2. Effectiveness of a Web-Based Provider Communications Platform in Reducing Hospital Readmissions Among Patients Receiving Dialysis: A Pilot Pre-Post Study.

RATIONALE & OBJECTIVE: Suboptimal care coordination between dialysis facilities and hospitals is an important driver of 30-day hospital readmissions among patients receiving dialysis. We examined whether the introduction of web-based communications platform (DialysisConnect) was associated with reduced hospital readmissions. STUDY DESIGN: Pilot pre-post study. SETTING & PARTICIPANTS: A total of 4,994 index admissions at a single hospital (representing 2,419 patients receiving dialysis) during the study period (January 1, 2019-May 31, 2021). INTERVENTION: DialysisConnect was available to providers at the hospital and 4 affiliated dialysis facilities (=intervention facilities) during the pilot period (November 1, 2020-May 31, 2021). OUTCOMES: The primary outcome was 30-day readmission; secondary outcomes included 30-day emergency department visits and observation stays. Interrupted time series and linear models with generalized estimating equations were used to assess pilot versus prepilot differences in outcomes; difference-in-difference analyses were performed to compare these differences between intervention versus control facilities. Sensitivity analyses included a third, prepilot/COVID-19 period (March 1, 2020-October 31, 2020). RESULTS: There was no statistically significant difference in the monthly trends in the 30-day readmissions pilot versus prepilot periods (-0.60 vs -0.13, P = 0.85) for intervention facility admissions; the difference-in-difference estimate was also not statistically significant (0.54 percentage points, P = 0.83). Similar analyses including the prepilot/COVID-19 period showed that, despite a substantial drop in admissions at the start of the pandemic, there were no statistically significant differences across the 3 periods. The age-, sex-, race-, and comorbid condition-adjusted, absolute pilot versus prepilot difference in readmissions rate was 1.8% (-3.7% to 7.3%); similar results were found for other outcomes. LIMITATIONS: Potential loss to follow-up and pandemic effects. CONCLUSIONS: In this pilot, the introduction of DialysisConnect was not associated with reduced hospital readmissions. Tailored care coordination solutions should be further explored in future, multisite studies to improve the communications gap between dialysis facilities and hospitals.

3. A Web-Based, Provider-Driven Mobile App to Enhance Patient Care Coordination Between Dialysis Facilities and Hospitals: Development and Pilot Implementation Study.

BACKGROUND: We piloted a web-based, provider-driven mobile app (DialysisConnect) to fill the communication and care coordination gap between hospitals and dialysis facilities. OBJECTIVE: This study aimed to describe the development and pilot implementation of DialysisConnect. METHODS: DialysisConnect was developed iteratively with focus group and user testing feedback and was made available to 120 potential users at 1 hospital (hospitalists, advanced practice providers [APPs], and care coordinators) and 4 affiliated dialysis facilities (nephrologists, APPs, nurses and nurse managers, social workers, and administrative personnel) before the start of the pilot (November 1, 2020, to May 31, 2021). Midpilot and end-of-pilot web-based surveys of potential users were also conducted. Descriptive statistics were used to describe system use patterns, ratings of multiple satisfaction items (1=not at all; 3=to a great extent), and provider-selected motivators of and barriers to using DialysisConnect. RESULTS: The pilot version of DialysisConnect included clinical information that was automatically uploaded from dialysis facilities, forms for entering critical admission and discharge information, and a direct communication channel. Although physicians comprised most of the potential users of DialysisConnect, APPs and dialysis nurses were the most active users. Activities were unevenly distributed; for example, 1 hospital-based APP recorded most of the admissions (280/309, 90.6%) among patients treated at the pilot dialysis facilities. End-of-pilot ratings of DialysisConnect were generally higher for users versus nonusers (eg, I can see the potential value of DialysisConnect for my work with dialysis patients: mean 2.8, SD 0.4, vs mean 2.3, SD 0.6; P=.02). Providers most commonly selected reduced time and energy spent gathering information as a motivator (11/26, 42%) and a lack of time to use the system as a barrier (8/26, 31%) at the end of the pilot. CONCLUSIONS: This pilot study found that APPs and nurses were most likely to engage with the system. Survey participants generally viewed the system favorably while identifying substantial barriers to its use. These results inform how best to motivate providers to use this system and similar systems and inform future pragmatic research in care coordination among this and other populations.

