Abstract

BACKGROUND: While experimental data suggest that selective intracoronary hypothermia decreases infarct size, studies in patients with ST-elevation myocardial infarction (STEMI) are lacking. AIMS: We investigated the efficacy of selective intracoronary hypothermia during primary percutaneous coronary intervention (PCI) to decrease infarct size in patients with STEMI. METHODS: In this multicentre randomised controlled trial, 200 patients with large anterior wall STEMI were randomised 1:1 to selective intracoronary hypothermia during primary PCI or primary PCI alone. Using an over-the-wire balloon catheter for infusion of cold saline and a pressure-temperature wire to monitor the intracoronary temperature, the anterior myocardium distal to the occlusion was selectively cooled to 30-33°C for 7-10 minutes before reperfusion (occlusion phase), immediately followed by 10 minutes of cooling after reperfusion (reperfusion phase). The primary endpoint was infarct size as a percentage of left ventricular mass on cardiovascular magnetic resonance imaging after 3 months. RESULTS: Selective intracoronary hypothermia was performed in 94/100 patients randomised to cooling. Distal coronary temperature decreased by 6°C within 43 seconds (interquartile range [IQR] 18-113). The median duration of the occlusion phase and reperfusion phase were 8.2 minutes (IQR 7.2-9.0) and 9.1 minutes (IQR 8.2-10.0), respectively. The infarct size at 3 months was 23.1±12.5% in the selective intracoronary hypothermia group and 21.6±12.2% in the primary PCI alone group (p=0.43). The left ventricular ejection fraction at 3 months in each group were 49.1±10.2% and 50.1±10.4%, respectively (p=0.53). CONCLUSIONS: Selective intracoronary hypothermia during primary PCI in patients with anterior wall STEMI was feasible and safe but did not decrease infarct size compared with standard primary PCI. 10. Antiplatelet response to aspirin and clopidogrel in patients with coronary artery disease undergoing percutaneous coronary intervention

Abstract

Aspirin and clopidogrel are cornerstone therapies in cardiovascular disease. In particular, they are almost universally prescribed in patients undergoing percutaneous coronary intervention (PCI). Evidence has emerged of a variation in the antiplatelet effects of aspirin and clopidogrel between individual patients with a suggestion of an increased risk of adverse cardiovascular events. However, the optimal method of measuring response to aspirin and clopidogrel remains uncertain. In light of this, the antiplatelet effects of both aspirin and clopidogrel were studied in patients with coronary artery disease, concentrating on patients undergoing PCI. Initially, a pilot study of 40 patients investigated the use of thromboxane B2 (TxB2), VerifyNow Aspirin, VerifyNow P2Y12, platelet fibrinogen binding and intra-platelet vasodilator-stimulated phosphoprotein levels (VASP-PRI) to measure response to aspirin and clopidogrel. This was followed by a larger study assessing aspirin and clopidogrel response in 323 patients attending for coronary angiography with a view to PCI. These patients were tested by measuring TxB2, VerifyNow P2Y12, VASP and whole blood impedance platelet aggregation (WBPA). The primary objective was to investigate whether measures of aspirin or clopidogrel efficacy predicted peri-procedural myocardial necrosis following PCI. In addition, a small series of 10 patients had aspirin and clopidogrel response measured following stent thrombosis. A wide variation in the antiplatelet effects of both aspirin and clopidogrel was found by all measures. Correlation between assays ranged from moderate to poor. Of particular interest, it was found that measurement of [TxB2] may facilitate the assessment of aspirin response in patients already taking clopidogrel. There was a high incidence of myocardial necrosis following coronary intervention assessed by elevation of troponin I. Only VerifyNow P2Y12 and VASP-PRI were associated with a significantly increased frequency of myocardial necrosis following PCI. The data of this thesis confirm a wide variation in response to aspirin and clopidogrel. Good response to clopidogrel was associated with reduced myocardial necrosis during PCI. TxB2 may be the best measure of aspirin response for patients taking both therapies. How these measures may be incorporated into clinical practice remains uncertain.

