Background: Current national guidelines on caring for hip fractures recommend early mobilization. However, this recommendation does not account for time spent immobilized waiting for surgery. We sought to determine timing of mobilization following hip fracture, beginning at hospital admission, and evaluate its association with medical complications and length of hospital stay (LOS). Methods: We performed a retrospective review of prospectively collected data for 470 consecutive patients who underwent surgery for a hip fracture between September 2019 and August 2020 at an academic, tertiary-referral hospital. Outcomes of interest included time from hospital admission to mobilization, complication rate and LOS. We used a binary regression analysis to determine the effect of different surgical and patient factors on the risk of a postoperative medical complication. Results: The mean time from admission to mobilization was 2.8 [+ or -] 2.3 days (range 3 h-14 d). There were 125 (26.6%) patients who experienced at least 1 complication. The odds of developing a complication began to increase steadily once a patient waited more than 3 days from admission to mobilization (odds ratio 2.15, 95% confidence interval 1.42-3.25). Multivariate regression analysis showed that prefracture frailty ((3 = 0.276, p = 0.05), and timing from hospital admission to mobilization ((3 = 0.156, p < 0.001) and from surgery to mobilization ((3 = 1.195, p < 0.001) were associated with complications. The mean LOS was 12.2 [+ or -] 10.7 days (range 1-90 d). Prolonged wait to mobilization was associated with longer LOS (p = 0.01). Conclusion: Comprehensive guidelines on timing of mobilization following hip fracture should account for cumulative time spent immobilized. Nous avons cherché à déterminer le temps écoulé de l'hospitalisation à la mobilisation après une fracture de la hanche et à évaluer les effets de ce délai sur les complications médicales et la durée de séjour à l'hôpital. Background: The shape of the association between preoperative hemoglobin level and outcomes after primary arthroplasty has not been adequately described. This study aimed to characterize the association between preoperative hemoglobin level and important outcomes after primary hip and knee arthroplasty and how this association is influenced by other key confounders. Methods: Using de-identified, population-based health administrative data for Ontario housed at ICES, we identified all primary hip and knee arthroplasty procedures performed in Ontario between April 2007 and March 2017. Preoperative hemoglobin level, age, sex, Charlson Comorbidity Index score, American Society of Anesthesiologists score, preadmission living status, Hospital-patient One-year Mortality Risk (HOMR) score, and serum sodium and creatinine levels were extracted. All relevant postoperative outcomes that could be measured accurately were identified. We performed multivariable logistic regression and restricted cubic splines analyses. Results: A total of 188176 patients clustered within 532 surgeons were studied. The adjusted likelihood of transfusion increased notably and progressively when the preoperative hemoglobin level was below 135 g/L; duration of surgery, patient age and HOMR score amplified this association. Risk of postoperative admission to critical care showed a linear association with preoperative hemoglobin level. Risks of unplanned 30-day emergency department visit, 30-day readmission and 1-year all-cause mortality showed curvilinear associations with baseline hemoglobin level, with risks being notably greater as the level deviated from 137 g/L to 141 g/L. Conclusion: Preoperative hemoglobin levels, both high and low, were independently significantly associated with primary arthroplasty outcomes, and levels at which outcome risks started to increase exceeded threshold values commonly used to define 'normal.' Preoperative hemoglobin level should be considered in future bundled payment models that aim to account for case-mix when grading postarthroplasty outcomes. Cette étude visait à caractériser le lien entre les taux d'hémoglobine préopératoires et certains paramètres importants après l'arthroplastie