5. 11258 Evaluation and Management of Right Upper Quadrant Endometriosis.

Abstract

The objective of this video is to highlight a case of right upper quadrant endometriosis, a rare manifestation of extra-pelvic endometriosis, and discuss the considerations and techniques used for successful surgical management. Intraoperative footage was edited and annotated to illustrate the key learning points of this case. The case took place in the main operating room, with the patient placed under general endotracheal anesthesia and positioned in dorsal lithotomy. The patient was a 36-year-old G0 who initially presented with severe right upper quadrant and flank pain occurring at the time of her menses for the last 6 months. Imaging demonstrated a small subcapsular liver lesion, otherwise abdominal and pelvic imaging was unremarkable. She was recommended to proceed with diagnostic laparoscopy. The surgery begins with a thorough inspection of the upper abdomen where the video highlights where endometriosis lesions are found in relation to key anatomy and where expert knowledge of the anatomy guided decision-making on whether or not to resect or fulgurate the identified lesions. We then demonstrate a variety of surgical techniques for subsequent management including excision as well as fulguration with both monopolar heat sources and argon beam ablation. This case highlights the importance of a thorough abdominal survey for patients undergoing diagnostic laparoscopy for cyclic pelvic and abdominal symptoms. The video also showcases the importance of collaboration with a multidisciplinary surgical team to successfully manage patients in whom right upper quadrant endometriosis lesions are identified. 11. Tuneable Emission of Polyhedral Oligomeric Silsesquioxane Based Nanostructures that Self-Assemble in the Presence of Europium(III) Ions: Reversible trans-to-cis Isomerization

Abstract

Hybrid nanostructures with switchable and reversible “blue-red-green” emission were efficiently synthesized. These nanostructures comprise polyhedral oligomeric silsesquioxanes (POSS) that behave as a nanocage that can be functionalized with terpyridine-based organic ligands, which can be easily complexed with europium (III) ions. The complexes were characterized by UV-Vis and fluorescence spectroscopy and their stoichiometry was also confirmed by H NMR spectroscopy. In the presence of the Eu(III) ions, the octafunctionalized nanocages self-assemble to form 3D architectures that display an intense red-emission, especially in the solid state. The presence of an alkenyl group bridging the inorganic core to the organic moiety was employed to tune the emission properties by trans-cis isomerization of the double bond. In the case of the octafunctionalized nanocages (O-POSS), this isomerization was monitored in the presence of Eu(III) cations and was accompanied by an evident colour change from blue (trans-O-POSS) to red (Eu@trans-O-POSS) and finally to green (cis-O-POSS) as consequence of the release of the metal cations. This behaviour, together with the easy dispersion of the dry powder and the possibility of coating as a film in presence of small amounts of solvent, makes the emissive solid promising for applications in materials science.

Abstract

Several anti-6-substituted 1,3-dioxepan-5-ols were kinetically resolved using an immobilized lipase (Amano PS–C II) in toluene in the presence of vinyl acetate at 30 C. This approach provided, in some cases, the alcohol and the acetate in high enantiomeric purity, depending on the nature of the substituent (R = N3, SePh, I, OBn) and the acetal group (unsubstituted or dimethyl). The role of the size of substituents is also discussed. Enantiopure anti-6-substituted 1,3-dioxepan-5-ols are useful building blocks.

Abstract

Halloysite nanotubes (HNTs) represent a versatile core structure for the design of functional nanosystems of biomedical interest. However, the development of selective methodologies for the site-controlled functionalization of the nanotubes at specific sites is not an easy task. This study aims to accomplish a procedure for the site-selective/specific, “pin-point”, functionalization of HNTs with polydopamine (HNTs@PDA). This goal was achieved, at pH 6.5, by exploiting the basicity of ZnO nanoparticles anchored on the HNTs external surface (HNTs@ZnO) to induce a punctual polydopamine polymerization and coating. The morphology and the chemical composition of the nanomaterial was demonstrated by several techniques. Turbidimetric analysis showed that PDA coating affected the aqueous stability of HNTs@PDA compared to both HNTs@ZnO and HNTs. Notably, hyperthermia studies revealed that the nanomaterial induced a local thermic rise, up to 50 °C, under near-infrared (NIR) irradiation. Furthermore, secondary functionalization of HNTs@PDA by selective grafting of biotin onto the PDA coating followed by avidin binding was also accomplished.
