This study evaluated the soft tissue stability around 10 single-tooth implants. All cases were treated following the same protocol, which included guided bone regeneration and connective tissue grafting. One year after prosthesis insertion the soft tissue shrinkage on the buccal side of the implant crown was 0.6 mm on average. A total of 264 implants was placed in 143 patients using different immediate or delayed-immediate implant placement techniques in 12 different centers participating in a prospective multicenter study. The reason for tooth extraction was evaluated; bone quality and quantity were classified; socket depths were registered; and data on implant type, size, and position were collected. One hundred thirty-nine suprastructures were placed on 228 implants in 126 patients. A follow-up evaluation was done on 125 patients after 1 year of loading and on 107 patients after 3 years of loading. Clinical parameters (bleeding or not bleeding, pocket depth, and implant mobility) were evaluated after 1 and 3 years, and the marginal bone level after 1 year of loading was measured on radiographs. Clinical comparisons were performed to evaluate implant loss in relation to implant type, size, position, bone quality and quantity, socket depth, reason for tooth extraction, and placement method. In addition, life table analysis was done for cumulative implant survival rates. There was no clinical difference with respect to socket depth or when comparing the different placement methods. A higher failure rate was found for short implants in the posterior region of the maxilla and when periodontitis was cited as a reason for tooth extraction. Mean marginal bone resorption from the time of loading to the 1-year follow-up was 0.8 mm in the maxilla and 0.5 mm in the mandible. The article presents a study on the role of attached gingiva in the health of peri-implant tissue in dogs. It examines the peri-implant health status of endosseous oral implants with and without bucchal attached gingiva in relation to plaque control and plant accumulation in the dogs. The impact of removing the keratinized gingiva through excision or replacement of the excised tissue with a free graft around implants with a reduced supporting apparatus is also evaluated. Several parameters were used, including plaque index, sulcus bleeding index and pocket probing depth.

The article describes the condition of art in suprastructure design using endosseous oral implants. It is noted that the procedures for implant-retained overdenture and bone-anchored fixed bridge are designed to have an optimal aesthetic result. Such procedures include treatment planning, surgical procedures, prosthodontic procedures and oral hygiene and recall. The fabricating possibilities for suprastructure are presented in normal patients and in a patient with less than adequate bone support. The technique presented here combines the knowledge of the reciprocal action between oral biology, dental technology and optimal esthetics.

The aim of this chart review was to obtain an objective, quantitative assessment of the clinical performance of an implant line used in an implantological office setting. Implants with hydrophilic (INICELL) and hydrophobic (TST; both: Thommen Medical AG, Grenchen, Switzerland) enossal surfaces were compared and the cumulative implant survival rate was calculated. The data of 1063 patients that received 2918 implants (1337 INICELL, 1581 TST) was included. The average follow up time was 2.1 (1.1-5.4) years for INICELL and 4.5 (1.3-5.9) years for TST implants (Thommen Medical AG, Switzerland). In the reported period 7 implants with INICELL (0.5%) and 23 TST implants (1.5%) failed. This difference was statistically significant. The analysis of cases treated and followed up in a single implantological office for 6 years confirmed the very good clinical outcome that was achieved with both used implant lines. Within the limitations of this retrospective analysis, the overall early failure rate of the hydrophilic implants was significantly lower than that of hydrophobic implants. The use of hydrophilic implants allows the clinician to obtain less early failures, hence the interest of an up-to-date surface for the daily work of an implant practice.

