Abstract

One of the key open questions in Cosmology is the nature of the sources that completed the cosmological hydrogen Reionization at z~5.2. High-z primeval galaxies have been long considered the main drivers for Reionization, with a minor role played by high-z AGN. However, in order to confirm this scenario, it is fundamental to measure the photo-ionization rate produced by active SMBHs close to the epoch of Reionization. Given the pivotal role played by spectroscopically complete observations of high-z QSOs, in this paper we present the first results of the RUBICON (Reionizing the Universe with BrIght COsmological Nuclei) survey. It consists of a color selected sample of bona-fide z~5 QSO candidates from the Hyper Suprime-Cam Subaru Strategic Survey. Our QSO candidates have been validated both by photometric redshifts based on SED fitting and by spectroscopic redshifts, confirming that they lie at 4.5

Abstract

The redshift drift is a small, dynamic change in the redshift of objects following the Hubble flow. Its measurement provides a direct, real-time, model-independent mapping of the expansion rate of the Universe. It is fundamentally different from other cosmological probes: instead of mapping our (present-day) past light-cone, it directly compares different past light-cones. Being independent of any assumptions on gravity, geometry or clustering, it directly tests the pillars of the Lambda-CDM paradigm. Recent theoretical studies have uncovered unique synergies with other cosmological probes, including the characterization of the physical properties of dark energy. At the time of the original proposal by Sandage (1962) the expected change in the redshift of objects at cosmological distances appeared to be exceedingly small for reasonable observing times and beyond technological capabilities. In the last decades progress in the spectrographs (e.g. ESPRESSO), in the collecting area of telescopes and in the samples of cosmic beacons, enabled by new datasets and new machine-learning-based selections, have drastically changed the situation, bringing the Redshift Drift Grail within reach. Abstract

Motivated by evidences favoring a rapid and late hydrogen reionization process completing at z~5.2-5.5 and mainly driven by rare and luminous sources, we have reassessed the estimate of the space density of ultra-luminous QSOs at z~5 in the framework of the QUBRICS survey. A ~90% complete sample of 14 spectroscopically confirmed QSOs at M1450<-28.3 and 4.53. Adopting our z~4 QSO luminosity function and applying a mild density evolution in redshift, a photo-ionization rate of GammaHI=0.46+0.17-0.09x10^-12 s^-1 has been obtained at z=4.75, assuming an escape fraction of ~70% and a steep faint-end slope of the AGN luminosity function. The derived photo-ionization rate is ~50-100% of the ionizing background measured at the end of the reionization epoch, suggesting that AGNs could play an important role in the cosmological reionization process., Comment: Accepted for publication in ApJ. 15 pages, 3 figures, 3 tables

Abstract

Based on results by recent surveys, the number of bright quasars at redshifts z>3 is being constantly revised upwards. Current consensus is that at bright magnitudes ($M_{1450}\le -27$) the number densities of such sources could have been underestimated by a factor of 30-40%. In the framework of the QUBRICS survey, we identified 58 bright QSOs at 3.6$\le z \le $4.2, with magnitudes $i_{psf}\le$18, in an area of 12400$deg^{2}$. The uniqueness of our survey is underlined by the fact that it allows us, for the first time, to extend the sampled absolute magnitude range up to $M_{1450}= -29.5$. We derived a bright-end slope of $\beta=-4.025$ and a space density at $=-28.75$ of 2.61$\times 10^{-10} Mpc^{-3}$ comoving, after taking into account the estimated incompleteness of our observations. Taking into account the results of fainter surveys, AGN brighter than $M_{1450}=-23$ could produce at least half of the ionizing emissivity at z$\sim$4. Considering a mean escape fraction of 0.7 for the QSO and AGN population, combined with a mean free path of 41.3 proper Mpc at $z=3.9$, we derive a photoionization rate of $Log(\Gamma [s^{-1}])=-12.17^{+0.13}_{-0.07}$, produced by AGN at M$_{1450}<-18$, i.e. ~100% of the measured ionizing background at z~4., Comment: Accepted for publication in ApJ, 15 pages, 5 figures, 4 tables