Abstract

Objectives-\ud \ud The aim of this study was to determine the safety of selective intracoronary hypothermia during primary percutaneous coronary intervention (PPCI) in patients with anterior ST-segment elevation myocardial infarction (STEMI).\ud \ud Background-\ud \ud Selective intracoronary hypothermia is a novel treatment designed to reduce myocardial reperfusion injury and is currently being investigated in the ongoing randomized controlled EURO-ICE (European Intracoronary Cooling Evaluation in Patients With ST-Elevation Myocardial Infarction) trial (NCT03447834). Data on the safety of such a procedure during PPCI are still limited.\ud \ud Methods-\ud \ud The first 50 patients with anterior STEMI treated with selective intracoronary hypothermia during PPCI were included in this analysis and compared for safety with the first 50 patients randomized to the control group undergoing standard PPCI. In-hospital mortality, occurrence of rhythm or conduction disturbances, stent thrombosis, onset of heart failure during the procedure, and subsequent hospital admission were assessed.\ud \ud Results-\ud \ud In-hospital mortality was 0%. One patient in both groups developed cardiogenic shock. Atrial fibrillation occurred in 0 and 3 patients (P = 0.24), and ventricular fibrillation occurred in 5 and 3 patients (P = 0.72) in the intracoronary hypothermia group and control group, respectively. Stent thrombosis occurred in 2 patients in the intracoronary hypothermia group; 1 instance was intraprocedural, and the other occurred following interruption of dual-antiplatelet therapy consequent to an intracranial hemorrhage 6 days after enrollment. No stent thrombosis was observed in the control group (P = 0.50).\ud \ud Conclusions-\ud \ud Selective intracoronary hypothermia during PPCI in patients with anterior STEMI can be implemented within the routine of PPCI and seems to be safe. The final safety results will be reported at the end of the trial.

Novel parameters that detect failed microvascular reperfusion might identify better the patients likely to benefit from adjunctive treatments during primary percutaneous coronary intervention (PCI).The aim of this study was to test the hypothesis that a novel invasive parameter, the thermodilution-derived temperature recovery time (TRT), would be associated with microvascular obstruction (MVO) and prognosis.TRT was derived and validated in two independent ST-elevation myocardial infarction populations and was measured immediately post PCI. TRT was defined as the duration (seconds) from the nadir of the hyperaemic thermodilution curve to 20% from baseline body temperature. MVO extent (% left ventricular mass) was assessed by cardiovascular magnetic resonance imaging at 2-7 days.In the retrospective derivation cohort (n=271, mean age 60±12 years, 72% male), higher TRT was associated with more MVO (coefficient: 4.09 [95% CI: 2.70-5.48], p0.001), independently of IMR32, CFR ≤2, hyperaemic Tmnmedian, thermodilution waveform, age and ischaemic time. At five years, higher TRT was multivariably associated with all-cause death/heart failure hospitalisation (OR 4.14 [95% CI: 2.08-8.25], p0.001) and major adverse cardiac events (OR 4.05 [95% CI: 2.00-8.21], p0.001). In the validation population (n=144, mean age 59±11 years, 80% male), the findings were confirmed prospectively.TRT represents a novel diagnostic advance for predicting MVO and prognosis. Abstract

Objectives: The aim of this study was to determine the safety of selective intracoronary hypothermia during primary percutaneous coronary intervention (PPCI) in patients with anterior ST-segment elevation myocardial infarction (STEMI). Background: Selective intracoronary hypothermia is a novel treatment designed to reduce myocardial reperfusion injury and is currently being investigated in the ongoing randomized controlled EURO-ICE (European Intracoronary Cooling Evaluation in Patients With ST-Elevation Myocardial Infarction) trial (NCT03447834). Data on the safety of such a procedure during PPCI are still limited. Methods: The first 50 patients with anterior STEMI treated with selective intracoronary hypothermia during PPCI were included in this analysis and compared for safety with the first 50 patients randomized to the control group undergoing standard PPCI. In-hospital mortality, occurrence of rhythm or conduction disturbances, stent thrombosis, onset of heart failure during the procedure, and subsequent hospital admission were assessed. Results: In-hospital mortality was 0%. One patient in both groups developed cardiogenic shock. Atrial fibrillation occurred in 0 and 3 patients (P = 0.24), and ventricular fibrillation occurred in 5 and 3 patients (P = 0.72) in the intracoronary hypothermia group and control group, respectively. Stent thrombosis occurred in 2 patients in the intracoronary hypothermia group; 1 instance was intraprocedural, and the other occurred following interruption of dual-antiplatelet therapy consequent to an intracranial hemorrhage 6 days after enrollment. No stent thrombosis was observed in the control group (P = 0.50). Conclusions: Selective intracoronary hypothermia during PPCI in patients with anterior STEMI can be implemented within the routine of PPCI and seems to be safe. The final safety results will be reported at the end of the trial.

