Abstract

According to the World Health Organization1,2 the percentage of healthcare dependent population, such as elderly and people with disabilities, among others, will increase over the next years. This trend will put a strain on the health and social systems of most countries. The adoption of robots could assist these health systems in responding to this increased demand, particularly in high intensity and repetitive tasks. In a previous work, we compared a Socially Assistive Robot (SAR) with a Virtual Agent (VA) during the execution of a rehabilitation task. The SAR consisted of a humanoid R1 robot, while the Virtual Agent represented its simulated counter-part. In both cases, the agents evaluated the participants’ motions and provided verbal feedback. Participants reported higher levels of engagement when training with the SAR. Given that the architecture has been proven to be successful for a rehabilitation task, other sets of repetitive tasks could also take advantage of the platform, such as clinical tests. A commonly performed clinical trial is the Timed Up and Go (TUG), where the patient has to stand up, walk 3 m to a goal line and back, and sit down. To handle this test, we extended the architecture to evaluate lower limbs’ motions, follow the participants while continuously interacting with them, and verify that the test is completed successfully. We implemented the scenario in Gazebo, by simulating both participants and the interaction with the robot3. A full interactive report is created when the test is over, providing the extracted information to the specialist. We validate the architecture in three different experiments, each with 1,000 trials, using the Gazebo simulation. These experiments evaluate the ability of this architecture to analyse the patient, verify if they are able to complete the TUG test, and the accuracy of the measurements obtained during the test. This work provides the foundations towards more thorough clinical experiments with a large number of participants with a physical platform in the future. The software is publicly available in the assistive-rehab repository4 and fully documented.

Abstract

Background: The majority of stroke survivors experiences significant hand impairments, as weakness and spasticity, with a severe impact on the activity of daily living. To objectively evaluate hand deficits, quantitative measures are needed. The aim of this study is to assess the reliability, the validity and the discriminant ability of the instrumental measures provided by a robotic device for hand rehabilitation, in a sample of patients with subacute stroke.Material and Methods: In this study, 120 patients with stroke and 40 controls were enrolled. Clinical evaluation included finger flexion and extension strength (using the Medical Research Council, MRC), finger spasticity (using the Modified Ashworth Scale, MAS) and motor control and dexterity during ADL performance (by means of the Frenchay Arm Test, FAT). Robotic evaluations included finger flexion and extension strength, muscle tone at rest, and instrumented MAS and Modified Tardieu Scale. Subjects were evaluated twice, one day apart, to assess the test-retest reliability of the robotic measures, using the Intraclass Correlation Coefficient (ICC). To estimate the response stability, the standard errors of measurement and the minimum detectable change (MDC) were also calculated. Validity was assessed by analyzing the correlations between the robotic metrics and the clinical scales, using the Spearman's Correlation Coefficient (r). Finally, we investigated the ability of the robotic measures to distinguish between patients with stroke and healthy subjects, by means of Mann-Whitney U tests.Results: All the investigated measures were able to discriminate patients with stroke from healthy subjects (p < 0.001). Test-retest reliability was found to be excellent for finger strength (in both flexion and extension) and muscle tone, with ICCs higher than 0.9. MDCs were equal to 10.6 N for finger flexion, 3.4 N for finger extension, and 14.3 N for muscle tone. Conversely, test-retest reliability of the spasticity measures was poor. Finally, finger strength (in both flexion and extension) was correlated with the clinical scales (r of about 0.7 with MRC, and about 0.5 with FAT).Discussion: Finger strength (in both flexion and extension) and muscle tone, as provided by a robotic device for hand rehabilitation, are reliable and sensitive measures. Moreover, finger strength is strongly correlated with clinical scales. Changes higher than the obtained MDC in these robotic measures could be considered as clinically relevant and used to assess the effect of a rehabilitation treatment in patients with subacute stroke. Abstract

This document presents an analysis of the state-of-the-art of clinical practices for attending patients with different degrees of Cognitive Impairment, both in general and in the settings where the DECI solutions will be tested. A thorough literature analysis was conducted, aimed at identifying the most widespread, evidence-based treatments for patients with MCI and dementia. Different integrated care models within different settings were also explored in the detail. The provision of care for patients with Cognitive Impairment and dementia was analyzed in four different settings: Italy, Sweden, Spain and Israel. A cross-analysis was finally conducted among all four studies, aiming at identifying the barriers and facilitators that could be addressed using DECI’s technology in the four pilot sites.

