Abstract Background and objective Evidence for the treatment of multisystem inflammatory syndrome in children (MIS-C) is lacking. Anakinra, which targets IL-1-mediated inflammation, is reserved for refractory cases of MIS-C; however, its use in the treatment of MIS-C is not clearly established. Patients and methods To examine a role for anakinra in MIS-C, we performed a single center observational cohort study of all MIS-C patients diagnosed at our children’s hospital from May 15 to November 15, 2020. Demographics, clinical features, diagnostic testing, and cardiac function parameters were compared between MIS-C patients treated with intravenous immunoglobulin (IVIG) monotherapy and IVIG with anakinra (IVIG + anakinra). Results Among 46 patients with confirmed MIS-C, 32 (70%) were in the IVIG + anakinra group, of which 9 (28%) were also given corticosteroids (CS). No patients were treated with anakinra alone. MIS-C patients in the IVIG + anakinra group were enriched in a CV shock phenotype (p = 0.02), and those with CV shock were treated with higher doses of anakinra for a longer duration. Furthermore, MIS-C patients in the IVIG + anakinra group exhibited improvements in fever and cardiac function with or without CS. No significant adverse events were observed, and no differences in IL-1β levels were found among MIS-C patients in the IVIG + anakinra group. Conclusions Anakinra treatment, which was co-administered with IVIG primarily in patients with severe MIS-C, was associated with improvements in fever and cardiac function, and demonstrated a favorable side-effect profile. These findings suggest a role for adjunctive anakinra in the treatment of severe MIS-C.

Objective Determine the pharmacokinetics (PK) and exposure–response of hydroxychloroquine (HCQ) and desethylhydroxychloroquine (DHCQ) in paediatric SLE (pSLE).Methods We conducted an exploratory phase 2, direct-to-family trial. Children enrolled in the Childhood Arthritis and Rheumatology Research Alliance (CARRA) Registry with a diagnosis of pSLE were eligible if they were receiving HCQ as standard of care for ≥3 months. Biological samples were collected at up to four visits over a 6-month period. At each visit, plasma was obtained to measure the concentrations of HCQ and DHCQ, as well as cytokines. HCQ and DHCQ plasma PK data were analysed using a population PK modelling approach.Results Twenty-five subjects provided a total of 88 plasma concentrations for PK analysis. There was a poor linear fit between HCQ concentrations and total body weight (R2=0.03). There was a decline in both interferon (IFN)-alpha and IFN-gamma with higher concentrations of HCQ and DHCQ. Volume of distribution for HCQ in plasma was higher in children compared with published values in adults (73 000 L vs 44 000 L), but clearance values in children were similar to adults.Conclusions We report the first population PK model for HCQ and DHCQ in children using data from a novel direct-to-family clinical trial. We observed high interindividual variability in HCQ PK and found that weight-based dosing for HCQ is poorly correlated with drug concentrations, suggesting the need to use therapeutic drug monitoring to individualise dosing. Furthermore, our results suggest that the current weight-based dosing paradigm for HCQ may result in suboptimal drug exposures, particularly for children with obesity. Accordingly, additional studies of HCQ are needed in pSLE to determine the optimal drug concentration and dosing to reduce disease activity and improve outcomes.Trial registration number NCT04358302.

Heavy metal exposure from wastewater is an important environmental issue worldwide. In the search for more efficient treatment technologies, biosorption has been presented as an alternative for contaminant removal from wastewaters. The aim of this work is to determine the operation parameters of copper adsorption followed by biosorbent regeneration. The algae Durvillaea antarctica and Lessonia trabeculata were used as biosorbents in batch experiments. These biosorbents were exposed to different conditions, such as pH, copper concentration, exposure time, mass-to-volume ratios and regeneration reagents. Batch sorption tests revealed an adequate pH of 4.5–5.0. The selected biosorbent was D. antarctica due to a considerably higher copper retention capacity. As a regenerating reagent, sulfuric acid was more efficient. For diluted copper solutions (10 to 100 mg L−1), a biosorbent particle size of between 1.70 and 3.36 mm showed better retention capacity than larger particles and a biosorbent mass-to-volume ratio of 10 g L−1 was desirable for these metal concentrations.

