than vaginal delivery. Abstract

IMPORTANCE: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale. OBJECTIVES: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis. DESIGN, SETTING, AND PARTICIPANTS: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017. MAIN OUTCOMES AND MEASURES: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis. RESULTS: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Abstract

Introduction: vaso-occlusive crisis (VOC) is the most common manifestation of sickle cell disease and the leading cause of hospitalization among affected children. The purpose of this study is to describe the clinical features of severe VOCs, to determine the etiologies of infectious syndromes that accompany them and to describe their management., Methods: we conducted a descriptive cross-sectional study of 137 adult patients with sickle cell disease hospitalised for severe VOC in the Paediatric Department of the Sylvanus Olympio University Hospital from 1 st January 2009 to 31 st December 2011., Results: the majority of patients (n=98; 71.5%) had homozygous sickle cell (SS), followed by double heterozygous SC disease (n=28; 20.5). The median of consultation time was 4.7 ± 4.4 days. Treatment before admission was based on antibiotics (28.5%). VOCs were mainly osteoarticular (70.8%). In 98.5% of cases, an associated bacterial infection was confirmed (48.9%) or suspected (49.6%). The main etiologies included acute chest syndrome (26.3%), acute osteomyelitis (10.9%), urinary tract infection (6.6%) and septicaemia (3.6%). One germ was isolated from 14.6% of patients: Escherichia coli (30%), followed by Klebsiella pneumoniae (25%), Staphylococcus aureus (15%), Salmonella typhi (10%), Streptococcus pneumoniae (5%), Streptococcus D (5%), Enterobacter (5%) and Acinetobacter (5%). Mortality rate was 2.2%. The average length of stay in hospital was 11.4 ± 8.8 days., Conclusion: severe sickle cell-related vaso-occlusive crisis is mainly associated with bacterial infections in tropical environments. 10. Management of acute sickle cell priapism in an African (Togo) pediatric department includes conservative measures and intracavernous epinephrine which is safe and efficacious.

Abstract

Priapism is a well-known urologic complication of sickle cell anemia. This study describes the results of a protocol for the treatment of acute priapism by intracavernous injection of epinephrine due to unavailability of etilefrine. A descriptive cross-sectional study of 18 cases of acute priapism in sickle cell patients treated in the pediatric department of the Sylvanus Olympio CHU from January 1 to December 31, 2020. The average age was 21.7 ± 7.7 years, the youngest patient was 8 and the oldest was 32 years old. Students represented 61.1% of the patients. The hemoglobin profiles were homozygous SS ( n  = 14) and double heterozygous SC ( n  = 4). Most of the crisis (83.3%) occurred at night. Most of the patients (66.7%) came to the hospital before the sixth hour of crisis, one patient came by the 48th hour. Walking was the most self-relief method tried by patients (67%). It was followed by a cold penile bath, attempted urination, body bath, and lastly lukewarm bath. Fourteen patients had a history of chronic intermittent priapism. The average pain intensity was 9.5 ± 0.9 with restlessness (33.3%) and crying (33.3%). Fifteen patients were treated upon admission with an intracavernosal injection of epinephrine, and three patients were first drained. Thirteen patients achieved remission immediately, while five patients required a second injection and only one had to be drained before remission. Tolerance was good. One patient had a borderline systolic blood pressure. One erectile weakness case was noticed and no cases of sexual impotence. Epinephrine by intracavernosal injection is an efficient treatment for acute priapism in sickle cell patients. Abstract

Progeroid syndromes (PS), including Hutchinson-Gilford Progeria Syndrome (HGPS), are premature and accelerated aging diseases, characterized by clinical features mimicking physiological aging. Most classical HGPS patients carry a de novo point mutation within exon 11 of the LMNA gene encoding A-type lamins. This mutation activates a cryptic splice site, leading to the production of a truncated prelamin A, called prelamin A ∆50 or progerin, that accumulates in HGPS cell nuclei and is a hallmark of the disease. Some patients with PS carry other LMNA mutations and are named "HGPS-like" patients. They produce progerin and/or other truncated prelamin A isoforms (∆35 and ∆90). We previously found that MG132, a proteasome inhibitor, induced progerin clearance in classical HGPS through autophagy activation and splicing regulation. Here, we show that MG132 induces aberrant prelamin A clearance and improves cellular phenotypes in HGPS-like patients' cells other than those previously described in classical HGPS. These results provide preclinical proof of principle for the use of a promising class of molecules toward a potential therapy for children with HGPS-like or classical HGPS.