Abstract

Featured Application: The proposed methodology can support the choice of robotic rehabilitation devices, based on rehabilitation aims, and their use in clinical practice. Robot-mediated therapy is a viable approach for upper limb rehabilitation. The upper limb is a highly complex segment and the identification of the appropriate devices capable of rehabilitating it globally (from the shoulder to the hand) in clinical practice is crucial. In this work, we aimed: (i) to describe an approach used in identifying a set of technological and robotic devices to globally treat the upper limb, and (ii) to evaluate the feasibility of the identified set in clinical practice. Using an ad-hoc form, a multidisciplinary team identified a set of four robotic and sensor-based devices to treat globally the upper limb. Then, 30 stroke patients were enrolled and assigned to two groups: the robotic group (RG), where patients were treated with the robotic set, or the conventional group (CG). All patients were evaluated before and after the treatment. In the RG the patients used all the devices (one in each rehabilitation session); the treatment was well accepted, without drop-outs or adverse events. Using a multidisciplinary approach, we identified a set of technological and robotic devices to treat the upper limb globally, and then we experimented to ascertain its feasibility, in a pilot study. The market of mobile technologies has considerably increased in the past few years and the costs have consequently decreased. This rapid technological evolution can be seen in two different ways from the perspective of people with disability: on the one side it represents a great opportunity to create new solutions for improving independence; on the other it may represent a source of social exclusion if appropriate assistive solutions are not available to make technology usable by people with disability. This paper describe three case studies of persons with disabilities that have undergone an Assistive Technology assessment at the DAT service of Fondazione Don Gnocchi (Milan, Italy) involving the use of mobile ICT based Assistive Technologies. In all the three cases the appropriate solution for performing the desired activities is represented by a combination of mainstream products and assistive products. The three use cases described support the idea that mobile technologies can be powerful and versatile instruments to create assistive solutions for improving independence in daily life. The DAT project is a research initiative that aims at building up a smart home environment where people with disabilities can improve their abilities to cope with daily life activities by means of technologically advanced home automation solutions. The project has a threefold purpose. The smart home will be used as a physical setting, where clients with disabilities can follow individual programs aimed at improving their independence in the home environment. The smart house will also be used as a demonstration an educational laboratory where anybody interested can get knowledge of the latest advancements in the field of home automation and tele-care. Finally, the smart home will be used as research laboratory for testing and developing new clinical protocols and innovative solutions in the field of environmental control and home care. Abstract

This deliverable aims to define the target population of the DECI Project, by giving the general definitions and boundaries that will be used during the Project, as well as specifying the variables and tests that will be used to analyze the target population in the different pilot sites and, then, subdivide it into various clusters. In particular, the DECI Minimum Data Set (MDS) is defined representing the minimum set of instruments (tests, scales, questionnaires, etc) needed for describing the target population during the pilot phase. MDS is used to standardize the data that each clinical Partner will collect during the pilot phase in order to allow comparisons across the test sites. Finally, the last paragraph includes a preliminary analysis of the target population in the four pilot sites.

Abstract

Mild cognitive impairment (MCI) is a state related to ageing, and sometimes evolves to dementia. As there is no pharmacological treatment for MCI, a non-pharmacological approach is very important. The use of Information and Communication Technologies (ICT) in care and assistance services for elderly people increases their chances of prolonging independence thanks to better cognitive efficiency. Robots are seen to have the potential to support the care and independence of elderly people. The project ENRICHME (funded by the EU H2020 Programme) focuses on developing and testing technologies for supporting elderly people with MCI in their living environment for a long time. This paper describes the results of the activities conducted during the first year of the ENRICHME project, in particular the definition of user needs and requirements and the resulting system architecture.

Abstract

The aim of the present study is a better comprehension of strategies of motor coordination during complex movements. In this field of research microgravity represent a unique experimental condition for the investigation of the role of equilibrium control in movement planning. Namely, here we focus on two important issues: the centre of mass control and the endpoint trajectory. Preliminary results of the center of mass position and the finger path curvature during pointing movements performed under normal and transient microgravity conditions are presented.
