The emergence of a modern teacher education system in Singapore can be traced to 1950 when the Teachers Training College (TTC) was established. The TTC was a separate entity from the School of Education at the University of Singapore. Rapid economic growth in the 1970s and 80s necessitated changes to upgrade the education system, such that school leavers could be better prepared for work in a post-industrial economy. It was recognised then that teacher preparation had to be upgraded to represent the field as an evidence-based profession, thus leading to the establishment of the Institute of Education in 1973; this was a significant first step in the journey towards universitising teacher education in Singapore. Continued and transformative economic growth in the 1980s and 90 s necessitated further changes in education goals, best represented by the Thinking Schools, Learning Nation initiative in 1997, the Teach Less Learn More initiative in 2004, and the ICT MasterPlans initiatives. In response, in 1991, the National Institute of Education (NIE) was established, as an institute of Nanyang Technological University. The move to universitise teacher education in Singapore enabled the profession to attract a better calibre of teacher-students, develop rigorous graduate and postgraduate programmes in education, recruit faculty with postgraduate qualifications, and invest in Singapore-centric education research. Though the journey has been long, steady progress towards universitisation has contributed significantly to Singapore's development, and the NIE can today claim to be an internationally recognised Institute of Distinction.

BACKGROUND: Mechanical thrombectomy (MT) has revolutionized the treatment of acute ischemic stroke (AIS) due to large vessel occlusion (LVO), but its efficacy and safety in medium vessel occlusion (MeVO) remain less explored. This multicenter, retrospective study aims to investigate the incidence and clinical outcomes of vessel perforations (confirmed by extravasation during an angiographic series) during MT for AIS caused by MeVO. METHODS: Data were collected from 37 academic centers across North America, Asia, and Europe between September 2017 and July 2021. A total of 1373 AIS patients with MeVO underwent MT. Baseline characteristics, procedural details, and clinical outcomes were analyzed. RESULTS: The incidence of vessel perforation was 4.8% (66/1373). Notably, our analysis indicates variations in perforation rates across different arterial segments: 8.9% in M3 segments, 4.3% in M2 segments, and 8.3% in A2 segments (p = 0.612). Patients with perforation had significantly worse outcomes, with lower rates of favorable angiographic outcomes (TICI 2c-3: 23% vs 58.9%, p

Modern micromanipulation techniques typically involve trapping using electromagnetic, acoustic, or flow fields that produce stresses on the trapped particles thereby precluding stress-free manipulations. Here, we show that by employing polyhedral symmetries in a multichannel microfluidic design, we can separate the tasks of displacing and trapping a particle into two distinct sets of flow operations, each characterized and protected by their unique groups of symmetries. By combining only the displacing uniform flow modes to entrain and move targeted particles in arbitrary directions, we were able to realize symmetry-protected, stress-free micromanipulation in 3D. Furthermore, we engineered complex, microscale paths by programming and controlling the flow within each channel in real time, resulting in multiple particles simultaneously following desired paths in the absence of any supervision or feedback. Our work therefore provides a general symmetry-group-based framework for understanding and engineering microfluidics and a novel platform for 3D stress-free manipulations. The costs of foraging can be high while also carrying significant risks, especially for consumers feeding at the top of the food chain. To mitigate these risks, many predators supplement active hunting with scavenging and kleptoparasitic behaviours, in some cases specializing in these alternative modes of predation. The factors that drive differential utilization of these tactics from species to species are not well understood. Here, we use an energetics approach to investigate the survival advantages of hunting, scavenging and kleptoparasitism as a function of predator, prey and potential competitor body sizes for terrestrial mammalian carnivores. The results of our framework reveal that predator tactics become more diverse closer to starvation, while the deployment of scavenging and kleptoparasitism is strongly constrained by the ratio of predator to prey body size. Our model accurately predicts a behavioural transition away from hunting towards alternative modes of predation with increasing prey size for predators spanning an order of magnitude in body size, closely matching observational data across a range of species. We then show that this behavioural boundary follows an allometric power-law scaling relationship where the predator size scales with an exponent nearing 3/4 with prey size, meaning that this behavioural switch occurs at relatively larger threshold prey body size for larger carnivores. We suggest that our approach may provide a holistic framework for guiding future observational efforts exploring the diverse array of predator foraging behaviours.