Abstract

Background: The travel distance from home to a treatment centre, which may impact the stage at diagnosis, has not been investigated for retinoblastoma, the most common childhood eye cancer. We aimed to investigate the travel burden and its impact on clinical presentation in a large sample of patients with retinoblastoma from Africa and Europe., Methods: A cross-sectional analysis including 518 treatment-naïve patients with retinoblastoma residing in 40 European countries and 1024 treatment-naïve patients with retinoblastoma residing in 43 African countries., Results: Capture rate was 42.2% of expected patients from Africa and 108.8% from Europe. African patients were older (95% CI -12.4 to -5.4, p<0.001), had fewer cases of familial retinoblastoma (95% CI 2.0 to 5.3, p<0.001) and presented with more advanced disease (95% CI 6.0 to 9.8, p<0.001); 43.4% and 15.4% of Africans had extraocular retinoblastoma and distant metastasis at the time of diagnosis, respectively, compared to 2.9% and 1.0% of the Europeans. To reach a retinoblastoma centre, European patients travelled 421.8 km compared to Africans who travelled 185.7 km (p<0.001). On regression analysis, lower-national income level, African residence and older age (p<0.001), but not travel distance (p=0.19), were risk factors for advanced disease., Conclusions: Fewer than half the expected number of patients with retinoblastoma presented to African referral centres in 2017, suggesting poor awareness or other barriers to access. Despite the relatively shorter distance travelled by African patients, they presented with later-stage disease. Abstract

The aim of this article is to describe the first case of Hutchinson-Gilford Progeria Syndrome (HGPS) in Togo and review all Africans cases. Our patient was a 12.8-year-old Togolese boy followed in our unit till he was 15-year-old for HGPS. He was the only child of non-consanguineous parents. The phenotypic findings were craniofacial dysmorphy, dwarfism, lipodystrophy, diffusely scattered hyperpigmented foci, pyriform thorax, nail dystrophy, decreased joint mobility, and camptodactyly. He had characteristic facies with prominent forehead, prominent eyes, absent ear lobule, thin nasal skin, convex nasal profile, micrognathia, and crowded teeth. Radiologicals findings were bilateral coxa valga, pyriform thorax, and acro-osteolysis. We sequenced the entire coding region of LMNA gene, and mutation analysis revealed a heterozygous mutation c.1824C>T (p.Gly608Gly). Our patient is therefore the fifth African and the fourth with classical mutation, first of Western Africa, and second of (sub-Saharan) African black race. Abstract

