Arkansas (AR) consistently has higher ischemic heart disease (IHD) death rates than the US, which is believed to be due to a higher prevalence in AR of major, modifiable risk factors. We examined the difference in IHD death rates between AR and the US as consequences of differences in the prevalence of nine risk factors between pre-Medicare age group (45-64) and Medicare age group (65-84). We modeled IHD deaths attributable to differential prevalence between AR and the US using mortality data and prevalence data from AR and US for years 2000-2010, and relative risk measures obtained from the INTERHEART and Atherosclerosis Risk in Communities studies. From 2000-2010, our study showed that if we were to reduce the prevalence of significant risk factors to US levels, we would reduce AR IHD deaths by 26.6% in the pre-Medicare age group and 15.9% in the Medicare age group. Most of the increased mortality was explained by higher prevalence of smoking and hypertension in AR. Other socioeconomic factors that contributed to an increased risk of poor health outcomes were education, income, and the lack of health insurance; with AR having worse outcomes than the US for the pre-Medicare age group. The importance of risk factors depended on race, sex, and age. The excess mortality in AR relative to the US for the two age groups can largely be explained by prevalence differences in smoking, hypertension, cholesterol, education and income.

Risk is defined as the exposure to the chance of injury or loss. Developing models or methods to evaluate the level of risk for a particular exposure comes from controlled experimentation. Cumulative trauma disorders are one type of loss that contributes to significant pain and suffering for people and for the industry in which these people work. However, no simple model exists which show how physical risk factors may contribute to the development of cumulative trauma disorders. This paper concentrates on efforts to evaluate ergonomic risk and associated outcomes on manufacturing and assembly jobs using existing ergonomic assessment tools to accurately assess risk. This paper concentrates on preliminary results as it relates to the upper extremity.

The incidence of invasive female breast cancer declined 6.7% in the United States from 2002 to 2003 following a nationwide decrease in the use of hormone replacement therapy that began in 2002. We used data from the Arkansas Central Cancer Registry to look for similar changes in the incidence of female breast cancer in Arkansas. There was a 9.3% decline in the incidence of invasive female breast cancer in Arkansas in 2003, and the decline continued through 2005. The decline was significantly higher (p0.05) among invasive cancers, particularly among women ages 50 and older and for those having an estrogen-receptor positive tumor.

This paper compares the ergonomic risk assessment of a task for the upper extremities as determined by Rapid Upper Extremity Assessment (RULA) and the Strain Index (SI). The ergonomic risk to the upper extremities of 244 automotive assembly plant tasks were evaluated using RULA and SI. The outcomes of each tool were compared for each task. Results from this study provide practical insight into the methods used in each tool. This study compared only the ergonomic risk outputs from each tool; it does not pursue the question of which tool best predicts injury. The kappa score was 0.11, indicating little agreement between the outputs of the two tools. This is supported by the lack of monotonicity with a gamma score of 0.1. These results indicate that the risk assessment outcome of these two ergonomic assessment tools for the upper extremities do not agree.

We describe an outbreak of Shigella sonnei infection among 24 persons who had eaten raw oysters in restaurants in southeastern Texas within five days before the onset of symptoms. The oysters in all eight restaurants were supplied by the same dealer, but examination of a routine water sample collected six days before the probable time of contamination showed the waters where the oysters were harvested to be free of fecal contamination, making widespread sewage contamination unlikely. The suspect oysters were traced to a single boat. Stool swabs from that boat's oyster harvesters allowed the identification of one asymptomatic carrier who had a strain of S. sonnei (determined by colicin typing, plasmid analysis, and testing for susceptibility to antibiotics) that was similar to or the same as that infecting the patients. Although the source of this man's infection was unknown, he reported having eaten no oysters. Investigation revealed that 5-gallon (19-liter) pails were used as toilets aboard the oyster boats. Sewage collected in these pails was often dumped overboard into the harvesting area. We conclude that this outbreak of S. sonnei resulted from poor sanitary procedures that probably allowed stool from a carrier to contaminate oysters either just before or after they were taken aboard the boat.
