Abstract

Current follow-up policies for early breast cancer aim to detect loco-regional recurrences and manage treatment-related adverse effects. Their "one size fits all" approach does not take into account differences in subtypes at initial diagnosis, individual prognosis and treatments received. They are derived from clinical trials conducted when early detection means - other than mammography - and treatment options were limited. Herein, we address the arguments for re-evaluating current breast cancer follow-up strategies starting from recent advances in breast cancer local and systemic treatments and discussing individual risk of recurrence prediction models, time-adapted imaging and biomarker assessment for disease diagnostic anticipation. This change in perspective would transform breast cancer follow-up into an integrated, multidisciplinary team medical practice. Hence we discuss the important role of patient-centered approaches, but also of general practitioners and other health professionals, in the final promotion of personalized surveillance programs and patient education.

Abstract

Ventricular tachycardia (VT) in patients with structural heart disease is associated with reduced quality of life and poor prognosis. Therapeutic options include medication, anti-tachycardia pacing or shock by implantable cardiac devices and catheter-based ablation of heart arrhythmogenic substrates. STereotactic Arrhythmia Radioablation (STAR) tested in a phase I/II trial by Robinson and colleagues offers a novel approach. Data from a retrospective series of 5 high-risk patients with VT refractory to catheter ablation and medication, treated with STAR are reported from a single referral center. CTV was defined to encompass the arrhythmogenic substrate by a team of a radiation oncologists and treating electrophysiologists, based on clinical and electro-anatomical information derived from CT scan and catheter ablation maps. ITV was added to CTV to compensate for heart and respiratory movement. PTV was then defined by adding an isotropic margin of 2-3 mm to the ITV. Volumetric Modulated Arc Therapy (VMAT) plans were generated, optimized, and delivered using medical linear accelerator. The prescription dose to the PTV was 25 Gy in 1 fraction. Mean CTV, ITV and PTV volumes, were 141.1cc, 187.7cc, 298.1cc, respectively. Mean heart volume was 1740.6 cc. All 5 patients completed STAR procedure and treatment. There were no acute treatment-related adverse events. Clinical and treatment-efficacy data are summarized in Table. STAR significantly reduced or abrogated arrythmia at a median time of 24 weeks (range = 4-48) post-treatment. Patient n.1 and n.3 showed a remarkable reduction of VT episodes at 4- and 8-weeks post-treatment, respectively. Patients n.2, n.4 and n.5 were free of VT episodes at 6-months post-treatment. At a median follow-up time of 11 months (range = 1-19), 2/5 patients are alive (patient n.2 and n.4), both free of VT events at 1-year post-treatment. Patient n.1 died due to complication after cardiac transplantation, patients n.3 due to sepsis and multiorgan failure and patient n.5 due to COVID pneumonia, at 11-, 1- and 5-months post-STAR, respectively. These data suggest that LINAC-based STAR is a safe and effective treatment option in high-risk patients with VT refractory to catheter ablation and medication. Abstract

To investigate the present attitude of the Italian Radiation Oncologists in the management of breast cancer (BC) concerning hypofractionated radiotherapy (hRT), partial-breast irradiation (PBI), re-irradiation (rRT) and radiotherapy after neoadjuvant chemotherapy (post-NAC RT). A nationwide, 21-point questionnaire was distributed online via SurveyMonkey. Seventy-four Italian Radiotherapy Centers answered to the survey. In most cases, the responding centers treated more than 100 BC patients/year between January 2016 and December 2017. Almost half of responding centers (49%) treated patients with hRT, out of these, 95% as routine practice for early-stage BC. Dose prescriptions ranged between 39 and 45 Gy indicating a high use of moderate hRT. The chest wall and regional lymph nodes were irradiated with hRT by 13% and 15% of the responding centers, respectively. PBI was used by 60% of responders, with different techniques. Only 0.6% of participants perform rRT after BC recurrence. Finally, only 11% of the interviewed centers responded to their attitude toward post-NAC RT, which, however, was indicated in 97% of patients after breast-conserving surgery. This survey shows a fairly good use of hRT and a moderate practice of PBI in Italy. Some practices like hRT to the chest wall and regional lymph nodes as well as rRT need further verification. Likewise, the management of post-NAC RT is very heterogeneous. Future national clinical collaborative studies are advocated in order to investigate these controversial topics about breast cancer radiotherapy.

Abstract

To describe left anterior descending coronary artery (LAD) and heart dosimetry in patients with left-side breast cancer undergoing adjuvant hypo-fractionated whole breast irradiation with a concomitant boost with a dedicated prone position cardiac-sparing protocol. In a consecutive series of 524 women, LAD and heart doses were reviewed from their radiation treatment plans retrospectively. For all patients, the setup was in the prone position and the medial edge of the breast tangents was placed at least 2.5 mm from the contoured LAD. A descriptive variables statistical analysis was performed. The Spearman's coefficients and linear regression models were applied to explore correlations between LAD and heart doses with breast and boost volumes. For LAD dosimetry, the mean values for Dmean, Dmax, and D2% were 2.20 Gy, 4.44 Gy, 3.57 Gy, respectively. For heart dosimetry, the mean values for Dmean, Dmax and D2% were 0.69 Gy, 7.92 Gy, 2.35 Gy, respectively. The mean volumes for PTV-breast and PTV-boost were 699.6 cc and 110.8 cc, respectively. No correlations were found among the cardiac dosimetry indices and breast/boost volumes. A linear relationship was found between Heart D2% and LAD Dmean, LAD D2% and LAD Dmax. A constraint of maintaining the Heart D2% dose less than 3 Gy achieved a LAD Dmean, LAD D2% and LAD Dmax less than 2.77, 4.36 and 5.16 Gy, respectively. A dedicated prone position cardiac-sparing protocol for hypo-fractionated breast irradiation minimizes LAD and heart exposure, regardless of individual body conformation and treatment volumes. The approach results in dosimetry superior or equivalent to that of more complex and costly radiotherapy techniques. Keeping the medial edge of the breast tangents at least 2.5 mm from the contoured LAD while limiting Heart D2% dose to <3 Gy will best prevent long term radiation associated cardiotoxicity.