Importance: Early diagnosis of retinoblastoma, the most common intraocular cancer, can save both a child's life and vision. However, anecdotal evidence suggests that many children across the world are diagnosed late. To our knowledge, the clinical presentation of retinoblastoma has never been assessed on a global scale., Objectives: To report the retinoblastoma stage at diagnosis in patients across the world during a single year, to investigate associations between clinical variables and national income level, and to investigate risk factors for advanced disease at diagnosis., Design, Setting, and Participants: A total of 278 retinoblastoma treatment centers were recruited from June 2017 through December 2018 to participate in a cross-sectional analysis of treatment-naive patients with retinoblastoma who were diagnosed in 2017., Main Outcomes and Measures: Age at presentation, proportion of familial history of retinoblastoma, and tumor stage and metastasis., Results: The cohort included 4351 new patients from 153 countries; the median age at diagnosis was 30.5 (interquartile range, 18.3-45.9) months, and 1976 patients (45.4%) were female. Most patients (n = 3685 [84.7%]) were from low- and middle-income countries (LMICs). Globally, the most common indication for referral was leukocoria (n = 2638 [62.8%]), followed by strabismus (n = 429 [10.2%]) and proptosis (n = 309 [7.4%]). Patients from high-income countries (HICs) were diagnosed at a median age of 14.1 months, with 656 of 666 (98.5%) patients having intraocular retinoblastoma and 2 (0.3%) having metastasis. Patients from low-income countries were diagnosed at a median age of 30.5 months, with 256 of 521 (49.1%) having extraocular retinoblastoma and 94 of 498 (18.9%) having metastasis. Lower national income level was associated with older presentation age, higher proportion of locally advanced disease and distant metastasis, and smaller proportion of familial history of retinoblastoma. Advanced disease at diagnosis was more common in LMICs even after adjusting for age (odds ratio for low-income countries vs upper-middle-income countries and HICs, 17.92 [95% CI, 12.94-24.80], and for lower-middle-income countries vs upper-middle-income countries and HICs, 5.74 [95% CI, 4.30-7.68])., Conclusions and Relevance: This study is estimated to have included more than half of all new retinoblastoma cases worldwide in 2017. Children from LMICs, where the main global retinoblastoma burden lies, presented at an older age with more advanced disease and demonstrated a smaller proportion of familial history of retinoblastoma, likely because many do not reach a childbearing age. Given that retinoblastoma is curable, these data are concerning and mandate intervention at national and international levels. Further studies are needed to investigate factors, other than age at presentation, that may be associated with advanced disease in LMICs.

Abstract

Objective: To determine the etiological and evolutionary profile of renal failure of chidren in Togo., Methods: This is a cross-sectional study over the period of 12 months (2016-2017) including children aged 1 to 18 years hospitalized in the pediatric ward of Sylvanus Olympio university teaching hospital of Lome (Togo) for renal failure., Results: Of 2374 patients hospitalized in our unit, 58 (2.4%) had renal failure. The mean age was 8.17±4 years with a sex ratio of 1.32. The average consultation time was 11.9 days. The mean duration of hospitalization was 12.7±7.7 days. Thirty-seven patients (63.8%) were referred from a peripheral center. Thirty-seven children out of 58 (63.1%) were oligoanuric. Renal failure was acute in 94.8% and chronic in 5.2%. Anemia was found in 84.4% of children. The main etiologies found were severe malaria (63.8%), glomerulonephritis (10.3%) and nephrotic syndrome (10.3%). Thirteen children (22.4%) benefited from dialysis sessions. The evolution was favorable in 79.3% of the cases., Conclusion: The renal failure of child is relatively common in our daily practice. Abstract

Purpose: To evaluate the results of an intensive polychemotherapy regimen for Burkitt lymphoma (BL) in sub-Saharan African pediatric centers., Patients and Methods: Children with advanced-stage BL (stages II bulky, III, and IV) treated with the GFAOP-Lymphomes Malins B (GFALMB) 2009 protocol in 7 centers between April 2009 and September 2015 were prospectively registered. Treatment regimen contained a prephase with cyclophosphamide followed by 2 induction courses (cyclophosphamide, vincristine, prednisone, high-dose methotrexate [HDMTX]), 2 consolidation courses (cytarabine, HDMTX), and a maintenance phase only for stage IV. HDMTX was given at the dose of 3 g/m 2 ., Results: Four hundred patients were analyzed: 7% had stage II bulky, 76% stage III, and 17% stage IV disease. Median age was 7.3 years, and sex ratio was 1.9:1 (male:female). A total of 221 patients received the whole protocol treatment and 195 achieved complete remission (CR), 11 of them after a second-line treatment. Treatment abandonment rate was 22%. One hundred twenty-five patients died, of whom 49 deaths were related to treatment toxicity. A total of 275 patients are alive, including 25 despite treatment abandonment, but only 110 are known to be in CR with a follow-up > 1 year, indicating a high rate of loss to follow-up. Twelve-month overall survival (OS) was 60% (95% CI, 54% to 66%) and 63%, 60%, and 31%, respectively, for stage II bulky, III, and IV. Patients with stage III disease who started second induction course within 34 days had OS of 76%, versus 57% ( P = .0062) beyond 34 days., Conclusion: The GFA-LMB2009 protocol improved patients' survival. Early dose intensity of treatment is a strong prognostic factor. Improving supportive care and decreasing loss to follow-up are crucial.

