Introduction Mentalization or reflective functioning (RF) is the capacity to interpret oneself or the others in terms of internal mental states. Its failures have been linked to several mental disorders and interventions improving RF have a therapeutic effect. Mentalizing capacity of the parents influences the children’s attachment. The Reflective Functioning Questionnaire (RFQ-8) is a widely used tool for the assessment of RF. No instrument is available to assess general RF in Spanish-speaking samples. The aim of this study is to develop a Spanish version of the RFQ-8 and to evaluate its reliability and validity in the general population and in individuals with personality disorders. Methods 602 non-clinical and 41 personality disordered participants completed a Spanish translation of the RFQ and a battery of self-reported questionnaires assessing several RF related constructs (alexithymia, perspective taking, identity diffusion and mindfulness), psychopathology (general and specific) and interpersonal problems. Temporal stability was tested in a non-clinical sub-sample of 113 participants. Results Exploratory and confirmatory factor analyses suggested a one-factor structure in the Spanish version of the RFQ-8. RFQ-8 understood as a single scale was tested, with low scorings reflecting genuine mentalizing, and high scorings uncertainty. The questionnaire showed good internal consistence in both samples and moderate temporal stability in non-clinical sample. RFQ correlated significantly with identity diffusion, alexithymia, and general psychopathology in both samples; and with mindfulness, perspective taking, and interpersonal problems in clinical sample. Mean values of the scale were significantly higher in the clinical group. Discussion This study provides evidence that the Spanish version of the RFQ-8, understood as a single scale, has an adequate reliability and validity assessing failures in reflective functioning (i.e., hypomentalization) in general population

Sus fallos se han vinculado a distintos trastornos mentales y las intervenciones que la mejoran han demostrado ser terapéuticas. La FR de los padres influye en el apego de sus hijos. El Reflective Functioning Quetionnaire (RFQ8) ha sido ampliamente utilizado en la evaluación de la FR. Incluye una escala de incertidumbre (RFQi) y otra de certidumbre (RFQc). No existen instrumentos que midan la FR general en población española. El objetivo del estudio es desarrollar una versión española del RFQ y evaluarla psicométricamente en población general y trastornos de personalidad (TP). Metodología: 602 participantes no clínicos y 41 con TP completaron una traducción española del RFQ y una batería de cuestionarios autoaplicados evaluando constructos relacionados con la mentalización (alexitimia, toma de perspectiva, difusión de identidad y mindfulness), psicopatología general y específica, y problemas interpersonales. Se evaluó la estabilidad temporal en una submuestra no clínica de 113 participantes. Resultados: En el análisis factorial exploratorio un modelo bifactorial explicó el 53,02% de la varianza. El análisis factorial confirmatorio sugirió un modelo de dos factores, aunque la RFQi solo ajustó parcialmente al modelo. Ambas escalas mostraron buena consistencia interna en ambas muestras y una estabilidad temporal adecuada. La RFQi correlacionó significativamente con difusión de identidad, alexitimia, mindfulness, toma de perspectiva y psicopatología general. La RFQc mostro correlaciones inversas y también correlacionó con problemas intepersonales. Ambas escalas diferenciaron entre grupos conocidos, incluidos el grupo clínico y el no clínico. Discusión: Se proporciona la primera evidencia de una adecuada fiabilidad y validez de la versión española del RFQ en población general y TP.

[EN] Background Healthcare workers (HCWs) exposed to coronavirus 19 (COVID-19) are at high risk of developing mental health concerns across several domains. The aim of this study is to determine the updated, global frequency of these outcomes. Methods A multistep literature search was performed from database inception until March 1, 2021. PRISMA/MOOSE-compliant systematic review and PROSPERO protocol were used to identify studies reporting on depression, anxiety, acute stress, post-traumatic symptoms, insomnia, and burnout in HCWs exposed to COVID-19. A quantitative meta-analysis with random effects was conducted to analyze the proportion rate of the mental health disorders. Sensitivity analyses were performed to investigate the effect of the different continents and scales. Meta-regression analyses were conducted to examine the effect of gender, age, and work position. Results 239 articles were included (n = 271,319 HCWs, mean age = 36.08 +/- 8.33 (66.99% female). 33% HCWs exposed to COVID-19 reported depressive symptoms (95% confidence intervals [CI] = 28-38%), 42% anxiety features (95% CI = 35-48), 40% acute stress (95% CI = 32-47), 32% post-traumatic symptoms (95% CI = 26-37%), 42% insomnia (95% CI = 36-48), 37% burnout (95% CI = 31-42). Sensitivity analyses did not show statistically significant differences. Meta-regressions found a statistically significant lower prevalence of post-traumatic symptoms in Asia. Conclusions HCWs exposed to COVID-19 were found to have a significant prevalence of mental health concerns in all domains analyzed. The effects of COVID-19 on HCWs' mental health could be underestimated and the future consequences dismissed.

