We aimed to investigate the involvement of pathogenic leptospira in the cerebrospinal fluid (CSF) of meningitis-suspected children in Sudan.Methods: A total of 153 CSF specimens were collected over 5 months from patients attending a reference pediatric hospital in Omdurman, Sudan. All patients had provisionally been diagnosed with meningitis on admission. Demographic, clinical, and conventional laboratory findings were obtained. DNA was extracted using a QIAamp mini kit, and the secY gene investigated using real-time PCR.Results: Nine of 153 (6%) CSF specimens were positive for pathogenic leptospiral DNA. All these patients were male (seven infants and two toddlers aged ˂4 years). Typical conventional laboratory findings for aseptic meningitis (ie, CSF turbidity/pleocytosis, normal or reduced CSF glucose, normal or elevated proteins) were seen in five (56%). All patients presented with fever and seizures, 56% vomiting and stiff neck, and 29% bulging fontanel. Most (67%) patients presented in summer (March to May). Polymicrobial infections were identified in three patients (33%).Conclusion: We conclude that pathogenic leptospira are probably a common cause of meningitis in children in Sudan; therefore, we recommend including leptospirosis in the differential diagnosis of CNS infections and other undifferentiated febrile illnesses in this country.Keywords: leptospirosis, infectious meningitis, PCR, pediatrics, Africa

Abstract

Hospitalized elderly patients frequently suffer from delirium, especially in the context of sepsis-associated encephalopathy. Current treatments of delirium are merely symptomatic. Calorie restriction (CR) is both a promising strategy to protect against sepsis and has beneficial effects on aging-induced neurodegeneration. In this study, we investigated whether six weeks of 30% CR had protective effects on lipopolysaccharide (LPS) induced (neuro)inflammation in wild-type (WT) and progeroid mice deficient in the DNA excision-repair gene Ercc1(Ercc1Δ/−). While CR did not affect the LPS-induced inflammatory response in WT mice, CR exaggerated the peripheral inflammatory response in Ercc1Δ/−mice, as evidenced by an increase of pro-inflammatory serum cytokines (TNF-α, IL-1β, and IFN-γ) and kidney injury marker Ngal. Neuroinflammatory effects were assessed by RNA-sequencing of isolated microglia. Similarly, CR did not affect microglia gene expression in WT mice, but increased neuroinflammation-associated gene expression in Ercc1Δ/−mice. In conclusion, CR increases the peripheral and brain inflammatory response of Ercc1Δ/−mice to a systemic inflammatory stimulus.

Abstract

In what ways is agroecology a means to food sovereignty? In Brazil, claiming land rights was the first step along one group of farmers’ pathway to autonomy. The next was to develop and maintain agroecological practices. To achieve this goal, these farmers never worked alone. Strong selforganisation and long-lasting partnerships enabled them to redesign their farming system and set up alternative markets that value their produce and way of life.

Abstract

Het is met de ontwikkelde (e)DNA-methodes mogelijk om de aanwezigheid van leptospiren en de bruine rat in oppervlaktewatermonsters aan te tonen. Uit het huidige onderzoek blijkt echter ook dat de sporen van de bruine rat en leptospiren zowel gezamenlijk als los van elkaar waargenomen kunnen worden. Door per locatie meerdere monsters te combineren kan een nauwkeurig beeld van de bron worden opgebouwd, hiermee kunnen specifieke beheersmaatregelen goed onderbouwd en gericht worden gedefinieerd.

Leptospirose is een zoönose die bij mensen soms zeer ernstige verschijnselen veroorzaakt.. Autochtonen liepen de infectie voornamelijk op tijdens recreatie of beroepsuitoefening. Contact met oppervlaktewater was in bijna twee derde van de autochtone gevallen de meest waarschijnlijke bron van infectie; daarnaast werd direct contact met dieren genoemd (met name ratten). Abstract

Leptospirosis is a zoonotic disease of global importance with a mortality of up to 50% among patients requiring intensive care medicine. Meningitis or meningoencephalitis due to leptospires are considered to be an immunologically mediated disease and to occur mainly during the second phase of disease. Herein, we report a patient with severe meningo-encephalitis due to leptospirosis with evidence of direct bacterial invasion into the central nervous system (CNS). A 36-year-old otherwise healthy male farmer, without recent travel history, presented with recurrent fever, peaking >39°C, cough and conjunctivitis 10 days after high-pressure cleaning of a piggery without eye protection. Diagnostic work-up revealed meningoencephalitis due to Leptospira species (spp.). Serology (microscopic agglutination test) yielded elevated antibody titers (1:400) for Leptospira interrogans. Polymerase chain reaction (PCR) and electron micrograph of cerebrospinal fluid (CSF) demonstrated direct invasion of Leptospires into the CSF. Magnetic resonance imaging (MRI) of the brain and spine did not reveal any pathologic findings; electroencephalography (EEG) indicated right-hemispheric slowing. Due to severe abdominal pain, gastroscopy was performed. Histology showed signs of vasculitis. Conventional abdominal angiography demonstrated vasculitis resembling panarteriitis nodosa. Combined antibiotic and steroid therapy lead to clinical improvement.