primaire de la hanche et du genou, et à analyser l'influence de certains facteurs de confusion clés sur ce lien. Méthodes : A partir de données administratives anonymisées sur la santé des populations en Ontario hébergées par l'ICES, nous avons recensé toutes les interventions pour arthroplastie primaire de la hanche et du genou effectuées en Ontario entre avril 2007 et mars 2017. Nous en avons extrait les taux d'hémoglobine préopératoires, l'âge, le sexe, l'indice de comorbidité de Charlson, le score de l'American Society of Anesthesiologists, la situation de vie préadmission, le score HOMR (Hospital-patient One-Year Mortality Risk) et les taux sériques de sodium et de créatinine. Nous avons identifié tous les paramètres postopératoires pertinents qu'il était possible de mesurer avec précision. Nous avons procédé à des analyses de régression logistique multi-variées et au calcul des splines cubiques restreintes. Résultats : En tout, nous avons étudié 188176 patients répartis entre 532 chirurgiens. La probabilité ajustée de transfusion augmentait nettement et progressivement lorsque les taux d'hémoglobine préopératoires étaient inférieurs à 13 5 g/L; la durée de la chirurgie, l'âge du patient et le score HOMR ont amplifié ce lien. Le risque d'admission dans une unité de soins intensifs après la chirurgie a été en lien linéaire avec les taux d'hémoglobine préopératoires. Les risques de consultation imprévue aux urgences dans les 30 jours, la réhospitalisation dans les 30 jours et la mortalité de toute cause à 1 an ont montré des liens curvilinéaires avec les taux d'hémoglobine au départ, les risques étant sensiblement plus élevés lorsque les taux s'éloignaient des valeurs de 137 g/L à 141 g/L. Conclusion : Les taux d'hémoglobine préopératoires élevés et bas ont été indépendamment en lien significatif avec certains paramètres post-arthroplastie primaire, et les taux à partir desquels le risque de survenue des paramètres mesurés commençait à augmenter excédaient les valeurs seuils communément considérées normales. The association between preoperative pain catastrophizing and postoperative patient-reported outcome measures of patients with pre-arthritic hip disease was evaluated. All patients scheduled for joint-preserving surgeries of the hip (JPSH) at our institution were approached. Patient demographics (age, sex, body mass index (BMI)), pain intensity (Numeric Pain Scale (NPS)) and pain catastrophizing (Pain Catastrophizing Scale (PCS)) were collected preoperatively. Patient function (12-Item International Hip Outcome Tool (iHot-12)) and physical and mental health (Patient-Reported Outcomes Measurement Information System (PROMIS-10) mental/physical) were collected preoperatively, three-month and one-year postoperatively. The analysis consisted of multivariate linear regression models fitted for continuous scores of outcome measures at three-month and one-year. Correlation between preoperative PCS and iHot-12 was assessed using the Pearson correlation coefficient. A total of 274 patients completed the PCS and were included in the multivariate linear regression models. Most patients were females (66.8%), mean age was 33 (SD 9), mean BMI was 26.5 (SD 5.8) and most were diagnosed with femoro-acetabular impingement (46.0%) and underwent arthroscopy (77.0%). There were statistically significant correlations between PCS and iHot-12 (preoperatively −0.615, P  < 0.001; three-month −0.242, P  = 0.002). Statistically significant associations were found for function (three-month PCS P  = 0.046, age P  = 0.014, NPS P  = 0.043; one-year BMI P  = 0.005, NPS P  = 0.014), physical health (three-month BMI, P  = 0.002, NPS P  = 0.008; one-year BMI P  = 0.002, NPS P  = 0.013) and mental health (three-month BMI P  = 0.047; one-year BMI P  = 0.030). There is an association between function and preoperative pain catastrophizing in patients with pre-arthritic hip disease undergoing JPSH. Background: Guidelines for urinary catheterization in patients with hip fracture recommend limiting catheter use and using intermittent catheterization preferentially to avoid complications such as urinary tract infection (UTI) and postoperative urinary retention (POUR). We