Abstract

We present the results of the spectroscopic follow up of the QUBRICS survey. The selection method is based on a machine learning approach applied to photometric catalogs, covering an area of $\sim$ 12,400 deg$^2$ in the Southern Hemisphere. The spectroscopic observations started in 2018 and identified 55 new, high-redshift (z>=2.5), bright (i<=18) QSOs, with the catalog published in late 2019. Here we report the current status of the survey, bringing the total number of bright QSOs at z<=2.5 identified by QUBRICS to 224. The success rate of the QUBRICS selection method, in its most recent training, is estimated to be 68%. The predominant contaminant turns out to be lower-z QSOs at z<2.5. This survey provides a unique sample of bright QSOs at high-z available for a number of cosmological investigations. In particular, carrying out the redshift drift measurements (Sandage Test) in the Southern Hemisphere, using the HIRES spectrograph at the 39m ELT, appears to be possible with less than 2500 hours of observations spread over 30 targets in 25 years., Comment: Accepted for publication in ApJS, 24 pages, 8 figures, 4 tables

Abstract

Allergic contact dermatitis (ACD) is a common inflammatory skin disease caused by delayed hypersensitivity to chemical and biotic contact allergens. ACD significantly affects the patients' quality of life negatively impacting both occupational and non-occupational settings. Patch testing is the gold standard diagnostic in vivo test to precise the ACD etiology and to correctly perform prevention. According to the Italian Medicines Agency (AIFA) legislative decree no. 178 of 29th May 1991, allergens are defined as medicines and therefore they are subject to strict regulation. In 2017, AIFA (decree no. 2130/2017) started a procedure to regulate contact allergens on the Italian market and actually the contact allergens temporarily authorized are reported in AIFA decree no. 98/2022, valid until November 2023. The availability on the market of contact allergens to diagnose ACD and continuous updating on the basis of new epidemiological trends are mandatory, jointly with the continuous update of the baseline and integrative series for patch testing. For this reason, the scientific community represented in Italy by the Skin Allergies Study Group of SIDeMaST (Italian Society of Dermatology and Venereology) and SIDAPA (Italian Society of Allergological, Occupational and Environmental Dermatology) are constantly working, in close relationship with the European scientific communities with large expertise in this important sector of the modern Dermatology. Herein, we report the setting up of regulatory legislation by AIFA and the new Italian Adult Baseline Series for patch testing.

Abstract

The emergence of new SARS-CoV-2 variants poses challenges to global surveillance efforts, necessitating swift actions in their detection, evaluation, and management. Among the most recent variants, Omicron BA.2.86 and its sub-lineages have gained attention due to their potential immune evasion properties. This study describes the development of a digital PCR assay for the rapid detection of BA.2.86 and its descendant lineages, in wastewater samples. By using this assay, we analyzed wastewater samples collected in Italy from September 2023 to January 2024. Our analysis revealed the presence of BA.2.86 lineages already in October 2023 with a minimal detection rate of 2% which then rapidly increased, becoming dominant by January 2024, accounting for a prevalence of 62%. The findings emphasize the significance of wastewater-based surveillance in tracking emerging variants and underscore the efficacy of targeted digital PCR assays for environmental monitoring.