36. Success factors of social innovations by a community-based learning course (CBLC)

Abstract

This is the success story of a community-based learning course (CBLC) project addressing the concerns of the international community of students and staff of Wageningen University and Research Centre (WageningenUR). A joint effort of this community, WageningenUR and social entrepreneurs resulted in social innovations for this community: an informative website and film about the available healthcare, explaining cultural differences. The film had 1,671 views (at 28-02-2014) and became part of the introduction programme of WageningenUR. At WageningenUR every year over 150 teams execute a ‘real-life’ project in the CBL course ‘Academic Consultancy Training’. In this course advanced Master students execute assignments commissioned by external commissioners in multidisciplinary and multicultural teams. What can be learned from this particular case of CBLC that successfully resulted in social innovations? The following factors contributed to the results of this CBLC project: 1) Collaboration and/or involvement of stakeholders in the different steps, before, during and after the CBLC project 2) Every step fitted within an interest or existing initiative of stakeholders involved 3) The CBLC team made the stories of international students about healthcare explicit 4) The stories reached the WageningenUR board 5) The CBLC team proposed constructive recommendations which supported follow-up action 6) The social action-oriented commissioner with network enabled follow-up action 7) The commissioner discussed sensitive issues. Lessons learned for the selection and guidance of future CBLC projects: 1) This case confirms the importance of attention for stakeholder involvement in the projects; 2) Projects in which stories of a community are made explicit, can be powerful in raising concern for this community; 3) Importance of formulation of constructive action-oriented recommendations; 4) Probe potential commissioners about how they expect they will share and use the results within their network.

Een internationaal netwerk als Participand in de regio rond Wageningen heeft vele mogelijkheden. Het biedt kansen voor het internationaal georiënteerde bedrijfsleven, Wageningen University & Research centre en voor de vele particuliere initiatieven van burgers. Door goed gebruik te maken van de kennis en contacten die regionaal aanwezig zijn, kunnen alle partijen optimaal profiteren van goed afgestemde projecten en producten op de lokale situatie elders. In veel netwerk initiatieven op het gebied van internationale samenwerking zijn internationale studenten en migranten ondervertegenwoordigd. Hetzelfde geldt voor een deel van de bedrijven en instituten die internationaal werkzaam zijn. Een internationaal netwerk als Participand laat zien dat het anders kan. Het maakt een omslag mogelijk van traditionele ontwikkelingshulp naar duurzame internationale samenwerking. De duurzaamheid van producten en projecten wordt bevorderd door de samenwerking met mensen uit die landen/regio’s waar men producten/projecten heeft of wil initiëren. De aanwezige expertise, capaciteiten en contacten onder nieuwkomers in de regio worden zo benut en erkend. Om een internationaal netwerk soepel te laten draaien is het belangrijk ruimte te bieden voor persoonlijk contact, diversiteit en een ‘broker’ te hebben voor de communicatie en de continuïteit van het netwerk. Hulpmiddelen daarbij zijn intervisie, online matching van vraag en aanbod, een informatieloket en een onderzoeksloket. Het rapport beoogt te bereiken dat verschillende partijen voor wie het netwerk waardevol is zich in enige vorm verbinden aan het netwerk. Belangrijke partijen in deze zijn Wageningen University & Research centre, Gemeente Wageningen, Business Café, de initiatiefnemers COS Gelderland en OIKOS en niet in de laatste plaats de doelgroep, (inter)nationale studenten, migranten, particuliere initiatieven en het bedrijfsleven.

Abstract

Leptospirosis and haemorrhagic fever with renal syndrome (HFRS) are hard to distinguish clinically since these two important rodent-borne zoonoses share hallmark symptoms such as renal failure and haemorrhage. Leptospirosis is caused by infection with a spirochete while HFRS is the result of an infection with certain hantaviruses. Both diseases are relatively rare in the Netherlands. Increased incidence of HFRS has been observed since 2007 in countries that border the Netherlands. Since a similar rise in incidence has not been registered in the Netherlands, we hypothesise that due to overlapping clinical manifestations, hantavirus infections may be confused with leptospirosis, leading to underdiagnosis. Therefore, we tested a cohort of non-travelling Dutch patients with symptoms compatible with leptospirosis, but with a negative diagnosis, during 2010 and from April to November 2011. Sera were screened with pan-hantavirus IgG and IgM enzyme-linked immunosorbent assays (ELISAs). Sera with IgM reactivity were tested by immunofluorescence assay (IFA). ELISA (IgM positive) and IFA results were confirmed using focus reduction neutralisation tests (FRNTs). We found hantavirus-specific IgG and/or IgM antibodies in 4.3% (11/255) of samples taken in 2010 and in 4.1% (6/146) of the samples during the 2011 period. After FRNT confirmation, seven patients were classed as having acute Puumala virus infections. A review of hantavirus diagnostic requests revealed that at least three of the seven confirmed acute cases as well as seven probable acute cases of hantavirus infection were missed in the Netherlands during the study period.
