Abstract

The emergence of new SARS-CoV-2 variants poses challenges to global surveillance efforts, necessitating swift actions in their detection, evaluation, and management. Among the most recent variants, Omicron BA.2.86 and its sub-lineages have gained attention due to their potential immune evasion properties. This study describes the development of a digital PCR assay for the rapid detection of BA.2.86 and its descendant lineages, in wastewater samples. By using this assay, we analyzed wastewater samples collected in Italy from September 2023 to January 2024. Our analysis revealed the presence of BA.2.86 lineages already in October 2023 with a minimal detection rate of 2% which then rapidly increased, becoming dominant by January 2024, accounting for a prevalence of 62%. The findings emphasize the significance of wastewater-based surveillance in tracking emerging variants and underscore the efficacy of targeted digital PCR assays for environmental monitoring.

Abstract

The SARS-CoV-2 Omicron variant emerged in South Africa in November 2021, and has later been identified worldwide, raising serious concerns. A real-time RT-PCR assay was designed for the rapid screening of the Omicron variant, targeting characteristic mutations of the spike gene. The assay was used to test 737 sewage samples collected throughout Italy (19/21 Regions) between 11 November and 25 December 2021, with the aim of assessing the spread of the Omicron variant in the country. Positive samples were also tested with a real-time RT-PCR developed by the European Commission, Joint Research Centre (JRC), and through nested RT-PCR followed by Sanger sequencing. Overall, 115 samples tested positive for Omicron SARS-CoV-2 variant. The first occurrence was detected on 7 December, in Veneto, North Italy. Later on, the variant spread extremely fast in three weeks, with prevalence of positive wastewater samples rising from 1.0% (1/104 samples) in the week 5–11 December, to 17.5% (25/143 samples) in the week 12–18, to 65.9% (89/135 samples) in the week 19–25, in line with the increase in cases of infection with the Omicron variant observed during December in Italy. Similarly, the number of Regions/Autonomous Provinces in which the variant was detected increased from one in the first week, to 11 in the second, and to 17 in the last one. The presence of the Omicron variant was confirmed by the JRC real-time RT-PCR in 79.1% (91/115) of the positive samples, and by Sanger sequencing in 66% (64/97) of PCR amplicons. In conclusion, we designed an RT-qPCR assay capable to detect the Omicron variant, which can be successfully used for the purpose of wastewater-based epidemiology. We also described the history of the introduction and diffusion of the Omicron variant in the Italian population and territory, confirming the effectiveness of sewage monitoring as a powerful surveillance tool.

Abstract

Introduction The COVID-19 pandemic is having a deep impact on emergency surgical services, with a significant reduction of patients admitted into emergency surgical units world widely. Reliable figures of this reduction have not been produced yet. Our international audit aimed at giving a precise snapshot of the absolute and relative changes of emergency surgical admissions at the outbreak of the pandemic. Materials and methods Datasets of patients admitted as general surgical emergencies into 45 internationally distributed emergency surgical units during the months of March and April 2020 (Covid-19 pandemic outbreak) were collected and compared with those of patients admitted into the same units during the months of March and April 2019 (pre-Covid-19). Primary endpoint was to evaluate the relative variation of the presentation symptoms and discharge diagnoses between the two study periods. Secondary endpoint was to identify the possible change of therapeutic strategy during the same two periods. Results Forty-five centres participated sent their anonymised data to the study hub, for a total of 6263 patients. Of these, 3810 were admitted in the pre-Covid period and 2453 in the Covid period, for a 35.6% absolute reduction. The most common presentation was abdominal pain, whose incidence did not change between the two periods, but in the Covid period patients presented less frequently with anal pain, hernias, anaemia and weight loss. ASA 1 and low frailty patients were admitted less frequently, while ASA>1 and frail patients showed a relative increase. The type of surgical access did not change significantly, but lap-to-open conversion rate halved between the two study periods. Discharge diagnoses of appendicitis and diverticulitis reduced significantly, while bowel ischaemia and perianal ailments had a significant relative increase. Conclusions Our audit demonstrates a significant overall reduction of emergency surgery admissions at the outbreak of the Covid-19 pandemic with a minimal change of the proportions of single presentations, diagnoses and treatments. These findings may open the door to new ways of managing surgical emergencies without engulfing the already busy hospitals.

