Sports psychiatry and psychotherapy is a relatively young field and is comprised of two key segments: the special features of the diagnostics and therapy of mental disorders in elite athletes and the use of exercise and sports in the development and treatment of mental disorders. Although all mental disorders can in principle also occur in (elite) athletes, there are additionally sport-specific mental disorders, such as anorexia athletica and other eating disorders, chronic traumatic encephalopathy, misuse of and dependency on performance-enhancing substances (doping) and muscle dysmorphia. Many high-quality clinical trials over the past two decades have been able to demonstrate a therapeutic efficacy of physical activity and sport in the treatment of various mental disorders. All clinicians active in psychiatry and psychotherapy should possess a basic knowledge of sports psychiatry. Die Sportpsychiatrie und -psychotherapie ist ein relativ junges Arbeitsgebiet, das sich insbesondere mit zwei Schwerpunkten befasst: zum einen mit den Besonderheiten in Diagnostik und Therapie psychischer Erkrankungen bei Leistungssportler:innen sowie Bewegung und Sport in der Entstehung und Behandlung psychischer Erkrankungen. Während alle psychischen Erkrankungen prinzipiell auch bei (Leistungs‑)Sportler:innen auftreten können, gibt es darüber hinaus sport(art)spezifische psychische Erkrankungen, wie z. B. die Anorexia athletica und andere Essstörungen, die chronisch traumatische Enzephalopathie, Missbrauch und Abhängigkeit von leistungssteigernden Substanzen (Doping) oder die Muskeldysmorphie. In qualitativ hochwertigen klinischen Studien konnte die therapeutische Wirksamkeit von Bewegung und Sport bei verschiedenen psychischen Erkrankungen belegt werden. Ein sportpsychiatrisches Basiswissen sollten alle in Psychiatrie und Psychotherapie klinisch Tätigen besitzen.

Syftet med denna rapport är att närmare analysera länsvisa skillnader i svenskarnas friluftsutövande. Analyserna bygger huvudsakligen på redan insamlat material, nämligen Naturvårdsverkets enkätstudie på temat från 2018. Ytterligare ett syfte är att undersöka hur friluftsutövandet ser ut i relation till resultaten från undersökningen Sveriges friluftskommun 20181 och utifrån olika kommungrupper. Sverige har uttalade friluftslän Analysen visar att det finns en rad skillnader mellan länen när det kommer till svenskarnas friluftsutövande och upplevelser i samband med det. Invånarna i Kronobergs, Gotlands, Kalmars och Jämtlands län framstår som särskilt aktiva i en rad avseenden, medan invånarna i Örebro och Västmanland är mindre aktiva. Intressant nog tillhör invånarna i Kronobergs, Gotlands och Kalmar län de som upplever just platsrelaterade faktorer som hinder för sitt friluftslivsutövande. Sammanställningen visar vidare att det är betydligt vanligare med positiva upplevelser i samband med naturvistelser jämfört med negativa. Likväl utmärkte sig Örebro, Västmanland och Skåne genom att en lägre andel invånare där rapporterade positiva naturupplevelser. Kunskapen om vad man får och inte får göra enligt allemansrätten är generellt god. Den är dock något lägre i Stockholms och Skåne län, och högre bland de som lever på Gotland och i Blekinge. När det kommer till medlemskap i friluftsföreningar finns inget tydligt mönster som i andra frågor från undersökningen. Befolkningen i Jämtland är dock överrepresenterad både vad gäller aktivitetsnivå och medlemskap i friluftsföreningar. Även om det förekommer en rad signifikanta skillnader i de olika analyserna bör betydelsen av dessa inte överdrivas. Att människor inte genomför friluftsaktiviteter på vardagarna, samtidigt som de upplever en rad platsrelaterade hinder, pekar på att närområdet har betydelse för de dagliga rutinerna. Däremot spelar den omedelbara omgivningen en underordnad roll i situationer då människor har mer tid och hinner förflytta sig längre sträckor, som under helger och längre ledigheter. Särskilt bristen på lämpliga platser tycker många är ett hinder för att komma ut i naturen i den utsträckning de önskar. Framför allt i Stockholms län har detta hinder stor betydelse. Storstadsbor har svårast att komma ut i naturen Kommunernas insatser på friluftsområdet verkar enbart ha en svag, om någon, koppling till invånarnas friluftsutövande. Andra faktorer tycks alltså ha avsevärt större betydelse för kommuninvånarnas aktiviteter. Däremot såg vi ett antal skillnader utifrån kommuntyp enligt den grova indelningen i områden som kan karakteriseras som storstäder, större städer och mindre orter. Likt vid analysen av aktiviteter i olika län framträder skillnader i det vardagliga friluftsutövandet, då det är i storstäder som lägst andel personer rapporterar aktiviteter. Men även på helger är åtminstone invånarna i mindre orter och på landsbygder aktiva i större utsträckning än dem som bor i storstäder