This study aims to meta-analytically characterize the presence and magnitude of within-group variability across neurocognitive functioning in young people at Clinical High-Risk for psychosis (CHR-P) and comparison groups. Multistep, PRISMA/MOOSE-compliant systematic review (PROSPERO-CRD42020192826) of the Web of Science database, Cochrane Central Register of Reviews and Ovid/PsycINFO and trial registries up to July 1, 2020. The risk of bias was assessed using a modified version of the NOS for cohort and cross-sectional studies. Original studies reporting neurocognitive functioning in individuals at CHR-P compared to healthy controls (HC) or first-episode psychosis (FEP) patients were included. The primary outcome was the random-effect meta-analytic variability ratios (VR). Secondary outcomes included the coefficient of variation ratios (CVR). Seventy-eight studies were included, relating to 5162 CHR-P individuals, 2865 HC and 486 FEP. The CHR-P group demonstrated higher variability compared to HC (in descending order of magnitude) in visual memory (VR: 1.41, 95% CI 1.02-1.94), executive functioning (VR: 1.31, 95% CI 1.18-1.45), verbal learning (VR: 1.29, 95% CI 1.15-1.45), premorbid IQ (VR: 1.27, 95% CI 1.09-1.49), processing speed (VR: 1.26, 95% CI 1.07-1.48), visual learning (VR: 1.20, 95% CI 1.07-1.34), and reasoning and problem solving (VR: 1.17, 95% CI 1.03-1.34). In the CVR analyses the variability in CHR-P population remains in the previous neurocognitive domains and emerged in attention/vigilance, working memory, social cognition, and visuospatial ability. The CHR-P group transitioning to psychosis showed greater VR in executive functioning compared to those not developing psychosis and compared to FEP groups. Clinical high risk for psychosis subjects shows increased variability in neurocognitive performance compared to HC. Insecure attachment has been described as mediating the relationship between childhood trauma and dysfunctional personality traits in different mental disorders. Despite the role insecure attachment and childhood trauma have independently demonstrated to play as determinants of borderline personality disorder, less is known about the mediating mechanisms explaining these associations. For the first time, we assessed adult attachment, childhood trauma and dimensional personality pathology in a sample of outpatients with borderline personality disorder and tested whether the association between childhood trauma and personality dysfunction was at least partially attributable to insecure attachment. The results showed that attachment anxiety fully mediated the relationship between specific types of trauma (emotional abuse and physical neglect) and emotional dysregulation. Further, emotional abuse was both directly associated with dissocial behaviour and indirectly via attachment anxiety (partial mediation). Emotional abuse has been described as an essential environmental factor for the development of borderline personality disorder and emotional dysregulation, on its part, as the core feature of the condition. Our results indicate that attachment anxiety explains the link between these central aspects of borderline personality disorder. Our findings are consistent with previous research and current etiological understanding of the condition and provide support for recommending a careful assessment of childhood traumatic experiences and adult attachment style to gain a more comprehensive insight into the symptoms and its heterogeneity. As a secondary aim, we assessed the effect parental mental illness may have in these mediation models, but no significant influence on childhood trauma, attachment or personality was found.