Objectives: To compare the clinical and histological outcomes of a resorbable modified polylactide/polyglycolide acid (PLGA) test membrane and a titanium-reinforced expanded polytetrafluorethylene (ePTFE) control membrane used for guided bone regeneration (GBR) around dental implants., Materials and Methods: A total of 40 patients with peri-implant dehiscence-type defects were randomly allocated to a GBR procedure using either a modified PLGA test or an ePTFE control membrane. Soft tissue condition, implant integration, adverse events and quality of life were recorded during the 6-month healing period. At re-entry peri-implant bone defect dimensions were measured and compared with values recorded at implant/GBR surgery. A biopsy was retrieved for qualitative and quantitative histological analyses. A comparison between the groups was conducted using non-parametric statistical tests., Results: Soft tissue complications were observed in five test patients and two control patients. Except for soft tissue complications and incomplete regeneration, no procedure- or device-related adverse events were observed. The vertical bone defect component was, in mean, reduced by-5.1 mm (95% CI -6.8, -3.3) in the test group and -6.9 mm (95% CI -8.2, -5.5 mm) in the control group. The mean residual vertical defect height measured 1.2 ± 2.4 mm in the test group and 0.3 ± 1.1 mm in the control group meaning a mean defect resolution of 81% in the test group and 96% in the control group (P = 0.161). The horizontal bone thickness at implant shoulder level decreased from a mean of 3.2 mm to 1.4 mm (-56%, mean -1.7 mm, 95% CI -2.3, -1.1) in the test group and from 3.3 mm to 2.5 mm (-24%, mean -0.8 mm, 95% CI -1.3, -0.3) in the control group (P = 0.022). Qualitative and quantitative histological analyses did not show significant differences in the tissue composition between groups., Conclusion: Peri-implant GBR was successfully performed using either of the membranes. The control membrane was able to better maintain the horizontal thickness of regenerated bone and revealed less soft tissue complications. No statistically valid evidence about the superiority of one membrane was found in any other parameters. The objective of this case series was to evaluate the clinical and histologic outcome of guided bone regeneration around simultaneously placed implants in sites with missing buccal bone walls. Eight weeks after tooth extraction, implants were inserted, and the sites were augmented in both the horizontal and vertical dimensions using a mineralized collagen bone substitute and a nonresorbable titanium-reinforced membrane. Six months later, small hard tissue biopsy specimens were harvested from the buccal bone walls at approximately mid-height of the original defect. The histologies revealed ongoing bone formation. Clinically, an adequate amount of hard and soft tissue volume had formed.

The esthetic outcome of an implant-supported restoration is first of all dependent on the soft tissue volume. Since the labial bone plate resorbs in every direction after tooth extraction, even when an implant is placed immediately, most patients end up with compromised esthetics. Twenty-four patients were treated consecutively with implants placed in the maxillary anterior area at the time of tooth extraction using two different treatment modalities. The first 12 patients were treated without raising a flap, whereas a subepithelial connective tissue graft was placed using the tunnel technique in the labial area of the subsequent 12 patients at the time of tooth extraction and implant placement. The dimension of the labial volume was measured before treatment and 6 months after implant placement. The results show an average loss of volume in the nongrafted group of 1.063 mm, whereas in the grafted group, there was a slight gain of 0.34 mm. These results demonstrate the effectiveness of placing a soft tissue graft at the time of immediate implant placement in the esthetic zone.

Objectives: the aim of the present study was to evaluate the dimensional changes of peri-implant tissues obtained by implant placement, bone and soft tissue augmentation, prosthetic reconstruction and 1 year of function using a new, non-invasive method for volumetric measurements., Materials and Methods: in 16 patients, the missing central or lateral maxillary incisor was reconstructed with an implant-supported single crown. Impressions were taken before (t1), after implant placement with guided bone regeneration using DBBM and a PTFE membrane (t2), after soft tissue augmentation (t3), immediately after crown placement (t4) and 1 year later (t5). The cast models were optically scanned and digitally superimposed allowing qualitative and quantitative analysis of alterations of the labial peri-implant tissue contour. In addition, the crown length and papilla height were measured at crown placement (t4) and after 1 year (t5)., Results: fifteen patients were available for recall after 1 year. During therapy, a mean gain in distance in the labial direction of 1.27 ± 0.67 mm was observed after the surgical procedures. One year after crown insertion, a mean loss of 0.04 ± 0.31 mm in the labial direction was recorded. During the same period, the crown length increased by a mean of 0.22 ± 0.57 mm and the papilla height by 0.07 ± 0.61 mm. The degree and pattern of tissue change following crown insertion were highly variable between individuals, irrespective of the amount and quality of previously augmented tissues., Conclusions: the clinical procedures were effective in augmenting peri-implant tissue volume that remained stable to a high degree within 1 year after crown insertion. Large inter-individual variations regarding the tissue alterations were observed.