Abstract

Purpose: Multidisciplinary management of Wilms tumor has been defined through multicenter prospective studies and an average expected patient cure rate of 90%. In sub-Saharan Africa, such studies are uncommon. After the encouraging results of the first Groupe Franco-Africain d'Oncologie Pédiatrique (GFAOP) study, we report the results of the GFAOP-NEPHRO-02 study using an adaptation of the International Society of Paediatric Oncology 2001 protocol., Patients and Methods: From April 1, 2005, to March 31, 2011, seven African units participated in a nonrandomized prospective study. All patients who were referred with a clinical and radiologic diagnosis of renal tumor were screened. Those older than age 6 months and younger than 18 years with a unilateral tumor previously untreated were pre-included and received preoperative chemotherapy. Patients with unfavorable histology or with a tumor other than Wilms, or with a nonresponding stage IV tumor were excluded secondarily., Results: Three hundred thirteen patients were initially screened. Two hundred fifty-seven patients were pre-included and 169 with histologic confirmation of intermediate-risk nephroblastoma were registered in the study and administered postoperative treatment. Thirty-one percent of patients were classified as stage I, 38% stage II, 24% stage III, and 7% stage IV. Radiotherapy was not available for any stage III patients. Three-year overall survival rate was 72% for all study patients and 73% for those with localized disease., Conclusion: It was possible to conduct sub-Saharan African multicenter therapeutic studies within the framework of GFAOP. Survival results were satisfactory. Improvements in procedure, data collection, and outcome are expected in a new study. Radiotherapy is needed to reduce the relapse rate in patients with stage III disease.

Abstract

Background: Monovalent rotavirus vaccine (RV1) was introduced in the immunization schedule of Togo in June 2014. We evaluated the impact of rotavirus vaccines on acute gastroenteritis (AGE) and rotavirus-associated hospitalizations in Togolese children., Methods: Sentinel surveillance for AGE (defined as ≥3 liquid or semi-liquid stools/24 h lasting <7 days) hospitalizations among children <5 years of age was conducted in two sites in the capital city, Lome. ELISA was used for diagnosis of rotavirus infection in children with AGE. Additionally, review of hospitalization registers was performed at five hospitals to assess trends in AGE hospitalizations among children aged <5 years. For the vaccine impact assessment, pre-rotavirus vaccine introduction (July 2010-June 2014) and post-rotavirus vaccine introduction (July 2014-June 2016) periods were compared for annual changes in proportions of hospitalizations associated with AGE and rotavirus., Results: During the pre-vaccine period, sentinel surveillance showed that 1017 patients were enrolled and 57% (range, 53-62%) tested positive for rotavirus, declining to 42% (23% reduction) in the first post-vaccine year and to 26% (53% reduction) in the second post-vaccine year; declines were most marked among infants. The patient register review showed that, compared with pre-vaccine rotavirus seasons, declines in hospitalizations due to all-cause AGE during post-vaccine rotavirus seasons were 48% among <1 year age-group in both first and second years following vaccine introduction. Among 1-4 year olds no reduction was noted in the first year and a 19% decline occurred in the second year., Conclusions: We report rapid and marked reduction in the number of AGE hospitalizations and the proportion of AGE hospitalizations attributable to rotavirus in the first two years post- RV1 implementation in Togo. Abstract

Introduction: The infection in pediatric HIV is the reason a lot of problems in Africa The objective of our study were to identify factors associated with mortality during follow-up of children receiving antiretroviral therapy in Togo., Methods: It was a cross-sectional study of 870 children aged files from 7 weeks to 15 years infected with HIV on antiretroviral treatment, covering the period 1 January 2001 to 31 December 2010 taking in 40 sites medical management in Togo. Data processing was done with the software Epi-Info 6.04d and duplicates were treated by the Software ESOPE., Results: All patients were infected with HIV-1. In total forty six (46) deaths is 5.29% of the overall cohort were reported in our series. The lethality of the overall cohort followed for 60 months was 5.29%. The survival rate of the overall effective monitoring in our study was 89.2%. Fifty-eight percent (58%) of deaths had affected children in a state of severe malnutrition and forty two percent (42%) in a state of moderate malnutrition. Sixty two percent (62%) of children under HAART treatment died benefited monitoring a psychologist., Conclusion: The diagnostic inadequacies of pediatric HIV strike the prognosis of infected children. Efforts still needs to be done to improve the load take pediatric HIV in Togo.