Abstract

Introduction: Bowel dysfunction (BD) is reported as a common and disabling symptom in multiple sclerosis (MS) patients. To date, no studies have explored the prevalence of these symptoms in a large multicenter outpatient setting. The aims of the present study are to assess: (i) the prevalence of BD in a large multicenter Italian MS population, and (ii) the correlation between clinico-demographic variables and the severity of BD. Methods: Each of the nine participating center screened MS patients prospectively: 1100 subjects were enrolled. All patients underwent the Expanded Disability Status Scale (EDSS) and completed the Neurogenic Bowel Dysfunction score (NBDs). Multivariable linear and logistic regression models were used to assess the association between NBDs and several clinico-demographic variables. Results: Fourteen percent of MS patients showed a moderate/severe BD (NBDs > 10); this percentage increased in patients with high disability, ranging from 26 to 32%. Moderate/severe BD was more frequent in MS patients with: progressive phenotypes, higher disability, older age, and longer disease duration. NBDs severity was predicted by female sex, ambulation impairment and bladder symptoms. Conclusion: This study confirms the relatively high prevalence of moderate/severe BD in a large, multicenter, unselected, outpatient MS population. BD appears to be mainly associated to female sex and MS-related disability.

Abstract

Since humans first appeared on earth, one of the essential conditions for basic survival has been our close relationship with water. Not by chance, all of the ancient civilizations known to history developed along rivers: the Sumerians (5500-4000 BC) between the Tigris and Euphrates in Mesopotamia, the Egyptians (3100-500 BC) along the Nile, the Indus valley civilization (2600-1900 BC) along the Indus river and its tributaries, and the Romans (753 BC - 476 AD) along the banks of the Tiber. Rivers were a direct source of life by providing water and food, and they also served these societies indirectly by allowing agricultural land to be irrigated and livestock watered.

Abstract

The European project Daylighting Rivers, co-funded by the Erasmus+ Programme, aimed at raising students' awareness and interest toward STEM (Science, Technology, Engineering and Math) in secondary education, by making connections between the school curriculum and the local territory on issues connected to rivers - with special emphasis on urban rivers - in Italy, Greece, Spain and Turkey. In order to raise awareness of appropriate urban planning, which recognises the importance of open rivers, a series of scientific inquiry activities was developed by teams of scientists and teachers to investigate different aspects connected to water and river basins. Rivers located in the areas of the Daylighting Rivers partner-schools became the particular objects of investigation by the teams of students. The methodology used in the project is based on Pedaste's model of inquiry-based learning (IBL), a flexible five-step learning cycle that engages students in multidisciplinary scientific investigation - and employs information technologies for the knowledge and the promotion of the territory. This paper describes the learning methodology that has been applied by more than 200 students in the project countries.

Abstract

Historically, water resources have allowed the development of urban settlements and water forms and availability - such as surface water or underground water (springs, rivers and streams, aquifers, lakes), have influenced the urban environment either in connection to danger and risk connected to the proximity of the water body, either for the functional and aesthetic value. The educational project Daylighting Rivers -co-funded by the European Union in 2017 (Project number 2017-1-IT02- KA201-036968), takes its cue from such theme to develop a teaching methodology that aims to facilitate STEM learning and at the same time to raise teachers' and students' awareness on the importance and vulnerability of water bodies, especially in a urban context. The project was implemented in Italy, Spain and Greece, countries with similar environmental characteristics and urban sprawl processes that have been emphasizing water issues especially in time of climate change. Over the first two years, the project involved three pilot secondary schools that tested an innovative, multidisciplinary and participatory teaching methodology, based on a model of Inquiry Based Learning. From the pedagogical point of view, the methodology fosters the students' centrality and curiosity for investigating the local river in own town or province. Twenty learning units were developed on specific topics connected to macro-themes that can be implemented in different school disciplines. The promotion of innovative digital tools such as Location Based Games have also allowed students to approach and work with georeferenced information, but also combine technical-scientific aspects and language to historical-humanistic-artistic aspects and storytelling. Students could also reflect on a variety of aspects related to rivers in town such as social, ecological, cultural and economic well-being aspects.

Abstract

This review is aimed at demonstrating the progesterone-like activity exerted by the active form of vitamin D, or calcitriol (1,25(OH)2D). To achieve this outcome, we compared the effects in vivo and in vitro exerted by progesterone and vitamin D, with a special focus on the female reproductive system and pregnancy.This is a literature review of the most important articles published in English on vitamin D as a hormone, mainly found by MEDLINE. Furthermore, a section of our review contains some unpublished data, concerning the analysis in silico of the similarities between the steric structure of progesterone and calcitriol, based on the availability of the experimental structures of progesterone and vitamin D3 receptors in complex with their physiological ligands in the RCSB Protein Data Bank.Vitamin D was shown to exert many physiological activities during the very early stages of gestation in perfect synchrony with progesterone. Both the molecules mutually help and reinforce the activity exerted by each one. A little bit later than progesterone is released, vitamin D secretion rises, but only if pregnancy occurs. Calcitriol contributes to prepare the endometrium to be receptive. Moreover, it supports the implantation process and the course of pregnancy through different but similar pathways to those used by progesterone, giving rise to a significant synergy of action. It is increasingly evident that vitamin D gives an essential support from the luteal phase onwards.Based on the evidence displayed in this review we may define appropriately vitamin D as a steroid hormone with progesterone-like activity.

