Background: Type 2 diabetes (DM2) has been related to development of macroangiopatic [coronary heart disease (CHD), peripheral vascular disease (PVD) and stroke] and microangiopatic complications [retinopathy, nefropathy, and distal sensory neuropathy (DSN)]. The aims of this study were to analyze the prevalence of complications in DM2 patients and to estimate their associated risk factors. Methods: Cross-sectional study, including 927 patients with DM2 (42% males, meanage: 59 ± 10 years; mean known duration of DM2: 12 ± 8 years). Retinopathy was identified by direct fundoscopy; CHD by WHO questionnaire and/or abnormal ECG and/or perfusion abnormalities on myocardial scintigraphy; DSN by compatible symptoms and absent sensation on 10 g monofilament and/or tune fork; PVD by the presence of claudication and absent foot pulses, stroke by presence of sequels and history, and nephropathy by urinary albumin excretion rate (³ 20 mg/min). Hypertension was defined by blood pressure (³ 140/90 mmhg) and/or use of antihypertensive drugs. Body mass index (BMI, kg/m2 ) and waist-to-hip ratio (WHR) were calculated. Results: CHD was present in 36% and PVD in 33%. Among microvascular, 37% had nefropathy (12% with macroalbuminuria); 46% retinopathy (15% proliferative retinopathy). DSN was present in 36%. Seventy three percent of the patients presented arterial hypertension. Cholesterol levels were >200 mg/dl in 64% and BMI > 30 kg/m2 in 36%. Twenty two percent of patients were smokers and 21% ex-smokers. Conclusion: Diabetic complications are frequent among out patients attending to general hospitals. Almost all patients presented at least one risk factor for cardiovascular disease, what valid the efforts to their identification and adequate control.&nbsp, Objetivos: O diabetes melito (DM) do tipo 2 está associado ao desenvolvimento de complicações macroangiopáticas [cardiopatia isquêmica (CI), doença vascular periférica (DVP) e acidente vascular cerebral (AVC)] e microangiopáticas [retinopatia diabética (RD), nefropatia diabética (ND) e neuropatia sensitiva distal (NSD)]. Os objetivos deste estudo foram avaliar a prevalência das complicações crônicas em pacientes com DM do tipo 2 e aferir os seus possíveis fatores de risco. Métodos: Estudo transversal, incluindo 927 pacientes com DM do tipo 2 (42% homens, média de idade: 59 ± 10 anos; duração média do DM do tipo 2: 12 ± 8 anos). A RD foi definida por fundoscopia direta; a CI através do questionário da OMS e/ou alterações eletrocardiográficas e/ou anormalidades perfusionais na cintilografia miocárdica; a NSD por sintomas compatíveis e ausência de sensação ao monofilamento de 10g e/ou ao diapasão; a DVP pela claudicação e ausência de pulsos pediosos; o AVC por seqüelas ou história compatível e a ND pela excreção urinária de albumina (≥ 20 µg/min). A hipertensão arterial sistêmica (HAS) foi definidapelos níveis pressóricos (≥ 140/90 mmHg) e/ou uso de drogas anti-hipertensivas. Foram calculados o índice de massa corporal (IMC, kg/m2 ) e a razão cintura-quadril (RCQ). Resultados: A CI estava presente em 36% e a DVP em 33% dos pacientes. Dentre as complicações microvasculares, 37% tinham doença renal (12% macroalbuminúricos) e 46% RD (15% retinopatia proliferativa). A NSD foi encontrada em 36% dos pacientes. HAS estava presente em 73% dos pacientes. O colesterol estava acima de 200 mg/dl em 64%, enquanto o IMC > 30 kg/m2 em 36%. Vinte e dois por cento dos pacientes eram fumantes atuais e 21% ex-tabagistas. Conclusão: As complicações crônicas do DM do 2 têm uma alta prevalência nos pacientes ambulatoriais de hospitais gerais. Praticamente todos os pacientes apresentavam pelo menos um fator de risco para doença cardiovascular, o que justifica o seu rastreamento e controle. Hereditary MTC can occur either alone – familial MTC (FMTC) – or as the thyroid manifestation of multiple endocrine neoplasia type 2 (MEN 2) syndromes (MEN 2A and MEN 2B) or others. Three phenotypic subtypes have been reported. MEN 2A(1), MEN 2A(2) and MEN 2A(3). Germline mutations in the RET proto-oncogene cause MEN 2 and recent studies suggest a relationship between specific mutations and different phenotypes in MEN 2 syndromes. The purpose of this study was to identify RET proto-oncogene mutations and analyze a possible relationship between genotype-phenotype in Brazilian kindred with MTC. A total of 57 patients with histopathological and immunohistochemistry diagnosis of MTC were included. This sample was formed from index cases and affected members of 16 familieswith hereditary MTC and 10 individuals with sporadic tumors. DNA was extracted from leukocytes of the affected individuals and relatives. Exons 10, 11, 13, 14, 15 and 16 were amplificated