Purpose: To validate automated counts of presumed anterior chamber (AC) cells in eyes with histories of uveitis involving the anterior segment using swept-source (SS) anterior segment optical coherence tomography (AS-OCT) against manual counts and compare automated counts against Standardized Uveitis Nomenclature (SUN) criteria., Methods: Eyes were imaged with the ANTERION SS AS-OCT device (Heidelberg Engineering). A fully automated custom algorithm quantified the number of hyper-reflective foci (HRF) in line-scan images. Automated and manual counts were compared using interclass correlation (ICC) and Pearson correlation coefficient. Automated counts were compared to SUN grades using a mixed-effects linear regression model., Results: 90 eyes (54 participants) were included; 67 eyes (41 participants) had histories of uveitis, while 23 eyes (13 healthy participants) served as controls. ICC comparing automated to manual counts was 0.99 and the Pearson correlation coefficient was 0.98. Eyes at each SUN grade with corresponding median HRF (interquartile range [IQR]) were: Grade 0, 42 eyes, 2 HRF (0,4); 0.5+, 10 eyes, 10 HRF (8,15); 1+, 9 eyes, 22 HRF (15,33); 2+, 3 eyes, 27 HRF; 3+, 2 eyes, 128 HRF; 4+, 1 eye, 474 HRF. For every 1-step increase in grade, automated count increased by 38 (p < 0.001) or 293% (Pearson correlation coefficient 0.80, p < 0.001). Automated counts differed significantly between clinically inactive eyes (2 HRF [0,4]) and controls (0 HRF [0,1], p = 0.02)., Conclusions: Our algorithm accurately counts HRF when compared to manual counts, with strong correlation to SUN clinical grades. SS AS-OCT offers the advantage of imaging of the entire AC and may allow detection of subclinical inflammation in eyes that appear clinically inactive., Competing Interests: Declarations. Background: Immune checkpoint inhibition (ICI) has changed the melanoma treatment spectrum. Few studies have examined the characteristics and long-term outcomes of patients achieving complete response (CR) under ICI., Materials and Methods: We evaluated patients with unresectable stage IV melanoma treated with first-line ICI. The characteristics of those achieving CR were compared with those not achieving CR. Progression-free survival (PFS) and overall survival (OS) were assessed. Late-onset toxicities, response to second-line treatment, the prognostic value of clinicopathologic features, and blood markers were examined., Results: A total of 265 patients were included; 41 (15.5%) achieved CR, while 224 (84.5%) had progressive disease, stable disease, or partial response. At the therapy start, those who had CR were more likely to be older than 65 years of age (p = 0.013), have a platelet-to-lymphocyte ratio below 213 (p = 0.036), and have lower lactate dehydrogenase levels (p = 0.008) than those not achieving a CR. For those who discontinued therapy after CR, the median follow-up time after CR was 56 months (interquartile range [IQR] 52-58) and the median time from CR to therapy end was 10 months (IQR 1-17). Five-year PFS after CR was 79% and 5-year OS was 83%. Most complete responders had a normalization of S100 at the time of CR (p < 0.001). In simple Cox regression analysis, age below 77 years at CR (p = 0.04) was associated with better prognosis after CR. Eight patients received second-line ICI; disease control was seen in 63%. Late immune-related toxicities occurred in 25% of patients, most being cutaneous immune-related toxicities., Conclusions: Response, according to the Response Evaluation Criteria in Solid Tumors (RECIST) criteria, is, until now, the most important prognostic factor, and CR is a valid surrogate marker for long-term survival in patients treated with ICI. Background: Anterior knee pain has been associated with sports activity, especially long-distance running and endurance sports. It is important to determine the incidence of anterior knee pain (AKP) in runners after a half-marathon race and identify possible risk factors associated., Methods: Cross-sectional study where runners from a half marathon race were randomly invited to participate. Participants were recruited at the race kit pickup site the day before the race. Eligible participants completed a survey regarding demographic information, running experience and training details. An orthopedic surgeon performed a physical examination and recorded the medical history. At the finish line, the participants were evaluated again for possible new injuries., Results: A total of 205 runners were included in the study, with a 98.5% follow-up rate (n = 203). 