The use of small-diameter implants is indicated when small missing teeth have to be replaced, especially in esthetic zones. Nevertheless, the small diameter can pose a limiting factor with respect to what materials can be used for the final crown. In most cases, full-ceramic crowns in combination with a ceramic abutment are usually the material of choice for final reconstructions. To date, based on mechanical considerations, a 3.5 mm implant diameter has been a contraindication for using ceramic abutments. The authors describe here the development, in vitro testing, and clinical use of a zirconium abutment with a 3.5 mm diameter. The advantages of this small-diameter zirconia abutment include a minimum platform height that offers optimal prosthetic flexibility, and an accurate transfer of the implant position on to the master model. Furthermore, a precise rotational orientation for single-tooth restorations, optimal mechanical stability, and optimal fatigue resistance can be achieved. The microgap is minimized and protection against overload is afforded. In the reported case, high patient satisfaction was achieved due also to an esthetically pleasing final result.

In a 40-year-old woman, a central incisor was to be replaced with an implant-supported crown. When the implant was uncovered, the soft tissue around the implant showed an unfavorable profile: The adjacent lateral incisor showed significant loss of periodontal attachment. Subsequent soft tissue and connective tissue grafts failed to correct the problem, and an alternative treatment plan was formulated. Errors in the initial treatment, including incorrect implant position and failure to analyze the cause of the periodontal attachment loss are discussed in detail; the options for further treatment measures are outlined; and the treatment that was subsequently carried out is presented.

There are biologic limits of the soft tissue dimension around implants; therefore, the limiting factor for the esthetic result of implant therapy is the bone level at the implant site. Clinicians must focus on the 3-D bone-to-implant relationship to establish the basis for an ideal and harmonic soft tissue situation that is stable over a long period. In some situations, missing bone is a limiting factor for esthetics; in others, it is possible to regenerate new bone around implants. As a certain amount of bone resorption occurs around implants as soon as the implant is in contact with the oral environment, the distance between an implant and adjacent tooth, as well as the distance between two implants, is as important as the bone volume on the buccal side of the implant head and in the papillary area, especially for the long-term result. This article discusses the 3-D bone-to-implant relationship and its influence on soft tissue esthetics around implants.

The purpose of this study was to evaluate the clinical success of immediate functional loading of immediate implants in edentulous arches. Five maxillary and five mandibular jaws were treated, and a total of 91 implants were placed; 66 of these implants were placed immediately after tooth extraction, and 25 were placed in healed sites. No bone substitutes or barrier membranes were used. Within 24 hours, fixed temporary restorations were inserted in all cases. During the entire 6-month healing time, all fixed temporary restorations were in normal function, after which the final fixed implant-supported restorations were inserted. After 24 months, the overall success rate of the implants was 92.31% (87.50% for the maxillary implants and 97.26% for the mandibular implants). The bone level measured mesially and distally was in 93.40% of all cases between the implant shoulder and the first thread. The present study shows that the immediate functional loading of immediate implants without the use of any bone substitutes or barrier membranes for fixed complete-arch reconstructions can be successful over a 2-year period.

Background: As a complement to the earlier reported 3-year results from a prospective multicenter study of immediate and delayed placement of implants into fresh extraction sockets, the 5-year results are reported., Purpose: The purpose of this 5-year report was to evaluate the immediate and long-term success of implants placed into fresh extraction sockets, with respect to implant size and type, bone quality and quantity, implant position, initial socket depth, and reason for tooth extraction., Materials and Methods: This paper presents the 5-year results of the original 12 centers that participated with 143 consecutively included patients. A total of 264 implants were placed either immediately after tooth extraction or after a short soft-tissue healing time (3-5 weeks). The patients were divided into five subgroups, depending on the type of insertion method used., Results: The outcome demonstrated that the cumulative implant survival rate after 5 years of loading has not changed and remains 92.4% in the maxilla and 94.7% in the mandible. No difference in failure rates can be seen between the groups when relating the failures to insertion method., Conclusion: This prospective study demonstrated that placing Brånemark implants into fresh extraction sites can be successful over a period of 5 years of loading. One of the outcomes of the study shows that there is a clinical correlation between implant failure and periodontitis as a reason for tooth extraction, even if it is difficult to give it a casual association. It can be hypothesized that periodontitis affected tissues might have a negative local influence because of the presence of infrabony defects that could possibly increase the gap between bone and implant or jeopardize achievement of primary stability.