35. Intestinal Yeast flora of honey bee in presence/absence of Nosema spp.: preliminary results

Abstract

In the last years, several research are aimed at the study of the intestinal microbiota of honey bee and their role in bee health. The literature reports that the flora of healthy bees is dominated by bacteria while only 1% is represented by yeasts and other fungi. Some authors describe that various diseases or stress increase the number of yeasts. Borsuk et al (2013 Med. Weter. 2013, 69, 726-729) studying the yeast microflora in bees experimentally infected by Nosema spp. noted an increase of yeasts in presence of a weak degree of Nosema infection, while heavy infections reduced the number of yeasts. In the present work, we describe preliminary result about the research of yeasts in bee guts in presence/absence of natural infection by Nosema spp. A total of 200 bees were collected from June to October 2015 in apiaries of Bologna province. The bees were euthanasized by placing their 4-5 minutes in freezer, then each single bee was dipped in ethyl alcohol, the gut was aseptically removed and put in 500 microliter of sterile saline in eppendorf and ground with pestle. One hundred microliter of the suspension were used for counting the number of Nosema spp spores per gut in a Burker chamber; other 100 microliters were spread onto the surface of Petri plates containing Sabouraud Dextrose Agar with Chloramphenicol and incubated at 30°C for 7 days to count the number of CFU (Colony Forming Unit)/gut. Nosema spp was found in 25/200 (12.5%) bee, with number of spore/gut ranging from 5000 to 34 million. Yeasts were found in 55/200 (27.5%) bee, with an approximate number of CFUs/gut ranging from 5 to 14000. The yeast CFU was significatively correlated to the number of Nosema spores (Spearman's rho p = 0.016). The species composition of yeast will be further elucidated.

Abstract

Purpose: Early and Intensive Behavioral Treatments are considered to be evidence-based interventions for children with Autism Spectrum Disorder (ASD). Nevertheless, children with ASD might not always have the opportunity to benefit from intensive treatment; new, more accessible and alternative treatment options need to be tested. The aim of this study was to evaluate the effectiveness of the Combined Low-intensive Psychoeducational Intervention (CLI-PEI) delivered to preschoolers with ASD at the end of the pre-primary school day. Methods: A quasi-experimental design study, namely a pretest–posttest alternative-treatment comparison groups design, was used. Treatment sessions were carried out over a period of 12 months. Forty-three individuals with autism were included in the study: 24 received the CLI-PEI and 19 were administered the Treatment As Usual. A pre- and posttreatment assessment was carried out using the Psychoeducational Profile-Third edition and the Vineland Adaptive Behavior Scale. Results: The children who received the CLI-PEI showed better gains in both developmental and maladaptive behaviors; furthermore, increased skills were found in all adaptive domains. Conclusions: The CLI-PEI might seems to be a viable treatment option for children with ASD, when intensive behavioral treatments are not accessible. Children with ASD might not always have the opportunity to benefit from intensive treatment. The identification of more accessible, less intensive and less expensive evidence-based psychoeducational interventions might represent an appealing challenge for rehabilitation therapists. Less intensive and less expensive evidence-based interventions might also represent a viable option for children and their families, especially in communities with limited resources for autism. A pragmatic approach including components from evidence-based treatments might guarantee flexibility and the possibility to implement an intervention well-tailored to the specific child needs. CLI-PEI for preschoolers with ASD seems to be a promising pragmatic approach, promoting improvements in developmental, adaptive and maladaptive domains. [ABSTRACT FROM AUTHOR]

Abstract

A 1-year study involving 157 gastroenteritis samples was conducted to investigate the role of human astrovirus, (HAstV) as a cause of gastroenteritis in Italian children aged < 2 years. The overall incidence of HAstV was 3.1%. Most cases occurred between March and May, and four of the five isolates were of the HAstV-1 type, the other being HAstV-3. Analysis of genetic variability showed that the three HAstV-1 isolates collected in 2000 clustered together, but separately from the 1999 isolate. The results indicated that HAstV should be considered as a potential diarrhoeal pathogen in Italian children. © 2004 Copyright by the European Society of Clinical Microbiology and Infectious Diseases