Introduction Direct-to-family clinical trials efficiently provide data while reducing the participation burden for children and their families. Although these trials can offer significant advantages over traditional clinical trials, the process of designing and implementing direct-to-family studies is poorly defined, especially in children with rheumatic disease. This paper provides lessons learnt from the design and implementation of a self-controlled, direct-to-family pilot trial aimed to evaluate the effects of a medication management device on adherence to hydroxychloroquine in paediatric SLE.Methods Several design features accommodate a direct-to-family approach. Participants meeting eligibility criteria from across the USA were identified a priori through a disease registry, and all outcome data are collected remotely. The primary outcome (medication adherence) is evaluated using electronic medication event-monitoring, plasma drug levels, patient questionnaires and pill counts. Secondary and exploratory endpoints include (1) lupus disease activity measured by a remote SLE Disease Activity Index examination and the Systemic Lupus Activity Questionnaire; and (2) hydroxychloroquine pharmacokinetics and pharmacodynamics. Recruitment of the initial target of 20 participants was achieved within 10 days. Due to initial recruitment success, enrolment was increased to 26 participants. Additional participants who were interested were placed on a waiting list in case of dropouts during the study.Discussion and dissemination Direct-to-family trials offer several advantages but present unique challenges. Lessons learnt from the protocol development, design, and implementation of this trial will inform future direct-to-family trials for children and adults with rheumatic diseases. Additionally, the data collected remotely in this trial will provide critical information regarding the accuracy of teleresearch in lupus, the impact of adherence to hydroxychloroquine on disease activity and a pharmacokinetic analysis to inform paediatric-specific dosing of hydroxychloroquine.Trial registration number ClinicalTrials.gov Registry (NCT04358302).

- Abstract
Background and objective: Evidence for the treatment of multisystem inflammatory syndrome in children (MIS-C) is lacking. Anakinra, which targets IL-1-mediated inflammation, is reserved for refractory cases of MIS-C; however, its use in the treatment of MIS-C is not clearly established. Patients and Methods: To examine a role for anakinra in MIS-C, we performed a single center prospective observational cohort study of all MIS-C patients diagnosed at our children’s hospital from May 15 to November 15, 2020. Demographics, clinical features, diagnostic testing, and cardiac function parameters were compared between MIS-C patients treated with intravenous immunoglobulin (IVIG) monotherapy and IVIG with anakinra (IVIG+anakinra). Results: Among 46 patients with confirmed MIS-C, 32 (70%) were in the IVIG+anakinra group, of which 9 of these patients (28%) were also given corticosteroids (CS). No patients were treated with anakinra alone. MIS-C patients in the IVIG+anakinra group were enriched in a CV shock phenotype (p=0.02), and those with CV shock were treated with higher doses of anakinra for a longer duration. Furthermore, MIS-C patients in the IVIG+anakinra group exhibited improvements in fever and cardiac function with and without CS. No significant adverse events were observed, and no differences in IL-1β levels were found among MIS-C patients in the IVIG+anakinra group. Conclusions: Anakinra treatment, which was co-administered with IVIG and primarily in patients with severe MIS-C, was associated with improvements in fever and cardiac function, and demonstrated a favorable side-effect profile. These findings suggest a role for adjunctive anakinra in the treatment of MIS-C.

- Abstract
El trabajo judicial ha estado sometido a intensos cambios en los últimos años. A las múltiples reformas implementadas, se han sumado la tramitación electrónica y la necesaria implementación de nuevas modalidades de trabajo asociadas al trabajo a distancia. El trabajo judicial ha debido integrar nuevas herramientas tecnológicas y ha tenido que ejecutarse en contextos productivos diferentes (presenciales, híbridos y a distancia). En este contexto de cambios, sigue siendo muy relevante para el trabajo judicial el manejo de programas de ofimática (Word, Excel) para la ejecución de las tareas diarias, independientemente de la materia jurisdiccional que trate la unidad judicial en que se desempeñe el participante del curso. Por ello la relevancia que reviste el desarrollo e incorporación de habilidades que potencien las capacidades en el uso de las herramientas que Office versión 2010 y 365 ofrecen en su nivel básico, poniendo especial énfasis en el uso de las herramientas Word y Excel para el trabajo en tribunales.

- Abstract
Carlos Lozier llegó a Chile en 1822 y se involucró inmediatamente en la vida educativa del país, siendo el primer director laico del Instituto Nacional y responsable de varios proyectos importantes. No obstante, la historiografía nacional, en particular la decimonónica, desmerece a menudo sus méritos y resultados. El objetivo de este artículo consiste en, a partir de fuentes francesas y chilenas, construir primero una crónica nueva de su vida que permita poner en valor su trabajo y, así, mostrar la relevancia de sus actuares y, en un segundo tiempo, explicar el porqué de esta imagen negativa o de esta ignorancia sobre el personaje. Las ciencias anexas de la historia utilizadas en este artículo como la biografía y la prosopografía permiten justamente calificar a los individuos, abordar y entender las diversas facetas mostradas por las personalidades estudiadas y, así, llegar a una comprensión distinta y renovada, aunque no siempre más positiva, lo que no ocurre con Lozier.