och större städer. En anledning för storstadsbor att inte vara ute i den utsträckning de önskar är framför allt bristen på lämpliga platser. Andra skillnader är att en större andel personer utanför de mest tätbebyggda kommunerna upplever just naturvärden som positiva, medan andra positiva upplevelser fördelar sig mer lika mellan kommuntyperna. Kunskaperna i allemansrätten skiljer sig också något mellan kommuntyperna där en större andel storstadsbor har luckor, även om de flesta har en god kunskapsnivå även bland dem. The purpose of this report is to analyse in more detail geographical differences in levels of outdoor recreation among people in Sweden. The analysis is primarily based on data already collected, namely from the Swedish EPA’s 2018 questionnaire on this theme. Another purpose is to study outdoor recreation in relation to the results from the study to determine Sweden’s best municipality for outdoor recreation in 2018 (Sveriges friluftskommun 2018) and based on various groups of municipalities. Sweden has counties that distinguish themselves in outdoor recreation The analysis shows that there are several differences between municipalities in terms of the extent of people’s outdoor recreation in Sweden and how they experience it. Residents in Kronoberg, Gotland, Kalmar and Jämtland counties appear to be particularly active in various outdoor activities, while the residents of Örebro and Västmanland counties are less active. It is interesting to note that residents of Kronoberg, Gotland and Kalmar counties are among those who in particular experience location-related factors as obstacles for outdoor recreation. This summary also shows that positive experiences in connection with being out in nature are far more common than negative ones. However, Örebro, Västmanland and Skåne counties stand out as having a lower percentage of residents reporting positive experiences in nature. Knowledge of what is permitted or not permitted under Sweden’s allemansrätten law (legal right of access to private land) is generally good. It is, however, slightly lower in Stockholm and Skåne counties but higher among those living on the island of Gotland and in Blekinge County. With respect to membership in outdoor recreation clubs, there is no clear pattern as in other areas covered in the study. The population of Jämtland County is, however, overrepresented – both in terms of activity levels and membership in outdoor recreation clubs. Although there are several significant differences between the various analyses, the importance of these differences should not be exaggerated. The fact that people are not involved in outdoor recreation on weekdays, while also experiencing multiple location-related obstacles, indicates that immediate surroundings are of significance to daily routines. On the other hand, immediate surroundings are less significant in situations where people have more time to travel longer distances, for example on weekends or during longer periods of time off work. Many feel that the absence of suitable spaces for outdoor recreation is a particular obstacle preventing them from getting out into nature to the extent they would like. This obstacle is particularly pronounced in Stockholm County. Hardest for people living in metropolitan areas to get out into nature Outdoor recreation initiatives by municipalities only seem to have a weak, or no, connection to the extent of outdoor recreation among their residents. Other factors seem to have a far greater impact on the activities of municipality residents. However, we identified a number of differences based on type of municipality according to the rough categorisation, with groups characterised as metropolitan cities, larger cities and smaller towns. As shown in the analysis of activities in different counties, there are also differences in daily outdoor recreational activity, with metropolitan cities having the lowest percentage of people reporting being engaged in outdoor activities. However, the data shows that, even on weekends, the residents of smaller towns and rural areas are more active than those living in metropolitan or larger cities. One of the main reasons for metropolitan residents not being outdoors to the extent they would wish is the absence of suitable spaces for outdoor recreation. Other differences include that a larger percentage of people outside the most densely populated municipalities experience the positive value of nature, with other positive experiences breaking down as more similar among the various types of municipalities. Knowledge on the legal right of access to private land (allemansrätten) in Sweden also differs among the various types of municipalities, with gaps in knowledge among a larger percentage of metropolitan residents, although the level of awareness of the law is good among these as well.