aimed to compare current practices to clinical guidelines, describe the incidence of POUR and UTI, and determine factors that increase the risk of these complications. Methods: We retrospectively reviewed the charts of patients with hip fracture who presented to a single large tertiary care centre in southeastern Ontario between November 2015 and October 2017. Data collected included comorbidities, catheter use and length of stay. We compared catheter use to guidelines, and investigated the incidence of and risk factors for POUR and UTI. Results: We reviewed the charts of 583 patients, of whom 450 (77.2%) were treated with a catheter, primarily indwelling (416 [92.4%]). Postoperative urinary retention developed in 98 patients (16.8%); however, it did not affect length of stay (p = 0.2). Patients with indwelling catheters for more than 24 hours after surgery had a higher incidence of POUR than those who had their catheter removed before 24 hours (65/330 [19.7%] v. 10/98 [10.2%]) (odds ratio [OR] 2.2, 95% confidence interval [CI] 1.06-4.38). A UTI developed postoperatively in 62 patients (10.6%). Catheter use was associated with a 6.6-fold increased risk of UTI (OR 6.6, 95% CI 2.03-21.4). Patients with indwelling catheters did not have a significantly higher incidence of UTI than those with intermittent catheterization (57/416 [13.7%] v. 2/34 [5.9%]) (p = 0.2). Patients who developed a UTI had significantly longer catheter use than patients who did not (p < 0.002). Conclusion: Indwelling catheters were used frequently, which suggests low compliance with clinical guidelines. Longer duration of catheter use led to higher rates of UTI and POUR. Further investigation of the reasons for the common use of indwelling rather than intermittent catheterization is needed. Nous avons voulu comparer les pratiques actuelles et les lignes directrices cliniques, établir l'incidence des cas de RUPO et d'infection urinaire et déterminer quels facteurs exacerbent le risque à l'égard de ces complications. Méthodes : Nous avons procédé à une analyse rétrospective des dossiers de patients victimes d'une fracture de la hanche ayant consulté dans un grand centre hospitalier universitaire du Sud-Est de l'Ontario entre novembre 2015 et octobre 2017. Les données recueillies incluaient les comorbidités, l'utilisation de cathéters et la durée du séjour. Nous avons comparé le recours au cathéter par rapport aux lignes directrices et évalué l'incidence des cas de RUPO et d'infection urinaire, et les facteurs de risque. Résultats : Nous avons analysé les dossiers de 583 patients, dont 450 (77,2%) se sont fait poser un cathéter, principalement à demeure (416 [92,4%]). Fragility fractures (FFs) are low-energy trauma fractures that occur at or below standing height. Among FFs, hip fractures are associated with the greatest morbidity, mortality and cost to Canadian health care systems. This review highlights the current state of medical care for hip fractures in Canada, with specific focus on the role of the multidisciplinary team. Gaps in care exist, as FFs represent a unique challenge requiring both acute and chronic management. Furthermore, there is a lack of ownership of FFs by a medical specialty. These gaps can be addressed through the use of multi-disciplinary teams, which have been shown to be efficacious and cost-effective. This model of care also addresses numerous patient-identified barriers to treatment, including inadequate patient counselling. However, there is still room for improvement in both the identification of patients at risk for hip fracture and patient adherence to therapy. Les fractures de fragilisation (FF) sont des fractures qui surviennent lors d'un traumatisme léger se produisant depuis la position debout ou d'une hauteur moindre. Les fractures de la hanche sont les FF associées aux plus grands taux de morbidité et de mortalité et aux plus grands coûts pour les systèmes de santé au Canada. La présente revue s'intéresse à l'état actuel des soins médicaux pour une fracture de la hanche au pays et porte une attention spéciale au rôle de l'équipe multidisciplinaire. Des lacunes dans les soins existent et sont mises en évidence par les FF, qui