3. The Forum BEVI Project: Applications and Implications for International, Multicultural, and Transformative Learning

Abstract

In a diverse portfolio of curricular and programmatic options in colleges and universities, global education has become highly prominent over the past 50 years. To take one of any number of indices in this regard, the Institute of International Education (2014) reported that a record number of global students, 886,052, studied at U.S. institutions in 2013-14 (up 8.1 percent over the previous year) and 289,408 American students studied abroad in 2012-13 (an increase of 2.1 percent over the previous year) (see also International Association of Universities, 2010; Knight, 2006). At a complementary level, institutions of higher education now are interested in "the internationalization of research, offering dual degrees with foreign partners, establishing a branch of campuses abroad," among other areas of emphasis (Marmolejo, 2011). Duru and Poyrazli (2007) contend that these sorts of trends demonstrate the important role that colleges and universities play in training workers for entry into the global marketplace. Likewise, Green (2013) notes that "higher education institutions in the United States are increasingly using the language of 'global citizenship' to describe the skills and habits they seek to cultivate in their students" (p. 52). Against this introductory backdrop, the authors begin this article with an overview of global education in the United States and a review of assessment instruments and approaches. They then go on to present 15 implications from the Forum-BEVI Project, a multi-year, multi-site assessment initiative that examined the processes and outcomes of international, multicultural, and transformative learning. Ultimately, it is the authors' hope that this project and its fifteen implications contribute to: (1) a deeper understanding of the complex interactions and fascinating processes that are associated with who learns what and why, and under what circumstances as well as; (2) greater appreciation for the demonstrable fact that such complexities can, and must, be assessed with depth, persistence, and rigor.

4. Endothelial dysfunction in the development of vascular complications in Systemic Sclerosis

Abstract

Systemic sclerosis (SSc) is an autoimmune, connective tissue disease characterised by endothelial dysfunction, collagen deposition and fibrosis. The leading causes of mortality, contributing to over 55% of deaths, are pulmonary hypertension (PH) and pulmonary fibrosis (PF). This thesis explored the contribution of endothelial cells in the development of pulmonary vascular complications in SSc. Blood-outgrowth endothelial progenitor cells (EPCs) are considered to support vascular repair, however their ability to do so in SSc patients is unclear. This thesis developed a robust method to culture EPCs from SSc and healthy donor (HC) blood, and explored their phenotype compared to pulmonary artery endothelial cells (PAECs). EPCs exhibited a number of endothelial characteristics including morphology, the ability to form barriers, and respond to permeability-inducing factors such as TNFα. EPCs exhibited greater Rac-1 activity and formed stronger cellular barriers which was Rac dependent. HC-EPCs were also more resistant to apoptosis compared to SSc-EPCs and PAECs. These observations suggest that HC-EPCs may aid vascular repair by providing apoptotic resistance and reducing endothelial permeability. In contrast to HC-EPCs and PAECs, SSc-EPCs supported greater levels of leukocyte trafficking. Thus SSc-EPCs may fail to ‘heal’ the endothelium and exacerbate endothelial dysfunction in SSc patients. Further, the differences between SSc-EPCs and HC-EPCs support their use as a surrogate for exploring endothelial dysfunction in SSc. The contribution of endothelial to mesenchymal transition (EndoMT) in SSc was also explored. Transitioning EndoMT cells were present in pulmonary arteries of SSc-PAH patients and pre-clinical models. The functional impact of EndoMT on endothelial function was explored by establishing a cytokine induced-EndoMT (I-EndoMT) model in vitro. I-EndoMT cells ‘lost’ endothelial and ‘gained’ mesenchymal cellular markers. EndoMT cells failed to form effective cellular barriers and secreted elevated levels of pro-inflammatory cytokines. Collectively this suggests that EndoMT may contribute to endothelial dysfunction and pathological remodelling in SSc-PAH patients.

Abstract

Cessation of one component of dual antiplatelet therapy (DAPT) following percutaneous coronary intervention (PCI) has been associated with increased risk of ischemic events but it is uncertain whether discontinuation of aspirin is preferable to discontinuation of the oral P2Y12 inhibitor. The GLOBAL LEADERS study compared two antiplatelet strategies following PCI, cessation of aspirin at 1 month with continued ticagrelor monotherapy for 23 months versus standard DAPT for 12 months followed by aspirin monotherapy for a further 12 months. We assessed recovery of platelet reactivity after withdrawal of either aspirin or ticagrelor at 1 month and 12 months, respectively, in this study. Platelet aggregation (PA) was assessed before cessation of DAPT (‘baseline’) and after 2, 7, and 14 days post-cessation using Multiplate whole-blood aggregometry with collagen, thrombin-receptor-activating peptide (TRAP), adenosine diphosphate (ADP) and arachidonic acid (AA) as agonists. Following cessation of aspirin at 1 month, there was marked recovery of PA induced by AA (baseline [mean ± SD]: 11.1 ± 7.4 U vs. 14 days: 64.9 ± 19.6 U, p < .0001) and collagen (37.4 ± 22.9 U vs. 79.8 ± 13.8 U, p < .0001), whereas PA induced by ADP (18.6 ± 6.6 vs. 69.1 ± 20.5, p < .0001) and collagen (34.4 ± 18.7 U vs. 43.0 ± 21.0, p = .0018) recovered following cessation of ticagrelor at 12 months. There were no significant changes in TRAP-induced PA in either group. In conclusion, cessation of either component of DAPT leads to substantial increase in platelet reactivity with differential effects on different pathways of platelet activation when aspirin or the P2Y12 inhibitor is stopped. Further work is required to determine which patients receive net benefit from long-term continuation of DAPT.