Negativ miljöpåverkan av plast är ett omfattande problem. Som ett led i Sveriges arbete för en mer hållbar plastanvändning har regeringen genom ramanslaget gett Naturvårdsverket i uppdrag att använda upp till 78 miljoner kronor årligen mellan 2018 och 2020 för att minska plaster i hav och natur. Inför Naturvårdsverkets fortsatta arbete mot en mer hållbar plastanvändning önskade myndigheten att satsningens resultat och effekter skulle belysas ytterligare. Regeringen har dock inte angett några särskilda insatser eller specificerade effekter som satsningen skulle uppnå, utan Naturvårdsverket har haft ett relativt fritt formulerat uppdrag utifrån givna ekonomiska ramar. Syftet med utvärderingen är därför att specifikt sammanfatta erfarenheter av plastsatsningens genomförande som grund för Naturvårdsverkets fortsatta arbete mot en mer hållbar plasthantering. Utöver det vill vi mer allmänt öka kunskapen om hur resultaten av att ta emot medel med relativt lite styrning kan se ut. Det har vi gjort genom att analysera kopplingen mellan projektens utfall eller resultat och de styrdokument som finns att tillgå. Vi har även intervjuat de medarbetare som arbetat med satsningen för att fånga upp erfarenheter och vad de identifierar som effekter. En mängd prestationer har uppnåtts, men det finns brister Naturvårdsverket har bidragit till att en mängd aktiviteter och projekt har genomförts som knyter an till de utmaningar som i olika styrdokument pekas ut som de mest centrala. Att många projekt startats först i samband med att aktören har fått finansiering från Naturvårdsverket kan indikera att projektet ifråga annars inte hade genomförts. Naturvårdsverket har i stora delar uppnått resultat som ligger i linje med de resultatmål som myndigheten formulerat för 2021 i sin interna prioriteringsordning för en hållbar plastanvändning. Dessa resultatmål fångar i sin tur upp de områden som lyfts fram i den nationella strategin för en cirkulär ekonomi.1 De två resultatmål som ännu inte uppnåtts sorterar under rubriken ”Plast i naturen” och handlar båda om implementering av åtgärder som till stor del ligger utanför Naturvårdsverkets mandat och möjlighet att påverka då de kräver företags respektive hela samhällets medverkan. Sett i ljuset av uppdraget att agera nationell plastsamordnare bedömer vi att Naturvårdsverket har goda förutsättningar för att nå dessa mål i samspel med de aktörer som involveras i detta arbete och det arbete som samordningen kan vara en katalysator för. Den interna prioriteringsordningen verkar dock inte kontinuerligt ha använts som ett verktyg för att välja och skapa projekt, utan målen tycks ha applicerats till viss del i efterhand på redan uppnådda resultat. Det har inte funnits förutsättningar för att initiera större projekt av mer strategisk karaktär. En planering utifrån tematiska portföljer har inte etablerats förrän emot slutet av de tre undersökta åren av satsningen till följd av att det inte verkar ha funnits någon tydlig verksamhetslogik. I stället för på effekter har fokus legat på aktiviteter. Flera intermediära effekter i form av en ändamålsenlig organisation och grundläggande övergripande kunskap har uppnåtts. De flesta effekter har antagligen ännu inte hunnit inträda då det har gått för kort tid eller då det enbart tagits fram förslag på åtgärder utan konkreta steg mot implementering och genomförande. Vidare finns det utrymme att systematisera omhändertagandet av prestationerna i större utsträckning än vad som görs idag. Hur väl Naturvårdsverket lyckas med sitt uppdrag som nationell plastsamordnare är här antagligen avgörande för det framtida arbetet. Otydlighet och kortsiktighet har fördröjt kraftfulla prioriteringar Satsningen är till stora delar otydligt avgränsad och formulerad. Därför har Naturvårdsverket behövt lägga relativt mycket tid på att hitta en lämplig inriktning samtidigt som medel skulle användas under respektive budgetår. Naturvårdsverket har under den utvärderade perioden inte satt agendan för arbetet mot en mer hållbar plastanvändning i Sverige utan har många gånger agerat på ett ad hoc-artat sätt som kan beskrivas som snarare reaktiv än proaktiv. Myndighetens agerande karakteriseras dock av att vara pragmatiskt och lösningsfokuserat där medarbetarna inom givna ramar försökt hitta kostnadseffektiva lösningar i mindre skala i stället för omfattande projekt med större osäkerhet i resultaten. Vidare är Naturvårdsverkets position och angreppssätt avsevärt tydligare i den färdplan som publicerats i maj 2021. Lärdomar om omfattande satsningar Det tar tid och resurser att introducera ett ämnesområde och tvärsektoriella arbetsrutiner i en stor organisation som Naturvårdsverket. Användningen av aktörer från existerande ramavtal kan ses som