- Abstract
Health disparities in childhood‐onset systemic lupus erythematosus (SLE) disproportionately impact marginalized populations. Socioeconomically patterned missing data can magnify existing health inequities by supporting inferences that may misrepresent populations of interest. Our objective was to assess missing data and subsequent health equity implications among participants with childhood‐onset SLE enrolled in a large pediatric rheumatology registry. We evaluated co‐missingness of 12 variables representing demographics, socioeconomic position, and clinical factors (e.g., disease‐related indices) using Childhood Arthritis and Rheumatology Research Alliance Registry childhood‐onset SLE enrollment data (2015–2022; n = 766). We performed logistic regression to calculate odds ratios (ORs) and 95% confidence intervals (95% CIs) for missing disease‐related indices at enrollment (Systemic Lupus Erythematosus Disease Activity Index 2000 [SLEDAI‐2K] and/or Systemic Lupus International Collaborating Clinics/American College of Rheumatology Damage Index [SDI]) associated with data missingness. We used linear regression to assess the association between socioeconomic factors and SLEDAI‐2K at enrollment using 3 analytic methods for missing data: complete case analysis, multiple imputation, and nonprobabilistic bias analyses, with missing values imputed to represent extreme low or high disadvantage. On average, participants were missing 6.2% of data, with over 50% of participants missing at least 1 variable. Missing data correlated most closely with variables within data categories (i.e., demographic). Government‐assisted health insurance was associated with missing SLEDAI‐2K and/or SDI scores compared to private health insurance (OR 2.04 [95% CI 1.22, 3.41]). The different analytic approaches resulted in varying analytic sample sizes and fundamentally conflicting estimated associations. Our results support intentional evaluation of missing data to inform effect estimate interpretation and critical assessment of causal statements that might otherwise misrepresent health inequities.

Background: SARS-CoV-2 hampered the resolution of multiple diseases, including cancer. Aim: To show that a multidisciplinary program of Strategies to Advance Recovery (STAR) can be implemented in a public hospital in Chile, despite the global pandemic and state of a national catastrophe, to provide a solution to cancer patients. Material and Methods: A retrospective descriptive study, of patients requiring an elective resolution of a colorectal cancer. Patients met the inclusion criteria, established in the STAR program. A total of 24 perioperative interventions were performed in the protocol. Demographic variables, days of hospitalization, complications, mortality, and readmissions were described. Results: The 24 interventions of the protocol were successfully implemented, although some partially. Sixteen patients aged 53 to 83 years (50% women) were operated. The median length of hospitalization was four days (range 2 to 9). Four complications were recorded, all were grade I or II according to the Clavien-Dindo classification. Two patients were readmitted. There were no reoperations or mortality. One patient was infected with coronavirus, diagnosed at the time of readmission. Conclusions: The STAR protocol reduces the length of hospital stay. In a pandemic context such as COVID-19 it becomes a useful resource and can be implemented in cancer patients, as herein reported.

Este artículo estudia las características de la novela Durante la Reconquista de Alberto Blest Gana como género histórico, en el contexto de la vigencia realista de la narrativa en Chile y como particular expresión de la realidad hispanoamericana del siglo XIX. Enfoca la perspectiva del héroe en la limitación de su conciencia y hacer ante el mundo y a su vez, analiza las estructuras del discurso y la novela. Se demuestra la interpretación y finalidad de la historia en el código romántico hispanoamericano y las particularidades que diferencian este género de la novela europea. This article studies the characteristics of the novel Durante la Reconquista by A. Blest Gana as an historical genre, in the context of the novel's realistic period and as a particular expression of the Spanish American reality of the XIX century. It focuses on the hero's perspective within the limitation of his conscience and his doings in the world, and at the same time it analyzes the structures of the narrative voice and of the novel. The interpretation and finality of the story in the Spanish American romantic code and the features which differentiate this gen re from the European novel are shown.

Se revisan en él las críticas desde la primera edición de la novela hasta nuestros días y algunos alcances teóricos que ha merecido el estudio de la novela histórica. De la obra se comentan aspectos de su estructura y función literaria, la relación con el programa narrativo del autor y su vínculo con la literatura hispanoamericana romántica. This artcle celebrates the hundred years of publication of the novel Durante la Reconquista of the Chilean writer Alberto Blest Gana. It includes a critical estimate of the different reviews and criticism from the first edition to the present, as well as theoretical considerations concerning the historical novel itself. About the novel by Blest Gana, aspects of structure and literary function are commented upon, including its relationship to the narrative project of the author with its specific connections to romantic Hispanic American writing.

El objetivo es trazar el camino que sigue el autor desde su reflexión ensayística, la perspectiva asumida en su lectura del poema Martín Fierro y los cuentos en los que el gaucho aparece como protagonista modelo de un modo de ser argentino. This article investigates the relationship of J.L. Borges' s early essays, of 'Evaristo Carriego' and the writings about gaucho literature, to the author's own fiction. The objective is to show the progress of the author from the reflexive essay form, the perspective assumed in his reading of the poem 'Martín Fierro', and the stories in which the gaucho appears as model protagonist of a typical Argentine way of being.