posent un défi bien particulier en nécessitant une prise en charge à la fois aiguë et chronique. De plus, cette prise en charge ne relève d'aucune spécialité médicale. La correction de ces lacunes peut passer par le recours aux équipes multidisciplinaires, dont l'efficacité et la rentabilité ont été démontrées. Lumbar stiffness leads to greater hip dependence to achieve sagittal motion and increases instability after total hip arthroplasty (THA). We aimed to determine parameters that influence lumbar stiffness among patients with hip pathology. We hypothesized that modifiable (degenerative changes, spinal canal stenosis, facet orientation) and nonmodifiable factors (muscle condition) would be associated with lumbar spine stiffness. In this retrospective case‐cohort study from a tertiary referral center, consecutive patients presenting at a hip specialist clinic underwent standing and deep‐seated radiographic assessment to measure lumbar lordosis (∆LL) (stiffness: ∆LL < 20°), hip flexion (∆PFA: pelvic femoral angle), and degree of degenerative‐disc‐disease (DDD) (facet osteoarthritis, disc height, endplate proliferative changes). Of these, 65 patients were selected with previous lumbar spine magnetic resonance imaging, allowing to determine lumbar facet orientation, spinal canal stenosis (Schizas classification), and flexor‐ and extensor‐muscle atrophy (Goutallier classification). Mean ∆LL was 45° (range: 11°–72°) and four patients (6%) exhibited spine stiffness. Patients with multilevel DDD (n = 22) had less ∆LL than those with no/single level (n = 43) DDD (34° [range: 11°–53°] vs. 51° [21°–72°]; p < 0.001). Number of DDD levels correlated strongly with ∆LL (ρ = −0.642; p < 0.001). Spinal stiffness was only seen in patients with ≥4 DDD levels. There was no correlation between ∆LL and facet orientation (p > 0.05). ∆LL correlated strongly with extensor atrophy at L3–L4 (ρ = −0.473), L4–L5 (ρ = −0.520), and L5–S1 (ρ = −0.473) and poorly with flexors at L4–L5 (ρ = −0.134) and L5–S1 (ρ = −0.227). Lumbar stiffness is dependent on modifiable (muscle atrophy) and nonmodifiable (extend of DDD) factors. This can guide nonoperative management of hip pathology, emphasizing the relevance of core muscle rehabilitation to improve posture and stiffness. Introduction: This study aims to: (1) describe perioperative complications amongst patients who underwent primary total hip arthroplasty (THA) using a short cementless, titanium, flat, tapered stem; (2) estimate this stem's early- to mid-term survival; (3) identify factors associated with revision arthroplasty; and (4) describe femoral remodelling at minimum 6 years postoperatively. Methods: A retrospective review of consecutive patients who underwent THA using a Taperloc Microplasty stem (Zimmer-Biomet, Warsaw, Indiana, USA) with minimum 2-year follow-up was performed. Surgeries were performed by 1 of 6, non-designer, arthroplasty surgeons between 2014 and 2018. Outcomes included perioperative complications including revision arthroplasty, and survival. Cox analysis was used to analyse the effect of different factors on risk of revision arthroplasty. Radiographs with 6-year follow-up served to describe femoral remodelling. Results: In 1205 patients, followed for 5.1 ± 1.4 years, the incidence of perioperative complication was 5.2% for which 29 patients (2.4%) required revision arthroplasty. The 5- and 7-year survival rates were 97.8% (95% CI, 96.9–98.5) and 97.0% (95% CI, 95.6–98.0), respectively. The only factor associated with revision arthroplasty was proximal femur morphology, as per Dorr classification (HR 1.24 [95%CI, 1.09–1.41]; p = 0.005). During radiographic assessment, 12% of patients showed ⩾25% of relative change in cortical thickness in Gruen zones 3 or 5. We observed calcar remodelling in 50% of radiographs while 10% showed presence of a pedestal sign. Conclusions: The 7-year survivorship of the Taperloc Microplasty stem is within National Institute for Health and Care Excellence (NICE) guidelines. Patients ⩽65 years with osteoarthritis and Dorr A/B femoral morphology may be ideal candidates for THA with this stem. Femoral remodelling is common and not associated with adverse outcome.