ett effektivt sätt att relativt snabbt få önskat resultat med hög kvalitet. Likväl finns en risk med att använda sig av på förhand givna leverantörer i de fall då en alternativ utförare hade kunnat göra ett bättre arbete. Vidare är kommunikation och kunskapsdelning internt och framför allt mot externa aktörer är en nyckelfaktor för att effekter ska kunna uppnås. Strategisk samverkan med externa aktörer tycks inte ha skett förrän under 2020 efter att Naturvårdsverket hade fått ett explicit samverkansuppdrag av regeringen. Utöver ett aktivt och kontinuerligt kommunikationsarbete menar vi att den strategiska samverkan är central för att kunna öka räckvidden och att lägga grunden för att olika samhällsaktörer ska vilja implementera och tillämpa de förändringar som till exempel olika sorters styrmedel syftar till. The environmental impact of plastics is a major problem. As part of Sweden’s efforts to achieve a more sustainable use of plastics, the Government tasked the Swedish Environmental Protection Agency (EPA) to spend up to SEK 78 million annually between 2018 and 2020 to reduce plastics in the sea and in nature. This funding has been included in the agency’s allocated framework funding. To prepare for the Swedish EPA’s continued efforts to achieve a more sustainable use of plastics, the agency wanted to analyse the results and outcomes of the initiative in more detail. However, the Government did not specify any special measures or outcomes for the initiative. Instead, the Swedish EPA had relatively free hands to formulate the assignment based on the defined financial framework. As such, the purpose of the evaluation is to specifically summarise experiences from implementing the plastics initiative to help in the agency’s continued efforts to achieve a more sustainable use of plastics. More generally, we also want to gain a better understanding of what happens when funding is provided with relatively limited guidance. We have done this by analysing the connection between the project’s outcome or results and the available policy documents. We have also interviewed staff who worked with the initiative so we could include their experiences and what they identify as outcomes. Much progress has been made but there are areas in need of improvement The Swedish EPA has contributed to the organisation of many activities and projects related to the most central challenges as defined by various policy documents. That many projects were only begun because of funding from the Swedish EPA may indicate that these projects would otherwise not have taken place. The Swedish EPA’s achieved outcomes are largely in line with the agency’s expected outcomes formulated for 2021 in its internal priority for the sustainable use of plastics. These expected outcomes, in turn, encompass the areas highlighted in the national strategy for a circular economy.2 The two expected outcomes not yet achieved fall under the category “Plastics in nature” and both deal with implementing measures that are largely outside the Swedish EPA’s mandate and ability to influence as they require the participation of companies and society at large. Viewed in light of the assignment to serve as the national plastics coordinator, we believe that it is well within the Swedish EPA’s ability to achieve these goals by working with relevant stakeholders and through the efforts that our coordinating role can promote. However, the Swedish EPA’s internal priority has not always been utilised when selecting and creating projects and the goals seem to have been applied to some extent after the fact on outcomes that had already been achieved. It has not been possible to initiate larger projects of a more strategic nature. No thematic planning was done until near the end of the initiative’s three years since there does not appear to have been any clear operational logic. Instead of outcomes, the focus was on activities. Several intermediate outcomes in the form of an appropriate organisation and basic overall knowledge have been achieved. Most of the outcomes have probably not yet come to fruition since the period was too short or only proposals for measures had been developed without concrete steps to implement these. There is also room for systematising how outcomes are utilised more than is currently the case. How well the Swedish EPA succeeds with its assignment as national plastics coordinator is likely decisive for future efforts. Forceful prioritisation has been delayed by vagueness and short-term thinking The initiative is largely vague in delimitation and formulation. This has resulted in the agency spending a relatively large amount of time finding a suitable focus all the while that funds were supposed to be used during