Abstract

Aims: Interferon‐beta (IFNβ), the most widely prescribed medication for multiple sclerosis, is generally considered safe. Nevertheless, rare serious and/or life‐threatening side effects have been reported such as thrombotic microangiopathy. A few mechanisms have been proposed to explain how interferon causes thrombotic microangiopathy, but immunological studies have been unable to pin this phenomenon down to a single pathophysiologic pathway. The aim of this article was to report a new mechanism explaining Interferon beta related thrombotic microangiopathy. Methods: We report thrombotic microangiopathy in a 28‐year‐old male receiving interferon‐beta treatment for multiple sclerosis. Results: After three years of starting interferon beta therapy, the patient presented with malignant hypertension causing seizures, rapidly progressive renal failure requiring haemodialysis and haemolytic anaemia. Corticosteroid and plasma exchange sessions permitted haemolysis control. Nonetheless, the patient remained hemodialysis‐dependent. Exploration of the complement system found a complement factor I deficiency whose activity normalized at the control carried out after 2 years. Conclusion: IFNβ treatment may cause complement factor I deficit, which can lead to thrombotic microangiopathy and severe renal failure. [ABSTRACT FROM AUTHOR]

Abstract

Context and objective. Renal thrombotic microangiopathy (TMA) is a histological lesion that can be observed in several pathological settings. The present study aimed to describe the different etiological contexts in which TMA has been observed and provide particularities.Methods. A retrospective and descriptive study over a 16-year period, from January 2000 to January 2016 was conducted at the Tunis University Hospital. All patients aged over 16 years and presenting with TMA at renal biopsy were analyzed.Results. 100 cases of renal TMA were collected. The mean age of patients at the time of renal biopsy was 38.5 ± 12.9 years. The clinical picture was dominated by hypertension observed in 70 patients. Results of the laboratory investigations were as follows: kidney injury was noted in 92 patients and biological TMA was observed in 10 patients. At the renal biopsy, arteriolar TMA was seen in all patients and glomerular TMA was seen in 15 patients. Seventy-three patients reached end stage renal disease. TMA was observed mainly in association with glomerular nephropathy outside lupus nephropathy in 47 cases, lupus nephropathy in 19 cases, malignant hypertension in 18 cases and post-renal transplantation in 9 cases. TMA was related to atypical hemolytic uremic syndrome in one case and 6 sporadic cases were observed.Conclusion. Renal TMA has been observed in several pathological situations. Comparative studies with matched control cases in each group but without TMA lesions should be performed to identify risk factors for developing TMA and the influence of TMA on renal prognosis in each etiological setting. Contexte et objectif. La microangiopathie thrombotique (MAT) rénale est une lésion histologique caractérisée par la présence de microthrombi obstruant les vaisseaux de la microcirculation rénale. Actuellement, les patients atteints de MAT rénale sont répartis en fonction du contexte étiologique. L’objectif de la présente étude était de décrire les différents contextes pathologiques au cours desquels une MAT rénale a été observée.Méthodes. Il s’agissait d’une étude documentaire (16 ans). L’étude a inclus tous les patients adultes ayant des lésions de MAT à la ponction biopsie rénale (PBR).Résultats. Parmi 3240 PBR réalisées, la MAT a été observée chez 100 patients soit une incidence moyenne de 6,7 cas/an. L’âge moyen était de 38,5+/-12,9 ans avec une prédominance masculine. Les lésions de MAT rénale ont été associées à plusieurs contextes pathologiques: les néphropathies glomérulaires (47%), les néphropathies lupiques (19%), l’HTA maligne (18%), post transplantation rénale (9%), postgreffe de cellules souches hématopoïétiques (1%), post partum (2%), un contexte néoplasique (1%), une origine médicamenteuse (1%), un syndrome des antiphospholipides primaire (1%), et un syndrome hémolytique et urémique (1%).Conclusion. La MAT rénale est fréquemment secondaire et s’associe à plusieurs contextes pathologiques. Cependant un SHUa/PTT sous-jacents devrait toujours être recherchés.