Throughout history, coronaviruses have posed challenges to both public health and the global economy; nevertheless, methods to combat them remain rudimentary, primarily due to the absence of experiments to understand the function of various viral components. Among these, membrane (M) proteins are one of the most elusive because of their small size and challenges with expression. Here, we report the development of an expression system to produce tens to hundreds of milligrams of M protein per liter of Escherichia coli culture. These large yields render many previously inaccessible structural and biophysical experiments feasible. Using cryo-electron microscopy and atomic force microscopy, we image and characterize individual membrane-incorporated M protein dimers and discover membrane thinning in the vicinity, which we validated with molecular dynamics simulations. Our results suggest that the resulting line tension, along with predicted induction of local membrane curvature, could ultimately drive viral assembly and budding.

Abstract Background A thorough psychosocial assessment is time-consuming, often requiring multiple sessions to uncover the psychological factors contributing to mental illness, such as depression. The duration varies depending on the severity of the patient’s condition and how effectively the psychotherapist can establish rapport. However, prolonged assessment periods pose a significant risk of patient deterioration. Methods The comprehensive psychosocial intervention, led by the Multi-Criteria Decision-Making (MCDM) approach utilizing the Multi-Objective Optimization by Ratio Analysis (MOORA) method, played a pivotal role in identifying the key psychological factors contributing to the depression of the client among the 21 factors specified by BDI-II analysis. Results The integration of the MOORA strategy compared to traditional psychotherapy on 254 samples demonstrates a Jaccard similarity coefficient of 0.8, with a minimum error margin of 7% (vulnerability index = 0.57), indicating a significant agreement between the two approaches, both converging towards a similar solution. For patients with extreme depression, the number of sessions reduced from 18 ± 2 to 11 ± 2, showing a 33–35% reduction (χ 2 = 6.94, p = 0.008). Severe depression patients experienced a reduction from 14 ± 2 to 8 ± 1 sessions i.e., 34–39% reduction (χ 2 = 8.32, p = 0.004). Moderate depression patients saw sessions drop from 9 ± 1 to 5 ± 1, i.e., 37–43% reduction (χ 2 = 0.29, p = 0.001). The accuracy for detecting dominant psychological factors improved to 82.88% for extreme, 86.74% for severe, and 90.34% for moderate depression, respectively. Conclusion The implementation of MOORA facilitated the identification and prioritization of key psychosocial intervention strategies, making the process significantly faster compared to traditional methods. This acceleration greatly enhanced the precision and efficacy of the work. Additionally, critical vulnerable factors were identified through ordered statistics and correlation analysis [Pearson (r) = 0.8929 and Spearman’s rank (ρ) = 0.7551] on the Beck Depression Inventory-II model. These findings were supported by other MCDM schemes such as EDAS and TOPSIS, demonstrating high stability and robustness in dynamic decision-making environments, maintaining consistency across scenarios adapted by different psychotherapists. Overall, the combined application of MCDM (MOORA) and targeted psychological interventions yielded substantial positive outcomes in enhancing the well-being of individuals with psychological illnesses, such as depression, cognitive, affective, and somatic syndromes.

Abstract Background Few randomized trials of public health and social measures were carried out during the COVID-19 pandemic. We report on the major barriers we faced when we tried to run such trials. Methods We reviewed all randomized trials proposed and initiated by the Centre for Epidemic Interventions Research in Norway during the COVID-19 pandemic. Results Of the 18 proposed trials, 11 trials were not implemented. One of the key legal and ethical barriers to conducting the trials was the Norwegian Health Research Act, which demands informed consent from all participants who are exposed to an intervention. A lack of sufficient political support was also a challenge, as was unpredictability, due to shifting disease incidence and changing recommendations from the authorities. Conclusion Strengthening the evidence base for public health and social measures in pandemics will require political and public understanding and support and a legal framework that allows for the conduct of such trials.