Abstract

The surveillance network RotaNet-Italia was established in 2007 in order to investigate the diversity of co-circulating rotavirus strains in Italy, and to provide a baseline for future assessment of possible effects of vaccine implementation in selecting novel versus common rotavirus strains. A total of 2,645 rotavirus strains from pediatric patients with acute diarrhea were collected over three consecutive seasons from September 2006 through August 2009, and partially characterized by standardized multiplex RT-PCR. Most of strains (89.1%) belonged to genotypes G1–G4, and G9, associated with either P[8] or P[4], commonly found in humans worldwide. However, in at least 2.0% of cases, viruses exhibited either a G or P type typical of animal viral strains, suggesting gene reassortment events between rotaviruses of different origin. Mixed infections with two or more rotavirus strains were observed frequently (7.6% of patients), and depended on the frequencies of co-circulating rotaviruses of one particular genotype. The numbers and genotypes of likely natural reassortants of common genotype rotaviruses were found to be correlated with the observed numbers and genotypes of mixed infections. Large variation in the relative frequency of different rotavirus genotypes was observed between different seasons and/or areas of Italy, suggesting independent evolution or differential introduction of viral strains with respect to both time and space. Abstract

Objectives: Among human noroviruses (NoVs), a few genogroup II strains of genotype 4 (GII.4) are dominant worldwide. GII.4 NoVs evolve rapidly and in 2006 two new epidemic variants have been identified. To investigate the circulation of GII.4 NoV variants in Italy a sequence analysis was performed on NoV strains obtained from children hospitalized for sporadic viral gastroenteritis in Palermo. Methods: A total of 465 faecal specimens were collected from children (< 5 years) hospitalized from January 2005 to December 2006. The presence of NoVs was detected by RT-PCR using primers JV12/JV13, targeting the region A of the RNA-dependent RNA-polymerase (RdRp) gene. NoV strains were genotyped by RdRp restriction fragment length polymorphism (RFLP) with XmnI, BstXI and sequence analysis of region A and of the ORF1/ORF2 junction region obtained with primer pair GIISKR/GIIFBN1-2-3. Phylogenetic analysis including isolates from the 2002-2004 surveillance was carried out using the software MEGA version 4. Results: Viral gastroenteritis surveillance resulted in the detection of NoV strains in 20.9% of the patients admitted to hospital. RFLP and sequence analysis of the RdRp gene allowed to successfully characterise 59 NoV strains. Eighty-one % of the strains were characterized as GII.4, 14% as GIIb/Hilversum and 5% as GI.1. Phylogenetic analysis of region A and of the ORF1/ORF2 overlapping region of the GII.4 strains recovered in Palermo in the years 2002-2006 revealed the sequential emergence of four variants, GII.4 2002, 2004, 2006a and 2006b. The variant GII.4 2006a was detected in June and July, 2006, while the variant 2006b first appeared in August, 2006, becoming predominant thereafter. Conclusion: The high detection rate of GII.4 NoVs in Italian children with gastroenteritis confirms their prominent role as human enteropathogens. At least four distinct GII.4 NoV variants appeared in Palermo in the last years and their dynamics of replacement and circulation in 2005-2006 appear to have matched the temporal pattern observed in Europe during the same period.

Abstract

A rotavirus sample collection from 19 consecutive years was used to investigate the heterogeneity and the dynamics of evolution of G1 rotavirus strains in a geographically defined population. Phylogenetic analysis of the VP7 gene sequences of G1P[8] human rotavirus strains showed the circulation of a heterogeneous population comprising three lineages and seven sublineages. Increases in the circulation of G1 rotaviruses were apparently associated with the introduction of novel G1 strains that exhibited multiple amino acid changes in antigenic regions involved in rotavirus neutralization compared to the strains circulating in the previous years. The emergence and/or introduction of G1 antigenic variants might be responsible for the continuous circulation of G1 rotaviruses in the local population, with the various lineages and sublineages appearing, disappearing, or cocirculating in an alternate fashion under the influence of immune-pressure mechanisms. Sequence analysis of VP4-encoding genes of the G1 strains revealed that the older strains were associated with a unique VP4 lineage, while a novel VP4 lineage emerged after 1995. The introduction of human rotavirus vaccines might alter the forces and balances that drive rotavirus evolution and determine the spread of novel strains that are antigenically different from those included in the vaccine formulations. The continuous emergence of VP7-VP4 gene combinations in human rotavirus strains should be taken into consideration when devising vaccination strategies.
