1. A modified DFC method based on full state and periodic gain for stabilization and chaos control

Abstract

A novel delayed feedback control based on full state is proposed. The designed scheme combines the difference between two delayed states and a periodic control gain. System stabilization is achieved in any hyperbolic unstable equilibrium point. The procedure to build the control is systematic and, the set of the stabilizing control parameters is rigorously featured by analytical arguments. An ad-hoc strategy based on the proposed scheme is implemented for control of chaos by stabilizing equilibrium points embedded in chaotic attractors. Simulated examples illustrate this strategy performance.

Abstract

Limitations of the delayed feedback control and of its extended versions have been fully treated in the literature. The oscillating delayed feedback control appears as a promising scheme to overcome this problem. In this work, two methods based on oscillatory delayed feedback control schemes for the continuous time case are dealt with. For both of them, stabilization of an equilibrium point in the general non-linear scalar case is rigorously proven. 21. Exploring the factors that affect the otucomes of a genre-based approach to listening

Abstract

The aim of this research is to explore the factors that affect listeners when they are introduced to the generic features of texts. In this intervention study, learners in a Mexican university are introduced to the linguistic features, generic structure and context of spoken narrative texts about a film, in the first and foreign language (L1) and (L2), in order to better understand the development of L2 listening skills. On the basis of participants’ stimulated recalls (SR) and semi-structured interviews, I analysed the listeners’ thought processes and listening strategies, after they are introduced to the generic features of narratives. While initially set up as experimental study with an experimental (EG) and a control group (CG), the qualitative data reveals that different listeners’ in both groups used different listening cognitive, metacognitive, affective and sociocultural strategies, during the intervention. The participants in this study are 34 undergraduates studying for an English language teaching degree. Seventeen students are enrolled in the fourth semester and take part in the CG and another seventeen in the EG. The CG attended a regular class, whereas the EG received instruction from a genre-based approach for listening purposes in a scaffolded way, going from guided deconstruction to independent reconstruction of spoken narratives. Participants analyse the texts’ social and textual elements including the context, language used, variations and organisation. The findings show that listening at a discourse level is multidimensional. Participants in the EG elaborate and incorporate different types of prior knowledge. While listening, they attempt to identify and contextualize social situations as well as structural elements based on knowledge gained through a genre-based approach. Findings unveil aspects affecting the outcomes of a genre based approach in listening and can help researchers appreciate the practicality of these findings for the understanding of listening from a genre-based perspective.

24. Social media mining for identification and exploration of health-related information from pregnant women

Abstract

Widespread use of social media has led to the generation of substantial amounts of information about individuals, including health-related information. Social media provides the opportunity to study health-related information about selected population groups who may be of interest for a particular study. In this paper, we explore the possibility of utilizing social media to perform targeted data collection and analysis from a particular population group -- pregnant women. We hypothesize that we can use social media to identify cohorts of pregnant women and follow them over time to analyze crucial health-related information. To identify potentially pregnant women, we employ simple rule-based searches that attempt to detect pregnancy announcements with moderate precision. To further filter out false positives and noise, we employ a supervised classifier using a small number of hand-annotated data. We then collect their posts over time to create longitudinal health timelines and attempt to divide the timelines into different pregnancy trimesters. Finally, we assess the usefulness of the timelines by performing a preliminary analysis to estimate drug intake patterns of our cohort at different trimesters. Our rule-based cohort identification technique collected 53,820 users over thirty months from Twitter. Our pregnancy announcement classification technique achieved an F-measure of 0.81 for the pregnancy class, resulting in 34,895 user timelines. Analysis of the timelines revealed that pertinent health-related information, such as drug-intake and adverse reactions can be mined from the data. Our approach to using user timelines in this fashion has produced very encouraging results and can be employed for other important tasks where cohorts, for which health-related information may not be available from other sources, are required to be followed over time to derive population-based estimates., Comment: 9 pages

Abstract

Adverse reactions caused by drugs following their release into the market are among the leading causes of death in many countries. The rapid growth of electronically available health related information, and the ability to process large volumes of them automatically, using natural language processing (NLP) and machine learning algorithms, have opened new opportunities for pharmacovigilance. Survey found that more than 70% of US Internet users consult the Internet when they require medical information. In recent years, research in this area has addressed for Adverse Drug Reaction (ADR) pharmacovigilance using social media, mainly Twitter and medical forums and websites. This paper will show the information which can be collected from a variety of Internet data sources and search engines, mainly Google Trends and Google Correlate. Abstract