Background & Objective: In clinical practice, obtaining informed surgical consent is rooted in respecting a person's dignity, and failure to obtain informed consent has been labeled as medical negligence. Teaching this process to undergraduates can improve doctor[ –patient relationships, thereby decreasing medico U -legal litigation. This study investigated whether a multimodal training module for teaching final-year medical students surgical informed consent was effective in improving this skill. It aimed to determine the students' perception of multimodal training in taking informed consent for surgical procedures. Material & Methods: This single-group quasi-experimental study was conducted in the authors' teaching hospital in Kerala, India, among 35 final-year medical students from January to July 2022. It evaluated their ability to obtain surgical informed consent and the improvement following an educational intervention based on a multimodal training module using a prevalidated Directly Observed Procedural Skills assessment. Results: Following the intervention, students showed an improvement in their ability to take surgical informed consent and in assessment scores, which was found to be statistically significant (p < 0.001). Male students showed a greater improvement in the Knowledge subcomponent of assessment score as compared to females, which was statistically significant (p < 0.001). All the students, in their feedback, felt the training was feasible, made learning interesting, helped identify weaker sections, and retained concepts in memory. They opined that it made them confident in taking consent and that they would prefer this type of learning in the future. Conclusion: Multimodal modular training enhanced the proficiency of final-year medical students in obtaining informed surgical consent from simulated patients. This enriched their professionalism and interaction with simulated patients. Utilizing faculty as simulated patients needs further exploration. We recommend that the undergraduate medical curriculum incorporate a prevalidated multimodal training module and assessment tool for surgical informed consent.

Cells self-organize into functional, ordered structures during tissue morphogenesis, a process that is evocative of colloidal self-assembly into engineered soft materials. Understanding how intercellular mechanical interactions may drive the formation of ordered and functional multicellular structures is important in developmental biology and tissue engineering. Here, by combining an agent-based model for contractile cells on elastic substrates with endothelial cell culture experiments, we show that substrate deformation-mediated mechanical interactions between cells can cluster and align them into branched networks. Motivated by the structure and function of vasculogenic networks, we predict how measures of network connectivity like percolation probability and fractal dimension as well as local morphological features including junctions, branches, and rings depend on cell contractility and density and on substrate elastic properties including stiffness and compressibility. We predict and confirm with experiments that cell network formation is substrate stiffness dependent, being optimal at intermediate stiffness. We also show the agreement between experimental data and predicted cell cluster types by mapping a combined phase diagram in cell density substrate stiffness. Overall, we show that long-range, mechanical interactions provide an optimal and general strategy for multicellular self-organization, leading to more robust and efficient realizations of space-spanning networks than through just local intercellular interactions.

Background: Linear scleroderma (LS) is a variant of localized scleroderma, which is termed as “en coup de sabre” (ECDS) when it involves the head and/or face. Although mainly a dermatological condition, extracutaneous manifestations have been infrequently reported. Clinical Description: A 13-year-old boy presented with fever, altered sensorium with seizures, right hemiparesis, and right-sided upper motor neuron type of facial palsy. He was noticed to have a depression in the left frontoparietal area of the scalp with skin changes. Management and Outcome: The child was initially managed along the lines of acute encephalitis supported with mechanical ventilation. Brain imaging revealed tiny calcification in the left temporal and parietal lobes with altered signal intensities ipsilateral to the scalp lesion. Suspecting an association with the overlying scalp skin lesion, biopsy of the skin lesion was done which confirmed LS. The erythrocyte sedimentation rate was elevated and the antinuclear antibody was positive. The child was started on pulse methylprednisolone following which encephalopathy improved facilitating extubation. Later, he was started on subcutaneous methotrexate and he improved on follow-up. Conclusion: This case creates awareness regarding the association between neurological manifestations and LS EDCS, which responds favorably to immunosuppressives and methotrexate.

The present report describes the management of tumorous growth in the ear canal of a dog with total ear canal ablation and lateral bulla osteotomy. Clinical examination revealed a severely stenotic ear canal along with palpable tumorous growth. The animal was premedicated with atropine, diazepam, and butorphanol. General anesthesia was induced with propofol and maintained using isoflurane with oxygen. Total ear canal ablation and lateral bulla osteotomy were performed according to the standard procedure. The closure of the surgical site was done in two layers using polyglactin 910. The skin was sutured by using polypropylene. Sutures were removed after 14 days. Post-operative antibiotics, analgesics, and nerve supplements were given. Complete recovery of the animal was seen without any post-surgical complications.