Abstract

Background and Aims Ig A nephropathy is the most frequent cause of glomerulonephritis worldwide. Renal-limited disease is the habitual clinical form of the disease. Pulmonary involvement may also be seen in rare cases. Capillaritis with pulmonary hemorrhage is the most frequent pulmonary involvement and is lifethreatening. We reported the case of patient presenting a rapidly progressive glomerulonephritis with pulmonary hemorrhage revealing Ig A nephropathy. Method A case report Results A 23 year-old man admitted for a rapidly progressesive glomerulonephritis with sever hypertension. He presented a respiratory distress with hemoptysis. At biology serum creatinine was at 2200µmol/l (eGFR = 2.4 ml/min) and Hemoglobin was at 5g/dl. Immunoassays of anti-glomerular basement antibodies and antineutrophil cytoplasmic autoantibodies were negatives. Computed tomography scan showed diffuse intra alveolar hemorrhage associated with pleural effusion. Bronchoalveolar lavage confirms this diagnosis. Renal histology showed Ig A nephropathy. Treatment consisted of daily depleted haemodialysis with blood transfusion and corticosteroids associated with monthly pulses of cyclophosphamide. His hemoptysis gradually resolved with a stable hemoglobin at 11 g/dL. However, renal failure persisted requiring maintenance hemodialysis regularly. Conclusion Pulmonary hemorrhage is a rare but serious complication of Ig A Nephropathy which is associated with significant morbidity and mortality. The exact cause for this “extra renal bleed” in Ig A Nephropathy is not known but it possibly represents IgA capillaritis. Though no evidence-based treatment protocol exists, therapeutic options includes steroids, plasmapheresis, cyclophosphamide and rituximab.

Abstract

Introduction: antineutrophil cytoplasmic antibodies (ANCA) associated Glomerulonephritis (GN) is rare but a life-threatening disease especially, particularly in patients with advanced renal failure at presentation. This study aims to evaluate the epidemiological, clinical and histopathological features of renal involvement and investigate factors associated with ESRD. Methods: patients with renal biopsy-proven ANCA associated glomerulonephritis were included retrospectively over a thirty years period. The renal survival, defined as time to reach ESRD, was evaluated based on clinical parameters, histopathological classification as well as the renal risk score. Results: a total of 65 patients with crescentic GN were included in the study. The mean age was 47.9 years ± 22.4 years (range: 18-78) with an M/F sex ratio at 1.13. Hematuria, proteinuria and oliguria were found in respectively 100%, 81.5% and 56.2% of cases. Sixty patients (92.3%) had renal failure at presentation, and 30 patients (46%) required initial hemodialysis (HD) therapy. The pattern of glomerular injury was categorized as mixed in 43.7% of cases, sclerotic in 34.3%, crescentic in 16.6%, and focal class in 6%. Regarding renal risk score, patients were classified in the category low risk, intermediate risk and high risk with respectively 16.9%, 44.6% and 38.4%. All patients received corticosteroids and immunosuppressive treatment. Complete, partial remission and relapses were noted in respectively 15.3%, 18% and 72% of cases. Factors associated with ESRD were serum creatinine level >500 μmol/l (P=0,0016), CRP level >60 mg/l (P = 0,0013), interstitial fibrosis (P=0,0009) and glomerulosclerosis> 10% of total glomeruli (P=0,001). The survival rate was 89%, 60.9% and 32.8% at respectively 1, 5 and 10 years. Death occurred in 10 cases (15%) caused mostly by infections (40%). Initial serum creatinine level>140 µmol/l (P=0,02), alveolar hemorrhage (P=0.001) and infections (P=0,0001) were associated with mortality. Conclusion: in our cohort of ANCA GN, confirms the data showing improved patient survival but constantly high relapse risk. In addition, we observed that ANCA GN classification was predictive, as the risk of progressing to ESRD increased with the ascending category of focal, crescentic, mixed and sclerotic GN.
