Abstract

Mutations in [Na.sup.+]-HC[O.sup.-.sub.3] cotransporter (NBC-1) cause proximal renal tubular acidosis (pRTA) associated with ocular abnormalities. One pRTA patient had increased serum amylase, suggesting possible evidence of pancreatitis. To further delineate a link between NBC-1 inactivation and pancreatic dysfunction, immunohistochemical analysis was performed on rat and human pancreas using antibodies against kidney-type (kNBC-1) and pancreatic-type (pNBC-1) transporters. In rat pancreas, the anti-pNBC-1 antibody labeled acinar cells and both apical and basolateral membranes of medium and large duct cells. In human pancreas, on the other hand, the anti-pNBC-1 antibody did not label acinar cells, although it did label the basolateral membranes of the entire duct system. The labeling by anti-kNBC-1 antibody was detected in only a limited number of rat pancreatic duct cells. To examine the effects of pRTA-related mutations, R342S and R554H, on pNBC-1 function, we performed functional analysis and found that both mutants had reduced transport activities compared with the wild-type pNBC-1. These results indicate that pNBC-1 is the predominant variant that mediates basolateral HC[O.sup.-.sub.3] uptake into duct cells in both rat and human pancreas. The loss of pNBC-1 function is predicted to have significant impact on overall ductal HC[O.sup.-.sub.3] secretion, which could potentially lead to pancreatic dysfunction. Abstract

The search for the endogenous sodium pump inhibitor (SPI) is crucial to the understanding of hypertension and to the development of antihypertensive drugs. Researchers have identified the oubain-like compound, digoxin-like compound, cardenolide-lipid conjugates and bufodienolide SPIs as possible regulators of blood pressure in humans and mammals. These compounds are believed to regulate sodium reabsorption by the kidney which leads to hypertension. Animal studies support the theory that SPI contributes to the development of hypertension.

Abstract

PURPOSE: On the assumption that renal tubular cells are more important as the target cells for a natriuretic factor than blood cells, we used a well-characterized cultured renal tubular cell line, Madin-Darby canine kidney (MDCK) cells, to monitor the circulating digitalis-like factor in human plasma and examine its role in the regulation of blood pressure and sodium balance. SUBJECTS AND METHODS: We investigated the effects of plasma on binding of radioactive ouabain to monolayered MDCK cells in order to determine the level of a circulating digitalis-like factor. First, we measured specific [.sup.3]H-ouabain binding to MDCK cells in the presence of plasma from 71 outpatients (34 normotensive subjects and 37 hypertensive patients) after incubation for 4 hours. Second, we measured specific [.sup.3]H-ouabain binding after incubation of cells with plasma from 16 hospitalized subjects (eight normotensive subjects and eight hypertensive patients) receiving low and high sodium diets. RESULTS: In Study 1, ouabain binding was lower by 30% with plasma from hypertensive patients than with plasma from normotensive subjects (p CONCLUSIONS: These findings suggest that a circulating digitalis-like factor, which may act on renal tubular cells as the ouabain-displacing compound, is increased in patients with essential hypertension and also demonstrate that plasma levels may be influenced by changes in dietary sodium intake., Many drugs are thought to be therapeutically active because they mimic the action of compounds already present in the body. This notion is illustrated by the discovery, in human tissues, of opioid proteins, which have effects similar to morphine. Digitalis is an important agent in the treatment of heart failure because it improves heart contraction and fluid excretion. Identification of a native digitalis-like factor would be very significant. Such a factor, which could alter kidney cell function, has been identified in human urine and mammalian blood. Eighty-seven blood specimens, assumed to contain a digitalis-like factor, were studied using cultured (laboratory-grown) kidney cells to determine if levels of the factor might differ in subjects with and without high blood pressure. The effect of dietary sodium on the factor was also explored. Digitalis-like factor activity was significantly higher in patients with high blood pressure, and activity tended to be correlated with blood pressure levels. Activity was higher in individuals with a high salt intake, and it correlated significantly with urinary sodium excretion. Although the results provide evidence for greater amounts of a digitalis-like factor in hypertensive patients, they can not rule out the possibility that increased digitalis-like factor might be caused by high blood pressure, rather than being a regulatory response to it.