24% of runners had an injury at the end of the race (n = 49). Anterior knee pain was the most frequent injury (n = 12), followed by iliotibial band syndrome (n = 10), muscle cramps (n = 7) and hamstring tears (n = 4). Anterior knee pain had a statistically significant association with insufficient stretching of the hamstrings (p = 0.048) and finishing the race in more than 2 h (p = 0.014)., Conclusions: Anterior knee pain was the most frequent new injury in the half-marathon runners after the competition. Spending more than 2 h to finish the race and stretching the hamstrings by less than 70° in the supine position were risk factors for anterior knee pain., (© 2021 Delhi Orthopedic Association. Although the need for the management of complex socio-technical systems (STS) to be compatible with the nature of those systems is widely recognized, there are few guidelines on how to determine the actual extent of this compatibility. The purpose of this study is to assess how compatible the management of standardized procedures (SPs) is with the nature of a complex STS. To this end, a case study was made of a control room in an oil refinery, involving the following stages: (a) delimitation of the investigated STS; (b) description of the STS according to a set of characteristics of complex STS; (c) application of two types of questionnaires to thirty workers - one of them to assess their perceptions about the applicability of seven principles of SPs management in complex STS and the other to determine their perceptions about the actual use of these principles; and (d) a feedback meeting with workers to discuss the results of the assessment. A lactic-acid producing bacterium was isolated from the rumen of lambs with rumen acidosis. The cells were gram-positive, nonmotile, nonsporing, catalase negative spherical, 1.5-2.0 μm in diameter, and occur in pairs and tetrads. Analysis of 16S ribosomal RNA indicated that the rumen bacterium was a strain of Pediococcus acidilactici with 99% of nucleotide homology. This bacterium was sensible to monensin and lasalocid at the unique dose tested of 300 ppm. The concentration of lactic acid and DM degradation decreased (P<0.05) when monensin or lasalocid were added to the culture media after 24, 48 and 72 h of incubation. In contrast, total VFA concentration and pH were higher (P<0.05) in the culture media added with the ionophores. Up to now S. bovis is considered the main ruminal bacterium related with rumen acidosis, but the importance of P. acidilactici should be also reconsidered in experimental studies focused on the control rumen acidosis., (Copyright © 2010 Elsevier Ltd. The diabetic phenotype caused by the deletion of insulin receptor substrate-2 (Irs-2) in mice displays a sexual dimorphism. Whereas the majority of male Irs-2(-/-) mice are overtly diabetic by 12 weeks of age, female Irs-2(-/-) animals develop mild obesity and progress less rapidly to diabetes. Here we investigated β-cell function and lipolysis as potential explanations for the gender-related differences in this model. Glucose-stimulated insulin secretion was enhanced in islets from male null mice as compared to male WT whereas this response in female Irs-2(-/-) islets was identical to that of female controls. The ability of α(2)-adrenoceptor (α(2)-AR) agonists to inhibit insulin secretion was attenuated in male Irs2 null mice. Consistent with this, the expression of the α(2A)-AR was reduced in male Irs-2(-/-) islets. The response of male Irs-2(-/-) islets to forskolin was enhanced, owing to increased production of cAMP. Basal lipolysis was increased in male Irs-2(-/-) but decreased in female Irs-2(-/-) mice, concordant with the observation that adipose tissue is sparse in males whereas female Irs2 null mice are mildly obese. Adipocytes from both male and female Irs-2(-/-) were resistant to the anti-lipolytic effects of insulin but female Irs-2(-/-) fat cells were additionally resistant to the catabolic effects of beta-adrenergic agonists. This catecholamine resistance was associated with impaired generation of cAMP. Consequently, targets of cAMP-dependent protein kinase (PKA) which mediate lipolysis were not phosphorylated in adipose tissue of female Irs-2(-/-) mice. In biology, materials science, radiography quality control or film dosimetry in radiotherapy, a transmission densitometer is useful for measurements of optical density. The design proposed here is oriented to quality control in radiographic films. The instrument described here utilizes low-cost solid-state devices and is easy to construct. The use of 1-watt white light-emitting diode in this densitometer enables low power consumption and a cold light source. Moreover, the instrument does not need a reference light, which results in decreasing the number of parts and reducing the overall size of the apparatus.