Abstract

The SARS-CoV-2 Omicron variant emerged in South Africa in November 2021, and has later been identified worldwide, raising serious concerns. A real-time RT-PCR assay was designed for the rapid screening of the Omicron variant, targeting characteristic mutations of the spike gene. The assay was used to test 737 sewage samples collected throughout Italy (19/21 Regions) between 11 November and 25 December 2021, with the aim of assessing the spread of the Omicron variant in the country. Positive samples were also tested with a real-time RT-PCR developed by the European Commission, Joint Research Centre (JRC), and through nested RT-PCR followed by Sanger sequencing. Overall, 115 samples tested positive for Omicron SARS-CoV-2 variant. The first occurrence was detected on 7 December, in Veneto, North Italy. Later on, the variant spread extremely fast in three weeks, with prevalence of positive wastewater samples rising from 1.0% (1/104 samples) in the week 5–11 December, to 17.5% (25/143 samples) in the week 12–18, to 65.9% (89/135 samples) in the week 19–25, in line with the increase in cases of infection with the Omicron variant observed during December in Italy. Similarly, the number of Regions/Autonomous Provinces in which the variant was detected increased from one in the first week, to 11 in the second, and to 17 in the last one. The presence of the Omicron variant was confirmed by the JRC real-time RT-PCR in 79.1% (91/115) of the positive samples, and by Sanger sequencing in 66% (64/97) of PCR amplicons. In conclusion, we designed an RT-qPCR assay capable to detect the Omicron variant, which can be successfully used for the purpose of wastewater-based epidemiology. We also described the history of the introduction and diffusion of the Omicron variant in the Italian population and territory, confirming the effectiveness of sewage monitoring as a powerful surveillance tool.

Abstract

Background: Few and small studies have described the management of immunomodulant/immunosuppressive therapies or phototherapy in atopic dermatitis (AD) patients during coronavirus disease 2019 (COVID-19) pandemic. Methods: A national registry, named DA-COVID-19 and involving 35 Italian dermatology units, was established in order to evaluate the impact of COVID-19 pandemic on the management of adult AD patients treated with systemic immunomodulant/immunosuppressive medications or phototherapy. Demographic and clinical data were obtained at different timepoints by teledermatology during COVID-19 pandemic, when regular visits were not allowed due to sanitary restrictions. Disease severity was assessed by both physician- and patient-reported assessment scores evaluating itch intensity, sleep disturbances, and AD severity. Results: A total of 1831 patients were included, with 1580/1831 (86.3%) continuing therapy during pandemic. Most patients were treated with dupilumab (86.1%, 1576/1831) that was interrupted in only 9.9% (156/1576) of cases, while systemic immunosuppressive compounds were more frequently withdrawn. Treatment interruption was due to decision of the patient, general practitioner, or dermatologist in 39.9% (114/286), 5.6% (16/286), and 30.1% (86/286) of cases, respectively. Fear of increased susceptibility to SARS-CoV-2 infection (24.8%, 71/286) was one of the main causes of interruption. Sixteen patients (0.9%) resulted positive to SARS-CoV-2 infection; 3 of them (0.2%) were hospitalized but no cases of COVID-related death occurred. Conclusions: Most AD patients continued systemic treatments during COVID pandemic and lockdown period, without high impact on disease control, particularly dupilumab-treated patients.

Abstract

Finding the brightest QSOs at high-z is important both for constraining cosmic evolution and fundamental physics. However, in the Southern Hemisphere, the number of bright QSOs is still relatively scarce. The advent of recent databases, including SkyMapper, Gaia, DESI, offers a golden opportunity to fill in this gap. The QUBRICS survey (QUasars as BRIght beacons for Cosmology in the Southern hemisphere) has been active since 2019, exploiting these databases through machine learning techniques (e.g. CCA, PRF, XGB) and so far over 400 new, bright (i < 18) and high-redshift (z > 2.5) QSOs have been spectroscopically confirmed. In this poster we highlight some of the multiple scientific applications enabled by such a dataset like (i) improving the estimate of the luminosity function; (ii) analyzing absorption features along the line of sight and (iii) improving the performance of the selection algorithms. In the future, QSOs confirmed by this survey will be the targets of subsequent studies using higher resolution spectrographs like MIKE, ESPRESSO, WINERED and will be prime targets for foreground galaxy redshift surveys with LLAMAS.