BackgroundOutcomes in patients with chronic myeloid leukemia in blast phase (CML-BP) are historically dismal. Herein, the authors sought to analyze the characteristics, prognostic factors, and survival outcomes in patients with CML-BP in the tyrosine kinase inhibitor (TKI) era.MethodsA total of 477 patients with CML-BP were treated with a TKI at some point during the course of their CML. Cox proportional hazard models identified characteristics that were predictive of survival. Overall survival and failure-free survival were assessed. Optimal cutoff points for specific parameters were identified using classification and regression tree (CART) analysis.ResultsThe median age of the patients was 53 years (range, 16-84 years) and 64% were male. Approximately 80% of patients initially were diagnosed in the chronic phase of CML at a median of 41 months (range, 0.7-298 months) before transformation to CML-BP. De novo CML-BP occurred in 71 patients. Approximately 72% of patients received TKI therapy before CML-BP. The initial therapy for CML-BP included a TKI alone (35%), a TKI with chemotherapy (46%), and non-TKI therapies (19%). The median overall survival was 12 months and the median failure-free survival was 5 months. Abstract

BackgroundSome patients with chronic myeloid leukemia (CML) have a history of previous malignancies. To the authors' knowledge, outcomes for CML diagnosed in these patients have not been well described. The current study was conducted to determine the outcome of patients with CML and a history of prior malignancies.MethodsThe current study included patients who were enrolled in clinical trials of tyrosine kinase inhibitors as initial therapy for CML in chronic phase from July 2000 to January 2014.ResultsOf the 630 patients with CML who were treated with frontline tyrosine kinase inhibitors, 626 had a known prior malignancy status. Of these, 45 patients (7%) had a prior malignancy other than nonmelanoma skin cancer whereas 17 patients (3%) had a history of nonmelanoma skin cancers alone. Characteristics of CML were similar between the patients with no prior malignancy, those with a prior malignancy, and those with nonmelanoma skin cancer. Patients with a prior malignancy were found to have an older median age compared with the other 2 groups. The most common prior malignancies were nonmelanoma skin cancer in 20 patients, breast cancer in 11 patients, melanoma in 7 patients, prostate cancer in 6 patients, and colorectal cancer in 5 patients. With regard to CML, the event-free survival, transformation-free survival, and failure-free survival rates were found to be similar between the groups. There was a statistically significantly decreased survival in the group with a prior malignancy versus the group with no prior malignancy versus the group with nonmelanoma skin cancer. In a multivariate analysis, advanced age and an elevated creatinine level were found to be associated with worse survival after a diagnosis of CML.ConclusionsPatients with CML with a history of prior malignancies appear to have the same excellent outcome as patients with no prior malignancies. In the few instances in which concomitant therapy for other malignancies was required during therapy with tyrosine kinase inhibitors, this was able to be accomplished without significant toxicity. Abstract

BackgroundDisease-related symptoms impair the quality of life of patients with chronic lymphocytic leukaemia (CLL) who do not require systemic therapy. Available therapies are not specifically aimed at symptom control. Because stimulation of the B-cell receptor activates JAK2 in CLL cells and the JAK2 inhibitor ruxolitinib improves symptoms in patients with myelofibrosis, we postulated that ruxolitinib would improve disease-related symptoms in patients with CLL. We did a phase 2 trial of ruxolitinib to test this hypothesis.MethodsSymptomatic patients with CLL who did not require systemic therapy were enrolled at MD Anderson Cancer Center (Houston, TX, USA) between Sept 15, 2014, and Sept 20, 2015. Participants were given 10 mg ruxolitinib orally twice a day. Scores on the Brief Fatigue Inventory (BFI), CLL module of the MD Anderson Symptom Inventory (MDASI) and symptom-associated interference in daily activities, were assessed before treatment and after 3 months. Abstract

BackgroundLittle is known about outcomes of acute myeloid leukemia (AML) in adolescents and young adults (AYA). The purpose of this study is to determine the characteristics and outcomes of AYA AML patients in comparison to older adult patients with AML.Patients and methodsWe retrospectively analyzed all AML patients treated at our institution from 1965 to 2009 who were aged 16 to 29 years.ResultsAmong 3922 adult AML patients treated during this period, 432 (11%) were identified as AYA. Median age was 23 years (range, 16-29 years); 73 (17%) patients had core binding factor (CBF)-AML [inversion (16), translocation (8:21)], and 51 (12%) had acute promyelocytic leukemia. Complete remission (CR) rates were 93% for CBF AML, 78% for APL, 77% with diploid karyotype, and 68% for other AML. Univariate analysis demonstrated higher rates of CR, CR duration, and overall survival (OS) in the AYA group compared with older patients. On multivariate analysis, AYA age group was independently associated with improved CR rate and CR duration, with a trend for longer OS (P = .085).ConclusionOutcome of AYA AML patients is overall better than for older adults with AML. Despite improvements in treatments and outcomes over time, there is still need for improvement in AYA with AML particularly for those with AML other than CBF and APL.