Mandibular distraction is the treatment of choice for the surgical correction of mandibular hypoplasia of different etiology. It is a surgical technique that is less invasive and time intensive; also, it has a significantly decreased morbidity rate.The experience of lengthening mandibles in the last 17 years had shown that in more severe mandibular hypoplasia, better results will be obtained in the long term with earlier treatment, especially in the more severe grades of hemifacial microsomia types IIB and III. Orthodontics plays a very important role to correct postdistruction occlusal changes and to promote a balanced growth.Besides the impressive aesthetic and functional results obtained in most cases, undoubtedly, the simultaneous expansion of the overlying soft tissues is the mayor advantage of this technique when it is compared with conventional osteotomies.With this clinical experience in the mandible using distraction osteogenesis, we confirm the farewell to major osteotomies to treat a wide range of deformities of this sophisticate anatomic structure.

This article presents our approach to performing an endoscopically assisted facelift. The advantages of the subperiosteal dissection are discussed. The procedure is expoused considering different approaches and variations depending on each particular case. Surgical procedures for the forehead, cheeks, and neck are discussed separately. Each procedure considers key maneuvers to liberate and relocate the deep soft tissues; for example, the transpalpebral approach to the forehead, the transpalpebral approach to the centrofacial area, and the subperiosteal mandibular dissection. Indications, limitations, and some advice to avoid complications also were presented.

Five Angus x Simmental steers (average BW 259 kg) cannulated in the rumen, proximal duodenum, and terminal ileum were fed five diets in a 5 x 5 Latin square design. Experimental periods were 14 d in length, with 10 d of diet adaptation and 4 d of sample collection. The basal diet contained (percentage of diet DM) ammoniated corn cobs (50%), alfalfa hay (22%), cornstarch grits (13%), corn (6.7%), cane molasses (5%), and urea (1.25%). Three canola seed-containing diets and a diet containing Ca salts of long-chain fatty acids (Ca-LCFA) were formulated by replacing cornstarch grits from the basal diet with the test feedstuffs. Whole canola seed untreated, crushed, or treated with a caustic alkaline solution and an oxidant were included at 10% of diet DM. The Ca-LCFA diet contained (percentage of diet DM) canola meal (5%) and Megalac (5%). Diets containing untreated, crushed, and treated canola seed and Ca-LCFA contained, on average, 5.6% more total fatty acids than the basal diet. Steers were fed 5.3 kg DM/d (2.05% of initial BW) in 12 equal portions (every 2 h). Ruminal fermentation characteristics and digestibilities of OM, GE, N, NDF, and ADF were unaffected (P > .05) by diet. Biohydrogenation of total 18-carbon unsaturated fatty acids was greater (P < .05) for steers fed the crushed canola seed-containing diet (72.0%) than for steers fed the untreated (27.9%) and treated (38.6%) canola seed-containing diets. Digestibility of total 18-carbon fatty acids in the small intestine was greater for steers fed the crushed canola seed (58.9% of duodenal flow) rather than the untreated canola seed (28.4% of duodenal flow) and intermediate for steers fed the treated canola seed (47.0% of duodenal flow). Chemical treatment of whole canola seed may be a viable method for the postruminal delivery of intestinally available unsaturated fatty acids to ruminants.

Prolactin (PRL) is an immunostimulatory hormone that is increased in serum of patients with systemic lupus erythematosus (SLE). On the other hand, SLE and AIDS patients share some clinical and immunologic manifestations and probably share similar pathogenic factors and etiology. Based on the above, we decided to investigate the serum PRL levels in 22 asymptomatic HIV-infected patients, 32 patients with AIDS, 9 seronegative homosexual men, and 66 healthy controls. Antibodies to HIV were determined by a micro-ELISA system and by HIV-1 Western blot. Serum PRL was assayed by monoclonal antibodies. Serum PRL levels in the control group were 5.07 +/- 1.36 ng/ml and in the seronegative homosexual men were 5.81 +/- 1.45 ng/ml. Serum PRL levels in asymptomatic HIV-infected patients were 9.22 +/- 3.3 ng/ml and in AIDS patients were 10.26 +/- 2.5 ng/ml (P < 0.01 and P < 0.001, respectively, for both groups when compared to healthy controls). The difference between seronegative homosexual men and AIDS patients was P < 0.05. In the present study we found that serum PRL levels in AIDS patients are higher than in seronegative homosexual men and healthy controls; these results are in agreement with a previous published report. Since PRL is related to lymphocyte activation and lymphoproliferation, the finding may have monitoring, prognostic, and therapeutic implications.