201 p., Mugako Nortasunaren Nahasmendua (MNN) atxikimenduaren zailtasunekin lotu izan da, helduaroan bereziki atxikimendu-antsietate maila altuarekin. Bestalde, MNN-n Aurpegi-Emozioen Antzematea (AEA) aztertu dituzten ikerketetan emaitza kontrajarriak argitaratu izan dira. Azkeneko ikerketen azterketek, MNN duten pazienteek aurpegi neutroetan emozioak ezartzeko joera izan dezaketela iradokitzen dute, emozio negatiboekiko isuria aurkeztuz. Horrez gain, atxikimendua eta aurpegi-emozioen antzematearen arteko asoziazioa aztertzen dituzten ikerketen gabezia dago. Tesi honen helburua, MNN duten pazienteen talde kliniko batean giroko aldagaiak (parentalitatea eta helduaroko atxikimendua) eta kognizio sozialeko prozesu jakin bat (AEA) aztertzea da, kontrol talde batekin konparatuz. Horrez gain, aipatutako giroko aldagaiek kognizio sozialeko aldagaiarekiko duten harremana aztertzea ere proposatzen da. Ikerketa proiektuan MNN duten pazienteen talde bat (n=120) kontrol talde (n=220) batekin konparatu egin zen giroko aldagaiak, kognizio sozialeko aldagai bat eta aldagai kliniko ezberdinak aztertuz. [EN] Background To determine the proportion of patients in symptomatic remission and recovery following a first-episode of psychosis (FEP). Methods A multistep literature search using the Web of Science database, Cochrane Central Register of Reviews, Ovid/PsychINFO, and trial registries from database inception to November 5, 2020, was performed. Cohort studies and randomized control trials (RCT) investigating the proportion of remission and recovery following a FEP were included. Two independent researchers searched, following PRISMA and MOOSE guidelines and using a PROSPERO protocol. We performed meta-analyses regarding the proportion of remission/recovery (symptomatic plus functional outcomes). Heterogeneity was measured employing Q statistics and I-2 test. To identify potential predictors, meta-regression analyses were conducted, as well as qualitative reporting of studies included in a systematic review. Sensitivity analyses were performed regarding different times of follow-up and type of studies. Results One hundred articles (82 cohorts and 18 RCTs) were included in the meta-analysis. The pooled proportion of symptomatic remission was 54% (95%CI [30, 49-58]) over a mean follow-up period of 43.57 months (SD = 51.82) in 76 studies. After excluding RCT from the sample, the proportion of remission remained similar (55%). The pooled proportion of recovery was 32% (95%CI [27-36]) over a mean follow-up period of 71.85 months (SD = 73.54) in 40 studies. After excluding RCT from the sample, the recovery proportion remained the same. No significant effect of any sociodemographic or clinical predictor was found. Conclusions Half of the patients are in symptomatic remission around 4 years after the FEP, while about a third show recovery after 5.5 years.

218 p., La hipótesis del continuum de la psicosis defiende que tanto sujetos de la población general, como pacientes con un trastorno de personalidad (TLP) o un brote psicótico manifiesto, comparten características cognitivas, comportamentales y emocionales. Por otro lado, los patrones de apego son muy importantes para comprender el papel que desempeñan los factores ambientales en la vulnerabilidad y el desarrollo de la psicosis. En la actualidad existe un número importante de estudios que exploran las asociaciones entre los estilos de apego, la cognición y los síntomas psicóticos. A pesar de ello, se requieren aún más estudios para poder aclarar cómo se relacionan entre sí.Este trabajo está orientado a tratar de conseguir un mejor conocimiento acerca del apego adulto y los trastornos mentales graves, como los primeros episodios psicóticos (PEP) y los TLP. Para ello se ha planteado un doble objetivo. Por un lado, y como principal objetivo, examinar la posible asociación de una variable ambiental como el apego adulto con alteraciones de la cognición social (concretamente las speech illusion y jumping to conclusions (JTC)) y las experiencias psicóticas (positivas, negativas y depresivas), en la muestra total y en los tres grupos de la muestra (PEP, TLP y controles). Y por otro lado, un segundo objetivo sería comparar las diferencias existentes entre los dos grupos clínicos y los controles, en relación al apego adulto, las alteraciones en la cognición social y las experiencias psicóticas.Se han utilizado para este estudio 293 sujetos, de ellos 70 eran pacientes con un PEP, 38 con TLP y 185 controles. Como principales instrumentos de medida utilizamos el ECR-S para el apego adulto, el White Noise para las speech illusion, el Beads para JTC y el CAPE para las experiencias psicóticas.En relación a los resultados, nuestros hallazgos no apoyaron la asociación entre el apego adulto inseguro y la experimentación de speech illusion. 305 p., El objetivo del trabajo fue analizar y describir la experiencia subjetiva como padre o madre de un adolescente que haya estado ingresado en una unidad de hospitalización de psiquiatría de niños y adolescentes. Participaron 10 padres y 12 madres en tres grupos focales que se realizaron para la obtención de la información.Del análisis cualitativo de la información emergieron seis categorías, Experiencia antes del ingreso, Experiencia durante el ingreso, y Experiencia después del ingreso, con una organización temporal de las mismas, y, las categorías de Propuestas de mejora, Culpa y Estigma que se encuentran presentes a lo largo de toda la experiencia relatada y modulan en el caso de las dos últimas la experiencia global.Esta investigación cualitativa aporta información sobre la experiencia de los padres de la que no se tiene conocimiento que hubiera sido recogida anteriormente mediante esta metodología en nuestro país. La utilización de esta información podría mejorar la atención a los padres y niños y adolescentes usuarios de las unidades de hospitalización psiquiátrica del niño y adolescente.