Abstract

This study assessed the relevance of 2013 European LeukaemiaNet (ELN) response categories on patients treated with common frontline tyrosine kinase inhibitors (TKI) in chronic myeloid leukaemia in chronic phase (CML-CP). Four hundred and eighty-seven patients treated with imatinib (400 mg; IM 400, n = 70; 800 mg; IM800, n = 201), dasatinib (n = 107) or nilotinib (n = 109) were analysed. Intention to treat (ITT) analysis indicated that the proportion of patients falling into optimal, warning and failure ELN categories were 89%, 6%, 6% at 3 months, 78%, 17% and 6% at 6 months, and 75%, 13% and 13% at 12 months, respectively. Rates of optimal response at 3 months were 75% for IM400, 90% for IM800, 89% for dasatinib and 97% for nilotinib; 41%, 80%, 86% and 89% at 6 months; and 47%, 77%, 76% and 87% at 12 months, respectively. Patients achieving optimal response had longer eventfree (EFS), failurefree (FFS), transformationfree (TFS) and overall survival (OS) compared to warning and failure responses at all-time points. Treatment with imatinib 800, dasatinib or nilotinib predicted for achieving an optimal response. Optimal response predicted for significantly longer EFS, FFS, TFS and OS at 3, 6 and 12 months, irrespective of the TKI modality used. ELN response categories reliably predicted outcomes in CML patients receiving commonly used TKIs.

Abstract

BackgroundSeveral tyrosine kinase inhibitors (TKIs) are available for treatment of patients with chronic myeloid leukaemia in chronic phase (CML-CP). We analysed long-term molecular and cytogenetic response and survival outcomes for four TKI modalities used as frontline therapy for CML-CP.MethodsIn a retrospective cohort analysis, we included data from patients with CML-CP treated in prospective clinical trials with frontline TKI modalities at a single institution between July 31, 2000, and Sept 10, 2013. The main aim of the study was to determine whether achievement of complete cytogenetic response or major molecular response had similar prognostic implications irrespective of the frontline TKI modality used. We analysed each TKI modality for response assessment and analysed survival endpoints (event-free, failure-free, transformation-free, and overall survival) with the Kaplan-Meier method. Univariate and multivariate analyses were done with Cox proportional hazard regression.FindingsOur analysis included 482 patients who were treated with imatinib 400 mg daily (n=68), imatinib 800 mg daily (n=200), dasatinib 50 mg twice daily or 100 mg daily (n=106), or nilotinib 400 mg twice daily (n=108). More patients receiving imatinib 800 mg or second-generation TKIs (ie, dasatinib or nilotinib) achieved complete cytogenetic response (58 [87%] of 67 for imatinib 400 mg vs 180 [90%] of 199 for imatinib 800 mg, vs 100 [96%] of 104 for dasatinib vs 99 [93%] of 107 for nilotinib), major molecular response (51 [76%] vs 171 [86%] vs 93 [90%] vs 97 [91%]), and 4·5 log or higher reduction in BCR-ABL transcripts (MR(4·5) response 38 [57%] vs 148 [74%] vs 76 [71%] vs 76 [71%]). This finding was consistent over time (3-60 months). 5-year event free survival significantly differed between the imatinib 400 mg group and the other TKI groups (imatinib 800 mg p=0·029, dasatinib p=0·003, nilotinib p=0·031). There was no significant difference in 5-year failure-free survival (p=0·32, p=0·075, p=0·332), transformation-free survival (p=0·053, p=0·038, p=0·493), or overall survival (p=0·563, p=0·162, p=0·981). Multivariate analysis showed that therapy with imatinib 800 mg (HR 0·51, 95% CI 0·29-0·88, p=0·016), dasatinib (0·28, 0·12-0·66, p=0·004), or nilotinib (0·42, 0·20-0·89, p=0·024) predicted for better event-free survival compared with imatinib 400 mg, but that failure-free, transformation-free, and overall survival were similar irrespective of the TKI used. 28 (41%) patients receiving imatinib 400 mg, 85 (43%) receiving imatinib 800 mg, 23 (21%) receiving dasatinib, and 27 (25%) receiving nilotinib discontinued treatment for any reason.InterpretationTreatment with imatinib 800 mg or the second-generation TKIs dasatinib or nilotinib resulted in superior and deeper responses than did standard-dose imatinib, which were maintained after 5 years of follow-up.
