Si le déclassement professionnel et plus largement le thème du «brain waste» est présent dans les études migratoires, peu d’études permettent de dresser un tableau général du déclassement professionnel en France: quels sont les immigrés et les descendants d’immigrés les plus concernés? Quels sont les métiers, les professions, les conditions de travail des personnes déclassées? Comment la situation est-elle perçue par les personnes? À partir d’une approche par méthodes mixtes qui couple l’analyse des données de l’enquête Conditions de Travail-Risques Psychosociaux 2016, et une enquête qualitative par entretiens semi-directifs avec des personnes immigrées en situation de déclassement professionnel, nous souhaitons décrire finement le phénomène de déclassement professionnel en France en termes de caractéristiques des personnes concernées et des espaces professionnels dans lesquels elles évoluent, ainsi que leurs expériences et discours autour du déclassement professionnel. https://www.ceped.org/wp &nbsp

Les personnes immigrées originaires d’Afrique subsaharienne traversent fréquemment des épisodes de grande précarité à leur arrivée en France. L’absence d’hébergement stable ou de titre de séjour peuvent notamment se traduire par une plus grande exposition aux risques sexuels et au VIH en particulier. Cette population représentait ainsi plus du tiers des nouveaux diagnostics d’infection par le VIH en 2018, infections par ailleurs acquises pour plus de la moitié après l’installation en France. Cet état de fait plaide pour une politique de prévention spécifique. C'est dans ce contexte que trois équipes de recherche et deux associations ont proposé une intervention innovante visant à renforcer la capacité d’agir (empowerment en anglais) en matière de santé sexuelle dans cette population, dans une démarche d’aller-vers. Nous présentons ici les coûts de l’intervention Makasi – la première du genre à être mise en place en France – dans le but d’informer les organisations souhaitant mettre en oeuvre ce type d’intervention.

International audience; INTRODUCTION: In France, combination prevention tools, particularly antiretroviral treatment for HIV prevention has been available for several years. We described the knowledge of these antiretroviral treatments among immigrants from sub-Saharan Africa, who are particularly affected by HIV, and the factors associated with this knowledge. METHODS: The data come from the Makasi study, which was conducted between 2019 and 2020 among precarious immigrants from sub-Saharan Africa recruited through a community-based outreach approach in the greater Paris area (n = 601). We described levels of knowledge of HIV treatment effectiveness (HTE), treatment as prevention (TasP), post-exposure prophylaxis (PEP), and pre-exposure prophylaxis (PrEP), by sex with chi2 test. We investigated factors associated with their knowledge with logistic regressions adjusted for sociodemographic characteristics, living conditions and sexual behaviors (p≤0.2). RESULTS: Respondents were mostly men (76%), from West Africa (61%), in precarious situation: 69% were unemployed, 74% were undocumented and 46% had no health coverage. Among this population, knowledge of HIV preventive treatments was heterogeneous. While HTE was well known (84%); TasP was known by only half of the respondents (46%), and PEP and PrEP were hardly known: 6% and 5%, respectively. Background We aimed to study whether social patterns of exposure to SARS-CoV-2 infection changed in France throughout the year 2020, in light to the easing of social contact restrictions. Methods A population-based cohort of individuals aged 15 years or over was randomly selected from the national tax register to collect socio-economic data, migration history, and living conditions in May and November 2020. Home self-sampling on dried blood was proposed to a 10% random subsample in May and to all in November. A positive anti-SARS-CoV-2 ELISA IgG result against the virus spike protein (ELISA-S) was the primary outcome. The design, including sampling and post-stratification weights, was taken into account in univariate and multivariate analyses. Results Of the 134,391 participants in May, 107,759 completed the second questionnaire in November, and respectively 12,114 and 63,524 were tested. The national ELISA-S seroprevalence was 4.5% [95%CI: 4.0%-5.1%] in May and 6.2% [5.9%-6.6%] in November. It increased markedly in 18-24-year-old population from 4.8% to 10.0%, and among second-generation immigrants from outside Europe from 5.9% to 14.4%. This group remained strongly associated with seropositivity in November, after controlling for any contextual or individual variables, with an adjusted OR of 2.1 [1.7–2.7], compared to the majority population. In both periods, seroprevalence remained higher in healthcare professions than in other occupations. Conclusion The risk of Covid-19 infection increased among young people and second-generation migrants between the first and second epidemic waves, in a context of less strict social restrictions, which seems to have reinforced territorialized socialization among peers.

International audience; BackgroundImmigrants and ethnic/racialized minorities have been identified as being at higher risk of coronavirus disease-19 (COVID-19) infection, but few studies report on their exposures and prevention behaviours. This study aims to examine the social distribution of COVID-19 exposure (overcrowding, working outside the home, use of public transport to go to work) and prevention behaviours (use of face masks, washing hands, respect for physical distance) in France during the first wave of the epidemic. Methods We used the EpiCov population-based survey from a random sample of individuals aged 15 years or more. We determined the distribution of the self-reported outcomes according to migratory status and sex, using χ2 tests. We modelled the probability of outcomes with logistic regression. Finally, we focused the analysis on the Greater Paris area and accounted for neighbourhood characteristics. Results A total of 111 824 participants were included in the study. Overall, immigrant groups from non-European countries were more exposed to COVID-19-related factors and more respectful of prevention measures. The probability of overcrowding and the use of public transport was higher for immigrants from sub-Saharan Africa [adjusted odds ratio (aOR) = 3.71 (3.19; 4.32), aOR = 6.36 (4.86; 8.32)] than for the majority population. Immigrant groups were less likely to have a non-systematic use of face masks and to breach physical distancing than the majority population [for immigrants from sub-Saharan Africa, aOR = 0.32 (0.28; 0.37) and aOR = 0.71 (0.61; 0.81), respectively]. Living in a neighbourhood with a higher share of immigrants was associated with higher exposure and better prevention behaviours. ConclusionsIn France, immigrants had a higher exposure to COVID-19-related factors and more systematic prevention behaviours.