Background\ud \ud Segmental extent of infarction assessed by late gadolinium enhancement (LGE) imaging early post‐ST‐segment elevation myocardial infarction (STEMI) has utility in predicting left ventricular functional recovery.\ud \ud Hypothesis:\ud \ud We hypothesized that segmental circumferential strain with displacement encoding with stimulated echoes (DENSE) would be a stronger predictor of infarct transmurality than feature‐tracking strain, and noninferior to extracellular volume fraction (ECV).\ud \ud Study Type:\ud \ud Prospective.\ud \ud Population:\ud \ud Fifty participants (mean ± SD, 59 ± 9 years, 40 [80%] male) underwent cardiac MRI on day 1 post‐STEMI.\ud \ud Field‐Strength/Sequences:\ud \ud 1.5T/cine, DENSE , T1 mapping, ECV , LGE.\ud Assessment\ud \ud Two observers assessed segmental percentage LGE extent, presence of microvascular obstruction (MVO), circumferential and radial strain with DENSE and feature‐tracking, T1 relaxation times, and ECV.\ud \ud Statistical Tests:\ud \ud Normality was tested using the Shapiro–Wilk test. Abstract

This paper introduces a novel algorithm for selecting the optimal wafers for measurement given a set of selection rules. The algorithm is based on assigning a penalty to each of the sampling rules and then using a mixed-integer linear program to pick the wafers which minimize the sum of the penalties. By waiting until the metrology step to determine the best wafers to measure, a real-time decision can be made based on which wafers have previously been measured, the tools and chambers on which wafers were processed, and other user-specified selection criteria. The penalties can also be increased after a rule violation to ensure that each of the selection rules are satisfied at a finite frequency. Abstract

In microprocessor manufacturing it is very important to monitor the processed material on certain metrology steps. This is done in order to characterize the manufacturing process by verifying correct processing and ensuring availability of adequate information on the contributors to process variance. In state-of-the-art technologies it has become increasingly challenging to measure all possible and relevant sources of variation. Because low cycle time (manufacturing duration) is of such high importance, it is necessary to minimize the amount of inspection operations/duration on processed material. This paper details the integrated lot-level and wafer-level sampling application deployed in AMD's Fab30 and Fab36, which is capable of providing the means to capture lot, wafer, and event-based sources of variation by best utilizing the available metrology resources. It will also highlight the connection of this application into the areas of advanced-process-control, run-to-run, fault-detection and classification (APC, RtR, FDC). Abstract

As production requirements in semiconductor manufacturing continue to escalate, it is rarely possible to perform quality measurements on a product before subsequent process operations are performed. This delay between manufacturing and metrology coupled with inaccurate process models can lead to closed-loop instabilities. This paper investigates the effect of metrology delay on the closed-loop stability of a multiple input--multiple output exponentially weighted moving average run-to-run controller. The generalized Routh--Hurwitz stability criteria are used to derive the necessary and sufficient conditions for stability. A sufficient condition for stability is then derived for systems with any length of metrology delay. This condition states that if all of the eigenvalues of a model-mismatch matrix fall inside a circle with unit radius and centered at {1,0} on the complex plane, then the closed-loop system is stable. Abstract

Aims Identifying novel mediators of lethal myocardial reperfusion injury that can be targeted during primary percutaneous coronary intervention (PPCI) is key to limiting the progression of patients with ST-elevation myocardial infarction (STEMI) to heart failure. Here, we show through parallel clinical and integrative preclinical studies the significance of the protease cathepsin-L on cardiac function during reperfusion injury. Methods and results We found that direct cardiac release of cathepsin-L in STEMI patients (n  = 76) immediately post-PPCI leads to elevated serum cathepsin-L levels and that serum levels of cathepsin-L in the first 24 h post-reperfusion are associated with reduced cardiac contractile function and increased infarct size. Preclinical studies demonstrate that inhibition of cathepsin-L release following reperfusion injury with CAA0225 reduces infarct size and improves cardiac contractile function by limiting abnormal cardiomyocyte calcium handling and apoptosis. Conclusion Our findings suggest that cathepsin-L is a novel therapeutic target that could be exploited clinically to counteract the deleterious effects of acute reperfusion injury after an acute STEMI.