Dupilumab was recently approved for treatment of adolescent AD. Objectives A multicentre, prospective, real-world study on the effectiveness and safety of dupilumab in adolescents (aged from >= 12 to <18 years) with moderate-to-severe AD was conducted. The main AD clinical phenotypes were also examined. Methods Data of adolescents with moderate-to-severe AD treated with dupilumab at label dosage for 16 weeks were collected. Treatment outcome was assessed by EASI, NRS itch, NRS sleep loss and CDLQI scores at baseline and after 16 weeks of treatment. The clinical scores were also evaluated according to clinical phenotypes. Results One hundred and thirty-nine adolescents were enrolled in the study. Flexural eczema and head and neck eczema were the most frequent clinical phenotypes, followed by hand eczema and portrait-like dermatitis. Coexistence of more than 1 phenotype was documented in 126/139 (88.5%) adolescents. Three patients (2.1%) contracted asymptomatic SARS-CoV-2 infection and 1 of the discontinued dupilumab treatment before the target treatment period. A significant improvement in EASI, NRS itch, NRS sleep loss and CDLQI was observed after 16 weeks of treatment with dupilumab. This outcome was better than that observed in clinical trials. Dupilumab resulted effective in all AD phenotypes, especially in diffuse eczema. Twenty-eight (20.1%) patients reported adverse events, conjunctivitis and flushing being the most frequent. None of patients discontinued dupilumab due to adverse event. Conclusions Dupilumab in adolescent AD showed excellent effectiveness at week 16 with consistent improvement of all clinical scores. Moreover, dupilumab showed a good safety profile also in this COVID-19 pandemic era.

Abstract

Necrobiosis lipoidica (NL) is a rare chronic granulomatous degenerative skin disease by unknown causes, which is mostly associated with diabetes mellitus, usually presenting with typical plaques of the shins. Although less common, some ulcerative forms may be seen in clinical practice. The occurrence of an atypical presentation in one of our patients was the occasion to review the pertinent literature.

Abstract

Patch testing is the standard procedure used to diagnose allergic contact dermatitis. It is an in-vivo test, which reproduces the reaction to a contact allergen. This in-vivo test aims to reproduce the elicitation phase of allergic contact dermatitis and is performed applying allergens under occlusion on the skin under standardized conditions. These guidelines for the best practice in performing patch test have been developed by an Italian group of experts taking in account the Italian legislation and local pharmacological governance. Guidelines are adapted from the original article under the guidance of the European Society of Contact Dermatitis (ESCD) and on the basis of the SIDAPA guidelines.

Abstract

Background: Treatment of moderate-to-severe atopic dermatitis (AD) in the elderly may be challenging, due to side-effects of traditional anti-inflammatory drugs and to comorbidities often found in this age group. Furthermore, efficacy and safety of innovative drugs such as dupilumab are not yet well known. Objectives: A multicentre retrospective, observational, real-life study on the efficacy and safety of dupilumab was conducted in a group of patients aged ≥65years and affected by severe AD. Their main clinical features were also examined. Methods: Data of elderly patients with severe (EASI ≥24) AD treated with dupilumab at label dosage for 16weeks were retrospectively collected. Treatment outcome was assessed by comparing objective (EASI) and subjective (P-NRS, S-NRS and DLQI) scores at baseline and after 16weeks of treatment. Results: Two hundred and seventy-six patients were enrolled in the study. They represented 11.37% of all patients with severe AD. Flexural eczema was the most frequent clinical phenotype, followed by prurigo nodularis. The coexistence of more than one phenotype was found in 63/276 (22.82%) subjects. Data on the 16-week treatment with dupilumab were available for 253 (91.67%) patients. Efficacy of dupilumab was demonstrated by a significant reduction of all the scores. No statistically significant difference regarding efficacy was found in elderly patients when compared to the group of our AD patients aged 18–64years, treated with dupilumab over the same period. Furthermore, only 18 (6.52%) patients discontinued the drug due to inefficacy. Sixty-one (22.51%) patients reported adverse events, conjunctivitis and flushing being the most frequent. One (0.36%) patient only discontinued dupilumab due to an adverse event. Conclusions: Therapy with dupilumab led to a significant improvement of AD over a 16-week treatment period, with a good safety profile. Therefore, dupilumab could be considered as an efficacious and safe treatment for AD also in the elderly.