Purpose – Few studies have examined psychosocial risks among immigrants or explored their association with mental health. Our study aims to 1) describe the prevalence of job strain and iso-strain according to migratory status and model the probability of exposure, 2) verify whether the association between job strain, iso-strain and anxiety holds for all immigrant groups.Methods – We used the nationally representative cross-sectional French Working Conditions Survey (N=24 640). Anxiety was measured with the GAD-Mini, a diagnosis tool to detect generalised anxiety. We described the prevalence of job strain and iso-strain according to migratory status and sex. We used multivariate Poisson regressions to model the probability of exposure to job strain and iso-strain. In each immigrant group, we modelled the probability of anxiety according to sociodemographic characteristics, lifetime suicidal attempt and job strain/iso-strain.Results – Overall, there were important variations in psychosocial risk prevalence, with immigrant groups more exposed than the majority population (neither first- nor second-generation immigrant). After adjust ment, being a first-generation immigrant from Africa remained associated with job strain (adjusted Incidence Rate Ratio: aIRR=1.21 [0.99-1.47]), and being a second-generation immigrant from Africa with iso-strain (aIRR=1.33 [1.05-1.69]). The prevalence of anxiety was highest in second-generation immigrants from Africa (12%). In this population, job strain and iso-strain were associated with anxiety (aIRR job strain=2.70 [1.22-6.01]; aIRR iso-strain 4.26 [2.29-7.92]).Conclusion – In line with previous international studies, our study shows that first- and second-generation immigrants in France are particularly exposed to job strain and iso-strain, which could could have a negative impact on their mental health, – Peu d’études se sont penchées sur les risques psychosociaux parmi les immigrés et les descen dants d’immigrés et leur association avec la santé mentale. L’anxiété a été mesurée par le score GAD-Mini, outil diagnostique pour repérer le trouble anxieux généralisé. Les prévalences du job strain et de l’iso-strain ont été décrites selon le statut migra toire et le sexe. Des régressions de Poisson ont été utilisées pour modéliser la probabilité d’être exposé au job strain et à l’iso-strain. La prévalence de l’anxiété a été ensuite décrite selon le statut migratoire et le sexe. Dans chaque groupe de population, nous modélisons la probabilité de l’anxiété en fonction des caractéristiques sociodémographiques, des antécédents de santé mentale et de l’expérience du job strain ou de l’iso-strain.Résultats – Les prévalences de risques psychosociaux varient fortement selon le statut migratoire, avec des immigrés plus exposés que la population majoritaire (ni immigrée, ni descendante d’immigrés). Après ajustement, être un immigré d’Afrique reste associé au job strain (ratio incidence risque ajusté : IRRa=1,21 ; IC95%: [0,99-1,47]), et être un descendant d’immigré d’Afrique à l’iso-strain (IRRa=1,33 [1,05-1,69]). La prévalence de l’anxiété était particulièrement haute parmi les descendants d’immigrés d’Afrique (12%). Dans cette population, le job strain et l’iso-strain étaient associés à l’anxiété (IRRa job strain=2,70 [1,22-6,01] ; IRRa iso-strain=4,26 [2,29-7,92]).

Six mois après un premier volet réalisé en mai 2020, l’enquête nationale Épidémiologie et Conditions de vie (EpiCov) révèle que 6,2 % de la population possède des anticorps contre le SARS-CoV-2 en novembre 2020, en France métropolitaine, chez les personnes de 15 ans ou plus hors Ehpad. Cette séroprévalence est inférieure à la proportion de personnes ayant été contaminées par le virus depuis le début de l’épidémie, car les anticorps peuvent disparaître avec le temps chez les personnes infectées. Entre mai et novembre 2020, on estime que 4,0 % de la population a développé des anticorps contre le virus, portant la part de la population contaminée par le virus depuis le début de l’épidémie à au moins 8,5 % en novembre 2020. En Île-de-France, le niveau de séroprévalence reste le plus élevé, avec 11 % des Franciliens de 15 ans ou plus ayant des anticorps en novembre 2020, devant l’Auvergne-Rhône-Alpes (8,4 %). Par ailleurs, contrairement à la première vague épidémique, les adolescents et les jeunes adultes ont été plus touchés par le virus que les adultes d’âges intermédiaires lors de la seconde vague : 10,1 % des 15 à 20 ans ont des anticorps, contre 6,7 % des 30 à 49 ans et 4 % des plus de 64 ans. Vivre dans une zone à forte densité de population ou exercer une profession essentielle dans le domaine du soin reste associé, comme lors de la première vague, à une séroprévalence plus élevée.
