Abstract

There has been a surge of interest in team coaching in recent years, however understanding of the process, based on the first-person perspective of team coaches, is lacking. This qualitative study was conducted using heuristic inquiry methodology, enhanced by the inclusion of focus groups. This process enabled shared learning, expanded thinking and knowledge development amongst the co-researchers over an eleven-month period. The study sheds light on the emergent practice of team coaching from the team coach’s perspective. The findings challenge concepts in the existing literature, add to existing knowledge as well as opening up new areas for debate. The findings have considerable potential to inform practice, and the coaching profession, by providing insight and a practical resource in the form of the PiE Team Coaching Model and accompanying framework. These resources detail universal elements, as well as typical variations, for team coaches to consider in their practice.

Abstract

The Yuzpe 2+2 regimen is recommended for emergency postcoital contraception by the Reproductive Endocrinology and Infertility Committee of the Society of Obstetricians and Gynaecologists of Canada (SOGC). In the Yuzpe 2+2 regimen, two tablets of estradiol and levonorgestrel are taken with an antiemetic; an identical dose is taken 12 hours later. The regimen has a 98% success rate and should be administered within 72 hours of unprotected intercourse. It should be regarded strictly as an emergency measure., Objective: A major reduction in the number of unwanted pregnancies and pregnancy terminations could be achieved if an effective method of emergency pregnancy interception (postcoital contraception) was readily available to women. This policy statement describes a method which is the Yuzpe 2 + 2 regimen; the use of two tablets of estradiol/levonorgestrel, with an antiemetic, followed by a second identical dose 12 hours later. Opinions: It must be stressed that this is the method of emergency contraception. Treatment should be reserved for those who have been exposed to unprotected coitus within the previous 72 hours only. Outcomes and Benefits: The success rate is 98 percent and is heralded by menstrual bleeding by the 21st day following treatment. For those women who do not qualify for this regimen, or with exposure in the previous seven days, the insertion of a copper intrauterine device (IUD) is highly effective in preventing pregnancy. Contraindications are those of the oral contraceptive pill. Evidence: Clinical trials have supported this statement. Harm and Costs: The rates of mild side effects, headaches, bloating and nausea and/or vomiting have been reduced by the reduction of the steroid dosage and the accompanying antiemetic. Reccomendations: Although the Yuzpe 2+2 method has been widely marketed in Europe and used in North America at physicians' discretion, there is currently no specific approval by the Health Protection Branch (HPB) or the Food and Drug Administration (FDA). However, the SOGC recommends this method to consumers as a highly effective method of emergency contraception. Abstract

The authors studied 740 consecutive in vitro fertilization (IVF) cycles over a 3-year period to compare the results of cycles in which an endogenous luteinizing hormone (LH) surge occurred with cycles in which human chorionic gonadotropin (hCG) was administered for induction of follicular maturation. Clomiphene citrate (100 to 150mg daily on cycle days 5 to 9) and human menopausal gonadotropin (hMG; 75 to 150IU daily from cycle day 6) were used for stimulation. Embryo transfer (ET) occurred in 164 (81.2%) of the LH surge cycles and 452 (84%; P=not significant [NS]) of the hCG cycles. The first urinary rise in LH was detected in the 6 or 9 A.M. collections in 78 (47.3%) of the LH surge cycles, a greater number (P<0.01) than expected if LH surge onset was random. A total of 107 pregnancies was achieved, for an overall pregnancy rate of 17.4% per ET. The pregnancy rate in the hCG-stimulated cycles was 13.9% per ET (63/452) and, in spontaneous LH surge cycles, was 28.8% (44/166; P<0.001). The spontaneous abortion rate was 9.1% in LH surge cycles, compared with 25.4% in hCG-triggered cycles (P<0.001). The result was a 2.4 times increase in live births for LH surge cycles compared with cycles in which hCG was administered. In this program, occurrence of an LH surge is a favorable event, associated with higher pregnancy and live birth rates than hCG-stimulated cycles, and usually occurring in the early morning, allowing oocyte retrieval during normal working hours.

Objectives: To review the etiology, evaluation, and treatment of hirsutism., Evaluation: A thorough history and physical examination plus selected laboratory evaluations will confirm the diagnosis and direct treatment., Treatment: Pharmacologic interventions can suppress ovarian or adrenal androgen production and block androgen receptors in the hair follicle. Hair removal methods and lifestyle modifications may improve or hasten the therapeutic response., Outcomes: At least six to nine months of therapy are required to produce improvement in hirsutism., Evidence: The quality of evidence reported in this guideline has been determined using the criteria described by the Canadian Task Force on the Periodic Health Examination., Recommendations: Hirsutism can be slowly but dramatically improved with a three-pronged approach to treatment: mechanical hair removal, suppression of androgen production, and androgen receptor blockade. Lifestyle changes including weight loss and exercise will lower serum androgen levels and improve self-esteem. The patient should be educated regarding associated health problems or long-term medical consequences of hyperandrogenism, including obesity, irregular menses, anovulation, infertility, pregnancy-induced hypertension, diabetes, hyperlipidemia, hypertension, and heart disease.