by PCR, using specific primers. The presence of mutation was determined by SSCP, enzymatic restriction analysis and/or automatic sequencing. The phenotypes of hereditary MTC were as follows: 7 MEN 2A, 3 MEN 2A associated with CLA, 3 MEN 2B, 2 FMTC and 1 other forms. We identified mutations at codon 634 in 6 families with MEN 2A, only one kindred had the mutation at codon 618. The 3 kindred with MEN 2A+CLA, both cases of FMTC and the only family classified as other hereditary forms of the MTC presented the mutation in codon 634. A mutation at codon M918T was identified in the 3 individuals with MEN 2B. The genetic screening was able to identified 23 assymtomatic carriers and determine the hereditary MCT pattern in 3 individuals with apparently sporadic tumors. In conclusion, genetic testing can identify affected and assymtomatic individuals with hereditary disease, allowing early diagnosis and treatment.&nbsp, O carcinoma medular de tireóide (CMT) hereditário pode apresentar-se como componente das síndromes de Neoplasia Endócrina Múltipla (NEM 2A e 2B) ou Carcinoma Medular de Tireóide Familiar (CMTF). Diferentes mutações no RET foram identificadas como responsáveis pelo CMT e estudos recentes sugerem uma correlação entre o genótipo-fenótipo, podendo existir uma grande variabilidade de síndromes clínicas associadas às diferentes mutações. O presente estudo realizou a análise molecular do RET em indivíduos com CMT e avaliou a correlação entre fenótipo-genótipo nos afetados e seus familiares. Foram incluídos 57indivíduos com diagnóstico histopatológico/imunohistoquímico de CMT (10 esporádicos e 47 hereditários, provenientes de 16 famílias). O DNA genômico foi extraído de leucócitos periféricos e os exons 10, 11, 13, 14, 15 e/ou 16 do RET amplificados por PCR com primers específicos. A presença de mutações foi determinada por SSCP, restrição enzimática e/ou sequenciamento. Das famílias com CMT hereditário, 7 apresentavam NEM 2A, 3 NEM 2A associada à Líquen Amilóide Cutânea (CLA), 3 NEM 2B, 2 CMTF e 1 outras formas hereditárias. Em 6 famílias com NEM 2A, nas 3 com NEM 2A+CLA e nas 2 com CMTF a mutação estavapresente códon 634. Enquanto que a outra família com NEM 2A apresentava a mutação no códon 618. Nos indivíduos com NEM 2B foi detectada uma mutação de novo no códon M918T. Na família classificada como outros, a mutação também localizava-se no códon 634. O diagnóstico molecular identificou mutações em todos indivíduos com doença hereditária, em 23 indivíduos carreadores sem evidência clínica da neoplasia e em 3 indivíduos com CMT aparentemente esporádico, destacando a importância do rastreamento genético como método diagnóstico.

Diet, exercise and weight reduction are the cornerstone of type 2 diabetes mellitus (DM) treatment, since most of these patients are obese. However, the majority of the individuals is not able to achieve and mantain a satisfactory glycemic control employing only these strategies, and the prescription of complementary pharmacological treatment is often necessary. The available oral agents for type 2 diabetes treatment are divided in 5 main classes: sulfonylureas, biguanides, thiazolidinediones, alfa-glucosidase inhibitors and glinides. Phase II studies are presently in course, evaluating the safety and efficacy of new drugs. Metformin belongs to biguanides’ class, and is the drug with the best profile, since not only it lowers glycemia, but it also reduces appetite and weight, and , most of all, it is the only drug that diminishes mortality rates. Data regarding indications, mechanisms of action and adverse effects of the above mentioned drugs are describe in the present review., Dieta, exercício e redução de peso são a base do tratamento do diabete melito (DM) tipo 2, uma vez que a maioria desses pacientes são obesos. No entanto, a maior parte dos indivíduos não consegue atingir e manter um controle glicêmico adequado apenas com estas medidas, sendo freqüentemente necessária a prescrição de tratamento farmacológico complementar. As drogas orais para o tratamento do DM tipo 2 são divididas em 5 classes principais: as sulfoniluréias, as biguanidas, as tiazolidinedionas, os inibidores da alfa-glicosidase e as glinidas. Estudos de fase II estão atualmente em curso, avaliando a eficácia e a segurançade novas drogas. A Metformina, representante das biguanidas, é a droga com o melhor perfil de ação, uma vez que além de controlar a glicemia, também diminui o apetite, causa diminuição do peso (ou evita seu aumento) e, sobretudo, é a única droga que promove redução de mortalidade. Dados principais sobre as indicações, mecanismos de ação e efeitos colaterais das drogas mencionadas estão descritos por categoria nesta revisão.