4. The role of informed consent in the veterinary clinic : a case study in companion animal neutering

In the UK, professional ethical guidance from the Royal College of Veterinary Surgeons requires that informed consent is obtained before treatment is given to animal patients. This consent should protect the patient from inappropriate treatment, the client from unexpected costs and the veterinary professional from complaints by evidencing the client's agreement to proceed. In this thesis, I utilise a socio-legal approach to conceptualise consent in veterinary practice. Using elective neutering of companion animal patients as a case study, I analyse relevant jurisprudence on informed consent in medicine to illustrate an ideal rooted in the autonomous human patient's right to choose, or refuse, treatment. Acknowledging the animal patient's lack of autonomy, I explore parallels with decision-making for young children, which usually incorporate a 'best interests' calculation. Tensions between autonomy and beneficence-based consent are explored via three linked empirical studies, involving analysis of veterinary consent forms, observation of consent consultations and interviews with key participants. Resulting data are interpreted in light of doctrinal research, demonstrating the value of interpretive description as a methodology for socio-legal studies. Finally, I propose a new model of consent for veterinary practice that recognises the appropriate balance between autonomy and beneficence, together with a re-designed consent form.

Using a popular model of shared decision making-choice talk, options talk, and decision talk-can help ensure that clients know they have choices, understand the risks and benefits of each choice, and can participate as much as they would like in deciding the treatment for their pet. Shared decision making helps ensure that clients are informed and satisfied with their treatment choice and that veterinarians are comfortable knowing they have received client consent. 14. Lay and professional constructions of childhood ADHD (Attention Deficit Hyperactivity Disorder) : a discourse analysis

Childhood ADHD is a contested yet rising public health phenomenon, due to greateruse of inclusive American diagnostic classification. In the UK ADHD is considered to be ‘incompletely medicalised’ with rising incidences predicted. A critical approach was adopted in this thesis, based on a number of social constructionist assumptions in order to examine the emergence and increased use of the construct and to contribute to broader critical debate in the field. Parents and teachers are key adults in childhood ADHD as they may identify and care for diagnosed children yet they have been relatively neglected in the literature. How such adults account for children’s difficulties was the focus of an empirical analysis. A ‘critical discursive psychology’ approach was adopted using Edley’s (2001) framework in order to examine culturally available talk by parents and teachers about ADHD, from semistructured interviews in Scotland. Analysis highlighted how parents deployed contradictory interpretive repertoires in talk using a Biological repertoire as a genetic explanation and an Environmental repertoire in relation to various parenting issues. Such talk was organised to attend to the ideological dilemma of parental moral adequacy and accountability and which sought to accomplish the ‘good parent’. Further analysis considered how parents accounted for competing versions of the difficulties and their positioning in relation to controversial medication talk. Teacher accounts of children’s difficulties deployed an ADHD repertoire as a medical condition and a Not ADHD repertoire as due to temporal difficulties. Through the ‘cases I know’ device, teachers managed their own experiential knowledge and thereby negotiated agency and control for childhood behaviours. Analysis considered accounts of (mis)diagnosis and (mis)treatment as alternative explanations for ADHD. This innovative focus on how health policy for children’s difficulties as ADHD were socially produced by lay parent and teachers accounts, highlighted the limitations for agency in ADHD diagnoses and implicated further critical debate about this topic. Parental talk which drew on current biopsychosocial models for ADHD was largely reductionistic and fragmentary. The reliance on discursive efforts about the ‘good parent’ identity meant that this was a temporal accomplishment in talk rather than achieved by a diagnosis. Analysis of teacher accounts originating from a Scottish context highlighted how they differed from a North American context and provided greater understanding of how teachers succeeded in offering robust alternative explanations to ADHD. The implications for health and education policy of ADHD efforts aimed at the ‘education’ of teachers may be limited in the face of the teacher talk. Finally, within methodological debate in discourse analysis, this work contributes to further arguments for an eclectic discourse analysis as applied to the field of ADHD.

23. Bridging the Teacher/Researcher Divide: Master's-Level Work in Initial Teacher Education

- Abstract
There is much current debate about the purpose and usefulness of educational research and the perceived communication gap between teaching professionals and academic researchers. UK government intervention into initial teacher education has in recent decades contributed to this divide by favouring school-based training. The most common route into teaching in England remains, however, the Postgraduate Certificate in Education, provided by higher education institutions and therefore required to comply with the higher education qualifications framework. The majority of initial teacher education in England therefore lies at the cusp of these two worlds, pulled in apparently opposing directions. The "teacher-as-researcher" movement is widely seen as a bridge spanning these tensions, though there is discussion about the quality of practitioner research as well as about the appropriateness of a rigorous academic approach for investigating practice. This article offers examples of the use of small-scale research projects as a valid means of "discovery learning" in pre-service teacher education. It argues that induction into research techniques as a means of exploring practical challenges can lead to knowledge production and ownership.