Abstract

Porcine circovirus associated disease (PCVAD) complex is caused by co-infections of PCV2 and other common pathogens and/or environmental stressors. Different vaccines have been developed to reduce PCV2 infections and PCVAD. Such vaccines are poorly standardized in terms of antigen payload and recognized correlates of protection. Therefore, we selected twenty, 40-day old piglets, and allocated them to 4 groups (5 animals each) with uniform levels of maternally-derived antibody to PCV2. Animals were vaccinated with 450/150/50/0 nanograms of an inactivated PCV2b strain, formulated in the same adjuvant of the commercial Circovac vaccine. Twenty-seven days later, all pigs were challenged intranasally with the homologous PCV2 strain. The main findings can be summarized as follows: 1) No clinical signs were observed in the pigs under study. 2) Viremia was observed in all the control pigs, as well as in 2 pigs of the 150 and 50 ng groups, respectively. No pigs of the 450-ng group developed viremia. 3) There was no correlation between protection and ELISA Ab titers in the single animals, even though the 450-ng group developed on average a stronger Ab response. 4) All the pigs with a PCV2-specific IFN-gamma response at 3 weeks after vaccination were fully protected against viremia. The IFN-gamma response at this time point was peculiar to CD4+, single positive T cells, whereas both CD8alpha and CD8 beta+ T cells were also positive after challenge infection. 5) In lymphoid tissues (mainly tonsils and ileum) the presence of sparse reactive hystiocytes and multinucleated giant cells was the only PCV2-associated feature and, by immunohistochemistry, only 2 out of 20 subjects (still viremic at 35 PID) had a mild viral load (grade 1 in 8 samples and grade 2 in only 1 sample). Our data point at the IFN-gamma release assay as a useful tool for monitoring efficacy of PCV2 vaccines.

Abstract

Helicobacter pylori is a Gram-negative bacterium identified for the first time about 30 years ago and commonly considered as the main pathogenic factor of gastritis and peptic ulcer. Since then, it was found to be associated with several gastrointestinal and extra-gastrointestinal diseases. Helicobacter pylori is also associated with many skin disorders including, but not limited to, chronic urticaria, rosacea, lichen planus, atopic dermatitis, psoriasis, pemphigus vulgaris, vitiligo, primary cutaneous MALT-type lymphoma, sublamina densa-type linear IgA bullous dermatosis, primary cutaneous marginal zone B-cell lymphomas and cutaneous T-cell pseudolymphoma. Literature up to September 2020 shows that clear evidence exists only for some of the mentioned associations, while in the majority of cases, data appear contrasting. The aim of this review is to summarize the available studies on the topic and draw possible conclusions. Further clinical and laboratory studies are needed to assess the real plausibility and relevance of these associations, as well as the possible role of Helicobacter pylori with the underlying pathogenic mechanisms.

Abstract

Sun exposure is the main external risk factor for keratinocytic non-melanoma skin cancer (NMSC). Outdoor workers are at increased risk, but the relationship of NMSC with occupational solar exposure is often confounded by concurrent recreational sun exposure. We compared the percentage of outdoor workers in NMSC patients versus controls without history of NMSC and assessed occupational and recreational sun exposure in both groups, evaluating also other risk factors and use of protective measures.Adult NMSC patients and controls without history of NMSC or actinic keratoses, matched for sex and age range, were recruited in the Departments of Dermatology of seven Italian University Hospitals, with a 1:2 patient/control ratio whenever possible. Data were collected using specifically designed questionnaires.Eight hundred thirty-four patients and 1563 controls were enrolled. History of outdoor work was significantly (P=0.033) more frequent in patients. Patients were more sun exposed from outdoor leisure activities (P=0.012) and sunbathed for longer periods (P=0.13) and between 12 pm and 3.30 pm (P=0.011). Cumulative sun exposure during hobbies was similar between patients and controls in outdoor workers, higher (P0.05) in patients among indoor workers. Patients and controls with history of outdoor work were more sun exposed at work than during leisure activities (P0.001). Use of sunscreens by outdoor workers was very low, particularly at work (19.9%). Patients used sunscreens more than controls (P=0.002).Occupational and recreational sun exposure are relevant risk factors for outdoor and indoor workers respectively. Sunscreens are alarmingly underused, particularly at work, and are used mainly by patients.