A 5-year prospective, multicenter study is in progress at four private dental practices to determine the cumulative implant survival rate and prosthetic outcome when using the Osseotite dental implant in posterior maxillary and mandibular areas. An interim evaluation after 34.4 months of study progress is presented. A total of 219 Osseotite implants were placed in 74 patients (34 women and 40 men with a mean age of 57.8 +/- 15.2 years) using a conventional two-stage surgical protocol and 3- to 6-month healing time. Subsequently, patients were restored with fixed or removable restorations. Nineteen of the 74 patients reported smoking an average of 13.2 cigarettes per day. Restorative treatments included 40 single-unit restorations; 53 splinted 2-, 3-, 4-, and 5-unit implant-supported maxillary and mandibular prostheses; 4 full-arch fixed maxillary prostheses; 1 mandibular fixed/detachable hybrid prosthesis; and 1 mandibular overdenture. The mean time from implant placement to second stage surgery was 6.2 +/- 2.0 months; from restoration and implant loading to the most recent follow-up evaluation was 20.9 +/- 6.8 months. Of the 219 implants placed, three posterior maxillary implants developed infections and were removed prior to second stage surgery. No implant failures occurred at second stage surgery or after implant loading. Using the Kaplan-Meier method, the cumulative implant survival rate was 100% for anterior implants and 98.4% for posterior implants at 28.5 +/- 5.7 months. The cumulative postloading implant survival rate was 100% for both anterior and posterior implants. The results of this study indicate that the Osseotite dental implant achieved a high rate of integration that remained stable during nearly 2 years of implant function. In addition, because no postloading implant failures have occurred, the Osseotite implant has provided a high level of prosthetic predictability.

The placement of a single tooth implant-supported restoration in the maxillary anterior area still presents a difficult challenge. To address this challenge, various methods and techniques have been proposed. The learning objective of this article is to present a particular clinical protocol for achieving predictable aesthetic restoration, even under the relatively difficult circumstances involving thin, highly scalloped gingiva and high lip line. A clinical case is used to illustrate the presentation of implant surgery, bone regeneration, connective tissue graft, provisionalization, and placement of the definitive prosthetic restorations. The use of pressure to expand soft tissue is described, and the importance of papillae protection is emphasized.

The purpose of this study was to determine the presence of suspected periodontal pathogens in the peri-implant microflora of osseointegrated implants exposed 3 and 6 months to the oral environment of patients previously treated for periodontal disease. Subgingival microbial samples were taken in the deepest residual pocket of each quadrant in 10 patients before placement of ITI implants and in 10 patients before abutment connection of Brånemark implants. The samples were cultured using continuous anaerobic techniques. 4 patients were positive for P. gingivalis, 13 for P. intermedia, 2 for A. actinomycetemcomitans, 16 for Fusobacterium, 7 for C. rectus and 12 showed microscopic evidence of spirochetes. After 3 and 6 months exposure of the implants to the oral environment, one sample was obtained from the peri-implant sulcus in each patient. P. gingivalis was found in 2 patients in the peri-implant samples after 3 and 6 months. 6 peri-implant samples were positive for P. intermedia after 3 months, 7 after 6 months. Fusobacterium was present in 13 peri-implant samples after 3, and in 12 samples after 6 months. 4 patients showed evidence of for C. rectus after 3 months, 2 after 6 months. Spirochetes were seen in 3 peri-implant samples after 3 and in 5 after 6 months. None of the implants were found to be colonized by A. actinomycetemcomitans, although the organism was detected on teeth in 1 individual. This study evaluated the treatment of ligature-induced peri-implantitis using guided tissue regeneration (GTR) around submerged and nonsubmerged implants in beagle dogs. Two titanium implants were placed bilaterally in the premolar regions of the mandible in each of 10 beagle dogs. Cotton floss ligatures were placed around the implant necks and no oral hygiene procedures were performed for 5 months. Periodontal surgery was performed, and in five dogs (submerged group) the abutments were removed and an e-PTFE barrier was placed to cover the test implants and their adjacent osseous defects, while the control implants received no barriers. In the other five dogs (nonsubmerged group), flap surgery was performed without removing the abutments. In the test sites, an e-PTFE barrier was placed around the abutment and adjacent defects, while on the control sites no barrier was placed. Nondecalcified specimens were prepared using standard histologic methods and analyzed. No differences were found between any of the clinical parameters in both the control and experimental sites from the submerged and nonsubmerged groups. Histologic and histomorphometric analyses also revealed no significant differences between groups with regard to new bone formation. In the experimental (GTR) groups, however, the amount of regenerated connective tissue was significantly greater than on the control groups. Under the conditions of this study, the treatment of peri-implantitis using GTR did not enhance clinical parameters or bone formation around "diseased" implants.