Maturity-onset diabetes of the young (MODY) é uma forma monogênica de diabetes, que se expressa a partir de herança autossômica dominante. Seis subtipos de MODY já foram descritos, baseados na identificação de seis genes distintos. Um deles é o regulador da glicoquinase, enzima chave na secreção de insulina, e os demais codificam fatores de transcrição expressos na célula β, onde possuem um papel crucial na diferenciação e metabolismo celular. O MODY é clinicamente caracterizado como um diabetes familiar, com pelo menos duas gerações consecutivas afetadas, sendo os indivíduos diagnosticados antes dos 25 anos de idade. Esse tipo de diabetes está associado com um defeito na secreção de insulina pelas células β do pâncreas. A sua identificação geralmente é feita através de dados clínicos e, em centros capacitados, é possível confirmá-lo com testagem genética. O reconhecimento do diagnóstico de MODY possibilita o emprego de tratamento mais específico e, futuramente, ao possibilitar o entendimento da homeostasia da glicose, poderá proporcionar a elucidação da patogênese do DM tipo 2. Maturity-onset diabetes of the young (MODY) is a monogenic form of diabetes with an autosomal dominant pattern of inheritance. Six MODY subtypes have been described, based on the identification of six distinct genes. One of them is the glucokinase regulator, the key enzyme in insulin secretion, the others are responsible for coding transcription factors expressed in beta cells, in which they play a crucial role in cellular differentiation and metabolism. At least two consecutive generations should be affected by diabetes, with symptoms onset before 25 years of age. This kind of diabetes is associated with a defect in pancreas beta cells that diminishes insulin secretion. Its detection is generally accomplished through clinical data, and, in specialized centers, it is possible to employ genetic testing. The identification of MODY allows a more specific treatment of the patient, and, in a near future, through the knowledge of glucose homeostasis, it will provide the basis for the understanding of type-2 diabetes pathogenesis.

SÍNDROME DE BECKWITH-WIEDEMANN

BACKGROUND In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. The aim of the study was to evaluate the effect of a structured group education program administered by a primary care nurse in patients with type 2 diabetes mellitus. The sample included 137 patients with type 2 diabetes mellitus, randomized into two groups: intervention (5-week educational course and reinforcements every 4 months for one year) and control (with no structured diabetes mellitus education) with an evaluation of metabolic control, weight, blood pressure, distress scores, and knowledge on diabetes. There were no differences between the two groups in HbA1c at 4, 8, or 12 months when compared to baseline values. An increase in HbA1c was observed in the control group after adjusting for baseline HbA1c and insulin dose (p = 0.044 between groups). Knowledge scores and diabetes-related distress improved after the intervention. A structured educational program administered to type 2 diabetes mellitus patients seen at a primary care unit improved the knowledge and distress associated with the disease. The results also suggest the prevention of an increase in HbA1c. Resumo: O objetivo deste estudo foi avaliar o efeito de um programa de educação estruturado em grupo, aplicado por enfermeira de cuidados primários a pacientes com diabetes mellitus tipo 2. A amostra contou com 137 pacientes com diabetes mellitus tipo 2, randomizados em dois grupos: intervenção, (curso educativo de cinco semanas e reforços a cada 4 meses, durante um ano) e grupo controle (sem educação estruturada) com avaliação do controle metabólico, peso, pressão arterial, escores de estresse e conhecimento relacionados ao diabetes. Não houve diferenças entre os grupos na HbA1c em 4, 8 ou 12 meses, quando comparado aos valores basais. Foi observado um incremento na HbA1c no grupo controle após ajustes para HbA1c no basal e dose de insulina (p = 0,044 entre grupos). Os escores de conhecimento e estresse relacionados ao diabetes melhoraram após a intervenção. Um programa educacional estruturado ministrado à pacientes com diabetes mellitus tipo 2 atendidos em atenção primaria melhorou o conhecimento e estresse associados à doença. Além disso, nossos resultados sugerem uma prevenção de elevação da HbA1c. La muestra incluyó a 137 pacientes con diabetes mellitus tipo 2, aleatorizados en dos grupos: intervención (curso de formación de 5 semanas, com refuerzos cada 4 meses durante un año) y el grupo de control (sin educación estructurada) para evaluar el control metabólico, peso, presión arterial, niveles de estrés y conocimientos relacionados con la diabetes. No hubo diferencias entre los grupos en HbA1c en 4, 8 ó 12 meses en comparación con los valores basales. Se observó un aumento de HbA1c en el grupo de control después del ajuste de HbA1c en la línea de base y a la dosis de la insulina (p = 0,044 entre grupos). Las puntuaciones de los conocimientos y del estrés relacionadas con la diabetes mejoraron después de la intervención. Un programa educativo estructurado ofrecido a los pacientes con diabetes mellitus tipo 2 tratados en la atención primaria mejoró el conocimiento y el estrés asociado con la enfermedad. Por otra parte, nuestros resultados sugieren una prevención elevada de la HbA1c.