Abstract

Background: There is currently no agreed cosmetic series for use across Europe.Objectives: To establish allergens currently tested in local and national cosmetic series.Method: Members of the European Surveillance System on Contact Allergy and the European Cooperation in Science and Technology project TD1206 ("StanDerm") were surveyed to establish their current practice.Results: A wide range of allergens was tested but there was significant variation between centres on the allergens considered to be important in screening for allergy to cosmetics. The number of allergens tested in addition to the baseline series varied between 2 and 50.Conclusions: There is a need for further investigation to establish the frequency and relevance of reactions to cosmetic allergens to enable an agreed evidence-based cosmetic series to be produced. Criteria for inclusion need to be established.

Abstract

BACKGROUND Studies specifically conducted to assess gender differences in genital lichen sclerosus (GLS) are not available. This multicenter study aimed to identify possible gender-related differences on GLS clinical features, history and course, through collecting data from a large mixed-sex sample of patients. METHODS This was a cross-sectional study on 729 subjects (53.8% females, 46.2% males) affected with GLS, consecutively observed within a network of 15 Italian dermatology units. The following information was specifically collected: clinical features and severity of symptoms related to GLS, extragenital involvement, previous therapies, diagnostic suspicion at referral, type of referring physicians, development of genital squamous-cell carcinoma (SCC). RESULTS Females complained of symptoms more frequent and severe than men; pallor and scarring-sclerosis-atrophy were the most frequent features without gender differences; itching- related signs were more frequent in females than in males as well as extragenital involvement; prior to receiving a definitive diagnosis, females received treatment more frequently than males; 40% of patients were referred with a misdiagnosis; the highest rate of correct suspected diagnosis at referral came from dermatologists than from other physicians; duration of the disease was found to predispose to SCC development. CONCLUSIONS Our findings highlighted several gender differences on clinical presentation and symptom profile of GLS. In spite of some characteristic features, misdiagnosis at referrals was frequent.

Abstract

Atopic dermatitis (AD) places significant burden not only on quality of life, but is also associated with considerable costs to healthcare systems. Diagnosis of AD may be challenging when it starts in adolescence or adulthood, and is further complicated as its manifestations are different from those generally seen in children. Accordingly, better definition of diagnostic criteria for adult onset AD is needed to avoid misdiagnosis and undertreatment in adult patients. to provide practical guidance for clinicians to reliably diagnose ad in adult patients, representatives from three Italian dermatology scientific societies (Italian Society of Dermatology and Venereology [SIDeMaST], Italian Association of Hospital Dermatologists [ADOI], Italian Society of Allergological, Occupational and Environmental Dermatology [SIDAPA]) carried out a joint consensus meeting to develop useful indications for improving diagnosis of moderate to severe ad in adult patients in routine clinical practice. the most representative criteria for morphological criteria, localization, clinical history, and differential diagnosis were identified by the experts. The most frequent clinical presentations are those on the flexural areas, hands, face/neck, and trunk, with itch and eczema as key manifestations. The diagnostic path defined herein can form a sort of “check list” for physicians to adopt when evaluating patients with suspected AD, which can help in refining a diagnosis and refer the patient for specialist dermatological care. It is hoped that the practical guidance developed by the consensus group will help to improve outcomes, lower overall costs of care, and ameliorate the patient's quality of life, even though validation in a large cohort of patients is still needed.

Pemphigus is a life threatening autoimmune epidermal blistering disease involving skin and mucous membranes. Pemphigus usually affects middle age men and women involving oral mucosa first and then spreading on the skin. It is caused by the presence of autoantibodies (IgG and less frequently by IgA) directed against desmogleins, and/or other glycoproteins that plays a critical role in cell-cell attachment. Upon a predisposing genetic background, different agents have been shown to act as triggers for the pathogenesis of pemphigus. This guideline for the diagnosis and treatment of pemphigus has been developed by an Italian group of experts taking in account the Italian legislation and local pharmacological governance. Guidelines are adapted from the original article under the guidance of the European Dermatology Forum in collaboration with the European Academy of Dermatology and Venereology. It summarizes evidence-based and expert-based recommendations (S2 level).