Abstract

To evaluate the hepatic and obstetric complications in pregnant women with cirrhosis. We report the cases of four pregnant women with cirrhosis treated in the gastroenterology and obstetrics-gynecology departments of the Lome Campus University Teaching Hospital between 2013 and 2015. The women's mean age was 32 years. Three were in the first trimester of pregnancy. Almost all had signs of advanced cirrhosis, including ascites (50%), lower-limb edema (75%), and jaundice (25%). All (100%) had liver failure and anemia. Cirrhosis was due to hepatitis virus B infection for 3 women. All had singleton pregnancies. Two mothers died; fetal outcome included one fetal loss and one stillbirth. This study shows the high risk associated with the combination of pregnancy and cirrhosis. Prognosis is poor for both mother and fetus.

Background: Anemia remains a major cause of morbidity and mortality of children in Togo despite of prevention effort, due to the parents lack of implication., Aim: To determine the knowledges, attitudes and practices of mothers, with anemia ofchildren under five years old Methods : Knowledge Attitudes and Practice survey from the first of february to 31 ofmarch 2012, about an interview of a hundred mothers with children under 5, randomly selected in the consultation, vaccination waiting rooms and in the hospitalisation., Results: Forty mothers had never heard about anemia. Health personnel was the main source of mother's information (29%), mass media represented 8% of mothers information source (radio 5%; television 3%). The decreased of blood in the body is the most given definition from the mothers (44%). Malaria (24%) and malnutrition (19%) were the main causes cited by mothers. Iron deficiency has been mentioned by 3% of the mothers. Palmar - plantar pallor (32%) was the clinical signe the best known by the mothers. Most of the mothers (90%) had never assist to an information education and communication message about anemia prevention. When their children had anemia, 25 mothers (65,8%) took their children to the health center, five mothers (13,2%) had given tomatoes. The use of iron in prophylactic treatment was known by 43% of the mothers. The blend tomatoes and milk was the prophylactic treatment mentioned by 3% of the mothers. Most of the mothers (77%) would advice a mother with a children suffering from anemia to take him to the hospital. The knowledge of anemia by the mothers was correlated to then level of instruction. But the knowledge of prevention did not depend on the instruction level., Conclusion: anemia is not well known by the mothers of under five children. It's causes, it's treatment are not well known. Mass media are not very implicated on the subject. The reduction of it's frequency goes by information education and communication activities.

Abstract

The renal complications of sickle cell anemia are well known. Gross hematuria is one of these, although it is associated more often with the sickle cell trait than with the disease. Nonetheless, in Togo, where the frequency of S gene is 16%, hematuria is rarely report in people with this gene. We describe 3 cases of gross hematuria in patients with sickle cell disease: primary hematuria associated with sickle cell disease. We found no promoting factors in our patients. The course was short, and after 4 to 15 years, there have been no recurrences.

Abstract

Here we report the association of giant aplasia cutis congenita in a newborn black male with Goltz syndrome. The cutis aplasia defect is extensive and circonscript at the vertex. The cerebral structures are visible through the lesions. In addition, the patient has a low birth weight, syndactyly, adactyly, cutaneous atrophy, and areas of hyperpigmentation on the legs and hypoplastic maxillary. We think that these signs are probably due to mosaic mutations in PORCN. We reviewed 18 cases of Goltz syndrome in 18 male neonates but none has aplasia cutis congenita. Such a combination of severe aplasia cutis congenita was not reported previously in Goltz syndrome.