- Abstract
Current government policy in the UK places teachers of all subject areas under pressure to embed ICT into their pedagogic practice to improve teaching and learning. This article attempts to compare that policy with the situated reality of a small number of Modern Foreign Language teachers striving to comply. The voices of four teachers are presented as they began to embed the use of ICT (primarily their newly acquired Interactive Whiteboards) into their existing classroom approach. The story of their developing practice is examined for signs of transformation; the complexity of a real situation is thus contrasted with a simplified ideal. By means of this investigation, examples of sophisticated pedagogical thinking are explored to demonstrate that it is perhaps not so easy to transform the learning process, with or without ICT.

- Abstract
BACKGROUND: We piloted a web-based, provider-driven mobile app (DialysisConnect) to fill the communication and care coordination gap between hospitals and dialysis facilities. OBJECTIVE: This study aimed to describe the development and pilot implementation of DialysisConnect. METHODS: DialysisConnect was developed iteratively with focus group and user testing feedback and was made available to 120 potential users at 1 hospital (hospitalists, advanced practice providers [APPs], and care coordinators) and 4 affiliated dialysis facilities (nephrologists, APPs, nurses and nurse managers, social workers, and administrative personnel) before the start of the pilot (November 1, 2020, to May 31, 2021). Midpilot and end-of-pilot web-based surveys of potential users were also conducted. Descriptive statistics were used to describe system use patterns, ratings of multiple satisfaction items (1=not at all; 3=to a great extent), and provider-selected motivators of and barriers to using DialysisConnect. RESULTS: The pilot version of DialysisConnect included clinical information that was automatically uploaded from dialysis facilities, forms for entering critical admission and discharge information, and a direct communication channel. Although physicians comprised most of the potential users of DialysisConnect, APPs and dialysis nurses were the most active users. Activities were unevenly distributed; for example, 1 hospital-based APP recorded most of the admissions (280/309, 90.6%) among patients treated at the pilot dialysis facilities. End-of-pilot ratings of DialysisConnect were generally higher for users versus nonusers (eg, I can see the potential value of DialysisConnect for my work with dialysis patients: mean 2.8, SD 0.4, vs mean 2.3, SD 0.6; P=.02). Providers most commonly selected reduced time and energy spent gathering information as a motivator (11/26, 42%) and a lack of time to use the system as a barrier (8/26, 31%) at the end of the pilot. 27. The Pros and Cons of Interactive Whiteboards in Relation to the Key Stage 3 Strategy and Framework

- Abstract
The article describes data emerging from a study of a group of language teachers integrating use of the interactive whiteboard (IWB) into their classroom practice. Data collection tools were developed which allowed participants freedom of action and expression whilst providing a framework for reflection designed to focus on pedagogy rather than technology. The teachers focused primarily on developing their use of PowerPoint for presenting and practising language, using a selection of interactive websites to provide further variety. Analysis revealed varied pedagogical awareness and a range of developmental needs. The research team linked emerging themes to principles underlying the "Key Stage 3 National Strategy: Framework for Teaching Modern Foreign Languages" (DfES, 2003). Participants' views largely reflected recent publications suggesting that use of the IWB can greatly enhance teaching by supporting classroom management, pace and variety and the drawing of attention to grammatical features and patterns. Participants also felt that use of the IWB had very positive effects on pupils' memorisation skills and writing development, though further long-term research is needed to test the accuracy of these intuitions. The study raised important caveats: the need to become comfortable with the technology before being ready to engage in pedagogical discussion; the dramatic increase in preparation time to support full exploitation; the danger of being lured into a "tell and show" interpretation of teaching; the need for IWB use to be underpinned by a sound understanding of language teaching methodology; the need for a balance of activities to ensure opportunities for kinaesthetic and tactile learners; and the difficulty of finding web-based material at the right level for language learning. Use of the IWB was linked firmly to participants' immediate needs for daily classroom survival. However, some participants were gradually becoming aware of the transformational potential of ICT. The definition of special education has been broadened to include all kinds of special help, training, institutionalization and intervention programs. Along with this has come a comprehensive system for financing and administering the program. The major dark cloud remaining seems to be the lack of trained personnel to carry out the intentions of the legislation. The example of the school psychologist is a good example. It will continue to challenge the ingenuity of psychologists and educators to develop innovations to make the regional psychological/educational centers is any indication, the challenge will be met in productive and imaginative ways.