each budget year. During the evaluated period, the Swedish EPA has not established an agenda for the efforts to achieve a more sustainable use of plastics in Sweden. Instead, it has often acted in an ad hoc manner that can be described as reactive rather than proactive. However, the agency’s actions are characterised by pragmaticism and a focus on finding solutions, where staff have tried to work within the framework to find cost-effective, smaller scale solutions instead of extensive projects with greater uncertainty about what will be achieved. Furthermore, the Swedish EPA’s position and approach are considerably clearer in the May 2021 roadmap. Lessons about larger initiatives It takes time and resources to introduce a subject area and cross-sectoral work routines in a large organisation like the Swedish EPA. The use of suppliers from existing framework agreements can be seen as an effective way of achieving the desired outcome relatively quickly while maintaining high quality. Nevertheless, there is a risk in using pre-chosen suppliers in cases where an alternative contractor could have done a better job. Furthermore, communication and knowledge sharing occur internally and, above all for external parties, are a key factor for achieving the expected outcomes. Strategic collaboration with external parties does not seem to have occurred before 2020 when the Swedish EPA was explicitly tasked by the Government to collaborate. In addition to active and continual communication efforts, we also believe that strategic collaboration is central to increasing impact and to laying the foundation for different societal stakeholders wanting to implement and apply the changes intended by various incentives.

Background The current COVID‐19 pandemic comes with multiple psychological stressors due to health‐related, social, economic, and individual consequences and may cause psychological distress. The aim of this study was to screen the population in Germany for negative impact on mental health in the current COVID‐19 pandemic and to analyze possible risk and protective factors. Methods A total of 6,509 people took part in an online survey in Germany from 27 March to 6 April. The questionnaire included demographic information and ascertained psychological distress, anxiety and depressive symptoms, and risk and protective factors. Results In our sample, over 50% expressed suffering from anxiety and psychological distress regarding the COVID‐19 pandemic. Participants spent several hours per day thinking about COVID‐19 (M = 4.45). Psychological and social determinants showed stronger associations with anxiety regarding COVID‐19 than experiences with the disease. Conclusions The current COVID‐19 pandemic does cause psychological distress, anxiety, and depression for large proportions of the general population. Strategies such as maintaining a healthy lifestyle and social contacts, acceptance of anxiety and negative emotions, fostering self‐efficacy, and information on where to get medical treatment if needed, seem of help, while substance abuse and suppression of anxiety and negative emotions seem to be associated with more psychological burden., The aim of this study was to screen the population in Germany for negative impact on mental health in the current COVID‐19 pandemic and to analyze possible risk and protective factors. Objectives Increasing the number of daily steps by using a pedometer and a diary leads to an activity increase and improved health outcomes in a variety of somatic disorders. Hence, for the inpatient treatment of depression, supervised exercise interventions are more widespread. We aim to examine if a self‐managed pedometer intervention (PI) with the option of being proceeded after discharge leads to reduction of depression and to a physical activity (PA) increase. Methods The Step Away from Depression (SAD) study is a multicenter randomized controlled trial targeting 400 patients with major depressive disorder. Treatment as usual (TAU) is compared to TAU plus PI after 4 weeks, at discharge, and 6 months after hospital admission. Primary outcomes are clinically rated depression severity and accelerometer‐measured step counts. Secondary outcomes include self‐reported depression symptoms and PA level, psychiatric symptoms, health‐related quality of life, self‐efficacy, and components of the Motivation Volition Process Model. Results We report the design of the SAD study considering several methodological aspects for exercise studies, in general. Conclusions Results of our study will provide information about efficacy of PI for inpatient treatment and about interrelating processes of change concerning depression, PA, and aspects of motivation and volition.
